Virus hits close to home In early March, the United States saw its first cases of COVID-19, a respiratory illness caused by the coronavirus that originated in Wuhan, China.

Soon, out of caution, the Centers for Disease Control and Prevention implemented several precautions to help reduce the spread of the virus.

Schools, restaurants, city and county facilities shut down and persons were urged to stay at home and quarantine.

Events and gatherings across the county, state and nation were canceled per recommendations from the CDC. Gillespie County The first case of COVID-19 in Gillespie County was reported in April and as of press time, the county has seen five total cases.

All are considered recovered.

Out of precaution, Hill Country Memorial converted a wing to negative pressure rooms in the intensive care unit.

“Normally, a closed door is really all that is necessary but if a patient needs specialized equipment or treatment,

this would prevent further spread and contamination,” Dr. James Partin, Chief Medical Officer at HCM, said.

HCM also limited visitation in the hospital and halted elective surgeries.

“We have learned to be agile,” Hill Country Memorial CEO Jayne Pope said. “Because we are in healthcare, caring for patients means needing to be agile as conditions change and we always need to be prepared.”

From early on, HCM worked with the Texas Hospital Association, Southwest Texas Regional Advisory Council, Texas Organization of Rural and Community Hospitals and other entities to develop a contingency plan.

“I think we had a lot of plans in place and (HCM) already has such a strong focus on reducing infections,” Pope said.

Healthcare changes The coronavirus pandemic has brought many changes to the entire healthcare system. Partin expects some of these changes to continue long after the pandemic is over.

“I think our virtual visits will outlive the pandemic,” Partin said. “Patients are going to become accustomed to that convenience and will demand for that to continue.”

HCM was the first hospital in the state to test thermal scans, a device that scans the face and takes the body’s temperature.

“I don’t see this technology going away and we want to ensure that we are looking to make sure our team is healthy and people coming into our organization are healthy,” Pope said.

U.S. Cases The first case of COVID-19 in the United States was reported on Jan. 21, 2020.

The current count of cases of COVID19 in the United States is available on CDC’s webpage at www.cdc.gov/ coronavirus/2019-ncov/cases-in-us. html.

As of press time, the U.S. reports 1.8 million cases and over 105,000 deaths.

New era for dental practices Local dentists discuss industry changes, latest technology

T

here’s no doubt that with recent changes in technology, dental practice is changing.

Zachary Hoerster, DDS, and Jay Lindsay, DDS, discussed how these changes have impacted dental practices and how they have been able to use new technology to make it more efficient.

Hoerster, a seven-year veteran in dentistry, said everything from medical charts to impressions have changed.

“Everything’s become digital,” he said. “It’s great because it makes dental visits faster for patients.

“No matter what anyone says, nobody likes going to the dentist, so the faster you can get a patient in and out, the happier they’ll probably be.”

More efficient materials One new techonology Hoerster uses is dental lasers.

“We don’t use the ‘big bad lasers that cut on teeth like some other dentists, but ours do a great job eliminating bacteria for cleanings,” Hoerster said.

Another type of new technology is an internal camera, which gives patients the opportunity to see what their dentists see.

“Back in the day, a dentist might tell a patient, ‘oh you have a crack on your tooth, you need a crown,’” Hoerster said. “Whereas now, we can go in and take a picture and blow it up on a screen for patients to actually see. It’s great for patient education, so it’s just a no-brainer for us to use.”

Lindsay said material science has greatly helped improve the products used to restore and replace teeth.

“For example, high-strength ceramics have allowed metal-free restorations to be developed, which can appear more vital and natural,”

Lindsay said.

He added bioactive materials have helped regenerage tissue within and around the tooth, and bonding technology improvements have contributed to less failures and sensitivity in treating teeth.

The most rapidly changing area, Lindsay said, has been in digital technology.

“Intra-oral scanners now exist that can replace traditional impressions in many instances,” Lindsay said. “Models of teeth can be printed from these scans on a 3D printer.

“In addition, the digital scan can be used by a dental lab or even Hoerster

in the dental office to fabricate a crown or other restoration from a solid block of ceramic using a 3D milling machine.”

