Inside these pages, you’ll find relevant, up-to-date and accurate information to assist you on your journey. From navigating the NDIS and learning about the different types of disabilities, right through to finding disability supports that suit your needs.

In the first part of the Guide, you will find information articles on a range of topics, alongside inspiring feature stories from fellow Australians who share their journey of navigating the system and seeking disability supports for themselves or their loved one.

In the back section of the book, starting from page 28, you’ll find five state and territory chapters covering: In-home & Community Support, Therapists & Specialists, Accommodation, Advisors, Equipment & Technology and Mental Health. In this Disability Support Guide, we’ll explain what NDIS-funded supports are, what items are not funded, how the changes may affect NDIS participants and what to do now.

It’s my pleasure to welcome you to the eighth edition of Disability Support Guide Eastern Australia — a publication to help you find your way through the many different options and considerations when learning about and searching for disability support.

Whether you need a quick QR code to learn more about certain diagnoses or you’re after a comprehensive and easy-to-navigate directory to start your support journey, this guide will offer you a wide range of support options organised by state and territory.

Each section will start with an overview of the different types of support you are looking for and the page range you can find them on. We’ve introduced this format following overwhelming feedback from the people who rely on us to continue delivering and improving a publication that they have used to connect with providers.

As the General Manager of DPS Publishing, I have seen our team’s passion and dedication to helping you, the reader, find information and navigate the sector with ease.

When emotions are high and you really should be able to focus your attention on making sure you and your loved one receive the best supports possible, you shouldn’t have to spend countless hours researching how an occupational therapist can help your child or who to contact for information about modifying your home to help you live independently.

Meet the model gracing our cover — Payge Hollis

ADVOCATE INDEPENDENT

FEARLESS MODEL

Payge Hollis was born prematurely on Christmas Eve of 1998 after her identical twin sister died in utero from complications with a twin transfusion. She was delivered through an emergency C-section to ensure her survival. At birth, she weighed 550 grams and spent over three months in the neonatal intensive care unit.

She had multiple complications associated with being born with extreme prematurity, such as a grade 4 intracranial haemorrhage, low oxygen saturation, multiple cardiac arrests and numerous blood transfusions.

Her parents, Leanne and David, were informed that Payge may have severe disability and could struggle to ever walk, talk or function ‘normally.’

However, as the face of Disability Support Guide Eastern and a rising star at Diversity Models, the now 25-year-old Ms Hollis has graduated from a mainstream high school with a modified program, learnt how to drive and moved out of home in March of 2024.

She expressed gratitude for how far she has come with the support network around her.

“It’s a challenge living on my own, but I have help from my family and my support workers. It’s another step forward for me to achieve my dreams,” she said.

“It was really hard finding work because people only saw my disabilities which is really disappointing.

“We should be seen, we should be heard and we should be respected!”

“It was a long time before I got a job, which made me sad. When I was 20 years old, I did get a little job in a supported disability coffee cart. This boosted my confidence and skills and now I work as an administration and project officer at a disability organisation called Parent-to-Parent QLD.”

Payge is a passionate self-advocate and advocates for the rights of people with disabilities. She is a member of Loud and Clear QLD which is a self-advocacy group for people with intellectual disabilities based on the Sunshine Coast.

Payge is the QLD representative of the Our Voice Committee which provides advice to the Inclusion Australia Board.

Her powerful work has forged some exciting opportunities to ask NDIS Minister Bill Shorten hard-hitting questions and attend Parliament House on International Day of People with Disabilities 2024.

Modelling is Payge’s latest adventure and she recently joined Diversity Models.

“My experience, so far, working with Diversity models has been thrilling and super fun. I want to build my confidence and this will help me with that,” she said.

Ms Hollis applauded Monique Jeremiah, the founder of disability modelling agency Diversity Models, for the inclusive initiative that landed her a spot on the cover of this guide, along with inspiring others to put themselves out there.

“I want to be a model because I want to show everyone that people with disabilities can achieve great things.

“We should be seen, we should be heard and we should be respected!

“My disability does not define who I am! When I have a dream, I don’t give up. I try hard, I work hard and I achieve great things!”

Supporting the support of others

Australia has a unique disability support system that uplifts others and brings us together as a nation.

The last edition of Disability Support Guide Eastern featured Diversity Models Director Monique Jeremiah on the front cover. Since then, she has appeared on national television, launched Mon Cheri Designs and continued to revel in the magic of her growing talent pool.

One of her models, Matthew, has found that his move from Canada to Australia has helped him fulfil his potential via the country’s unique support system.

Matthew is an inspiring young Egyptian in his early 20s, living with Down syndrome and gifted with a rare talent for music. Before he signed with Diversity Models, Matthew was a budding guitarist and drummer with a hidden talent. His dream had always been to become a musician and disability model. As his stars finally aligned, Diversity Models found a star themselves.

Matthew’s psychologist, Maryam Elyas from Trinity Psychology, had briefly mentioned to the agency that he had lived in Canada for more than 10 years before coming to Australia with his family. Secretly, Matthew always loved music and had aspirations to pursue music as his dream career with modelling.

