How to choose a dentist for your kids KETAN SUKKAWALA, DDS AND LARRY KALKE, DMD

Brazos Valley Pediatric Dentistry

How to select a dentist for your kids: What should parents consider in making their decision? Pediatric dentists are the pediatricians of dentistry. They are specially trained for children’s unique oral health needs. Their professional education includes two to three years of specialized study after becoming a dentist, emphasizing child psychology, growth and development, and special health care needs. Infants, preschoolers, children and adolescents require different approaches to manage their behavior, guide their dental growth and development, and help them avoid future dental problems. The pediatric dentist is best qualified to meet these needs. Questions to ask to choose the right dentist for your child: BEFORE THE VISIT • Does the dentist have special training or interest in treating children? • Is the dentist a member of the American Academy of Pediatric Dentistry and

the American Dental Association? • Is the dental office set up for children? For example, does it offer toys, books, games or child sized furniture? • How does the dental office manage emergencies? • Is the office conveniently located to your home or child’s school? • Does the practice accept your dental benefit plan? AFTER THE VISIT • Was your child seen promptly? • Were you asked for a complete medi-

cal and dental history for your child? • Was the dentist gentle, but thorough when examining your child’s mouth? • Did the dentist and staff talk to your child, encouraging their involvement in their dental health? • Were you informed about your child’s tooth development, the cause and prevention of dental disease and appropriate dental care at home? • Were your questions treated with concern and respect? • Was the visit positive for your child?

Education is key to diabetes prevention BECKY KUBICEK

Diabetes Coordinator Scott & White Hospital-Brenham

Diabetes affects nearly 26 million Americans, and an estimated 79 million Americans have prediabetes, a condition that increases their chances of developing type 2 diabetes. In Washington County alone, diabetes affects many of our residents, and it is a growing epidemic. Diabetes is a disease in which the body does not produce enough insulin or does not use insulin properly. This results in high blood glucose, also known as blood sugar, and too much blood glucose can lead to serious health problems, such as heart disease, blindness, kidney disease, amputation, and even death. With early diagnosis and treatment, however, individuals with diabetes may prevent the development of these health problems. Type 2 diabetes, formerly called adult-onset diabetes, is the most common form of diabetes. People can develop type 2 diabetes at any age, and with the recent increase in childhood obesity, young children are now being diagnosed with type 2 diabetes. Symptoms of type 2 diabetes may include fatigue, blurred

vision, cuts and sores that do not heal, and numbness and tingling of the hands, legs or feet. Some individuals have no signs at all. Risk factors for type 2 diabetes include being overweight, getting too little physical activity, and having a parent, brother, or sister with diabetes. If you have any of these risk factors for diabetes, ask your physician if you need to be tested. Good blood glucose control is achieved by daily attention to nutrition, physical activity, weight control, monitoring blood glucose, and taking medications as prescribed. Preventive care practices such as annual eye and foot exams are also essential to keeping individuals with diabetes healthy. Diabetes education, also known as diabetes self-management education, is an interactive ongoing process that involves the individual with diabetes with a diabetes educator. This intervention aims to achieve optimal health, better quality of life, and reduce the need for costly health care. Diabetes educators give people with diabetes the knowledge, skills, and tools needed to successfully manage their diabetes and avoid many of the complications associated with the disease.

Sinusitis affects 37 million Americans each year R. PAUL FULMER, MD FACS

Real Sinus Solutions

More Americans suffer from sinusitis than diabetes, asthma, or coronary heart disease. Sinusitis affects 37 million Americans each year, making it one of the most common health problems. Sinusitis impacts a person’s quality of life and accounts for over $8 billion in annual health care expenditures. Is it allergies, a cold or sinusitis? Your nose is congested and it’s hard to breathe. Thick mucus irritates the back of your throat. Your face, head and even your teeth hurt from the pressure. You’re losing your sense of smell and taste. You’re incredibly tired and irritable. You think that it must be yet another cold or allergy attack again this year. You take allergy or cold medicines to relieve your symptoms but they don’t help. Finally, you see your primary care physician or general practitioner. After listening to your history of symptoms and conducting an exam the doctor says you have acute sinusitis, which is a temporary inflammation of the sinus lining that is caused by a bacterial infection and commonly called a sinus infection. Your doctor may recommend saline nasal SINUSITIS continued on Page 7