Digital technology has also helped in implant surgeries, Lindsay said, as the entire case can now be planned on the computer.

“A CT scan is taken of the patient’s jaws, the implants are positioned on the computer and a surgical guide is fabricated in a 3D printer from the information,” Lindsay said. “Then, on the day of surgery, the guide is placed into the patient’s mouth and all of the implants are inserted using the guide into the patient’s mouth and all of the implants are inserted using the guide to determine the exact location, angle and depth of the implants.

“This can yield a much faster and more accurate surgery, improving both the surgical and prosthetic outcome for the patient.”

Hoerster also said materials have gotten much better. One example was the introduction of zirconia, which is a tooth colored, metal-free material used for crowns.

“Zirconia allows the preparation to be more conservative versus with traditional porcelain fused to metal

crowns,” Hoerster said.

Another new tool being utilized for medical charts is iPads.

“Patients come in and instead of filling out their information on paper, all they have to do is click a few buttons on the iPad and their information is immediately sent in,” Hoerster said. “It’s great because it’s more efficient and since it saves paper, it’s more environmentally friendly.”

While he said the platform is easy to use, his assistants will help patients who might have trouble.

Lindsay agreed, saying it’s one of the most important attributes of a successful dentist.

“It involves making some kind of connection with the patient, as well as making them comfortable with the dentist not only as a person, but as a technician,” Lindsay said.

“We allow them to select headphones, a blanket, just things to make the visit easier for them,” Hoerster said.

Lindsay’s practice has also adapted a more comfortable and patient approach through the years.

“Technologies that improve the patient experience, even simple ones like headphones or television and movies to watch during the treatment, (has come a long way),”

he said.

Another practice is distraction.

Hoerster said he’ll ask the patients questions about their day or things going on in their lives to help ease their anxiety.

Sedation has also become more routine, Lindsay said, which helps reduce the anxiety of a visit.

Lastly, patient and doctor communication has been enhanced with new technology.

“Systems that allow for texting information such as appointment reminders, questionnaires and other office communications back and forth between the patient and the office is a major improvement and can save a lot of time,” Lindsay said.

Best practices One cause of anxiety can be the fear of hearing bad news during a visit, like being notified of having a cavity or needing braces.

Hoerster said there are some ways that might be able to help prevent bad news.

“First off, be mindful of what you eat or drink,” he said. “Having one soda here and there is OK, but sipping on a 44-ounce drink all day is one of the worst things you can do to put teeth at risk for cavities.”

He also said to keep drinks like sweet tea and juice to a minimum, as they’re often high in sugar and acidity.

Another good practice includes brushing twice a day for two minutes. Hoerster recommends using an electric toothbrush with a timer.

Using floss a couple of times per day is also helpful. Lindsay agreed, saying floss or interdental brushes (not just a waterpick) are helpful.

For those prone to cavities, Hoerster recommended using toothpaste with extra fluoride.

Those prone to dry mouth should drink lots of water and consider chewing a sugar-free gum to keep saliva moving.

Lindsay and Hoerster both said it’s important to have teeth professionally cleaned twice a year and examined by a dentist once a year.

Give wellness Community wellness has always been a priority for Hill Country Memorial, going back to 1992 when it established one of the first hospitalaffiliated wellness centers in the nation.

Since that time, the Hill Country Memorial Wellness Center has been so successful that it has reached and gone beyond peak membership class capacity, even with later expansions.

This reflects the growth of the Texas Hill Country, and the increased awareness of the importance of maintaining a healthy and active lifestyle.

Now underway is an extensive upgrading of the Wellness Center that will help serve more constituents and keep the community even healthier.

The upgrades include increases both in size and amenities: Larger facility Total size will increase more than 60% to 31,830 square feet.

Added locker rooms The two men’s locker rooms will be updated, and a new women’s locker room added, along with a new family locker room.

More educational space The center will now have three classrooms, up from one.

This will allow the center to accommodate up to 80 hours of weekly class time.

Larger workout areas Circuit Training will increase from 1,300 to 2,500 square feet.

Cardio Room will add 400 square feet.

Aerobic/Exercise Classrooms will go from one small room to three flexible rooms with modern sound control systems.

