Implant Design & Surface Mini-threads with Double-lead Threads for Faster Insertion The body of all Spectra-System implants are tapered evenly down from the top to facilitate insertion in an undersized socket (stop at intermediate drill) for expansion in soft bone. This increases initial stability (G. Niznick: Clinical Oral Health 2000 “Achieving Success in Soft Bone”). The SBM textured surface extends over the entire endosseous portion of the Spectra-System implants. Mini-threads start 1mm below the crest of the bone and extend for 2-2.5mm before transitioning into double-lead threads to the apex of the implant. The mini-threads are half the depth and half the distance between adjacent peaks compared to the double-lead threads that extend to the apex of the implant. Mini-threads reduce stress in the critical crestal bone region while double-lead threads reduce the number of turns required to seat the implant. Vertical cutting grooves extend halfway up the implant from the apex for self-tapping insertion.

SBM and HA Coated Surface Options SBM surface is created by using a Soluble Blast Media of Hydroxylapatite (HA) crystals to create a medium-rough texture. We use the same vendor for our SBM and HA coated surface treatments as was used by Paragon Implant Company from 1992-2000 when these studies were conducted.

ScrewPlant & ScrewPlus Fixture-Mount Packaging The implant is mounted on a color-coded fixture-mount designed for use as a transfer & shortened for use as a straight abutment.

A snap-on comfort cap is provided for the modified abutment. A cover screw is included in each vial. The ScrewPlant & ScrewPlus also include a 2mm extender retained by the cover screw for use as a healing collar.

Clinical & Laboratory Procedures for Contoured Abutments Orienting the 15º Angled Abutment The flat side of the Fixture-Mount/Transfer [Fig. 1] & the angle of the 15° abutment [Fig. 2] are manufactured to align with the flat of the implant’s internal hexagon.

To establish the optimum angle of the abutment, index the flat surface of the Fixture-Mount/Transfer either toward or away from the desired direction of abutment angle at the time of implant placement.

1. Use a forceps to remove the comfort caps exposing the tops of the implants.

2. Select the Titanium Temporary Coping matching the prosthetic platform. Titanium Coping [4000-69] is supplied with fixation screws [1000-01] and [1000-05]. An optional longer screw [1000-10] is also available.

3. Place the fixation screw through the coping & tighten using the 1.25mmD Hex Tool.

Standard torque value for fixation screws is 30Ncm.

4. Confirm there is less than 40 degrees of relative divergence between copings to be splinted together.

5. Mark top of Titanium Temporary Copings with indelible marker. Place denture on top of copings to transfer their position to under surface of denture.

6. Confirm markings on under surface of denture. Use acrylic trimmer to drill holes through denture corresponding to marked positions.

7. Seat denture over Titanium Temporary Copings to confirm passive placement.

Mark copings to indicate required vertical reduction prior to luting to denture base.

8. Occlude screw access hole with cotton pellet. Use autopolymerizing acrylic or similar light cure material to attach prosthesis to Titanium Temporary Copings. Allow to set prior to removal from mouth.

9. Remove prosthesis from the mouth and flip over to expose tissue side. Use additional autopolymerizing resin to fill in any voids around the processed copings.

10a & b. Attach implant replicas to copings and tighten using 1.25mmD Hex Tool. Pour a working cast using a soft tissue replication material if required due to anatomical undercuts.

11. Remove prosthesis from working cast and trim to allow access for patient cleaning.

The prosthesis is now a fixed restoration, removable by clinician only.

12. Attach prosthesis to implants & tighten in place using the appropriate tooling. Occlude screw access holes with material of choice.

1. Use the black plastic component to carry the GoDirect implant to site. Initiate self-tapping of implant into osteotomy by rotating black carrier.

Remove black plastic carrier once implant is stabilized.

2. Use either the [GDT] or [GDIT for 4.7mmD] insertion tool or [GDD] insertion drill to rotate the GoDirect implant into the osteotomy.

Stabilize the assembly with downward finger pressure.

3. The gold colored section of the implant should extend a minimum of 1.5mm above the soft tissue.

4. Attach the Parallel Posts [1000-97] to the implants to determine the divergence If the relative divergence between two assemblies is less than 20° use the white, red, orange or green GPS Male.

05a. Chairside Process: Place the White BlockOut Spacer over the head of the implant. Attach the metal cap with Black Processing Male from Processing Cap/Kit [1000-98S] or [1000-98SI].

5b. Chairside Process: Mark location of components on under surface of prosthesis.

Relieve the areas within the prosthesis so it can be fully seated over the metal cap without interference. Process the cap into denture following standard procedures.

6a. Transfer Process: Place the Impression Coping [4500-02] onto the implant. Inject impression material around the coping & record a full-arch impression.

6b. Laboratory Procedure: Insert the Implant Analogs [4500-06] into the Impression Copings [4500-02]. The laboratory fabricates the working cast.

7. Place the white Block-Out Spacer over the head of the Implant Analog. Attach the metal cap with Black Processing Male. Process the denture following standard procedures.

8. Pull on the Black Processing Male at an angle & remove it from the metal cap. If needed, wedge a tool against the bottom inside of the black plastic to aid in removal.

9. Use the Male Seating Tool to firmly push the Replacement Male of choice into the empty metal cap located within the denture.

10. Insert the finished prosthesis. Instruct patient on standard cleaning instructions.