Sinusitis Continued from Page 6 sprays, antibiotics, nasal steroid sprays, decongestants and over the counter pain relievers to help relieve the condition. However, if you experience symptoms for longer than 12 weeks you could have chronic sinusitis. A look into the sinuses The sinuses are hollow spaces in the skull (i.e. the frontal, ethmoid, sphenoid and maxillary) which serve to lighten the skull and give resonance to the voice. The purpose of the sinuses, which open into the nasal cavity, is to generate mucus to keep the nose from drying out during breathing and to trap unwanted materials so that they do not reach the lungs. When you have acute or chronic sinusitis, the mucous membranes of your nose, sinuses and throat become inflamed, possibly from a pre-existing cold or allergies. Swelling obstructs the sinus openings and prevents normal mucus drainage, causing mucus and pressure to build up. Symptoms include: drainage of a thick, yellow or greenish discharge from the nose or down the back of the throat; nasal obstruction or congestion; tenderness and swelling around the eyes, cheeks, nose and forehead; and/or a reduced sense of smell and taste. The impact of chronic sinusitis on a person’s quality of life could include: throbbing facial pain or headaches, congestion, bad breath, irritability, fatigue or nausea. The treatment of chronic sinusitis If chronic sinusitis is suspected, your doctor most likely will refer you to an Ear, Nose and Throat (ENT) physician (an otolaryngology specialist). The ENT physician uses several methods to help screen for chronic sinusitis: visual SINUSITIS continued on Page 9

The new American dentist MICHAEL K. REECE, D.D.S., LVIM Reece Dentistry

Dentists are rapidly becoming an integral part of healthcare with innovative treatments not imaginable only a few years ago. Science has shown the clear relationship between oral health and overall physical wellness. Heart health, sleep apnea, headaches and diabetes can be reflected in the state of our teeth, gums and bite. Today, leading dentists are monitoring blood pressure, solving TMJ related headaches, using oral appliances to treat sleep apnea, and extending the useful life of our teeth far into our future using space age materials and technology. Aging and broken teeth are being replaced with fully functional, cosmetically beautiful teeth made of composites and secured in a variety of ways including implants that allow teeth to be as functional and solid as natural teeth. Even the dental experience has changed with the advent of pain-free sedation dentistry and one location, one-visit, convenience. This new dental field is called ‘comprehensive’ or ‘neuromuscular dentistry’ and is being taught at LVI Global, the leading post-graduate dental school in the world. Dentistry has evolved from ‘drill and fill’ to ‘diagnose and treat.’ The training and tools available today is progressing at light speed. In my own practice, we dedicate at least a week per month to training. It is personally rewarding being able to give our patients access to the latest proven techniques and technology that create functional, beautiful smiles that will last them a lifetime. As patients we want the best. By taking advantage of the latest advances in comprehensive dentistry we now have another resource in helping us stay active and healthy for a lifetime.

After diagnosing chronic sinusitis and identifying a possible cause, your ENT will often begin with medical management. Medical management of chronic sinusitis may include: • Decongestants • Nasal steroids • Antibiotics • Mucus thinning drugs • Oral steroids Healthcare professionals often fi nd it difficult to treat the majority of chronic sinusitis sufferers with medication. In fact, it is estimated that up to 60 percent of chronic sinusitis sufferers are not successfully treated with medication. Patients who do not respond well to medications become candidates for conventional sinus surgery, which is known as functional endoscopic sinus surgery (FESS). Since the 1980s, FESS has been the standard of care for sinus surgery. FESS aims to clear blocked sinuses and restore normal sinus drainage by removing bone and tissue to enlarge the sinus opening, which may lead to pain, scarring and bleeding. However, conventional sinus surgery no longer has to be the only option for chronic sinusitis patients who are not responding well to medications. Balloon Sinuplasty is a breakthrough procedure that relieves the pain and pressure associated with chronic sinusitis. To learn more about chronic sinusitis, visit the American Academy of Otolaryngology.

