INEOS’S SAFETY, HEALTH AND ENVIRONMENT POLICY The primary concern of Ineos Plant Management is safe production and the prevention of injuries and illnesses to all employees, their families and members of the community. Management ranks safety, health and the environment as highly as production, quality, or costs. The following outlines some of the basic parameters of our commitment to safety and the environment. 1.
Management will maintain the highest safety standards in the design of buildings, equipment and processes.
2.
Environmental concerns are a priority. Management will continually work to find ways to minimize waste and emissions and preserve the quality of the environment.
3.
Housekeeping is also an integral part of our program. The daily discipline of all employees in keeping their location clean and orderly contributes to our safe and healthful environment.
4.
High standards will be maintained in the development of job methods, the provision of protective equipment required by specific methods, the provision of protective equipment required by specific jobs, and the thorough training zof all employees in the safety health and environmental aspects of their work.
5.
Included in our commitment to safety and the environment are methods for minimizing property damage and for maximizing fire prevention and firefighting potential.
6.
Plant security is vital to providing safe conditions and will be maintained. Management will assure compliance with all current safety and environmental legislation.
The SHE Department has responsibility for providing advice, guidance, program audits, and any such assistance as may be needed by Supervision and Management in promoting and maintaining safety and environmental standards. Management's goal is to promote, within each employee, a state of mind which is conducive to safe and healthy habits, both on and off the job. It is, then, the personal responsibility of each employee to constantly strive to maintain the prescribed standards for safe production, safe living and preservation of environmental quality.

SAFETY PRACTICES The following practices are designed to assist you in choosing the safe course of action in situations where no specific safety rule applies and where your personal judgment can prevent injury to you and your fellow workers. 1. Correct Unsafe Conditions Yourself. If you see tools, materials, and other objects lying in aisleways, on platforms, or around work areas, remove them to eliminate slipping and tripping hazards. Take the necessary steps to eliminate all unsafe conditions within your work area. 2. Study Your Job From the Safety Angle. Take time to think before starting work. Make every effort to search out the hazards and take precautions to prevent accidents from happening. Be sure that you have all necessary protective equipment with you when you start work. 3. Report Hazards and Near-Miss Incidents. The Company provides safe working conditions for all employees. You can assist in this effort by reporting to your supervisor any unsafe conditions you observe in your work. Nearmiss accidents often reveal such conditions. Report all near-miss incidents. 4. Communicate Work Area Conditions. Always inform your fellow workers of changes in the work area which create unusual hazards or operating conditions. Teamwork is essential to safety and we encourage everyone to watch out for each other. 5. Move About the Plant Safely. Use regular walkways, passageways, stairs and ladders. Do not take shortcuts. Do not walk through work areas and operating areas unless you are authorized to do so. Be constantly alert for moving power equipment. Certain areas may require special protective clothing and equipment, such as coveralls, ear plugs, or respirators. Do not enter these restricted areas without the required clothing and equipment. 6. Avoid shortcutting procedures. Many knowledgeable employees participate in writing and approving the plant procedures. They were written to define the best known way to do a job. To circumvent or bypass these procedures would be unwise and unacceptable. Follow all procedures as written and if you believe they are incorrect or need to be revised, initiate the proper actions to do so, but follow them until they are changed.

CONFINED SPACE ENTRY Many of you may be required to perform work activities involving Confined Spaces. If you are going to be required to enter into a Confined Space then it is important for you to read this information. Confined Space Entry can be dangerous and you need to understand these rules. All permitted confined spaces at Ineos will have a sign posted that says “Danger – Do Not Enter. Confined Space Entry Permit Required”. Never enter a space marked with this sign unless all confined space entry requirements are met. Confined Space Entry can be dangerous and you need to understand these rules. Permit-required confined space means a confined space that has one or more of the following characteristics: 1. Contains or has a potential to contain a hazardous atmosphere. 2. Contains a material that has the potential for engulfing an entrant. 3. Has an internal configuration such that an entrant could be trapped or asphyxiated by inwardly converging walls or by a floor which slopes downward and tapers to a smaller cross-section. 4. Contains any other recognized serious safety or health hazard.
CONFINED SPACE ENTRY RULES 1. Safe preparation of a confined space PRIOR to entry is essential. You must not enter any confined space unless it has been isolated, tagged/permitted, cleaned, ventilated, inspected and tested. There are many hazards involved with entering confined spaces and therefore many rules to assure proper preparations are complete before anyone is allowed to enter. 2. A Confined Space Entry Permit is required before any person may enter a closed vessel or an open confined space deeper than four (4) feet. At Ineos the Production group will coordinate all Confined Space Entry activities and rules will not be bypassed or short-cut. 3. All entrants must be sure they know how to: • recognize potential hazards in confined spaces. • understand how to communicate effectively in confined spaces.
< 20 >
• know what protective equipment may be needed inside the space. • know plant emergency alarm sounds and evacuation procedures. 4. An Attendant or outside stand-by person is required before anyone can enter a confined space. This attendant’s dedicated duties are: • to remain at the entry at all times and stay in communication with the entrants inside to verify they are safe. • be sure all entrants sign in and out. • monitor outside area activities for situations that might pose a potential hazard for those inside. • understand the plant emergency action plan and employee alarms systems including what to do at confined spaces during alarms. • perform continuous O2 monitoring.
