Before her baby, Amber, was born four months early, Adriana Zepeda had never heard of the clinical trials that would save her daughter’s life. But thanks to the expertise and relentless care of University Health System’s Level IV NICU team, Amber was one of the first babies in the country to benefit from a new medication that would correct her life-threatening digestive problem. In partnership with UT Health San Antonio, we are always thinking beyond conventional medicine, so that we can find new ways to save lives and restore health.

What are Life Care Planning Steps? We’ve Got a Problem…A GrownUp Problem. Adulting: Encourage your kids to be their own health advocates Eco-Impressionist – Grant Maniér I’m an ADULT!...Am I ready for this? Supported Decision Making Agreements, Powers of Attorney, Guardianship, Oh My! Be Prepared for Flu Season
Dear Readers, I attended the Texas Parent to Parent Conference and had an opportunity to meet a very talented young man, Grant Maniér and his Mom, Julie Coy Maniér. Grant is a young talented Eco-Artist combining his art form and autism . They have also co-authored their first children’s book; Grant the Jigsaw Giraffe ~Different is MORE. Please read their amazing story on pages 26 & 27. Transitioning from a teenager to an adult can be very overwhelming. Once your child turns the age of 18 they will have a change in physicians, benefits from insurance to social security and/or disability benefits to name a few. And, for parents, planning for the future, what happens if I am no longer able to take care of my son or daughter. Is a life care plan set up? We have physicians and professionals who have written some very informative articles to address many of these concerns for both you and your adult child. If you are looking for a Pediatrician check out our website at www.ExceptionalKidsTexas.com. We could not publish Exceptional Kids without our wonderful advertisers. What are Life Care Planning Steps? A Special Care Planner specializes in working with caregivers to provide counsel, direction, and solution’s for special needs caregivers to ensure that both the current and future sustainable needs of the dependent are met. These professional advisors do this by: • Identifying and addressing the primary issues that special needs caregivers have with regards to sustainable care for the child/dependent • Create a life care plan vision by discussing goals, dreams, wishes and visions for the future care of your loved one. • Discussing guardianship and alternatives. Because this is such a personal decision it is critical that the entire support group/family be included in these discussion • Identifying available resources that will be available to perpetuate the funding of the child/dependent • Determining the cost of life care! Because dependents will often times outlive a caregiver, it is critical that the quantification of future costs be assessed so that the assets left behind to help supplement the ongoing need is sufficient to cover these supplemental needs. • Preparing a Letter of Intent that essentially provides a set of ‘instructions’ to offer the custodian/caregiver a set of intimate insights to the child/dependent’s
EK53_07-07.indd 1
needs, desires, limits, expectations, etc. This document is a living, breathing, liquid document that provides any future caregiver guidance on how best to meet the dependents needs. • Helping to provide insight and guidance in setting up and preparing the legal documents necessary to help ensure the smooth transition at the passing of the primary caregiver. These documents will require the use of a qualified special needs planning attorney in setting up guardianship, last will and testament, and special needs trusts. The effort in creating a life care plan for a loved one is a difficult journey. The benefits of creating one however ensure that those that we care for have the best possible chance for sustained care. For more information on how the Special Care Planning Team can help you with your planning needs, contact Darren Scrimpshire, Special Care Planning Team leader, dscrimpshire@financialguide.com or 210-384-5366. The Special Care Planning Team of MassMutual South Texas is an innovative outreach initiative. We have members of our team with the Chartered Special Needs Consultant (ChSCP) designation from the American College. Local firms are sales offices of Massachusetts Mutual Life Insurance Company (MassMutual), and are not subsidiaries of MassMutual or its affiliated companies. Neither MassMutual South Texas nor any of its employees or agents are authorized to give legal or tax advice. Consult your own personal attorney legal or tax counsel for advice on specific legal and tax matters.

