Happy Fall Y’all!

It’s been difficult to imagine that our new virtual reality will continue through the Fall. I thrive when I’m busy and have always been busy attending fun community activities from sports events to Fall markets, to dinner with friends. I was really hoping we would all be back out and about enjoying all things we are used to, but COVID continues to be present. So, I’m trying to embrace slow. Some days I’m better at it than others. One thing I have enjoyed is more family time. We have spent more time together cooking, listening to music, puzzling, playing games, and most Kay Garrett spent some time with her family walking along the beach in Galveston: Matt, recently, we spent a wonderful weekend at Lindsey and Canon Kiehl, Carley, Kay, Reid the beach together. Family time is the best and Jim Garrett. time!

Don’t Let COVID-19 Fear Postpone Your Cancer Screening The hospital is safe, and coming in for a screening can save your life.

C

ancer has not stopped during the COVID-19 pandemic. But what has come to a stop – almost completely – is regular cancer screening. One study, from the electronic medical records system Epic, found that screening mammograms across the U.S. dropped 94 percent in March when concerns over the spread of COVID-19 intensified.

Another study by the IQVIA Institute in April found similar results: since February, colonoscopies were down 90 percent; Pap smears, down 83 percent; PSA testing, used to detect prostate cancer, down 60 percent; and CT scans for lung cancer, down 39 percent.

Patient Safety is Paramount

Guidelines You Should Know

Since February, many people have put off annual screenings, such as mammograms due to fear of going to the hospital, or uncertainty about changes in appointment processes. But Houston Methodist Sugar Land has developed key safety protocols that protect patients throughout their screening. Since March, the Houston Methodist Breast Care Center at Sugar Land has performed more than 2,800 mammograms, ensuring the safety of every patient even during the pandemic.

“Getting a mammogram or other test today is no different than it was before, except that we have additional layers of protection and safety,” said Michelle O’Shea, M.D., boardcertified breast surgeon at Houston Methodist Breast Surgery Partners at Sugar Land. “There is no reason for patients to worry. If you missed your annual mammogram during the first part of the year, you can feel safe coming in now. The staff continues to put patient safety first, which now includes extra precautions and safety measures.”

The hospital’s focus on safety includes constant deep cleaning and the use of CDC-approved disinfectants – including UV lighting sanitizers – along with the following new safety processes:

Screening guidelines are based on years of data and are designed to ensure that patients are checked regularly for the most common forms of cancer.

In some cases, it may be beneficial to follow a more frequent screening schedule than is recommended.”

• A new in-car check-in process, as well as reconfigured waiting rooms and check-in lines to support social distancing.

• Mandatory masking and social distancing guidelines for all physicians, employees, patients and visitors.

• Screening all patients to ensure those suspected or confirmed to have COVID-19 are scheduled for a later date.

• Daily screening and frequent testing of employees as recommended by the CDC. • Mandatory personal protective equipment rules for all staff and physicians.

• Additional sanitation processes to disinfect all equipment and surfaces.

Here are some basic guidelines for the most common forms of cancer screening: Breast Cancer: Women ages 20-39 should have a clinical breast exam every three years. At the age of 40, women should have both a clinical breast exam and a mammogram annually. High-risk women should utilize additional screening methods at their doctor’s discretion.

“Here at Houston Methodist Sugar Land, we have the latest and most sophisticated imaging technology available, including 3D screening and diagnostic mammography; breast MRI and ultrasound; and biopsies, all read by specialized breast radiologists,” said O’Shea. “That gives us a significant advantage in diagnosing tumors at the very earliest stages, which means that women have the best chance for complete recovery – all at one location close to home.”

Cervical Cancer: Women ages 21-29 should have a pelvic exam annually, and a Pap test every three years.

