BBUK's National Project: 3 Years On...

Development and Transformation of Children's Services

Support provided to over 50 services throughout the UK In 2010 NICE produced Guidelines for the management of both bedwetting and for constipation in childhood; these were followed in 2014 by a Quality Standards for both conditions. However, from emails and calls to our helpline it was apparent that many areas were struggling to implement the key recommendations from NICE. Consequently, children and young people (C&YP) were not receiving appropriate care and NHS resources were not being used cost effectively, as attendance at A&E and referrals to secondary care were increasing.

Background research identified that the poor implementation of NICE guidelines and recommendations was largely due to a lack of community based services for C&YP with bladder and bowel problems.

This issue was compounded when commissioning of School Nurses and Health Visitors moved to Public Health England in September 2015. This resulted in Local Authorities in many areas assuming responsibility for these universal services and continence promotion came to be seen as a 4

specialist area and therefore no longer part of their remit. At the same time the Children’s Specialist Nurses from the Bladder & Bowel UK team were linking in to a number of national organisations and working groups. These included the Paediatric Continence Forum (a multidisciplinary campaigning group set up to raise awareness of bladder and bowel problems in C&YP and improve service delivery), the NHS England Excellence in Continence Care Programme Board, the National Institute for Health and Care Excellence (NICE) and NHS RightCare.

An approach from one area resulted in our providing advice and support regarding development of their paediatric continence service to improve treatment outcomes. The positive results of that involvement demonstrated that with basic advice and support, often simple changes could lead to more effective service delivery with potential cost savings.

The team realised how much difference could be made, which resulted in this initial work being instrumental in the development of a model of care, that not only reflected the NHS RightCare approach, but also fitted well within the ‘Leading change- adding value’ framework. The requests for assistance from other areas increased and with the support of an unconditional educational grant from Ferring Pharmaceuticals Ltd the ‘National Project’ was born.

Since the project was initiated 3 years ago we have given support to more than 50 services from every corner of the UK. This has ranged from simply highlighting relevant documentation and signposting clinicians, to visiting areas, meeting with commissioners, managers and clinicians to understand their issues and concerns and suggesting ways to address these, including development of a new service where there was none, or transformation or development of an existing service.

Examples of work we have done included bringing together Commissioners, Providers and Service Leads from NHS Vale of York, York teaching hospital NHS

Trust and NHS Hambleton, Richmondshire and Whitby CCG into one really proactive and enthusiastic group committed to provide the best service for children and young people with bladder and bowel problems.

A major piece of work spanned over 12 months and involved working with all 5 Health Boards in Northern Ireland, bringing everyone together to develop a national consensus approach to service delivery for all children and young people with bladder and bowel problems.

An important part of the National Project has been ensuring that all relevant professionals have improved understanding of the impact of continence problems in children and young people including how to assess and treat them.

Providing evidence-based training has been a large part of our work, with well over 2000 clinicians having attended sessions provided in their areas. We have provided training sessions to update knowledge base and promote best practice and the NICE recommendation in a number of CCG’s and Trusts across the length and breadth of the UK including Guernsey, Nottingham, York, Isle of Man, Shropshire and Scarborough.

We have also published nationally, provided new pathways and resources on our website, spoken at national conferences and events and developed a new biannual newsletter ‘Enuresis Update’, which is available to clinicians. This work will continue into 2019.

BBUK's Symposium Programme 2019

The education programme for BBUK’s symposium has just been released and promises to provide excellent CPD and re-validation opportunities through 3 learning streams.

The diverse range of seminars and workshops delivered by experts in their field, will present the latest evidence based research and clinical data. The annual symposium is relevant to all HCPs working in the field of continence, bladder and bowel care. Due to venue capacity numbers places are limited. The symposium is always oversubscribed with a waiting list, so early booking is essential.

Night Time Bladder Problems Clinicians - We Need Your Help!

Please can you display the poster (available on the next page) in your waiting room or have them in your clinic to discuss with patients? Your help is much appreciated.

BBUK in conjunction with a pharmaceutical company are currently looking to recruit individuals who want to share their stories about night time bladder problems.

We are actively recruiting individuals who have nocturia symptoms. These may be individuals you see in your clinics or clinical practice.

The project will involve producing informative videos to be used as part of an educational website and during training sessions to raise awareness of night time bladder problems,

encouraging individuals to seek help and know support is available.