Want to prolong your life? Start swimming Let me tell you a few benefits of swimming. Regular swimming can offer anyone, especially older adults, a wide range of health benefits — including feeling and looking younger. The ability to do more with less Swimming offers something no other aerobic exercise does: the ability to work your body without harsh impact to your skeletal system. When the human body is submerged in water, it automatically becomes lighter. When immersed to the waist, your body bears just 50 percent of its weight; dunk yourself to the chest and that number reduces to around 25 to 35 percent; with water all the way to the neck, you only have to bear 10 percent of your own weight. The other 90 percent is handled by the pool. This means that the pool provides an ideal place to work stiff muscles and sore joints, especially if you suffer from arthritis. In its recommendation for the right types of exercise for people with arthritis, “the Arthritis Foundation suggests those that stretch muscles, those that strengthen muscles, and those that provide an aerobic workout. A few laps in the pool combine all three!” If you have access to a pool that is heated, the warm water can help loosen stiff joints. In fact, people with rheumatoid arthritis receive greater benefits to their health after participating in hydrotherapy than with other activities. It’s also been proven that water-based exercise improves the use SWIMMING continued on Page 14

Is there a chiropractor in the house? MICHAEL REID, DC

Austin County Chiropractic

Chiropractic treatment is the most sought after therapy for those with back pain. About 27 million Americans visit chiropractors annually and this number continues to grow. There are a variety of causes of back pain, such as accidents, sports injuries, muscle strains, herniated discs, and

sciatica. Chiropractic care is not limited to the treatment of back pain, but also treats headaches and neck, arm, and leg pain. Additionally, there has been success in the treatment of ear infections with chiropractic care. So who is the chiropractor? A Doctor of Chiropractic (DC) or chiropractor is educated in a nationally accredited, four-year doctoral graduate

school program through a curriculum that includes a minimum of 4,200 hours of classroom, laboratory, and clinical internship. They use hands-on spinal manipulation and other alternative treatments that focus on treating disorders of the musculoskeletal system and the nervous system. The approach to treatment is drug-free and hands-on and includes patient examina-

tion, diagnosis, and treatment. Chiropractors can also recommend therapeutic exercises as well as provide nutritional, dietary, and lifestyle counseling. Spinal manipulation or chiropractic adjustment is the most commonly used therapeutic treatment performed by the chiropractor. The purpose of this manipHOUSE continued on Page 13

Midlife emotions and weight gain LETE PHILLIPS

Lete Phillips Exercise Program

Are you a woman in your 40s or 50s? The physical and psychological changes that occur during these midlife years can cause emotional stress, which may lead to unwanted weight gain. What’s going on, and how can you navigate more easily through your middle years? Get insights below from Cathy Cohen, MA, a nutrition counselor at a hospital-affiliated wellness center and a certified personal trainer and fitness instructor at a rehabilitation hospital adult fitness facility, both of which are in Westchester County, New York. A Time of Change The midlife years can be particularly challenging for women. You face perhaps the most emotionally charged-and quite possibly most frustrating-time of your life in terms of trying to maintain an ideal body weight. Emotional issues that may have been suppressed for years can surface during midlife. Divorce, financial burdens, retirement, empty nesting, a parent’s illness or death, career moves and residential changes often

occur, along with unpleasant menopausal symptoms, such as hot flashes, memory loss, slowed metabolism, mood swings, redistribution of body weight and sexual difficulties. These issues are only made worse by the constant barrage of unhealthy, unrealistically thin images foisted on women by the media. Failure to meet society’s ideal female body image can be another major source of emotional stress. How Your Body Responds These physical and psychological changes can cause stress. And during situations perceived as stressful, your sympathetic nervous system sets in motion a series of physiological responses, according to Hans Selye, MD, who pioneered the concept of emotional stress. Hormones produced by the adrenal cortex, including cortisol and epinephrine, prepare the body for an instance state of readiness in the classic “fight or flight” response. Selye theorized that once the stress-invoking threat passed, the body returned to a state of homeostasis, or normalcy and balance. However, recent research shows that chronic outpouring of stress hormones may result in undesirable body changes.

