Publisher’s note and disclaimer At Support at Home, we go to great lengths (and make more than a few phone calls) to ensure the information in this guide is accurate, relevant, and genuinely useful. We source our content from government departments, industry bodies, aged care providers, and direct interviews — all with the aim of helping you and your family make informed decisions.
This guide is proudly published by Care & Co Media, Australia’s largest platform dedicated to helping people find the right care. We’re also committed to protecting the planet we all call home. That’s why this guide is printed locally on recycled paper, using environmentally responsible production processes to reduce our footprint wherever possible.
That said, aged care is a fast-moving sector. Prices, services, and policies can — and do — change. While we make every effort to publish accurate information at the time of printing, we accept no liability for any errors, omissions, or changes that occur after publication. All information provided is general in nature and should not be considered personal, financial, legal, or clinical advice. Welcome to the 3rd edition of the Support at Home Guide.
Message from COTA The aged care system is undergoing long-overdue reform - driven by the Royal Commission and shaped by the voices of older Australians who want their rights respected, their choices heard, and their care delivered with dignity. COTA Australia continues to advocate for a system built on transparency, accountability and individual choice - one where everyone has access to clear, trustworthy information to make decisions about their care including support at home or retirement living. Resources like the Aged Care Guide play an important role in helping people understand what’s available and how to find support that suits their needs. Together, we’re committed to empowering older Australians with trustworthy, easy-tounderstand information. Your voice matters and as the new Aged Care Act and reforms continue to take effect, we encourage you to stay informed and share your experiences, as we focus on older Australians' rights and empowering people to live their best life as we age.

“Mum wants to stay home. Where do I start?”
When Dad passed, Mum said she wanted to stay put. I became the researcher, the organiser and (quietly) the worrier. The jargon felt heavy. So we did one simple thing first: we wrote down what a good week looks like for Mum. Safe shower. Fresh meals. Bowls on Tuesdays. A check-in if I’m late from work. We called My Aged Care for an in-home assessment and kept a little notebook with meds, falls, and Mum’s goals. While we waited, we jumped onto agedcareguide.com.au to see who helps in our suburb — personal care, cleaning, meal prep, transport, physio, even small safety tweaks. That made the system feel human again.
6
Western Australia Edition 03
Waitlists happen, so we booked a couple of privately paid visits to keep Mum safe and confident. When our Support at Home budget (transitioned from her old package) kicked in, we compared providers, looked at published prices, and chose a team that reviews progress, not just rosters. We didn’t do everything. We did the right things: rails in the bathroom, two meal-prep visits, transport to bowls, monthly physio. Mum’s still home. Still herself. If you’re where we were: breathe, write down what matters, book the assessment, and use Aged Care Guide to shortlist providers. You don’t have to wait to start help if you can fund a little privately. Kathy Western Australia

Support at Home

Support at Home: What’s changing? Australia has simplified in-home care. From 1 November 2025, in-home care is delivered under Support at Home. The help itself is familiar — personal care, help around the house, nursing and allied health, equipment and home modifications — but Support at Home brings clearer service categories, published price lists and more regular plan reviews so you can see exactly what you’re paying for.
If you previously had a Home Care Package (HCP) You don’t need to re-apply as you will move across automatically. Your budget will be aligned to your current Support at Home classification, and any unspent funds will carry over. Your provider should contact you beforehand to review your goals, explain how their prices map to the new categories and update your service agreement. If something no longer fits, ask about alternatives under the new rules.
If you use the Commonwealth Home Support Programme (CHSP) Nothing changes yet. CHSP continues for now and is expected to transition to Support at Home later (CHSP continuing until 30 June 2029). Keep using the services that work for you and stay in touch with your provider about timing.

What you can do now » Take stock. Note what supports are helping and what’s not quite right. » Talk to providers. Ask how they’re preparing, how they will price services under Support at Home, and how often they’ll review your plan. » Bridge any gaps. If you’re waiting for services or need more hours, you can pay privately for care in the interim and transition that support into your plan later if eligible. » Keep records. Save quotes and care notes — they make reviews easier. Need a hand comparing providers or sense-checking fees? Visit: agedcareguide.com.au to explore options and get practical guidance tailored to your situation.