This will allow them to offer 125 classes per week.

Addition of an indoor track, and outdoor activity area.

New community space For the first time, the public will be able to enjoy indoor and outdoor spac-

es dedicated to community gatherings, including juice and snack bars.

Increased community outreach Doubling the number of Community Outreach programs from 10 to 20.

Assisting and encouraging active lifestyles means an increase in the overall health of the community, resulting in fewer hospital stays and quicker recovery times from major procedures.

The Wellness Center Revitalization is one more way HCM can fulfill its mission to “Give Wellness.”

Virus spread The virus that causes COVID-19 is thought to spread mainly through close contact from person to person.

“Given the increases of community spread, all individuals are at some risk of exposure,” a spokesman said.

“Everyone should monitor themselves for symptoms, practice social distancing and stay at home if they are sick, except to seek medical care.”

The CDC states that the virus is spreading more efficiently than influenza but less efficiently than measles, another highly contagious disease.

Some people who show no symptoms are able to spread the virus.

Because of this, the CDC recommends when coming in close contact with others, to stay six feet apart.

It is likely spread through respiratory droplets when an infected person coughs, sneezes or talks. These droplets land in the mouth or on noses of others who are nearby and can be inhaled through the lungs.

The virus can also be spread by touching a surface or object that has the virus on it and then touching the face,

mouth, nose or eyes.

How to prevent spread The CDC recommends the following ways to prevent the spread of COVID19: • Maintain social distance from others, at least six feet apart.

• Wash hands regularly with soap and water for at least 20 seconds. If soap and water are not available, use a hand sanitizer that contains at least 60% alcohol.

• Routinely clean and disinfect frequently touched surfaces.

• Cover the mouth and nose with a cloth face covering.

“Practicing social distancing and good personal hygiene remain our best tools in this fight,” Partin said.

Persons who have had symptoms, but been isolated for 14 days and had additional time being asymptomatic are now considered negative.

He said the makeup of the virus can remain after it is inactive, resulting in a positive test, even if the person is considered recovered.

Symptoms Patients with COVID-19 have had

mild to severe symptoms. Older adults and those with underlying health conditions like lung disease, heart disease or diabetes are at higher risk of developing complications.

Symptoms may appear two to 14 days after exposure.

“We all need to be careful,” Partin said. “We can catch it from people with mild symptoms, even two days before they show symptoms.”

Those include: • fever or chills; • cough; • shortness of breath; • fatigue; • muscle or body aches; • headache; • new loss of taste or smell; • sore throat; • congestion or runny nose; • nausea or vomiting; • diarrhea.

Testing The Centers for Disease Control and Hill Country Memorial offer various self-check tools to evaluate symptoms for testing. Most tests are offered through prescription only or on recommendation from a doctor.

For me it has been to recognize and manage toward that balance.

Maybe your wheel is slightly different, that’s OK. Next, reach out and vulnerably share with someone safe about how you are doing.

And ask them the same.

There are all kinds of resources.

If you are married, there is your spouse. Maybe your siblings, parents (if they are still living), or your adult children.

There are friends.

If things are serious and you find yourself really down, there are pastoral counselors at the churches, the chaplains at Hill Country Memorial, or counseling resources around town.

There is also your doctor.

And the State of Texas set up a number that anyone can call 24/7 to just talk. It is 833-986-1919.

Finally, look out for your children, parents, friends and neighbors. Be skeptical when they tell you they are doing “just fine” and gently draw them out a little more.

Remember: No one is untouched.

In the end, your relationship with them can be a source of encouragement and balance for you, as well.

There is a lot of power in community, and if we tackle this together, we can make the pain of this pandemic count for something.

We might even come out of it stronger for the experience because we’ve picked up a few new healthy habits along the way.

Keeping health a top priority County health division oversees regulations in a fast-changing environment

elli Olfers, Gillespie County’s Director of Health Services, and Rebekah Cathey, food safety inspector, have jobs that are changing by the day.

Three months in to America’s response to the coronavirus pandemic, the governor is issuing nearly weekly changes regarding the reopening of the economy.