Women ages 30-65 should have a pelvic exam annually and a Pap test every three to five years, along with an HPV screen every five years. Women over the age of 65 should have pelvic exams and Pap tests at their doctor’s discretion. Colon/Rectal Cancer: Men and women younger than age 45 should talk with their doctor about when to begin screening and screening frequency.

“Your doctor will want to know about high risk factors, such as family or personal history of polyps, colorectal or uterine cancers, or a personal history of chronic inflammatory bowel disease,” said Kirtan Nautiyal, boardcertified hematologist-oncologist at Houston Methodist Oncology Partners at Sugar Land.

Men and women age 45-75 without specific risk factors should be screened regularly per their doctor’s recommendation.

“An annual fecal blood test is the minimum screening I would recommend,” said Nautiyal. “That Kirtan Nautiyal, M.D Hematologistshould be combined with regular Oncologist colonoscopies as indicated.”

Lung Cancer: Regular lung cancer screening for asymptomatic patients is relatively new, but it is making a significant difference in doctors’ abilities to successfully treat the disease. Men and women between the ages of 5577 who have a 30-pack year history of smoking – defined as one pack a day for 30 years or two packs a day for 15 years – and those who currently smoke or who have smoked within the past 15 years should be screened using low-dose CT scans. Prostate Cancer: In general, men who have more than one first-degree relative (father, brother or son with prostate cancer at an early age) should begin screening at age 40.

Beginning at age 45, men with one first-degree relative diagnosed with prostate cancer at an early age, and AfricanAmerican men, should begin screening. All men 50 and above should be screened regularly.

“Men who are 40 and above should talk with their physician about the need for regular prostate cancer screening,” said Patrick E. Prath, M.D., hematologist-oncologist at Houston Methodist Oncology Partners at Sugar Land. “As with colon cancer, family history plays a big role in your individual risk, and your doctor can help you decide on the right approach.

An annual digital rectal exam, along with a prostate specific antigen (PSA) test, can identify most prostate cancers early.

Seabourne Creek Nature Park, located at 3831 Hwy 36 South, is now home to 14 more Black-Bellied Whistler Ducks.

The ducks were brought to the park by the Texas Wildlife Rehabilitation Coalition (TWRC) after spending 75 days in the homebased rehabilitation facilities of Rosenberg-area residents Jennifer Bellanger and Teya Miller.

TWRC cares for both native and non-native Texas wildlife found in and around the Houston area, with a specialty in small mammals, migratory songbirds, small raptors and reptiles.

Their goal is to rehabilitate with the intent to release back into the wild.

“We were honored to have Seabourne Creek Nature Park selected by TWRC to be the new home of these Black-Bellied Whistler Ducks,” said Parks Director Darren McCarthy. “The park is a perfect location for these ducks. It is over 209 acres of parkland, and to date, we have sited over 212 bird species.

That, along with our many piers and pavilions, make it the perfect location for bird and duck watching.”

2020 Fort Bend County Fair Cancelled Rodeo action, live music and BBQ cookoff fun will not be part of the 2020 Fort Bend County Fair experience. Due to concerns about the COVID-19 pandemic, the Fort Bend County Fair Board of Directors announced the cancelation of the 2020 Fort Bend County Fair and Rodeo; this is the first cancellation of the entire Fair since World War II. Currently, the board and staff are working with community partners to have a livestock show and auction, and 14 young ladies will continue their journey to be crowned 2020 Fort Bend County Fair Queen.

Details are being finalized, and an altered schedule will be released soon. As of August, 339 livestock exhibitors intend to show during the fair’s altered run. Steers, Pen of Two Commercial Heifers, Halter Heifers, Goats, Lambs, Swine, Turkeys, Pen of Broilers, Roasters, Bucket Calf, Scramble Heifer and Rabbits are expected to be shown.

You can even shop products to be used in your project virtually since online catalogs typically show a variety of colors and finishes along with prices. For items you need to see or feel to choose, like flooring, request samples to test in your home. To make your virtual meetings most productive, prepare by providing a detailed scope of work and be ready to provide measurements and show the space cleared of clutter.

ith the extra time many Americans have spent at home over the past few months, there have been plenty of opportunities to think about how that living space could be improved.