Nutritious meals made easy. Mealtimes should be something to look forward to and with the right support, they can be! While many older Australians face challenges like reduced appetites, cooking difficulties, or the hassle of shopping, there’s an easier way to enjoy delicious, nutritious meals every day. With Lite n’ Easy’s My Choice range, older adults can enjoy tasty, nourishing meals designed to support health, independence and quality of life.
Why good nutrition matters As we age, our bodies need the right fuel to stay strong and healthy. Including enough protein in your diet is important for maintaining muscle mass and supporting overall wellbeing. That’s why My Choice dinners are designed to provide the protein you need in a meal size appropriate for smaller appetites, without the oversized servings that can feel overwhelming. Every meal in the My Choice range has been developed by Accredited Practising Dietitians and prepared by chefs, so you can trust that each meal meets strict nutritional standards while delivering the flavour you love.
Trusted by thousands of older Australians It's not just about convenience Lite n' Easy is helping older Australians feel healthier and more independent every day. A recent survey of Home Care Package (HCP)* customers found that 89 per cent believe their quality of life has improved since ordering Lite n' Easy.
"I still love my food but grew tired of cooking for myself, so when a friend introduced me to Lite n' Easy, I gave it a try. I was immediately impressed," said Bill. "The My Choice meals are delicious, easy to prepare and being frozen means there is no waste and plenty of variety to choose from. "My Home Care Package covers most of the cost, which allows me to order every week. With the right nutrition and support, I can continue to enjoy the activities that keep me healthy."
How to get started If you’re receiving Support at Home funding, Lite n’ Easy’s My Choice meals can be delivered to your door with up to 70 per cent of the meal-preparation and delivery cost covered by your package. The remaining 30%, which covers the cost of ingredients, is paid upfront by you to Lite n’ Easy. Once your Support at Home Provider has set you up as a Lite n’ Easy customer, you can start ordering meals straight away and have them delivered to your door within the week. That means you can enjoy delicious, dietitian-designed and chef-prepared meals without delay - a simple way to take stress out of daily living while making the most of your government-funded home care support from day one. What are the first steps when finding home care?
Support at Home

Starting the journey to organise home care for yourself or a loved one can feel overwhelming — but you’re not alone. Whether you’re looking for government-funded services or private help at home, knowing where to begin makes the process much more manageable.
Step 1:
Step 2:
Understand what “home care” is
Check your eligibility
Home care supports older Australians to stay independent in their own home for longer. Services can include:
To access government-funded support you generally need to be:
» Help with cleaning, cooking or showering
» be 50 or older if you are an Aboriginal or Torres Strait Islander person, or
» Transport to appointments » Nursing or allied health support » Social connection and companionship You can choose government-funded home care or pay for services privately. Many people do a mix of both.
» 65 or older, or
» be 50 or older if you are homeless or at risk of homelessness. If you're under 65, you may still be eligible in limited circumstances, such as if you're already receiving government-funded aged care services. If you meet the eligibility criteria, you'll need a free aged care needs assessment, which you can arrange through My Aged Care.

Step 3: Get assessed
(Single Assessment System) Call My Aged Care on 1800 200 422 or apply online at myagedcare.gov.au to organise an assessment. An assessor will visit your home to talk through your situation and understand the level of support you need. Australia now uses a Single Assessment System, which replaces previous assessment services and provides one consistent process.
If you are approved for the Restorative Care Pathway or End-of-Life Pathway, there is no waitlist for this funding.
Step 5: Choose a provider Once approved, compare providers on: » Services offered » Costs and published prices
Depending on your needs, you may be approved for:
» Availability in your area
» Entry-level support at home, or
Use agedcareguide.com.au/compare to find and compare providers near you, then contact your shortlist directly. (Providers publish standard prices under Support at Home, so ask for a one-page example showing your monthly hours and any out-of-pocket costs.)
» More comprehensive, ongoing care through the Support at Home program.
Step 4: Wait times and funding If you are approved for ongoing Support at Home services, you may need to wait for your funding to become available. Funding is allocated through the Support at Home Priority System, based on your priority category and the date you were approved. There are four priority categories, with current estimated wait times: » Urgent: within 1 month » High: 1 month » Medium: 5–6 months » Standard: 7–8 months Your priority category will be included in your assessment outcome.

These wait times are estimates and may change, so check My Aged Care for the latest information.