For example, some researchers believe that chronically elevated cortisol levels can lead to weight gain, especially in the belly. Causes for Weight Gain You may also be gaining weight if you are reaching for food as comfort. While there is definitely a psychological basis for emotional eating, physiological issues also play a role. A groundbreaking study showed that foods high in fat and carbohydrate may actually fight stress by reducing cortisol levels in the body (Dallman et al, 2003). Deeper emotional issues can be the underlying reason for weight gain, as well. You may harbor unresolved issues well into midlife and beyond, perhaps striving to remain “protected” by your fat and thereby rendering future weight loss efforts futile. Stress Management Techniques Consider these strategies for decreasing stress during midlife years. • Practice Mind-Body Techniques. Yoga, Pilates, tai chi, meditation, breathing techniques, biofeedback, relaxation tapes, guided imagery, journal writing and other forms of self-expression are effective stress busters. There are many factors that can cause tissue injury, which many do not realize. For example improper lifting of a heavy object, repetitive stresses, sitting in awkward positions, poor spinal posture, carrying backpacks and purses can all cause stress and injury to tissue. The pain caused by tissue injury can be treated by manipulation, or adjustment of that affected joint and tissue. By doing so, mobility is restored, thereby alleviating pain and muscle tightness, and allowing tissues to heal. Quick facts about your chiropractor: • Recent studies have shown treatment for low back pain initiated by a DC costs less than more traditional methods. • DCs are designated as physician-level providers in the vast majority of states and federal Medicare program. • DCs are the highest rated healthcare practitioner for low-back pain treatments. • All 32 National Football League teams utilize DCs in optimizing the functionality, endurance and overall conditioning of their players in the treatment of neuromusculoskeletal injuries. Swimming of affected joints and decreases pain from osteoarthritis. Increased muscle tone and strength Ever see a flabby dolphin or a weak-looking competitive swimmer? Didn’t think so!! That’s because swimming is a great way to increase muscular strength and muscle tone — especially compared to several other aerobic exercises. Take running, for example. When a jogger takes few laps around the track, that jogger is only moving his or her body through air. A swimmer, on the other hand, is propelling himself through water — a substance about 12 times as dense as air. That means that every kick and every arm stroke becomes a resistance exercise — and it’s well known that resistance exercises are the best way to build muscle tone and strength. Improved flexibility Unlike exercise machines in a gym that tend to isolate one body part at a time (like a bicep curl machine, for example), swimming puts the body through a broad range of motion that helps joints and ligaments stay loose and flexible. The arms move in wide arcs, the hips are engaged as the legs scissor through the

Continued from Page 9 water, and the head and spine twist from side to side. Plus, with every stroke, as you reach forward, you’re lengthening the body, which not only makes it more efficient in the water, it also helps give you a good stretch from head to toe. T o improve your flexibility beyond the natural gains you’ll make by swimming, you might also want to fi nish your pool workout with a series of gentle stretches. The support of the water should help you maintain positions involving tricky balance — such as a quadriceps stretch — for longer periods of time. A healthier heart In addition to toning visible muscles like pectorals, triceps and quads, swimming also helps improve the most important muscle in our bodies: the heart. Because swimming is an aerobic exercise, it serves to strengthen the heart, not only helping it to become larger, but making it more efficient in pumping — which leads to better blood flow throughout your body. If that’s not enough to get you moving in the pool, the American Heart Association reports that just 30 minutes of exercise per day, such as swimming, can re-

duce coronary heart disease in women by 30 to 40 percent. Additionally, it is shown that regular aerobic exercise can reduce blood pressure as well. Weight control For some time, some people thought that because water is generally cooler than our body temperatures, it would be difficult to lose weight with a water workout. Like many old ideas about exercise, this has since been revised: Swimming is now recognized as one of the biggest calorie burners around, and it’s great for keeping weight under control. The exact number of calories you burn, of course, depends on your own physiology and the intensity with which you exercise, but as a general rule, for every 10 minutes of swimming: the breast stroke will burn 60 calories; the backstroke torches 80; the freestyle lights up 100; and the butterfly stroke incinerates an impressive 150. To boost the calorie-burning component of swimming, consider using intervals in which you work your hardest for short bursts and then recover. One way to structure this kind of work-