While this may not be the ideal time to tackle a full gut and renovate project, it may still be possible to do some smaller scale remodeling projects that update and improve your home. ift giving is best when a gift is made with someone special in mind and by your own hands. I love making special gifts for friends, and hand scrubs are one of my favorites.

I’m using hand scrubs so much these days due to my latest hobby: gardening. The one thing they don’t tell you when you start gardening is how dirty you’ll get. I am not kidding when I say no amount of soaking can get off the soil that gets packed around your nails and into your heels. It’s a never-ending task to stay neat and tidy!

Lemony and Lavender Gardener’s Scrub

The Agave at Gruene Rapids, a 15-unit luxury condominium complex in the Gruene Historic District, broke ground on May 14th. Located just west of the Gruene Road bridge, the condominiums are the first waterfront condo development in New Braunfels in more than 10 years. All units are located at 1228 Ervendberg Avenue, a 2.3-acre riverfront lot that is just two lots from Gruene Road.

egular health screenings with precautions are a valuable tool to help keep people healthy during the COVID-19 pandemic. However, screenings including annual mammograms have come to a stop since the pandemic began.

One study, from the electronic medical records system Epic, found that screening mammograms across the U.S. dropped 94 percent in March when concerns over the spread of COVID-19 intensified.

“These are very significant numbers because this could represent a delay in finding and treating breast cancer early on,” said Sandra Templeton, M.D., breast surgeon at Houston Methodist Breast Surgery Partners in Sugar Land. “One of our most powerful tools is early detection through screening mammograms.”

It is recommended that women ages 20-39 should have a clinical breast exam every three years. At the age of 40, women should have both a clinical breast exam and a mammogram annually. High-risk women should utilize additional screening methods at their doctor’s discretion.

Focus on the Cure Don’t Be Alarmed if You Need Another Mammogram or Ultrasound Masses

here are numerous reasons why a radiologist may elect to ask for additional views or ultrasounds after reading your mammogram. They are viewing a three-dimensional object in two dimensions. Overlapping tissue can create densities on the mammogram that appear as a mass or area termed “architectural distortion.”

The main reasons a patient may be called back are for areas of architectural distortion, masses or grouped microcalcifications, which are tiny spots of calcium in the breast.

Mammograms are categorized into groups termed BIRADS, or Breast Imaging Reporting and Data System. BIRADS 1 is negative; BI-RADS 2 is benign; BI-RADS 3 is probably benign; BI-RADS 4 is suspicious; BI-RADS 5 is highly suspicious and BI-RADS 0 is incomplete, requiring additional imaging including additional views, with or without spot compression, spot magnification views – typically for microcalcifications – and ultrasound. The focus here is on the BI-RADS 0.

Architectural Distortion This is a very common occurrence but a potential sign for a true lesion. Additional views with slightly different projections and particularly spot compression views are utilized for evaluation of this abnormality. The spot compression views spread the overlapping tissue and remove the summation artifact if there is no true lesion. If a persistent abnormality is seen, an ultrasound is commonly requested to exclude an underlying lesion in the breast. The majority of the time, there is no lesion, and routine follow-up may be performed.

Doctors typically ask for spot compression views and ultrasound on any new mass or enlarging mass from prior mammograms. The spot compression views give an idea of the borders of the lesion with smooth borders typically indicating benign lesions such as cysts and benign fibroadenomas (non-cancerous tumors), whereas irregular borders are more worrisome and may indicate a more aggressive lesion.

Ultrasound is then performed after the spot compression views for evaluation of the consistency of the lesion, primarily cystic or solid.