With every change, Olfers and Cathey are communicating with “customers,” which include all eating and bar establishments in the county. The two also convey information from the state and federal Departments of Agriculture and Texas Restaurant Association, and update the city/county website, and send any other pertinent information needed by the service industry in this heavy tourism area.

“There are lots of good ideas for keeping up with changing regulations,” Olfers said. “It’s just finding them and making sure we make it easy for our customers to understand.”

Olfers had also just gotten off the phone with several hotel customers who were inquiring about changes to rules for swimming pools and when they may open up their facilities. She also renewed permits for mobile food units.

Cathey was on site for a safe reopening of the Fredericksburg Farmers Market.

Olfers’ and Cathey’s jobs were always busy in a heavily traveled county, but it got compounded with COVID-19 and all the additional demands.

Olfers and Cathey said they also have to keep up with the changing rules regarding hand-washing, occupancy and recommendations from the Centers for Disease Control and Prevention.

And while the office isn’t responsible for enforcement — that falls to law enforcement or the Texas Alcoholic Beverage Commission — the two regularly field lots of calls from anxious business owners inquiring about the state of change.

“Is it 25% occupancy or 50% occupancy? If most of my

Thrive sales come from food (or alcohol), how much capacity can I open?” Olfers said.

“We direct people to the most current information.

Everyone is so anxious about their business or job.”

Olfers praised the community partners she also relies on, including the Fredericksburg Chamber of Commerce and the Fredericksburg Convention Olfers and Visitor Bureau. Both have also been a well-spring of business- and tourismrelated material on a rapidly changing situation.

She and Cathey also send out helpful information geared toward businesses that had not been open during the shelter-in-place time.

“Those owners need to have a checklist of issues that might crop up while their facilities were not being used,” she said. “For instance, are the pipes clean, or is their ice machine free of mold?”

Much of the information that is needed to reopen a clean facility as safely as possible during an illness outbreak can be found on the website link to her office.

“Our job is to get out as much information to customers as we can,” she said.

Touch ValuMed Pharmacy’s history cements customer relationships

M

att Roberts heads up the longest-serving pharmacy in Fredericksburg. And the legacy of customer service formed by his predecessors guides his business.

Hugo Kallenberg was pharmacist in connection with the Keidel Hospital and later opened Kallenberg Pharmacy in 1931. His daughter, Emmie, became the first female to graduate from the University of Texas College of Pharmacy and helped run the business.

In 1958, the pharmacy was purchased by Forest Rees and the name was changed to Rees Pharmacy. Rees, a business leader in the community, ran the pharmacy which was once located on the corner of Main and Adams.

Rees brought on partner William “Mike” Roberts and changed the name to Roberts Pharmacy in 1976. The name was changed to Roberts Pharmacy and became ValuMed Pharmacy a decade later.

“Forest passed a couple of years ago, but I grew up working with him and my father,” said Matt Roberts, second-generation owner. “I started working here when I graduated from Texas Tech in 2000 and I bought out Forest in 2007 and bought my dad’s half in 2012.”

Roberts learned from “all the old-timers.”

His father said he would pay for his education, “so long as you don’t go into pharmacy,” Matt said, laughing. The younger Roberts’ degree is actually in business.

Thrive “I feel like it’s become more of a business. The world has just changed — there is so much dealing with insurance and government regulation,” Roberts said.

Roberts was born and raised in Fredericksburg. He and wife Kim have three children in eighth, sixth and second grades.

For him, the pharmacy is like a family.

“It’s taken me 20 years to understand there is such a need, especially in a community like this, to have a personal relationship with people in the medical industry,” he said. “With the healthcare profession, people really do need to trust who is giving them advice. We’re lucky to have great doctors in town and they are loyal to us and their patients.

“Everybody who walks through the door or is in the drive-through, the staff knows their name before they say it. The pharmacists know their medical histories and conditions and their families,” Roberts said. “That’s important from a community angle and from the compassion side.”

ValuMed carries a lot of stock not found at larger stores, and they work with many nursing homes in proximity as the only pharmacy north of Main Street.

mental to independent pharmacies.

“And we’re quick. We’re fast. We can give the same medicine with the same price and we’re faster,” he said.