Costs of home care explained Support at Home is subsidised by the Australian Government. Depending on the services you receive and your financial circumstances, you may be asked to contribute toward the cost of some services. Your contribution depends on the type of service you receive:
Clinical supports Clinical services are fully government funded, so you do not pay a participant contribution for approved clinical supports.
Independence services You may pay a contribution of between 5% and 50% of the service price, depending on your income and assets.
Everyday living services You may pay a contribution of between 17.5% and 80% of the service price, depending on your income and assets.
Private or additional services If you choose services outside your approved Support at Home funding, or services that are not covered by the program, you may need to pay the full cost privately.
20
Western Australia Edition 03
Check your contribution estimate Services Australia will assess your financial circumstances and advise you and your provider of the contribution rate that applies to you. Some people who were receiving, approved for, or waiting for a Home Care Package before 12 September 2024 may be covered by the Government’s “no worse off” arrangements and may pay different contribution rates. For current contribution rates and to estimate what you may pay, visit myagedcare.gov.au. (scan QR code).
Important Update: From 1 October 2026, approved personal care services will move into the clinical supports contribution category. Getting help from a home care expert Navigating aged care can be complex, with financial, legal, and care decisions that affect your comfort, independence, and long-term wellbeing. Getting help from the right professionals can make this process smoother and give you confidence in the decisions you make.
Financial advisers The type of help you need depends on your situation. Some people seek financial advice to plan for costs, others need to find the support quickly, and many families look for legal advice to manage important documents. When selecting an aged care expert, consider: » Experience and qualifications – Look for professionals with proven experience in aged care and appropriate certifications (e.g. CFP for financial advisers, legal accreditation for solicitors). » Memberships – Check that they belong to recognised professional associations such as the Financial Advice Association of Australia or the Law Society of Western Australia. » Client feedback – Read reviews on the Aged Care Guide or similar platforms to understand how others have rated their services.
Financial advisers – understanding and funding aged care The financial side of aged care can be complicated. A specialist aged-care financial adviser can help you: » Plan for in-home care costs. » Maximise your Age Pension or DVA entitlements. » Ensure your chosen payment plan supports your lifestyle and estate goals. Financial advisers can also help families compare government support through the Support at Home Program (for care at home) or residential care that best fits their needs. 24

Support at Home: Priorities and waitlists Australia has simplified in-home care. Support at Home commenced on 1 November 2025, replacing Home Care Packages and ShortTerm Restorative Care. If you were already on a Home Care Package, you moved across automatically; your services and any unspent funds continued under the new rules. The Commonwealth Home Support Programme (CHSP) has been extended to 30 June 2029 while the Australian Government consults on the longer-term design of entry-level aged care.
Your priority category is determined during your aged care assessment and will be included in your assessment outcome letter. These wait times are estimates and may change as new information becomes available.
Who gets priority?
» Stay in touch: keep your My Aged Care details up to date; tell them if your situation changes so your priority can be reviewed.
Everyone seeking subsidised support completes a Single Assessment System (formerly ACAT/ ACAS) through My Aged Care. Your assessor looks at your health, safety, living situation and supports. People with urgent needs are placed at the highest priority for assignment of funding.
How the queue works now Under the previous HCP system, eligible people entered a national queue (the National Priority System) and were assigned funding based on priority and time waited. Support at Home continues to allocate funding based on assessed need and priority; exact timeframes can vary by location, demand and available program funding. Providers must publish prices and review plans routinely, which helps you see what your budget can buy once funding is assigned. If you were waiting for a Home Care Package before 1 November 2025: you did not need to re-apply. Your place and priority carried into Support at Home. (My Aged Care notes that published HCP wait-time estimates up to June 2025 don’t apply to the new program.)
If your needs change while you are waiting, contact My Aged Care or your assessment organisation.
Ways to keep things moving
» Ask your provider how they’re preparing: providers were required to update operations, discuss new prices and review agreements with clients before transition. » Consider bridging with private services: if you have the means, you can pay privately for help at home while you wait, then transition those services under your Support at Home budget when it’s assigned. (This has always been allowed and remains available under the new program.)
Where CHSP fits If you receive entry-level help through CHSP, your services will continue as usual for now. CHSP will operate under the new Aged Care Act from 1 November 2025, with a separate transition to the Support at Home program expected in the future. CHSP has been extended to 30 June 2029 while the Australian Government consults on the longer-term design of entry-level aged care.
How long will I have to wait? Funding is allocated through the Support at Home Priority System. Current estimated wait times are: » Urgent: within 1 month » High: 1 month » Medium: 5–6 months » Standard: 7–8 months 32