In addition, the vascularity of the lesion can be assessed with the color Doppler with the more vascular lesions typically being more aggressive. Cysts do not contain internal vascularity. Cysts typically do not require any further followup unless they are painful or they may be aspirated under ultrasound guidance. The vast majority of solid lesions are benign fibroadenomas, but an ultrasound guided biopsy may be necessary for confirmation of the pathology. Short-term ultrasound follow-up may also be performed.

Microcalcifications The vast majority of microcalcifications are benign and typically either secretory or dermal (within the skin). Vascular calcifications in the arteries of the breasts may also present as microcalcifications. Spot magnification views are performed to determine the number and shape of the microcalcifications. Rounded, well-defined calcifications are almost always benign and compromise the vast majority of our findings. Irregular sharp or serpiginous microcalcifications – particularly if they are numerous and tightly grouped – generally require a biopsy, which may be performed with stereotactic technique and local anesthesia or may be performed with open technique with needle localization for the surgeon, who then completely removes the microcalcifications for pathologic evaluation. Stereotactic technique is much simpler and is used the vast majority of the time for these calcifications.

Over 95 percent of the BI-RADS 0 mammograms turn out to be benign. Having said this, it is crucial not to ignore the recommendations of returning to the radiology department for additional views or ultrasounds, as early detection and treatment of the worst case scenario – breast cancer – results in cure.

Focus on the Cure Catching an Evasive Killer: Inflammatory Breast Cancer

O

ften mistaken by women as a rash or skin infection, Inflammatory Breast Cancer (IBC) is one of the most aggressive and deadliest forms of breast cancer. Although IBC is rare, accounting for only one percent to five percent of all new breast cancer cases in the United States, it can be one of the most difficult to diagnose.

Unlike other breast cancers, there is no lump found in IBC cases. There are several symptoms that are unique to IBC that cause many people to second guess their symptoms and delay seeing a doctor.

In cases of IBC, your skin may look like it has a rash that doesn’t itch. Sometimes, it may be considered an infection without pain. Even though your breast is hard and firm to the touch, you may not feel any pain.

And since it involves the skin, IBC starts as a Stage IIIB cancer. It is often diagnosed at either a Stage III or IV because the disease can progress rapidly in just a matter of weeks or months.

According to the National Cancer Institute, IBC typically is diagnosed in younger women. It also tends to be more common among African-American women and obese women. IBC can also strike men.

IBC is most recognizable by changes in the appearance of your breast. These changes are caused by cancer cells that invade and block the lymphatic vessels. Contact your doctor if you experience any of these symptoms: • Thickening or swelling of the skin of the breast • Redness visible on the entire breast area • Skin of the breast has pitting or dimpling, and looks and feels like an orange peel • A retracted or inverted nipple • One breast looks larger than the other because of swelling • One breast feels warmer and heavier than the other Unlike other types of breast cancer, in cases of IBC a lump cannot be felt during a physical exam or seen in a mammogram. But IBC is visible to the eye. It’s good to be mindful of any changes in your breast area because the sooner you are able to get a diagnosis, the better your chance of beating it.

Due to the aggressive nature of the disease, IBC tumors in some cases are unable to be treated with hormone therapies. Instead, a multidisciplinary approach is used to treat patients diagnosed with the disease. Patients receive chemotherapy to help shrink and kill the cancer cells before having surgery to have the cancer removed. Surgery is usually followed by radiation therapy.

We know that IBC is a very aggressive cancer. Even with treatment, it can spread rapidly. Research has found that IBC patients who receive multiple forms of treatment may live longer.

Many factors come into play in terms of how the patient responds to treatment. A patient’s overall general health, stage of the disease and location of the cancer all play a role.

That’s why I encourage all women and men to seek the opinion of a specialist if they experience any symptoms of IBC or notice any changes in the breast area.

Although IBC is extremely rare, one in eight women in their lifetime will be diagnosed with some form of breast cancer. The earlier you start treatment, the better your chance of a cure. Practice your monthly self-exams and schedule your annual mammogram.