We’ve Got a Problem.. A Grown-Up Problem. Ruchi Kaushik, MD, MPH August 14, 2019 We’ve got a problem. A grown-up problem. It’s certainly a GOOD problem to have, but, nevertheless, still a problem, and similar to all problems, it needs a solution. A grown-up solution. Today’s medical knowledge and technology have allowed us to improve survival and quality of life for children with special health care needs. Fortunately, more than four out of five children born today with chronic conditions will live into adulthood.1 As of 2010, more than half of all individuals with cystic fibrosis were adults.2 And over the next decade, roughly 90,000 persons with congenital heart disease are expected to enter adulthood.3 Indeed, there are currently 18 million young adults 18-21 years of age, one in five of whom have a chronic condition.4 And, yet, pediatricians remain so attached to our patients. We are afraid to let go or afraid adult doctors will not be comfortable caring for our patients; similarly, our patients are afraid of moving on and not ready to trust a new provider.5 But it is time to face our fears. Evidence suggests that health outcomes for young adults with chronic conditions are improved when they have smoothly transitioned into an adult model of care.6 So, why can’t pediatricians just continue to see young adults with conditions that begin in the womb or early childhood? Well, while we are great at immunizing, developmental screening, and discussing safety topics, we may not be so great at recognizing signs of diseases of adulthood, discussing family planning, or recalling the timing of other important preventive screenings (eg mammograms, cholesterol tests). Certainly, these are all topics we can brush up on, but what’s more important than transferring to adult care is transitioning to the adult model of care. Fortunately, the National Alliance to Advance Child Health has developed “Got Transition”7, a partnership with the Maternal and Child Health Bureau aimed to help young adults and families with health care transition via its website, www. gottransition.org. The site’s resources can assist with understanding their pediatrician’s transition policy, checking to see if your teen is transition ready, keeping a portable medical summary, and transferring care. It’s helpful to keep a checklist. The Children’s Hospital of San Antonio Adolescent Transition and Adult Care Committee has created a list of the different aspects of health care transition:
EK53_09-09.indd 1
• Locating Adult Health Care Providers • Insurance and Care Coordination • Managing Appointments • Managing Medications • Managing Equipment and Therapies • Managing Medical Information/Record Keeping • Medical Decision-Making/Conservatorship This checklist and many other helpful resources are also available on our Transition Webpage, www. chofsa.org/transition. A great first step is to locate where you are in the process. Does your teen know the name of her specialist or the doses of her medicines? Can she find the phone number to her insurance company? Ask your teen to complete the Transition Survey (available in English and Spanish), which will highlight her strengths and her opportunities to learn. Other Health Care Transition resources include Navigate Life Texas (www.navigatelifetexas.org) and Texas Parent to Parent Pathways to Adulthood Transition Program (www.txp2p.org). You’ve been an advocate for your child for over a decade, and this has provided the greatest role modeling for her to now advocate for herself. Encourage her to make an appointment and talk to her pediatrician today about the next step in her future. Adulting: Encourage your kids to be their own health advocates Jackie Boyle, May 22, 2019 When we send kids off to college, or they leave home to experience life on their own, we expect them to attend class and get to work on time. We hope all that we taught them takes hold. For many, it’s the first time they have to balance a budget, go grocery shopping, do laundry, maintain a car, and so much more. Before the big send-off, parents discuss a lot of do’s and don’ts. But all too often, we forget to teach them to take the lead in protecting their own health. University Health System’s Teen Health Clinic, created in partnership with UT Health San Antonio, provides medical and educational services to those who are 10 to 25 years old. According to Medical Director, Dr. Kristen Plastino, it’s important for young adults to learn how to ask their own questions about their health. “We encourage our patients to open up and talk about sensitive, personal health issues ID16957312 © Lightpoet Dreamstime that need to be addressed,” Dr. Plastino said. “I routinely ask parents or guardians to step out of the room so I can understand the real concerns of my patients.” Experts recommend starting health discussions with youth in junior high or earlier. It’s helpful for parents to model how to interact with a doctor. Show teenagers how to focus on key points during a doctor visit, such as: • Making a list of questions ahead of time so you don’t forget anything • Writing down what the doctor says and repeating it to confirm that you understand • Paying attention to details of test results, required therapy, and necessary medications • Asking for instructions on how to access digital medical records Part of adulthood is talking about uncomfortable topics and taking action. Teenagers are best served by doctors who aren’t afraid to tackle conversations about sexually transmitted diseases, binge drinking, unsafe driving, anxiety and depression, bad relationships, or anything else that may be troubling the young adult.

