Caring for those with life-threatening illness Hospice is a philosophy of caring for those living with a life-threatening illness. The hospice philosophy holds that endof-life care should emphasize quality of life. The object is to treat the whole person and not just the disease. The hospice philosophy focuses on patient/family-centered care which addresses the whole person — body, mind and spirit. Every member of the care team supports clinical excellence and unwavering compassion to the patients and caregivers at every stage of life. Each health care professional works with the patient to design and implement a plan of care unique to the patient’s diagnosis. In addition, hospice provides all medications, services and equipment related to the terminal illness. Hospice care does not end with the patient’s death; it continues with up to 13 months of bereavement counseling for the family and loved ones. Hospice cares for people where they live. Although some hospice care is provided in hospitals, in-patient hospice facilities or nursing care facilities, the vast majority of patients are cared for in the place they call home.
Hospice is a Medicare benefits. Congress’s Medicare Hospice Benefits of 1983 was established to ensure that all Medicare beneficiaries could access high-quality end-of-life care. Americans
are now promised the opportunity to live the end of their lives free of pain and with emotional and spiritual support. More that 80 percent of hospice patients are Medicare beneficiaries. Hospice Care is not limited to Cancer Patients. Hospices now care for over half of all Americans who die from cancer and a growing number of patients with other chronic, life threatening illness, such as: lung disorder, cerebral, vascular and heart, and alzheimer and dementia. Hospice Care is Increasing. Twice as many seniors died in hospice care as in a hospital or nursing home compared with a decade ago. Furthermore, 44 percent of Americans with a terminal illness choose hospice care, and 97 percent of those people choose home hospice care, according to a recent study conducted by the National Hospice and Palliative Care Organization. The proportion of dying patients who used hospice care rose from 21.6 percent in 2000 to 42.2 percent in 2009. By choosing hospice care you and your family will have peace of mind knowing that there is a plan of care providing your loved one with dignity.

Is there a chiropractor in the house? Chiropractic treatment is the most sought after therapy for those with back pain. About 27 million Americans visit chiropractors annually and this number continues to grow. There are a variety of causes of back pain, such as accidents, sports injuries, muscle strains, herniated discs, and sciatica. Chiropractic care is not limited to the treatment of back pain, but also treats headaches and neck, arm, and leg pain. Additionally, there has been success in the treatment of ear infections with chiropractic care. So who is the chiropractor? A Doctor of Chiropractic (DC) or chiropractor is educated in a nationally accredited, fouryear doctoral graduate school program through a curriculum that includes a minimum of 4,200 hours of classroom, laboratory, and clinical internship. They use hands-on spinal manipulation and other alternative treatments that focus on treating disorders of the musculoskeletal system and the nervous system. The approach to treatment is drug-free and hands-on and includes patient examination, diagnosis, and treatment. Chiropractors can also recommend therapeutic exercises as well as provide nutritional, dietary, and lifestyle counseling. Spinal manipulation or chiropractic adjustment is the most commonly used therapeutic treatment performed by the chiropractor. The purpose of this manipulation is to re-
store joint mobility to joints that have become hypomobile or restricted in their movement as a result of a tissue injury. There are many factors that can cause tissue injury, which many do not realize. For example improper lifting of a heavy object, repetitive stresses, sitting in awkward positions, poor spinal posture, carrying backpacks and purses can all cause stress and injury to tissue. The pain caused by tissue injury can be treated by manipulation, or adjustment of that affected joint and tissue. By doing so, mobility is restored, thereby alleviating pain and muscle tightness, and allowing tissues to heal. Quick facts about your chiropractor: • Recent studies have shown treatment for low back pain initiated by a DC costs less than more traditional methods. • DCs are designated as physician-level providers in the vast majority of states and federal Medicare program. • DCs are the highest rated healthcare practitioner for low-back pain treatments. • All 32 National Football League teams utilize DCs in optimizing the functionality, endurance and overall conditioning of their players in the treatment of neuromusculoskeletal injuries. For more information, contact your doctor of chiropractic and start your road to less pain and better health.