However, like most people with disabilities, in the past, he was never allowed to be truly seen.

Monique said she had seen the nation's unique disability support system give him many positive opportunities to be surrounded by excellent psychologists, support coordinators and other NDIS support parties to help him flourish and achieve his personal goals.

“As a Diversity Models disability model with Down syndrome, we have seen him embrace being a model on the catwalk and be a live music performer at the Melbourne Disability Connections Expo in 2024,” she said.

“Step by step, he is fulfilling his dreams of modelling and music in Australia.

“This was his purpose of moving to Australia and, so, this has all been made possible thanks to the support of the NDIS system.

Ms Jeremiah expressed gratitude for those in her network and what the NDIS has offered people like herself, Matthew and Payge.

“I'm truly proud to have been on the cover with our disability models, as the magazine is clearly the leading publication of the disabilities sector,” she added.

“My philosophy has always been that, at Diversity Models, if I am seen, the agency is seen and our models are seen, therefore we grow and succeed together!

In 2022 Scott moved house and has been living independently for the last 3 years. With the help of his Support Workers, and members of the community, he’s built a new life that he loves.

I feel happy. I do everything on my own now. I’m very happy with myself and with all the people who help me out. My Support Workers help me out with shopping, cleaning up the house, and doing washing. I have been exploring places I’ve never been to before. I like going to shops like the coffee shop every morning to get my coffee. I do like going to the gym a lot. I actually like doing it!

When Scott first started coming in, he was very shy, but now it’s all him. He knows what he wants and how to get it.

I love doing recycling for the environment. The environment is no good because people throw rubbish everywhere. I like to donate the money that I get from the machine.

I love going for walks in the afternoon too, and helping on the barbecue at the homeless shelter. I do have some goals, yes. One of them is saving for my holiday. Another goal is maybe one day I might do some volunteer or work at Woolworths.

How an advisor can help

Disability support advisors, such as plan managers or support coordinators, can help you to use your funds wisely and achieve your goals as a National Disability Insurance Scheme (NDIS) participant.

Advisors are people who work with you to ensure the best outcomes through connecting with both formal and informal means of support to identify where you need assistance.

Plan managers assist NDIS participants with managing their funding and make sure that the participant’s NDIS plan is being implemented and that funds are being used effectively. Plan managers are also able to watch out for the new NDIS guidelines and ensure that budgets are being spent in compliance with reforms to the system.

Support coordinators can put you into contact with the right providers and understand more about your NDIS plan in order to build your capacity and manage your plan independently as you participate.

Whereas a plan manager would focus on financial planning and the support coordinator would focus more on the day-to-day delivery of support services, a case coordinator may tend to prioritise and help participants work towards a long-term support plan.

Some providers can offer independent assessments to ease your journey through the NDIS, whether through referrals to other specialists, learning more about how you are affected by your disability and how you can use assistive technology or, alternatively, determining what your support plan should look like.

In each state and territory, the Department of Education is responsible for the day-to-day supervision of students. This includes employing appropriately trained staff, providing learning aids, organising transport for school activities, adjusting the school curriculum and ensuring the school buildings are easily accessible. However, some supports may be covered through the NDIS. You may receive education supports through your NDIS plan if you need assistance with self-care at school, specialist transport, portable equipment or specialised, intensive, support when transitioning between schools.

Finding employment can help you develop new skills and gain a sense of independence or community participation. An employment support advisor can offer career advice and help you plan an employment pathway so you can work towards your employment goals. To help with some basics, employment services may assist with writing your résumé or finding other professional development opportunities. They may also support you by finding on-the-job training or support your employer to assist you in your work.

Intervention support is an umbrella term for all the different supports available to help you gain or regain your independence and increase your participation within your community. These supports could include strategies to prepare you for employment, such as assistance in looking for a job, training and skill building or support in finding community activities that interest you. Intervention support plays an important role in helping you live a fulfilling life and achieve your goals.

Table category

Each state and territory section will have several directories. These six service directories include: In-home & Community Support, Therapists & Specialists, Accommodation, Advisors, Equipment & Technology and Mental Health.

The tables list a number of ‘verified’ providers. These providers have confirmed with DPS that the information displayed is correct.

Service region: Providers are listed by service region. They’ll be listed under ‘Providers servicing all of [state or territory]’ if no region was selected or they service the whole state/territory. Please refer to the state/territory map for the boundaries of each service region.

Type of support and services: These lists include both registered and unregistered NDIS providers in alphabetical order, detailing whether they provide Core, Capital or Capacity supports under the NDIS. It also indicates some of the specific services they deliver.

Delivery of services and age groups: The tables show if the provider can come to your house or school to deliver services or works from a set location, such as a medical practice. It also indicates what age groups they can deliver services to.

Paid advertising: A featured listing, with or without a website, indicates the provider has paid to have their services listed in this guide.

The ‘DSG Web ID’ number: This number can be used as a unique identifier to find out more information about the provider on DisabilitySupportGuide.com.au. You can enter the ID number into the ‘Provider by Name’ search toolbar on the home page to view the full profile and range of services of the provider.
