ELIGIBILITY AND ENROLLMENT Who Is Eligible for Benefits

Qualifying Life Events

Employees who work 30 hours or more per week are eligible to enroll in benefits.

Examples include:

• Birth or adoption of a child • Loss of other coverage • Change in dependent eligibility

• Domestic partner (if applicable)

Benefit elections must typically be made within 30 days of the life event.

How to Enroll

Benefits typically begin on your hire date, unless otherwise specified. The benefit year spans from June 1 to May 31.

When You Can Enroll

Utilize www.employeenavigator.com to enroll in or adjust benefits such as health insurance, dental insurance, vision, life insurance, HSAs, HRAs, and Flexible Spending Accounts.

MEDICAL The School offers Cigna Open Access Plans. Key features of the OAPs include no referral requirements, no mandatory primary care provider (PCP) requirement, and access to a nationwide network of in-network physicians.

DENTAL The school utilizes DR Administrative Services, Inc. as the claims administrator for its dental program.

Summary The first $125 of dental expense is reimbursed at 100%.

Then a $50 deductible is applied.

Then 70% to the annual maximum of $1,800.

Orthodontic Benefit is part of the dental.

The School offers additional Vision coverage through EyeMed to help pay for routine eye care and corrective lenses.

HEALTH SAVINGS, HEALTH REIMBURSEMENT & FLEXIBLE SPENDING Health Savings Account (HSA) Employees enrolled in a High Deductible Health Plan through Eaglebrook may be eligible for an HSA. Employees with other health coverage (including Medicare, Medicaid, and Mass Health) cannot enroll in the HSA per IRS regulations. Eaglebrook will contribute to the HSA in order to partially subsidize the high deductible health insurance plans offered according to the schedule indicated in the “Health Insurance” section above.

HSA Key Points

Health Reimbursement Accounts (HRA) Employees who are enrolled in the high deductible health insurance plan, and who are also ineligible for the HSA, will be enrolled in the HRA so that Eaglebrook can partially subsidize the high deductible health insurance plans offered according to the schedule indicated in the “Health Insurance” section above.

HRA Key Points • Unused funds roll over each year, until the employee leaves Eaglebrook.

•E mployees can contribute up to $4,400 (individual) or $8,750 (family) (less Eaglebrook’s contribution).

•E mployee contributions reduce taxable income.

Flexible Spending Accounts (FSA)

•W ithdrawals for qualified expenses are tax-free (per IRS Section 213).

An FSA is an account that allows you to set aside pre-tax money from your paycheck to pay for eligible out-of-pocket healthcare or dependent care expenses.

•C an be used to pay for various out-of-pocket health care costs in retirement, including, Medicare Part B and Part D premiums, Medicare Advantage plan premiums, Deductibles, copayments, and coinsurance under any part of Medicare, Tax-qualified long-term care insurance premiums.

•T ransitions at age 65, from a strictly medical savings tool into a highly flexible retirement account, subject to certain restrictions.

FSA Key Points •A healthcare FSA can be used to pay for qualified medical expenses. Employees may contribute up to $3,400 per year.

•A dependent care FSA can be used to pay for child or elder care expenses. Employees may contribute up to $7,500 per year.

•A limited purpose FSA can be used for vision and dental care. Employees may contribute up to $3,400 per year.

• F unds must be used within the plan year.

RETIREMENT PLAN The School offers a retirement savings plan to help employees prepare for the future.

Changing Your Contribution

After two years of school experience (at Eaglebrook or another school or combined), the School contributes 8% of your salary to the plan if you contribute 5% or more of your salary. If you are in the maintenance or dining services departments, you are entitled to a contribution of 8% from the school after two years of service without being required to make your own contributions.

INCOME PROTECTION Life Insurance The School provides basic life insurance coverage at no cost to eligible employees through the Standard at a rate of one times annual salary up to $50,000. After the age of 65, this benefit begins to reduce.

Additional, optional supplemental life insurance available for employees and dependents through the Standard.

Employees are eligible to take this coverage with them when their employment at Eaglebrook ends.

Paid Family Leave The Massachusetts Paid Family and Medical Leave (PFML) program lets many workers in Massachusetts take paid time off for major life or health events. You can receive partial pay for up to 26 total weeks per year to: recover from a serious illness, bond with a new child, care for a seriously ill family member, or manage issues related to a family member’s military deployment.

Long Term Disability Insurance (LTD) Disability insurance provides income protection for eligible employees who are unable to work due to illness or injury. You are eligible for LTD if you are actively working at least 37.5 hours per week. LTD replaces 60% of your eligible earnings subject to certain limitations after a waiting period of 90 days.

PAID TIME OFF Paid time off is available for benefit eligible non-faculty employees.

These employees will accrue vacation time each month. Vacation time accrues 10 or 13.33 hours per month based on an employment period of less than eight years or more than eight years.

Benefit eligible non-faculty employees are awarded one personal day at the start of the year.

Benefit eligible non-faculty employees accrue four hours of sick time at the start of each month.

EAP & SEPARATION Employee Assistance Program (EAP) Eaglebrook has EAP programs through both Cigna (mycigna.com) and the Standard (workhealthlife.com/Standard3).

EAPs can help in areas such as emotional health, home life referrals (child care, senior care, adoption, etc.), legal services (family law, estate planning, real estate concerts, etc.), addiction, and identity theft.

Separation Leaving Eaglebrook?

Schedule an exit interview with Human Resources to discuss taking certain benefits with you.

Leaving Eaglebrook as of July 1?

Plan to enroll in benefits during open enrollment so that you are covered for the month of June.

Retiring?

Schedule an exit interview with Human Resources to discuss meeting with a Medicare consultant and your retirement plan benefits.