New American dentist MICHAEL K. REECE, D.D.S., LVIM Reece Dentistry
Dentists are rapidly becoming an integral part of healthcare with innovative treatments not imaginable only a few years ago. Science has shown the clear relationship between oral health and overall physical wellness. Heart health, sleep apnea, headaches and diabetes can be reflected in the state of our teeth, gums and bite. Today, leading dentists are monitoring blood pressure, solving TMJ related headaches, using oral appliances to treat sleep apnea, and extending the useful life of our teeth far into our future using space age materials and technology. Aging and broken teeth are being replaced with fully functional, cosmetically beautiful teeth made of composites and secured in a variety of ways including implants that allow teeth to be as functional and solid as natural teeth. Even the dental experience has changed with the advent of pain-free sedation dentistry and one location, one-visit, convenience. This new dental field is called ‘comprehensive’ or ‘neuromuscular dentistry’ and is being taught at LVI Global, the leading post-graduate dental school in the world. Dentistry has evolved from ‘drill and fill’ to ‘diagnose and treat.’ The training and tools available today is progressing at light speed. In my own practice, we dedicate at least a week per month to training. It is personally rewarding being able to give our patients access to the latest proven techniques and technology that create functional, beautiful smiles that will last them a lifetime. As patients we want the best. By taking advantage of the latest advances in comprehensive dentistry we now have another resource in helping us stay active and healthy for a lifetime.

Education is key to diabetes prevention BECKY KUBICEK
Diabetes Coordinator Scott & White Hospital-Brenham
Diabetes affects nearly 26 million Americans, and an estimated 79 million Americans have prediabetes, a condition that increases their chances of developing type 2 diabetes. In Washington County alone, diabetes affects many of our residents, and it is a growing epidemic. Diabetes is a disease in which the body does not produce enough insulin or does not use insulin properly. This results in high blood glucose, also known as blood sugar, and too much blood glucose can lead to serious health problems, such as heart disease, blindness, kidney disease, amputation, and even death. With early diagnosis and treatment, however, individuals with diabetes may prevent the development of these health problems. Type 2 diabetes, formerly called adult-onset diabetes, is the most common form of diabetes. People can develop type 2 diabetes at any age, and with the recent increase in childhood obesity, young children are now being diagnosed with type 2 diabetes. Symptoms of type 2 diabetes may include fatigue, blurred
vision, cuts and sores that do not heal, and numbness and tingling of the hands, legs or feet. Some individuals have no signs at all. Risk factors for type 2 diabetes include being overweight, getting too little physical activity, and having a parent, brother, or sister with diabetes. If you have any of these risk factors for diabetes, ask your physician if you need to be tested. Good blood glucose control is achieved by daily attention to nutrition, physical activity, weight control, monitoring blood glucose, and taking medications as prescribed. Preventive care practices such as annual eye and foot exams are also essential to keeping individuals with diabetes healthy. Diabetes education, also known as diabetes self-management education, is an interactive ongoing process that involves the individual with diabetes with a diabetes educator. This intervention aims to achieve optimal health, better quality of life, and reduce the need for costly health care. Diabetes educators give people with diabetes the knowledge, skills, and tools needed to successfully manage their diabetes and avoid many of the complications associated with the disease.

How to choose a dentist for your kids KETAN SUKKAWALA, DDS AND LARRY KALKE, DMD
Brazos Valley Pediatric Dentistry
How to select a dentist for your kids: What should parents consider in making their decision? Pediatric dentists are the pediatricians of dentistry. They are specially trained for children’s unique oral health needs. Their professional education includes two to three years of specialized study after becoming a dentist, emphasizing child psychology, growth and development, and special health care needs. Infants, preschoolers, children and adolescents require different approaches to manage their behavior, guide their dental growth and development, and help them avoid future dental problems. The pediatric dentist is best qualified to meet these needs. Questions to ask to choose the right dentist for your child: BEFORE THE VISIT • Does the dentist have special training or interest in treating children? • Is the dentist a member of the American Academy of Pediatric Dentistry and
the American Dental Association? • Is the dental office set up for children? For example, does it offer toys, books, games or child sized furniture? • How does the dental office manage emergencies? • Is the office conveniently located to your home or child’s school? • Does the practice accept your dental benefit plan? AFTER THE VISIT • Was your child seen promptly? • Were you asked for a complete medi-
cal and dental history for your child? • Was the dentist gentle, but thorough when examining your child’s mouth? • Did the dentist and staff talk to your child, encouraging their involvement in their dental health? • Were you informed about your child’s tooth development, the cause and prevention of dental disease and appropriate dental care at home? • Were your questions treated with concern and respect? • Was the visit positive for your child?