Self-managed Support at Home: How it works If you like to be hands-on, you can self-manage your Support at Home services. You'll still work with a registered provider, who holds your funding and remains responsible for quality, safety and ensuring your services meet the requirements of the Support at Home program. You can take on more of the day-to-day organising, such as choosing workers, scheduling visits and approving invoices.
What’s changed under Support at Home Support at Home commenced on 1 November 2025 and has replaced Home Care Packages and Short-Term Restorative Care. Existing HCP clients will move across automatically. CHSP will continue for now and transition later in stages.
Support at Home introduces: Standardised service categories » Mandatory published prices » Routine plan reviews » Greater transparency and flexibility for consumers
Your role vs your provider’s ou: Y » Choose and organise workers » Compare quotes and approve bills » Monitor your spending, records and goals Your provider: » Must agree to support self-management. » Hold and manage your funds » Ensure your workers (including platform or independent workers) meet safety and eligibility requirements » Check that services are appropriate for your plan » Remain accountable for all services delivered under your plan Friendly help If you’d like guidance comparing self-managed vs fully managed options (and real-world fees), talk to our team or browse providers on Aged Care Guide for a local, apples-to-apples view. 34
Western Australia Edition 03
What it costs Self-management isn't "fee-free". Under Support at Home, 10% of your ongoing quarterly budget is allocated to care management, even if you choose to self-manage. Providers must publish their prices so you can compare costs, including: » Care Management » Other services they deliver under the program Depending on the services you receive and your financial circumstances, you may also pay participant contributions. Your provider will explain which services attract a contribution and which are fully government funded.
What you can spend on Funds must go toward eligible supports, including: » Personal care » Cleaning and domestic assistance » Nursing and allied health » Equipment, assistive technology and home modifications approved in your plan Your provider should help you check eligibility before you commit to any service.
Is self-management right for you? Self-management suits people who feel confident coordinating supports and want maximum control over who delivers their care. If you prefer someone else to organise everything – or your needs are complex – you can choose a more fully managed option. Your plan will still be reviewed regularly and adjusted if your needs change.

How to compare home care providers after you’ve been assigned a package From 1 November 2025, all in-home aged care will be delivered under the Support at Home program. If you previously held a Home Care Package (HCP), you will move across automatically - with no need to reapply. Your plan and budget will now follow Support at Home rules. The Commonwealth Home Support Programme (CHSP) has been extended to 30 June 2029 while the Australian Government consults on the longer-term design of entry-level aged care. If you’ve recently been approved for Support at Home, your next step is choosing a provider to manage your plan, coordinate your supports, and help you stay safe and independent at home. Providers differ in pricing, flexibility and service approach - so it’s important to compare them carefully.
Step 1:
Step 3:
Match services to your needs
Look for flexibility
Start by identifying the support you require for example:
Can you choose your own support worker? Will they use your preferred physiotherapist or cleaner? Providers who offer flexibility and control usually deliver a more personalised experience.
» personal care » domestic assistance » transport » nursing and allied health » social connection or companionship Some providers specialise in complex care; others focus on wellness, reablement or social support. Ensure their service mix aligns with your goals.
Step 2:
Check for cultural fit & reputation Ask friends or family for recommendations, read reviews, and speak with providers about language or cultural preferences. A good provider will listen and tailor your plan.
Understand the fees – what’s standard vs optional
Step 5:
Under Support at Home, providers must publish clear, transparent prices for all services - including care management and plan administration.
You’re not locked in. If the fit isn’t right, you can switch. Your arrangements move with you and a new provider can support the transition.
Prices must show exactly what is included, such as labour, travel, scheduling and administration. Always ask for: » a full published price list, and » a simple monthly example showing what your total care hours will look like after all costs. 36
Step 4:

What Providers Might Differ On
Care management
For ongoing Support at Home services, 10% of your quarterly budget is allocated to care management. This applies whether or not you self-manage.
How care management is delivered, including the level of contact, coordination and support provided.
Service Rates (personal care, domestic assistance, nursing, allied health)
Providers set their own service prices. Prices must be reasonable, transparent and published. Separate administration fees cannot be charged.
Hourly/service rates, minimum visit times, cancellation arrangements and other agreed service pricing.
Third-party / self-managed services
Where third-party workers are used, your provider remains responsible for the services delivered.
Providers may charge an overhead for arranging third-party services. For self-managed third-party services, this is capped at 10% of the actual cost and must be included in the final service price.
Exit / Transfer Fees
No separate exit/transfer fee should be charged under Support at Home.
Ask your provider about the process and notice requirements for changing providers.
Top-up / Private‑Pay Services
Allowed - any private rates must be published, reasonable and agreed upfront.
Some providers offer flexible private-pay options or additional service bundles.
Tip: Instead of focusing on individual fees, ask: “How many hours of direct care will I receive each week after all costs are deducted?” Need help comparing? Respite care at home and in an aged care home
What is respite care? Respite care is short-term support so a carer can rest, work, travel or simply recharge. It can happen: » At home (a paid support worker visits), » In the community (day or overnight centre-based programs), or » In an aged care home (a temporary stay with 24-hour support). These options are designed to support both the older person and their carer, keeping routines safe and sustainable.
Who can get governmentsubsidised respite? Get assessed (for subsidised care): organise a Single Assessment (formerly ACAT/ACAS) through My Aged Care — the national assessment now used for all government-funded aged care, including Support at Home.
How much residential respite can you use? You can use up to 63 days of subsidised residential respite per financial year. If you need more, your assessor can approve extensions of up to 21 days at a time. These days can be taken in blocks or spread across the year.

Western Australia Edition 03
Costs at a glance » Residential respite (in an aged care home): You’ll usually pay a basic daily fee set by the Government at 85% of the single basic Age Pension. The exact dollar amount is indexed and updated periodically, so check the current rate when booking. Homes may also charge a small booking fee — ask the provider to outline this before you confirm dates. » At-home or centre-based respite: From 1 November 2025, these services are available through Support at Home (and some services continue under the Commonwealth Home Support Programme (CHSP) during transition). Providers publish their prices, and you’ll pay a client contribution based on those prices; ask for a written quote and any concession policies before you start. ood to know: If you need respite G quickly or prefer extra services, you can choose to pay privately without waiting for assessment or funded availability. Your funded supports (if any) can still be used later.

A guide to home safety & modifications for older people As we age, our homes should continue to be a place of comfort, not risk. But everyday activities - like getting in and out of the shower, climbing stairs, or simply reaching for a cupboard - can become more difficult and potentially dangerous over time. That’s why home safety and smart modifications are important considerations to remain independent for longer.

Common home hazards
Types of home modifications
Even the most familiar home can present safety risks. Here are some of the most common concerns:
Home modifications can range from basic equipment to major structural changes, depending on the needs of the individual.
» Slippery floors, especially in bathrooms or kitchens
Simple modifications
» Poor lighting, which increases the risk of tripping or falls » Steps or uneven surfaces at entrances or within rooms » Cluttered spaces that limit movement or access to mobility aids » Hard-to-reach storage that requires bending or stretching » Bathrooms without rails or seating, which can become high-risk zones A professional home safety assessment can help identify which modifications may reduce these risks and support your mobility and wellbeing.
46
Western Australia Edition 03
» Grab rails in bathrooms, toilets, or along hallways » Non-slip flooring or floor mats » Shower chairs, hand-held shower heads » Lever-style door handles and tap turners » Ramps or threshold wedges for easier entry
Complex modifications » Bathroom renovations (e.g. walk-in showers, raised toilets) » Stair lifts or platform lifts » Kitchen redesign for accessibility » Widened doorways for mobility equipment These changes don’t just support safety they promote independence and dignity.

