Community Action helps patients navigate breast cancer All too often, the very thought of a diagnosis of “breast cancer” is enough to knock women on their ears. The fear of the diagnosis, and the feeling of helplessness is sometimes too much to bear. Fear keeps women not only from seeking treatment, but from seeking the very diagnostic care that could lead them to life-saving measures. Community Action, Inc., has a way to help. Since 2004, funding provided by the Austin Affiliate of the Susan G. Komen Foundation has allowed Community Action, Inc., to expand their community-based programs to include “navigators:” advocates that provide assistance, support and advice from the first moments of diagnosis until the words “Cancer Free.” Community Action, Inc., has put a Navigation Team in place in Caldwell County with the mission of reducing the amount of time it takes for uninsured or under-insured women to move through the process of breast cancer diagnosis and treatment, beginning with diagnostic mammograms and ending with the recommended treatment. Within 10 days of the initial referral to Community Action, nearly all women are referred to a funding source. Funding is available for everything from diagnostic treatment to transportation assistance, and everything in between. Every step of the way, Navigators are on hand to provide emotional support, advice and treatment recommendations, with the assistance of a team of doctors, nurses, radiologists and other medical providers.
Community Action, Inc. of Central Texas has been an established Texas Breast and Cervical Cancer Services (BCCS) Provider through its multiple clinic sites since 1986. Since 2004, funding from the Austin Affiliate of Susan G. Komen for the Cure has allowed CAI’s community based program to expand
these services to utilize a model proven and recognized to provide effective and comprehensive breast cancer patient navigation throughout the wide continuum of breast health care. Community Action’s Patient Navigation Team guides women through breast diagnostics to reach resolution within 30 days. Women are referred to Community Action at any point throughout this continuum of care. Di-
agnostics include clinical breast exams, diagnostic mammograms and ultrasounds, surgeon consultations and biopsies. Services provided by Navigators include: Transportation to medical appointments, or gas vouchers to help pay for travel; interpretation at medical appointments; financial assistance for rent and bills, if available or referrals to established community assistance programs; referrals to cancer organizations; appointment scheduling and follow up to missed appointments; education regarding treatment; and childcare assistance during treatment. In addition, Navigators help breast cancer patients find the words to communicate with their families and caregivers regarding their needs, their fears and their diagnosis. In many cases, this part of the Navigation process is the most valuable – as mysterious as a cancer diagnosis is for a patient, the family often has even more questions and concerns. After the initial diagnosis, Navigators stand beside their patients and their families throughout the process of treatment, including surgeries, chemotherapy and radiation, and aftercare. Working in conjunction with Community Action, Inc., Lockhart Community Health Services provides access to Navigators, as well as other services for early detection and treatment of breast cancer. Groups, therapy available for substance abuse In Caldwell County, as in places throughout the United States, substance abuse and addiction is a problem that residents face. Too often, those battling an addiction don’t know where to turn. Under the blanket of the Texas Department of State Health Services, Caldwell County residents have access to the Outreach, Screening, Assessment and Referral (OSAR) program. This free program, funded by DSHS, provides confidential alcohol and drug screenings, and assessments, referrals for state-funded inpatient and outpatient drug and alcohol treatment, brief interventions, which include motivational counseling, education and support, and case management for clients who need assistance in accessing supportive services. All Texas residents who are seeking substance abuse services and information may qualify for OSAR assistance. Many insurance policies cover substance abuse treatment, and help is available through Bluebonnet Trails Community Services for those who need further assistance. After the assessment is complete, patients may have access to a variety of recovery programs, including inpatient and outpatient programs throughout Central Texas. For adolescents, the Bluebonnet Trails Recovery program serves is available for those between the ages of 13 and 17 and their families, who are actively experiencing substance abuse problems. The program goal is to help teens develop and practice “Real Life Skills” that can improve their understanding of the negative impact of their substance use and motivate them to decrease usage.
Additionally, area Outpatient Substance Abuse Treatment Services for adults have also recently become available through Bluebonnet Trails. The outpatient program provides an individualized focus within a program structure. Each client receives highly individualized treatment provided by licensed substance abuse counselors. The Bluebonnet Trails Recovery transitional program offers Intensive, Supportive and Aftercare treatment options, which provides a solid step-down from each level of care to ensure continued support and to promote long-term recovery. Assessments are generally provided at offices in Travis, Williamson, Bell, Brazos, Burnet and Bastrop counties, but arrangements can be made for assessments in other counties, if needed. For those who prefer a group approach to maintaining their sobriety,
there are several options for 12-step meetings available in Lockhart. Each evening, from Monday – Sunday, a AA group meets at 906 Mimosa, in Lockhart. Additionally, a separate AA group meets at 1011B W. San Antonio, providing a variety of open and closed meetings. The Conscious Contact Narcotics Anonymous Group meets at Emmanuel Episcopal Church, 118 N. CONTINUED FROM PREVIOUS PAGE Health Care program exists separately from the bulk of Seton’s health care mission, as it is state mandated and funded through a combination of state and local funding, rather than by the Seton Family. By that state mandate, according to the Texas Department of State Health Services, counties are required to provide the following health care services, which, in Caldwell County, Seton staff helps patients to navigate: (1) primary and preventative services designed to meet the needs of the community, including: (A) immunizations; (B) medical screening services; and (C) annual physical examinations; (2) inpatient and outpatient hospital services; (3) rural health clinics; (4) laboratory and X-ray services; (5) family planning services; (6) physician services; (7) payment for not more than three prescription drugs a month; and (8) skilled nursing facility services, regardless of the patient’s age. In addition, a series of optional services are available, based on patient need and qualifications. Those services include: A county may elect to provide these optional health care services: (1) ambulatory surgical center services; (2) diabetic and colostomy medical supplies and equipment; (3) durable medical equipment; (4) home and community health care services; (5) social work services; (6) psychological counseling services;