Want to prolong your life? Start swimming Let me tell you a few benefits of swimming. Regular swimming can offer anyone, especially older adults, a wide range of health benefits — including feeling and looking younger. The ability to do more with less Swimming offers something no other aerobic exercise does: the ability to work your body without harsh impact to your skeletal system. When the human body is submerged in water, it automatically becomes lighter. When immersed to the waist, your body bears just 50 percent of its weight; dunk yourself to the chest and that number reduces to around 25 to 35 percent; with water all the way to the neck, you only have to bear 10 percent of your own weight. The other 90 percent is handled by the pool. This means that the pool provides an ideal place to work stiff muscles and sore joints, especially if you suffer from arthritis. In its recommendation for the right types of exercise for people with arthritis, “the Arthritis Foundation suggests those that stretch muscles, those that strengthen muscles, and those that provide an aerobic workout. A few laps in the pool combine all three!” If you have access to a pool that is heated, the warm water can help loosen stiff joints. In fact, SWIMMING continued on page 16

5 reasons you need a primary care provider ERIC ALFORD, M.D. Brenham Clinic
Imagine this scenario: One summer, Mary feels irritable and anxious much of the time. She thinks she is having panic attacks, so she visits a counselor. The next winter, she goes to the emergency department for heart palpitations. After many months, she learns that an overactive thyroid was causing her symptoms. However, no single provider had every piece of the puzzle, so it took her a long time to get an accurate diagnosis. A primary care provider (PCP) — a physician who cares for overall health — would likely have been able to diagnose the cause of her symptoms much sooner. It’s important to develop a relationship with a PCP who gets to know you and your medical conditions over time. Specialists, urgent care providers and emergency room doctors are experienced in their area of medicine; however, they do not usually know you and your health history like your PCP will. They Make Your Life Easier In addition to seeing the “big picture” of your health, a PCP can: 1. Obesity still rising among U.S. adults despite government’s best efforts; women overtake men MIKE STOBBE
AP Medical Writer
NEW YORK — Obesity is still rising among American adults, despite more than a decade of public-awareness campaigns and other efforts to get people to watch their weight, and women have now overtaken men in the obese category, new government research shows. For the past several years, experts thought the nation’s alarming, decades-long rise in obesity had leveled off. But the Centers for Disease Control and Prevention said in a report Thursday that the obesity rate climbed to nearly 38 percent of adults in 2013-14, up from 32 percent about a decade earlier. “This is a striking finding” and suggests that a situation that was thought to be stable is getting worse, said Dr. Wil-
liam Dietz, an obesity expert at George Washington University. But another authority, the University of North Carolina’s Barry Popkin, urged caution, saying the participants selected for the study may not have been representative of the nation as a whole. Experts said they had no explanation for why the obesity rate appears to be rising. The report also found a tipping of the scales toward women. Obesity rates for men and women had been roughly the same for about a decade. But in the new report, the rate was significantly higher for women, at 38 percent, compared with 34 percent for men. Obesity — which means not merely overweight, but seriously overweight — is considered one of the nation’s leading public health problems. Until
the early 1980s, only about 1 in 6 adults were obese, but the rate climbed dramatically until it hit about 1 in 3 around a decade ago. The new figures come from a regular government survey that has about 5,000 participants each year. In interviews, people tend to understate their weight and overstate their height. But participants in this survey are actually weighed. Because of that, it is considered the gold standard for measuring the nation’s waistline. The news comes after years of government anti-obesity campaigns to encourage people to eat better and exercise. Also, soda consumption has dropped in recent years, and fast-food chains have adopted healthier menus. New federal rules have also been adopted to remove arti-
ficial trans fats from grocery store foods and to require chain restaurants to post calorie counts, though those have not gone into effect yet. The widening gap between men and women seems to be driven by what’s happening among blacks and Hispanics, said the study’s lead author, the CDC’s Cynthia Ogden. Obesity rates for white men and white women remain very close. But for blacks, the female obesity rate has soared to 57 percent, far above the male rate of 38 percent. The gender gap is widening among Hispanics, too — 46 percent for women, 39 percent for men. The report also looked at obesity in children but did not see much change. For young people ages 2 to 19, the rate has been holding at about 17 percent over the past decade or so.