ValuMed is also the only pharmacy in town that still performs compounding.

Tech changes With COVID-19, ValuMed uses a smartphone app that is easy to send in refills, with the drive-thru in high use due to social distancing.

“Customers are hitting the refill button on the app, and 10 minutes later, going through the drive-thru and grabbing their medicines,” he said. “It’s seamless.”

The business itself has been checking temperatures of anyone who enters the building and stressing hand-washing and additional cleaning.

“We’re in the nursing homes every day, and the idea of being a conduit into the nursing home or taking it home to our kids kept me up at night,” he said. “Fortunately, everybody here has laid low and made the adjustments and practices to stay safe.”

Roberts said the safety practices will continue after the virus fades.

Roberts jokes that his father is much more personable. “He still lives out near Willow City and looks in on us a couple of times a week,” he said.

“He and Forest both seemed to know everyone in town and built those relationships,” Roberts said.

Like his father and Rees, Roberts serves on the board of Heritage School, serves as a deacon at Hill Country Church and serves on the Quality Control Committee at Hill Country Memorial Hospital.

‘Neighborhood Fit Trails’ help make everyone stays in shape

local gym is shaking up the neighborhood.

Hill Country Memorial Wellness Center staff responded to the COVID-19 pandemic the only way they know how by making sure Fredericksburg stays in shape.

One way they’ve done this is by creating multiple “Neighborhood Fit Trails” in town.

Shannon Maenius, an ACE-certified personal trainer and AFFA-certified group instructor at the Wellness Center, says the responses have been positive.

“I have had lots of families who are using it as P.E. for their kids,” Maenius said. “Lots of people who exercise regularly, since the gyms have been closed, they’ve been looking for ways to work out and get more than just walking, biking or running. The strength training stations have given them the option for strength since they can’t get in the gym. Then, I’ve had people who’ve said they’ve just started exercising during this pandemic and they’ve been home.

“It’s been a perfect way to get out and have some instruction and some structure to their workout that they were doing for the first time. It’s kind of cool.”

The idea came when Wellness Center director John Phelps noticed a number of runners, walkers and cyclists out and about on the normal Wildflower Run route.

Thrive The Wellness Center has a fitness trail outside of the building, so the center’s trainers came up with an idea to bring it to the streets.

“I thought about what we could do at the Wellness Center to help them get a little more exercise in and a little more fitness during this time when everybody’s out,” Phelps said. “That’s where the root of the idea came in.

Many years ago, we did the fit trail that’s by the hospital. So how could I get it out in the community? I worked with our group trainers and we came up with 10 exercises that will do the whole body, are pretty easy to execute and didn’t need any extra equipment and could be done on the sidewalk or using the curb.”

The first Neighborhood Fit Trails went up in mid-April.

Maenius designed a 10-station, strength training trail in the neighborhood behind Fredericksburg Middle School, which runs two miles and incorporates part of the normal Wildflower Run 5K route.

“That’s why it’s called the Wildflower Neighborhood Fit Trail,”

Phelps said.

Exercises don’t have to be done in any order, so residents can start at any point and get a full body workout.

HCM’s marketing department pitched in by making plastic signs to post in yards throughout the trails.

Maenius said no one refused when asked if a sign could go in their yard and Fredericksburg Independent School District superintendent Jeffrey Brasher allowed Wellness Center staff to post signs at the middle school.

Phelps came up with a trail along Windcrest Street between Milam and Adams streets with hospital employees in mind.

“He put the 10 stations up and down the sidewalk at Windcrest,” Maenius said. “The CEO of the hospital (Jayne Pope) encouraged all the staff who are working during this time to take their break and go out and use that sidewalk to get some exercise.”

“What we found were that the Wellness Center employees like me, we were enjoying getting out and walking the block because we were

cooped up inside and having to adjust to the COVID surge and the prep,” Phelps said. “We put one there. It was really for the hospital workers, to get them out and get them in the sun.”

Another, a one-mile route was made for a neighborhood near Cross Mountain, and a trail also went in at Lady Bird Johnson Municipal Park.

Phelps is working with the City of Llano to install a Neighborhood Fit Trail at a riverside park Thursday, May 7.