Moving into care is about more than a place to live – it’s about support, dignity and peace of mind. Western Australia Edition 03
Planning ahead: Wills, powers of attorney & enduring power of guardianship Protecting your choices, values and future wellbeing As we age, having the right legal documents in place helps ensure your wishes are followed - and it takes pressure off your loved ones during difficult times. There are two main areas to plan for: » Your money and property » Your healthcare and lifestyle decisions Here’s a guide to the most important tools:
Enduring Power of Guardianship (financial) » What it does: Lets you appoint someone you trust to manage your money and property - such as paying bills, accessing bank accounts, or selling assets if needed. » When it takes effect: You choose. It can start immediately or only if you lose capacity. » Why it matters: Without this, no one - not even your partner - can legally access your finances unless they apply to the courts.
Enduring Power of Guardianship (personal, lifestyle and treatment) In Western Australia, medical and personal decision-making is covered by an Enduring Power of Guardianship (EPG). » What it does: Allows someone you appoint to make personal, lifestyle and medical treatment decisions on your behalf if you lose decision-making capacity. Financial decisions are covered separately under an Enduring Power of Attorney (EPA).
What does an Executor of a will do? Understanding how this role differs from Powers of Attorney and Guardianship. An Executor is the person named in a will to carry out the wishes of someone after they have died. It’s a role that only begins after death, and it is different from the roles of Enduring Power of Attorney and Enduring Guardian, which apply while someone is still alive but may have lost capacity.
The Executor’s key responsibilities: » Locate and secure the will » Apply for probate, if required (a legal process confirming the will is valid) » Arrange the funeral (if not already planned or prepaid) » Notify banks, Centrelink, super funds, and other institutions » Identify and value assets and debts » Distribute the estate according to the will » Manage tax returns and financial closures
» Where it applies: Used in Western Australia.
» Deal with disputes or legal claims against the estate
» Why it matters: Ensures someone who understands your values can make informed medical and personal decisions if you’re unable to speak for yourself.
It’s a job that can take months, sometimes over a year, depending on the complexity of the estate. agedcareguide.com.au

Who should you choose?
» You can appoint different people for each role, or the same person for all.
The Executor should be someone:
» These roles are legally binding and remain in effect if you lose capacity—unlike general powers of attorney.
» You trust to act responsibly
» Rules vary by state, so check local requirements or speak to a solicitor.
» Ideally, not much older than you. Many people choose a family member, close friend, or solicitor. You can also appoint joint Executors if needed.

» Make sure your appointed decision-makers know your wishes and have access to the documents when needed. Putting these documents in place now protects your independence and your peace of mind.

Legacy, funeral planning & honouring a life Talking about death isn’t easy - but planning ahead can ease the burden for everyone. It’s not just about logistics or legalities. It’s about honouring a life well lived and making sure that, when the time comes, things are carried out with care and clarity.
Why planning matters Just as we plan for a birth or a wedding, planning for end-of-life helps loved ones feel more prepared and less overwhelmed. Whether it’s choosing between a burial or cremation, deciding on music or readings, or documenting preferences for a memorial, having these conversations early ensures wishes are respected.
A personal legacy Legacy is more than just paperwork it’s about stories, values and memories. Many people choose to leave a personal mark: writing letters to loved ones, recording a short video, creating a memory book or nominating a cause for donations in their name. These are simple but meaningful ways to leave behind something lasting. 62
Western Australia Edition 03
Funeral planning basics You don’t need to know all the answers, but a few early decisions can make a big difference. Consider: » Appointing someone to manage your funeral arrangements » Pre-selecting a funeral director or service » Documenting music, poems, or religious/ cultural preferences » Noting if you’d prefer a smaller private ceremony or public celebration repaid funeral plans are also an option, P offering financial and emotional relief for families later.
Support when it’s needed For those supporting a loved one through this process, grief can begin long before the final goodbye. Take time to talk, listen and ask what’s important to them. When the time comes, having a plan already in place allows families to focus on what matters most - being there for each other. Preparing for the end doesn’t mean giving up - it means showing up, with love and intention.
Planning ahead: Advance Care Planning (ACP) Making sure your voice is heard, even if you can’t speak for yourself.
How do I get one?
Advance Care Planning is all about making your healthcare wishes known before you’re ever in a situation where you can’t make decisions yourself - such as during a serious illness or after a medical emergency.
» Talking to your doctor or aged care provider
It gives you the opportunity to say what matters to you, and it gives your loved ones peace of mind knowing they’re doing what you would want.
What is an Advance Health Directive? An Advance Health Directive (AHD) is the legal document you complete as part of your planning. It outlines: » The types of treatment you would or wouldn’t want (like life support, CPR, or feeding tubes) » Your preferences for comfort and care (such as pain relief, staying at home, or cultural/religious wishes) » Who you want to make decisions for you if you’re unable to
Do I need one? You don’t have to have one, but it’s strongly encouraged — especially if: » You’re moving into aged care » You’re managing a long-term illness or dementia » You simply want to stay in control of your future healthcare Aged care homes will usually ask about this when you move in. While it’s not required by law, it helps staff deliver care that respects your values and choices.
You can start the process by: » Visiting advancecareplanning.org.au to download forms for your state or territory » Appointing a trusted person to act on your behalf (often called a substitute decisionmaker or enduring guardian)
Things to include in your plan: » “If I can’t recognise my family, I want comfort care only” » “I would want every medical treatment if there’s a chance of recovery” » “I’d like music or prayer during my care” » “I do not want to be transferred to hospital unless necessary”
Advance Care Planning helps you stay in charge of your care - even when life doesn’t go to plan. It’s not just about preparing for the end – it’s about protecting your values and giving your loved ones confidence in doing what’s right for you.