New push for pint-sized medical devices to treat sick kids LAURAN NEERGAARD AP Medical Writer
WASHINGTON — Improvise isn’t a word parents want to hear from their kid’s doctor. Yet pediatric specialists too often have to jury-rig care because many of the medical devices needed to treat sick children were built for adults. Part of the problem is size. Doctors fixed Alice de Pooter’s faltering heartbeat by wedging an adult pacemaker into a baby’s body. But the device’s large battery bulged so badly under her rib cage that she struggled to sit upright until her first birthday. It’s also an engineering problem. Children aren’t just miniature adults; their bodies are growing and changing. When adult devices haven’t been formally studied in children, using them in youngsters can raise safety questions. “It affects patient care. We need to find a resolution,” Dr.
Matthew Oetgen, chief of orthopedic surgery and sports medicine at Children’s National Health System, said at a recent grants competition that the Washington hospital hosted to help spur development of innovative pediatric devices. There’s little financial incentive to create and test pint-sized devices because children overall are healthier than adults and make up a fraction of the treatment market. But families are starting to demand solutions. A birth defect left Miyah Williams with one leg missing at midthigh. The prosthetic leg she received as a toddler came with such a painful, sweat- and sore-inducing socket — a rigid cup connecting the leg to her thigh — that she refused to wear it. Her mother eventually found a small San Francisco company willing to design a softer, adjustable socket able to grow some as Miyah does. Miyah, now 3, dances to show
off a leg finally comfortable enough to wear all day. “She told them earlier today that her bone no longer hurts,” Tamara Williams of Fayetteville, North Carolina, said of manufacturer LIM Innovations. “They made her a socket that changed her life.” The industry acknowledges medical devices designed specifically for children often lag five years to 10 years behind new technology for adults, and Food and Drug Administration statistics illustrate the disparity. In 2013, eight of the 38 novel or higher-risk devices FDA approved were labeled for use by patients younger than 22. In 2014, six of 33 such device approvals were for pediatric use; so were two devices for rare disorders allowed to sell under a special fast-tracking program. Consider pacemakers. Children account for only about 1 percent of pacemaker implants, said Dr. Charles Berul, co-director of Children’s National
Heart Institute. Adult pacemakers such as the biventricular version implanted into Alice de Pooter when she was 4 months old can save lives, but improvising has drawbacks, he said. Adults typically have a pacemaker’s electrical leads, or wires, easily threaded into the heart through a vein, but babies require open-chest surgery attaching them to the outside of the heart, explained Berul, Alice’s longtime cardiologist. Faster child heartbeats wear out the batteries more rapidly, and the leads can stretch out of place as tots grow. Now 3 and living near Houston, Alice has a well-functioning heart and while the battery’s outline remains visible in her belly, “she’s running and happy,” said her mother, Charlotte Vanheusden. Manufacturers are developing a tiny wireless pacemaker for adults. If it works, Berul hopes eventually that could be adapted for children, too.

Swimming Continued from page 10 people with rheumatoid arthritis receive greater benefits to their health after participating in hydrotherapy than with other activities. It’s also been proven that water-based exercise improves the use of affected joints and decreases pain from osteoarthritis. Increased muscle tone and strength Ever see a flabby dolphin or a weak-looking competitive swimmer? Didn’t think so!! That’s because swimming is a great way to increase muscular strength and muscle tone — especially compared to several other aerobic exercises. Take running, for example. When a jogger takes few laps around the track, that jogger is only moving his or her body through air. A swimmer, on the other hand, is propelling himself through water — a substance about 12 times as dense as air. That means that every kick and every arm stroke becomes a resistance exercise — and it’s well known that resistance exercises are the best way to build muscle tone and strength. Improved flexibility Unlike exercise machines in a gym that
tend to isolate one body part at a time (like a bicep curl machine, for example), swimming puts the body through a broad range of motion that helps joints and ligaments stay loose and flexible. The arms move in wide arcs, the hips are engaged as the legs scissor through the water, and the head and spine twist from side to side. Plus, with every stroke, as you reach forward, you’re lengthening the body, which not only makes it more efficient in the water, it also helps give you a good stretch from head to toe. To improve your flexibility beyond the natural gains you’ll make by swimming, you might also want to fi nish your pool workout with a series of gentle stretches. The support of the water should help you maintain positions involving tricky balance — such as a quadriceps stretch — for longer periods of time. A healthier heart In addition to toning visible muscles like pectorals, triceps and quads, swimming also helps improve the most important muscle in our bodies: the heart. Because swimming is an aerobic exercise, it serves to strengthen the heart, not

only helping it to become larger, but making it more efficient in pumping — which leads to better blood flow throughout your body. If that’s not enough to get you moving in the pool, the American Heart Association reports that just 30 minutes of exercise per day, such as swimming, can reduce coronary heart disease in women by 30 to 40 percent. Additionally, it is shown that regular aerobic exercise can reduce blood pressure as well. Weight control For some time, some people thought that because water is generally cooler than our body temperatures, it would be difficult to lose weight with a water workout. Like many old ideas about exercise, this has since been revised: Swimming is now recognized as one of the biggest calorie burners around, and it’s great for keeping weight under control. The exact number of calories you burn, of course, depends on your own physiology and the intensity with which you exercise, but as a general rule, for every 10 minutes of swimming: the breast stroke SWIMMING continued on page 17

