Among other things, it defines the characteristics of the organization of the company and has the mission of establishing authority, hierarchy, organizational charts, and departments, among other things. All organizations must have an organizational structure according to the tasks or activities they intend to do, through an appropriate structure that allows them to set their functions, and areas with the intention of producing products or services through a correct order that facilitates the achievement of business objectives.

All workers must be clear about their role in the system, and through this order, a series of protocols for action and control results are specified (Huber, 2010). In order to follow an organizational structure, a leader or a manager is needed. Positive leadership is exercised by those who are capable of guiding the efforts of a work team towards achieving the objectives and goals of the organization (Karefoot, 2015). An organizational structure has a vision and mission a leader must follow in order to lead other employees in that direction. An organizational structure can shape a leader or manager into learning how to be confident, to promote dialogue within the employees and to become an innovator (Huber, 2010).

A leader or manager also improves the skill of decision-making, the person becomes a motivator, the one that takes the initiative, and further improves the skills of listening and delegation. A leader must identify with the mission and vision of an organization because this one will shape the path of the employees, company and the method of achieving the goals (Karefoot, 2015).

Organizational structure plays a pivotal role in shaping not only the operational efficiency of a healthcare organization but also its capacity to deliver quality patient care. Within the nursing sector, a well-defined organizational structure ensures clarity of roles, accountability, and a clear chain of command, which ultimately impacts patient outcomes. The importance of an effective organizational framework is underscored by its influence on leadership practices, staff motivation, and decision-making processes. In healthcare, especially nursing, organizational structures determine how roles and responsibilities are allocated, facilitating smooth communication and coordination among interdisciplinary teams. According to Van Wormer and Bartels (2018), an effective organizational structure fosters an environment where

nurses can exercise clinical judgment confidently, supported by clear protocols and leadership guidance. This clarity reduces errors, enhances patient safety, and improves overall care quality. Furthermore, it empowers nurses to participate actively in decision-making processes, fostering a culture of shared governance, which has been linked to increased job satisfaction and retention (Harper et al., 2019).

Leadership within nursing organizations is intrinsically tied to the organizational structure. Leaders are tasked with guiding their teams toward shared goals, ensuring adherence to organizational mission and vision. Effective leaders inspire confidence, promote open dialogue, and foster innovation among staff. As noted by Van Wormer and Bartels (2018), transformational leadership styles are particularly effective in healthcare settings because they motivate staff, promote professional development, and enhance team cohesion. These leadership qualities are critical in adapting to the constant changes and complexities prevalent in healthcare environments.

Decision-making is another core aspect influenced by organizational design. In nursing, decentralized structures often empower staff at various levels to make clinical decisions, which can lead to more responsive and patient-centered care. Conversely, highly centralized structures may inhibit initiative but ensure consistency. Research by Harper et al. (2019) indicates that nursing units with participative organizational frameworks tend to have higher staff morale and better patient outcomes, highlighting the centrality of organizational design in fostering effective decision-making.

Moreover, a nurse leader’s alignment with the organization's mission and vision plays a significant role in shaping staff behavior and organizational culture. Leaders must embody the core values of their institutions, modeling desired behaviors and promoting a shared sense of purpose. This alignment encourages staff engagement, accountability, and a collective pursuit of excellence in patient care (Van Wormer & Bartels, 2018).

In conclusion, organizational structure is fundamental in nursing practice because it directly influences leadership effectiveness, decision-making, communication, and ultimately, patient outcomes. Effective leaders who understand and navigate their organizational frameworks can foster environments where high-quality, patient-centered care is the norm. As healthcare continues to evolve, ongoing evaluation and optimization of organizational structures remain essential to meet the challenges of modern nursing practice.