Research published by the National Institutes of Health suggests that improving adolescent and young adult health is critical because the behaviors you start as a teenager build a foundation for future healthy or unhealthy lifestyle choices. Surprisingly, almost 20% of adolescents have experienced some type of behavioral or mental health disorder.Young adults experience this at a greater level, particularly in the areas of substance abuse, sexually transmitted infections and mental health problems. One way to promote good health is by being organized and having a plan for the unexpected. Dr. Plastino recommends young adults follow these healthy guidelines: • Schedule medical check-ups once a year and dental check-ups twice a year • Exercise, eat healthily and get at least seven to eight hours of sleep a night • Know your normal, physically and mentally • Before a crisis, establish a list of medical contacts • Have a trusted adult to go to for advice and support • Be aware of your own risky behaviors • Seek counseling services when needed The percentage of young Americans experiencing certain types of mental health disorders has risen significantly over the past decade, according to research recently published by the American Psychological Association. The research suggests that a higher percentage of adolescents and young adults may be experiencing mood disorders partially due to the increased use of electronic communications and digital media. Young adults are entering adulthood in a different technological culture compared to their parents. You can help your adolescent grow into a healthier adult by encouraging them to value self-care and engaging in more face-to-face communication. “Foster conversations, treatments, and preventive care that encourages healthy living for a lifetime,” Dr. Plastino said. “If you empower good decision making in your teenager, you’ll likely see good decision making as they become older.” Your health, at any age, is worth protecting. Dr. Kristen A. Plastino is Medical Director of the Teen Health Clinic. She also is a professor and Vice Chair of Clinical Operations for the Department of Obstetrics and Gynecology at UT Health San Antonio.

I'm an ADULT! . . Am I ready for this? It’s difficult to keep up in today’s world of technology and social media. It’s even far more challenging when you struggle with processing delays and autism. But, being left behind was never an option in my house. I was raised by a strong, independent mother whose primary goal was to make sure, somehow, I would be ready to navigate this world when I aged out of school. She was not waiting until I turned 18 to teach me how to work and use my talents. Being Independent Mom knew that being independent was a LEARNED BEHAVIOR; you’re not born with it. I was 13 years old when my mom heard Dr. Temple Grandin speak out to parents and young adults about building on talents and teaching them to work. She lives and breathes by what she calls “The Temple Grandin Golden Rules.” Mom created a life for me that would allow me to use my skills ... especially, if something happened to her. At an early age, it was obvious to people that I had a natural talent for art. My Mom could not afford formal art classes; instead, she would find affordable ways to help me develop my abilities. I attended free classes, discount classes, and even gathered recycled materials to stretch my imagination. Art is My Therapy Art became my form of therapy for my anxieties. Drawing, coloring, painting and even tearing paper for hours and days. Yes, it was an obsession; but it felt right. It became obvious that I had talent, some said a “Great Talent!” Mom wasn’t waiting; she seized the opportunity and decided to take my art to the people. She knew very little about the art world. But, she has courage and is very stubborn. She followed her heart, too. My future is brighter because of her, Dr. Temple Grandin, and so many others. A Lot Has Happened beyond the Art world.... Since starting my art career, a lot has happened. I have won and received honors for my artwork. We have built a successful Art and Literacy outreach program. I teach Eco-art using my techniques. Plus, we share our books, which feature an innovated dyslexie font for those struggling with reading. In 2018, we launched our first children’s book, “Grant the Jigsaw Giraffe ~ Different is More.” A heartfelt book about a young giraffe born different, but who follows his passion for art and rises above his challenges. Yes, it’s my story, but with a twist ... I am a giraffe! Within one year, we have sold over 12,000 books, won an “Outstanding Media and Communication Award” from the Governor of Texas, and been recognized by the Barbara Bush Literacy Program.