Funerals: A thoughtful approach to planning ahead

Why pre-planning matters Choosing a funeral director early means your wishes, or those of your loved one, are respected and carried out without urgency. It also ensures key details such as burial or cremation preferences, cultural or religious practices, and service arrangements - are documented and communicated in advance.

Choosing the right funeral director

Recommendations matter: Ask friends, family, aged care staff or your GP for referrals.
Planning & Preparation
4
Services provided: Ask what’s included - transport, paperwork, service coordination, viewing, or cultural rituals. When moving into an aged care home, families are often asked whether a funeral director has been nominated. While it may feel confronting, having these details organised in advance can ease emotional and practical pressure when the time comes. Making decisions during a time of grief can be overwhelming - planning ahead gives families clarity, confidence, and peace of mind.

Communication & care: Choose someone who listens, explains clearly, and offers compassionate support. Transparency on costs: Request a full itemised quote and clarify which services are optional. Cultural & personal fit: Make sure they can honour spiritual, cultural or family preferences. Use the directory - Aged Care Guide
What happens after a resident passes away? When a resident passes away in an aged care home, the staff will contact the nominated next of kin and, if a funeral director has been selected, arrange for them to be notified. The home will typically assist with initial paperwork and provide time for family and friends to visit, say goodbye, and begin making arrangements.
Over the past five years, more than 3,500 families across Western Australia have entrusted Purslowe & Chipper Funerals with the care of their loved ones. For many, it’s not just the service itself, but the way they are supported: gently, personally, and with quiet attention to what matters most. In-home care services
The following pages list approved in-home care service providers and the types of support they offer to help older Australians remain independent in their own homes. From government-funded options like CHSP and DVA programs to private care and specialist services, these tables outline what each provider offers at a glance. For additional information or to confirm service availability in your area, contact providers directly using the contact details listed, or visit agedcareguide.com.au and search for them by name.
CHSP funded services
DVA services
The Commonwealth Home Support Programme (CHSP) funds providers to deliver entry-level support that helps older people stay at home. Services may include domestic help, transport, nursing, home maintenance, and allied health. Providers must follow CHSP service guidelines to ensure appropriate care.
The Department of Veterans’ Affairs (DVA) supports eligible veterans and their families with a range of in-home care services. These include Veterans’ Home Care (VHC) for basic support, Community Nursing for more complex needs, respite care, and household assistance – to help veterans remain independent at home.
Private funding available Private funding means paying for home care services directly, without government subsidies. It offers faster access, more provider choice, and flexibility – ideal for those not yet approved for funding, waiting for a package, or needing extra support beyond what’s included in their current plan.
VHC services Veterans’ Home Care (VHC), funded by DVA, provides basic in-home help for eligible veterans and dependents. Services include personal care, domestic help, garden and home maintenance, and respite. VHC is best suited for those needing low-level support – not complex or high-care services.

Support at Home
If you’ve been approved for a government-funded Support at Home (SAH), you have the option to choose an approved provider to deliver the care and support you need at home. These services are designed to help you stay safe, comfortable, and independent in your own home for as long as possible. Some providers offer support in just one local area, while others may service a wider region – so it’s worth comparing what’s available near you.
How to use the following pages On the following pages, you’ll find an alphabetical list of approved Support at Home providers, organised by region to help make your search easier. To simplify things further, each of the 11 regions has been broken down into local council districts. Use the maps and lists on pages 80-81 as a quick reference to find your region and council group. For example, the Nungarin is grouped under Mid West and colour-coded to match. A simpler way to search The directory is arranged in alphabetical order to help you find services quickly and easily. Each provider is listed with a contact phone number – so you can reach out directly and speak to someone without needing to go online. Should you need more detailed information about any provider, visit agedcareguide.com.au and search for them by name.