(7) services provided by physician assistants, nurse practitioners, certified nurse midwives, clinical nurse specialists, and certified registered nurse anesthetists; (8) dental care; (9) vision care, including eyeglasses; (10) services provided by federally qualified health centers, as defined by 42 U.S.C. Section 1396d(l)(2)(B); (11) emergency medical services; (12) physical and occupational therapy services; and (13) any other appropriate health care service identified by department rule that may be determined to be cost-effective. To be eligible to participate in Cald-
well County’s Indigent Health Care program, one must, of course, have proof of residence in Caldwell County. Single individuals, as well as families, are eligible for the program based on income guidelines, provided no other qualification for Medicaid exists. Monthly income must not exceed 21 percent of the Federal Poverty Guidelines (see chart below). WIC helps feed families in need The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) is a health and nutrition program with a successful record for improving the diet of infants, children, and pregnant, postpartum and breastfeeding women who are at risk for nutrition-related illness. The main focus of the WIC program is to educate parents/caregivers on the proper nutrition for babies and young children. In addition to nutrition education, WIC clients also receive breastfeeding support and education, supplemental WIC foods, and referrals to other health and social services. To apply for WIC, a mother must schedule an appointment at the Lockhart clinic, located at 1003 W. San Antonio. Caseworkers are available Mondays and Wednesdays
from 8 a.m. – 4:30 p.m., Tuesdays and Thursdays from 8 a.m. – 7 p.m. and on Fridays from 8 a.m. – noon. To apply, required documents include a photo ID, proof of residency, proof of income, TANF, food stamps and Section 8 participation. Case workers can help potential recipients determine what documents are necessary to complete the application process. To be eligible for WIC in Texas, potential recipients must either: 1. Be a pregnant woman; 2. Be a breastfeeding woman with a baby who is under 1 year old; 3. Be a woman who has had a baby within the last six months; or 4. Be a foster parent, step-parent, guardian, or parent of a child who is under the age of 5. As this adult, you can apply for WIC for the child.

Camp HeartSong helps children cope “My grief is like a river, I have to let it flow, but I myself determine just where the banks will go.” Much like the words of this poem by Cynthia Kelley, Camp HeartSong helps children to let go of their grief and find peace and comfort with their loss. Each summer, CTMC Hospice provides a three-day sleep-away camp for children who have suffered a significant death in their life. Children are accepted through an application process, and families are not charged for the camps, held at John Knox Ranch each June. Camp HeartSong, Too!, an adolescent day camp, hosts teenagers aged 14-18. This camp is also a free camp to those who participate. Camp HeartSong provides bereaved children an opportunity to freely ex-
press grief in a safe, healing yet fun environment under the guidance of Abel Rodriguez, LMSW, CTMC Hospice bereavement coordinator and Camp HeartSong co-director, through support groups and expressive arts and crafts. pating in group activities including sports, gaming, ropes course and arts and crafts. Camp HeartSong, Too! is a day camp only. Breakfast and lunch are provided. While attendees enjoy regular camp activities such as swimming, arts and crafts and canoeing, they also get help coping with their grief. They create and decorate their own face mask and learn that it is okay to take the “mask” off and let out their feelings of grief. The campers also create paper boats with messages to their loved one and release them into the river on the last day of camp in a special ceremony called “Letting Go.” This bereavement camp allows the children to share their stories with other kids who are experiencing the same type of loss. “It’s really comforting to know that you’re not alone in this type of situation, especially at such a young age,” said Lisa Adams, Camp Director and Public Relations Coordinator with CTMC Hospice Care. “It’s amazing to see children who have lost their mother, father, sister or brother through illness or a tragic accident hugging and comforting one another. There are no words to describe this precious moment.” Camp HeartSong is completely funded through contributions from the CTMC employee annual Seasons of Sharing campaign. Bluebonnet Trails helps with mental, developmenal disabilities Bluebonnet Trails Community Mental Health Mental Retardation Center (the Center) was formed in 1996 through an interlocal governmental agreement among the six counties of Bastrop, Burnet, Caldwell, Fayette, Lee and Williamson. After wellattended public hearings, and at the request of these counties, the Center was legally established as a community mental health mental retardation center in 1997 by the Texas Department of Mental Health and Mental Retardation. During September 2000, Gonzales and Guadalupe Counties joined the Center establishing the current eight-county service area in which the Center is designated by the State of Texas as the local mental health and mental retardation authority. The Center is governed by a Board of Trustees appointed by the County Judges and Commissioners’ Courts from each of the eight counties. The Trustees work in responsible and accountable cooperation with local and state government and citizens of their counties to ensure the needs of the communities are heard and considered in the strategic planning and development for the Center. The members of the Board of Trustees volunteer their time, experience and talents during regular monthly meetings as well as during Center events and discussions with our community leaders. The Center is fortunate to have dedicated individuals to govern our Center in providing high quality,
efficient services to persons whose lives are seriously affected by mental disorders, substance abuse disorders and developmental disabilities. Today, the Center has been renamed to Bluebonnet Trails Community Services and serves as the local mental health and mental retardation authority in eight counties with a population density of over 696,000 persons and a land mass of approximately 6,910 square miles. Center services are provided to adults with serious mental illness and chemical dependency; to children and adolescents with serious mental illness or emotional disorders, chemical dependency, autism or pervasive developmental disorders; to persons with mental retardation; and to infants and toddlers with developmental delays. In Caldwell County, those services include: Mental Health Services for adults in Caldwell County are available through the Caldwell County Mental Health Center, located at 510 Pierce