Swimming Continued from page 16 will burn 60 calories; the backstroke torches 80; the freestyle lights up 100; and the buttery stroke incinerates an impressive 150. To boost the calorie-burning component of swimming, consider using intervals in which you work your hardest for short bursts and then recover. One way to structure this kind of workout would be to swim 50 yards then rest for 10 seconds, then 100 yards with a 10-second rest, then 150 yards — all the way up to 300 yards with rests in between. When you reach 300 yards, reverse the pattern. Improved asthma symptoms Unlike exercising in the often dry air of the gym, or contending with seasonal allergies or frigid winter air, swimming provides the chance to work out in moist air, which can help reduce exercise-induced asthma symptoms. Not only can pool workouts help you avoid asthma attacks if you’re prone to them, some studies have shown that swimming can actually improve the condition overall. According to a study published in the scholarly journal Science Daily, “Respirology, when a group of kids completed
a six-week swimming program, they saw improvements in symptom severity, snoring, mouth-breathing, and hospitalizations and emergency room visits.â€? Even those without asthma can beneďŹ t from swimming as the exercise can increase lung volume and teach proper breathing techniques. Improve cholesterol Being healthy is more about having the right ratio of cholesterol in your body than just having low amounts of the stuff in your blood. SpeciďŹ cally, it’s beneďŹ cial to have higher levels of “goodâ€? cholesterol (HDL) and lower levels of LDL, or “badâ€? cholesterol. Swimming can get these levels in the right balance thanks to its aerobic power, which has been proven to raise HDL levels. And for every 1 percent increase in HDL cholesterol, the risk of dying from heart disease drops by 3.5 percent. What’s more, studies have shown that aerobic exercises like swimming can also keep your endothelium in good shape. What’s your endothelium, you might wonder? It’s the thin layer of cells that lines your arteries, and it tends to lose exibility as you age.
The theory is that because aerobic exercise causes arteries to expand and contract, it keeps them ďŹ t. Low risk of diabetes When it comes to warding off diabetes, there are few prescriptions as powerful as aerobic exercise. In one study, men reduced their risk of diabetes by an average of 6 percent for every 500 calories a week they burned in aerobic exercise. With just 30 minutes of breaststroke swimming three times per week, you could burn 900 calories — reducing your risk of contracting type 2 diabetes by over 10 percent. A study that focused on women also suggested the same beneďŹ ts for the fairer sex: Vigorous exercise just once a week (like the kind derived from a robust swimming session) lowered their risk of contracting type 2 diabetes by 16 percent over inactive women. According to the American Diabetes Association, diabetics should get 150 minutes per week, spread across at least three days per week, of moderate-intensity physical activity like swimming to aid SWIMMING continued on page 18

edge of many conditions older adults experience. • A gynecologist cares for women’s health and may also offer general medical care to female patients. • An internal medicine doctor provides general medical care for patients with complex or chronic conditions. • A pediatrician offers general medical care for infants, children and adolescents.
Swimming Continued from page 17 glycemic control. Lower stress, higher spirits and better brain William Wilson wrote in the 1883 book, “The Swimming Instructor”: “The experienced swimmer, when in the water, may be classed among the happiest of mortals in the happiest of moods, and in the most complete enjoyment of the happiest of exercises.” Wilson probably didn’t
know this in the 19th century, but all that happiness was likely due to the release of feel-good chemicals known as endorphins — one of swimming’s most pleasant side effects. In addition to a natural high, swimming can also evoke the relaxation response the same way yoga works on the body. This is due in large part to the constant stretching and relaxing of your muscles com-
bined with deep rhythmic breathing. Swimming is also a meditative exercise, with the sound of your own breathing and the splash of the water. You might just live longer If the previous nine reasons weren’t enough to convince you of the health benefits of swimming, perhaps this one will: It can keep you from dying. Actually, we’re not promising Aquaman-like
immortality, but it seems that swimming can at least help you avoid dying prematurely. Researchers at the University of South Carolina followed 40,547 men, aged 20 to 90, for 32 years and discovered that those who swam had a 50 percent lower death rate than runners, walkers or men who got no exercise. The study authors concluded that the same benefits would be received by aqua-women as well as men.