I’m Kodi Roberts. I have been a disc jockey in local radio for the past 12 years. In September of 2010, I weighed 247 pounds. I was using the excuse that I had just had a child and that’s why I was heavy. But my son was five months old and I found I was putting on maternity pants every morning before I would leave the house. I had a difficult time tying my own shoes because of the extra weight. When I heard about the Brenham Fitness Bootcamp, I thought: I’ve found the answer to my problems, I’ll do the Bootcamp and lose all of my weight!” That way of thinking is a prime example of the common misconception that there is a six-week program, a diet, a pill... “a quick fix” that will help you reach your weight loss goals immediately. It takes hard work and dedication, focus and determination. It takes a support system and takes education in nutrition weight management and exercise. To date I’ve lost 93 pounds. No fads, no gimmicks, just hard work and the support and education I received from Brenham Fitness Center. Utilizing the flexible class schedule that Brenham Fitness Center offers free with membership helped keep me accountable, created a support group of other people who had similar goals and kept me on track. I am now a certified kettlebell instructor and very active as a Crossfit level 1 instructor. I also coach CrossFit Kids at Crossfit Red Hornet (located in the strip center BFC-Market Street location). CrossFit Kids is customized to the ability of each child. All kids, regardless of fitness experience, are encouraged to participate. We also teach proper nutrition. The time to change their lives is NOW, let us help!

Is knee replacement surgery right for you?

What is Balloon Sinuplasty?

JEFFERY SPAW, M.D.

College Station Orthopaedics

Each year, knee pain keeps thousands of active Americans from enjoying the life they wish to lead. For those who have tried various treatment plans without relief, knee replacement surgery has become an increasingly popular solution. According to the American Academy of Orthopedic Surgeons (AAOS), more than 189,000 people over the age of 65 undergo knee replacement surgery every year. If you are experiencing consistent knee pain, replacement surgery may be the right choice for you. Knee replacement offers many benefits for those experiencing long-term pain. During replacement surgery, a surgeon removes damaged bone and cartilage from the thighbone, shinbone and kneecap and replaces it with a prosthesis. The surgery not only relieves pain, but improves mobility in the knee joint. According to the AAOS, more than 95 percent of knee replacement recipients experience pain relief, increased movement and a better quality of life. However, not everyone with knee pain is a good candidate for replacement surgery. Consider knee replacement only if: • You have pain that interferes with daily life. A prime candidate for knee replacement experiences knee pain that makes it difficult to perform daily activities, such as walking, gardening or climbing stairs. • You have limited function and mobility. The knee stiffness makes it difficult to sit and bend the knee. • You have tried other methods to relieve pain. Knee replacement is major surgery, so it should not be considered until other prescribed methods to relieve pain have been exhausted, including weight loss, medication and physical therapy. • You have certain joint disorders. Patients with advanced joint conditions, such as osteoarthritis and rheumatoid arthritis, are good candidates for knee replacement. Osteoarthritis causes the wearing of the cartilage, which allows the surfaces of the bones to rub directly against each other.

Weight Continued from Page 12 • Exercise Regularly. Exercise is a powerful tamer of emotional stress and menopausal symptoms. A fitness program that includes cardio respiratory activity and strength training can help you lose body fat, build confidence, alleviate anxiety and depression, and boost self-esteem. (Work with a personal trainer if you need help developing a program). • Eat Healthfully. Eating healthy meals and snacks at regular intervals will keep your blood sugar levels in check and prevent hunger. However, occasionally indulging in your favorite treats is okay - and may actually prevent binge eating. • Monitor Your Eating. Self-monitoring is an effective way to manage and lose weight. Try keeping an “emotional eating” diary, where you record what you eat and when you eat it each day, along with any feelings you experience at the time. You will become aware of how and when your emotions affect your eating patterns and food choices. • Get Support From Professionals. A counselor or registered dietitian may be able to help you handle any emotional or eating issues that surface.

Balloon Sinuplasty safely and effectively dilates sinus openings, resulting in relief from sinus symptoms.

Dr. Paul Fulmer, a board certified Ear, Nose and Throat physician with 20 years experience, has moved back home to Central Texas, and he has focused his practice to helping patients with chronic nasal and sinus diseases. So if you are suffering from constant nasal obstruction, repeated trips to the doctor for sinus pressure headaches or infections, have taken multiple rounds of antibiotics and/or nasal sprays and nothing seems to help. You may be a candidate for Ballon Sinuplasty. In-office Ballon Sinuplasty is a simple procedure that affords the patient minimal down time. It can be conducted comfortably under local anesthesia in the familiar surroundings of a doctor’s office. Because this procedure does not involve removing tissue or causing any significant trauma, the patient is allowed to return home and then back to their normal activities within a few days.