We’re currently writing books about other animals with disabilities or challenges: autism, dyslexia, epilepsy, bullying, and more. I’m creating new character art for these books. My Mom reminds me often ... this is not the “typical” transition that most artists experience. It takes years, lots of years, to become famous and some artists aren’t recognized until after they die. I am fortunate to witness people, of all ages, enjoying my art and being inspired to save the planet by creating Eco-art. I have a Plan B However, I do know that making a living as an artist comes with risks. So, I have a plan B ... I have a part-time job at STAPLES. I do enjoy working outside the home. Plus, I need to take a break from art so that I don’t burn out. I meet new friends, I’m developing job skills and I’ve learned to take Uber as I am also working on my driver’s license. There’s a balance and peace in my life as I multitask and navigate my world of autism. What's Next? My friends on the spectrum are moving into their own apartments, buying homes, getting degrees, obtaining full-time jobs, having relationships, getting married, and having babies. For me, I think I will start with the apartment next ... onestep at a time. -- Grant Maniér Age 24 “Over the years, I have watched Grant develop both artistically and as a person. Getting out and doing art shows has helped him develop both his art and social skills.” -Dr. Temple Grandin “When Grant started working here, we were unsure where his best placement would be. Along the way, we discovered his strengths. He’s great with customers and his attention to details made him one of my best cashiers in the building.” – Reyna Ybarra/STAPLES Manager

Eco-Impressionists of today.. Grant Maniér Shredding with a Purpose Grant is not your typical artist. He took a compulsive habit, ripping paper, and turned it into something magical. Collaging is an “OLD” form of art, but now is a “NEW” art movement thanks to eco-friendly times and Eco-artists like Grant Maniér. Grant sees a medium that can be brought to life through hours of work, intense focus abilities, and remarkable patience. He follows the same method for every art piece: a brush, recycle materials, tear, cut, glue, shade, glaze and repeat. The paper is expertly applied to canvas by the hands of a genius. “I never know how each one will turn out. I just keep going and see what happens, “stay-in-action”, is just one of my mottos,” Grant says. Grant Maniér finds inspiration and ideas from nature, animals, academic subjects, friends, traveling, the World Wide Web and his greatest inspiration-his mother. Grant has coined his art “COOLAGES” because he says he uses cool colors, cool shapes, cool textures. His art has brought him peace and purpose. “Art is my meditation,” he explains, “When my anxieties get the best of me and I need to look for peace, all I have to do is come back to my art and I feel connected to something bigger.” Grant has a garage full of puzzles and it’s no coincidence that the puzzle piece is the symbol for autism. Recycled puzzles happen to be one of Grant’s greatest signatures marks. Grant not only recycles puzzles for his art, once he receives the box he proceeds to peel the print off of each puzzle piece, “I need the puzzle to be paper thin so I can apply it to the canvas easily, that’s why I need to peel the print off each piece,” Grant says. He is the only known puzzle artist to apply this painstaking puzzle process to his art. Although he teaches children and adults to recycle puzzles and create art, it takes a tremendous amount of patience and time to create the masterpieces that Grant does effortlessly. Grant travels, makes appearances, and raises money for schools, galas, conferences, and other great causes. Exhibiting and teaching the importance of recycling and protecting our planet. With his beautiful and breath-taking Eco-Art masterpieces, Grant captures the attention of everyone around him and has created a following for his art. He has worked alongside Dr. Temple Grandin, the most well-known person with autism (HBO movie, Time Magazine “100 most influential people in the world). Grant has also raised thousands of dollars for great causes with his Eco-art and motivational message. In 2017 Grant and his mother, Julie Coy Maniér, embarked on a new chapter in their lives and began writing and illustrating a series of children’s books that highlight special needs animals in a zoo. Their first book, Grant the Jigsaw Giraffe~Different is MORE, exceeded their expectations of acceptance

by the special needs community. Each animal has a special challenge and finds through their friendship with Grant the Jigsaw Giraffe they are Different… but MORE! Because they receive compassion and mentorship they are able to rise above their diversity and understand how to turn their challenge around and make it a greater purpose and message to share with others.
Topics such as Autism, Epilepsy, Bullying, Deaf, Dyslexia and other challenges will be addressed. Grant and Julie are writing their books after individuals they have met along their own personal journey. These are real-life stories and circumstances. Grant and Julie are graciously giving proceeds to the person they create their story after. The most exciting and innovated aspect of their books is not just the stories they write, but each book is printed with a dyslexia friendly font. A font that allows the reader to enjoy and those with reading challenges to read comfortably. In less than a year they have sold over 14,000 books (Hardback and paperback). The books success is due largely to the Barbara Bush Houston Literacy Foundation. In addition they received a children’s book award from the Governor of Texas for media and communication for people with disabilities. Their books and toys can be found in area gift shops and on the website: www.JigsawGrant.com Each book is complimented with a plush toy that relates to the main character in the book; Grant the Jigsaw Giraffe, Tori the Red Panda, Macy the Zebra, Johnathan’s Ice Cream Truck, and more. Grant Manier, stands for “Hope,” and for others, a “New Era.” As an Eco-artist, he’s protecting and teaching others to save the earth. As a person living with autism, he has become a voice for those who are not heard. He leads the way with his remarkable special talent and teaching the world, “It’s not what we can’t do... It’s what we can do, that makes the difference” www.JigsawGrant.com Social Media pages: Jigsaw Grant

