ASSERTIVE COMMUNITY TREATMENT (ACT) The ACT team provides frequent, intensive services to individuals as outlined in the person’s individualized, person-centered treatment plan. The number of contacts ranges from multiple contacts daily to once weekly, depending upon the person’s needs.

Who is ACT for?

Individuals with serious mental illness with difficulty managing medications without ongoing support, or with psychotic/affective symptoms despite medication compliance. Persons who exhibit co-occurring psychiatric and substance use related disorders. Persons who are frequent users of inpatient hospital services, emergency services, crisis residential services, or homeless shelters.

Services include (offered 1-4 times weekly): Case management Therapy Psychiatric services Nursing services Housing support Employment support