Step 1 ————————————————— A soft, flexible guidewire is inserted into the blocked sinus.

Step 2 ————————————————— The balloon is advanced over the guidewire and is inflated to gently expand the sinus opening.

Step 3 ————————————————— The balloon is removed, leaving the sinus open.

Swimming out would be to swim 50 yards then rest for 10 seconds, then 100 yards with a 10-second rest, then 150 yards — all the way up to 300 yards with rests in between. When you reach 300 yards, reverse the pattern. Improved asthma symptoms Unlike exercising in the often dry air of the gym, or contending with seasonal allergies or frigid winter air, swimming provides the chance to work out in moist air, which can help reduce exercise-induced asthma symptoms. Not only can pool workouts help you avoid asthma attacks if you’re prone to them, some studies have shown that swimming can actually improve the condition overall. According to a study published in the scholarly journal Science Daily, “Respirology, when a group of kids completed a six-week swimming program, they saw improvements in symptom severity, snoring, mouth-breathing, and hospitalizations and emergency room visits.â€? Even those without asthma can beneďŹ t from swimming as the exercise can increase lung volume and teach proper breathing techniques. Improve cholesterol Being healthy is more about having the

Continued from Page 14 right ratio of cholesterol in your body than just having low amounts of the stuff in your blood. SpeciďŹ cally, it’s beneďŹ cial to have higher levels of “goodâ€? cholesterol (HDL) and lower levels of LDL, or “badâ€? cholesterol. Swimming can get these levels in the right balance thanks to its aerobic power, which has been proven to raise HDL levels. And for every 1 percent increase in HDL cholesterol, the risk of dying from heart disease drops by 3.5 percent. What’s more, studies have shown that aerobic exercises like swimming can also keep your endothelium in good shape. What’s your endothelium, you might wonder? It’s the thin layer of cells that lines your arteries, and it tends to lose exibility as you age. The theory is that because aerobic exercise causes arteries to expand and contract, it keeps them ďŹ t. Low risk of diabetes When it comes to warding off diabetes, there are few prescriptions as powerful as aerobic exercise. In one study, men reduced their risk of diabetes by an average of 6 percent for every 500 calories a week they burned in aerobic exercise. With just 30 minutes of breaststroke

swimming three times per week, you could burn 900 calories — reducing your risk of contracting type 2 diabetes by over 10 percent. A study that focused on women also suggested the same beneďŹ ts for the fairer sex: Vigorous exercise just once a week (like the kind derived from a robust swimming session) lowered their risk of contracting type 2 diabetes by 16 percent over inactive women. According to the American Diabetes Association, diabetics should get 150 minutes per week, spread across at least three days per week, of moderate-intensity physical activity like swimming to aid glycemic control. Lower stress, higher spirits and better brain William Wilson wrote in the 1883 book, “The Swimming Instructorâ€?: “The experienced swimmer, when in the water, may be classed among the happiest of mortals in the happiest of moods, and in the most complete enjoyment of the happiest of exercises.â€? Wilson probably didn’t know this in the 19th century, but all that happiness SWIMMING continued on Page 20

Swimming Continued from Page 19 was likely due to the release of feel-good chemicals known as endorphins — one of swimming’s most pleasant side effects. In addition to a natural high, swimming can also evoke the relaxation response the same way yoga works on the body. This is due in large part to the constant stretching and relaxing of your muscles combined with deep rhythmic breathing. Swimming is also a meditative exercise, with the sound of your own breathing and the splash of the water. You might just live longer If the previous nine reasons weren’t enough to convince you of the health benefits of swimming, perhaps this one will: It can keep you from dying. Actually, we’re not promising Aquaman-like immortality, but it seems that swimming can at least help you avoid dying prematurely. Researchers at the University of South Carolina followed 40,547 men, aged 20 to 90, for 32 years and discovered that those who swam had a 50 percent lower death rate than runners, walkers or men who got no exercise. The study authors concluded that the same benefits would be received by aqua-women as well as men.