Supported Decision Making Agreements, Powers of Attorney, Guardianship, Oh My! As your child approaches age 18, the world of transition can be overwhelming. On their eighteenth birthday, your child becomes a legal adult whether they can function as such. One of the biggest challenges facing parents with children who have special needs is determining how best to support the child in adulthood. The Texas Legislature has placed some alternatives to guardianship, but the field is still murky and confusing. Let’s take and unpack the different options. First, you can have your child sign a “Supported Decision-Making Agreement.” This is basically an agreement which puts doctors, financial institutions, and anyone else that your child does business with on notice that your child wants and needs assistance in making decisions. This document does not allow parents to act alone on behalf of their child but does allow them to be present for appointments with physicians and participate in ARD meetings. For your child to sign one of these documents, the child must be able to understand the ramifications of the document. While Supported Decision Making Agreements can be helpful, they don’t work for everyone. Page 28

Second, there are Durable and Medical Powers of Attorney. Powers of Attorney are great documents, because they allow you to act for your child whether they are present or currently can answer questions. However, like with the supported decision-making agreement, your now adult child must have the legal capacity to execute the documents. That means they must understand the document and the possible consequences of signing it. The biggest issue with both powers of attorney and supported decision-making agreements is that they do not provide any protection for your child in the case of others taking advantage of them. This is where Guardianship comes in. Guardianship removes the ability of your child to enter into contracts on their own and instead gives that responsibility to the Guardian. Because this involves taking rights away from a person, the court must be involved and thus can be very costly. If you have a child approaching 18, you should discuss all options with an attorney who practices Guardianship and Estate Planning. For more information contact Weisinger Law Firm, PLLC 210-308-0800

Be Prepared for Flu Season Influenza is a viral infection that can cause mild to severe illness and, at times, death. People who have the flu are contagious even 1 day before their symptoms develop and up to 7 days after becoming sick. The most common type of flu is the seasonal flu. There are different strains of seasonal flu with the most common being Type A and Type B. Type A mutates rapidly while type B does not change much over time. Each year, a new form of the vaccine is developed to protect people against the strains that are expected to be most common that year. According to the CDC, the estimated number of flu illnesses in the last season was 49 Million. That’s more than the population of Texas and Florida combined! Flu Symptoms: • Fever • Cough • Sore Throat • Runny or stuffy nose • Muscle or body aches • Headaches • Fatigue • Vomiting / diarrhea Children and the Flu: Young children (under 5 years of age, and especially under 2 years old) are at high risk of serious flu-related complications. Complications from flu among children in this age
group can include: o Pneumonia o Dehydration o Worsening of long-term medical problems o Brain dysfunction o Sinus problems and ear infections o In rare cases, death. Prevention is key to decreasing the risk of influenza related illness. Be sure to make the time to get yourself and your children vaccinated. This will decrease the risk of your children getting the flu. The flu vaccine decreases the risk of severity of symptoms as well as hospitalizations and death in children. As much as possible, keep yourself and your children away from people who are sick. If you get flu symptoms, avoid all preventable contact with other people. Cover your nose and mouth with a tissue when you cough or sneeze, and wash your hands often with soap and water. If you are not near water, use an alcohol-based hand sanitizer. Try not to touch your eyes, nose and mouth as germs are often spread this way. Lastly, make sure to clean and disinfect the common areas around your home. As always, it is important to seek medical care as soon as possible. Your medical provider can provide medication to combat the infection as well as preventative medication for close family members living in the same household.
