Most people know complications with diabetes are serious.

But do you understand how diabetes relates to circulation and heart disease?

How to Keep Diabetes from Causing Bigger Problems

C

omplications of diabetes are divided into two categories, microvascular and macrovascular. While microvascular complications can lead to blindness, kidney failure, erectile dysfunction and even foot amputation, macrovascular complications can be serious, too.

Macrovascular complications are linked to cardiovascular disease, heart problems such as heart attacks, strokes and poor blood flow. In fact, the U.S. Department of Health and Human Services states that about 65 percent of people with diabetes die from heart attack or stroke.

What can a person with diabetes do to help prevent heart problems? w Maintain a healthy weight. Being overweight increases your risk for heart attack and stroke. w Get at least 30 minutes of exercise daily. Make a habit of taking a daily walk.

It can help lower blood pressure and increase weight loss. w Improve your diet. Eat foods low in saturated fats, trans fats, cholesterol, sodium and excess sugars. w Stay away from tobacco. Smoking is a major risk factor for heart attack and stroke. If you need help quitting, ask your doctor. w Take medications as prescribed. High blood sugar levels can cause damage to your blood vessels. Make sure to take your medications exactly as prescribed, this will help control your blood glucose levels and prevent further complications. w Get support. Ask your friends and family for help with your diabetes. Their support can help you reach your goals.

Worn Out? The solution could be a better night’s sleep.

S

ometimes, getting some shut eye can be harder than it seems, leaving you frustrated and watching the minutes and hours tick by on your clock.

Instead of counting sheep, try these tips for improving your sleep. By adopting these habits, a good night’s sleep won’t seem like such a pipedream.

SET A SCHEDULE Just like when you were a kid, set a bedtime for yourself every night. Try to get up at the same time every day, too.

Reinforcing your sleep schedule, even on weekends, will train your body and promote a better night’s sleep.

CREATE A ROUTINE Pick an activity to do before bed every night. This could be reading a book or taking a shower. By doing the same activity every night, your body will know it’s time to wind down.

GET ACTIVE Regular exercise can help you fall asleep faster and reach a deeper sleep. Just don’t exercise too close to bedtime; this can leave you too energized to fall asleep.

AVOID NAPS Long naps can really interfere with your nighttime sleep.

If you need to nap during the day, keep it between 10-30 minutes and in the middle of the day.

WATCH YOUR DIET BEFORE BED Discomfort can keep you up at night, so avoid going to bed hungry or completely stuffed. Also, avoid drinking before bed as nighttime trips to the bathroom can leave you wide awake.

Obviously, caffeine can keep you up at night, but so can alcohol. While alcohol can make you initially fall asleep, it can be disruptive to your sleep later in the night.

MANAGE STRESS When you have too much on your mind, your sleep can suffer. No one wants to lie in bed with his or her mind racing. If you feel like you have a lot on your mind, jot down your thoughts before bed and you can reassess them in the morning.

Do you want to receive the right health care in the most cost-effective way? These steps will lead to better coordination of care.

Let your primary doctor (often called your primary care provider, or PCP for short) know about any changes to your treatment. Your doctor needs to know if you have seen a specialist or started a new medication.

Contact your primary doctor after a visit to the emergency room.

If you are receiving behavioral health care services, make sure to give written consent for your behavioral health provider to share relevant health information with your doctor.

Network Health’s Quality Improvement Department ensures the accuracy of the data gathered and reviewed by Network Health. To make sure we gather and review the most accurate data about you, please let your health care concierge know if you change your doctor. hy is it that even the chattiest among us get so quiet in the doctor’s office? Open communication with your doctor is essential for good health. Your doctor needs to know what you are experiencing and how you’re feeling to be able to provide effective care.

So, don’t just sit there when your doctor asks if you have questions.

Come prepared. At your annual doctor visit, write down and bring your questions. Not sure what to ask? Start with the following.

• How can I maintain or improve my physical activity?

• Is it helpful to monitor my activity and what is the best way to do it?

• What steps do I need to take to maintain or improve my mental health?

• How can I protect my bone health and reduce the risk of falling?

To get the best care possible, make sure to keep track of any health concerns you have and talk to your doctor about them.

obody likes talking about urinary incontinence, not even with their doctor. But, according to the National Association for Continence (NAFC), over 25 million Americans are affected by a loss of bladder control. It is a common health problem, and it is treatable.

It may be uncomfortable to discuss, but your doctor needs to know if you are experiencing discomfort. Many people affected by urinary incontinence can be cured or notice an improvement with treatment.

It’s important to seek treatment, because urinary incontinence can be a symptom or sign of another health problem. In other cases, sometimes simple dietary changes or exercises can help stop urinary incontinence.

o you often feel as though you’re struggling for breath?

Or do you have trouble with a persistent cough? You may want to talk to your doctor about chronic obstructive pulmonary disease, or COPD. What is COPD? It’s a condition that affects your lungs and restricts airways.

People who suffer from COPD may have airways that are swollen, blocked and mucus production can increase.

How can I manage my COPD? Once you’ve been diagnosed, it’s more important than ever to take steps to improve your health. Read the following helpful management techniques.

Trying to Quit? One of the best things you can do for your lung health is to stop smoking. Network Health covers tobacco cessation counseling sessions twice per calendar year. To learn more, call your health care concierge.

Get a spirometry test (also called a pulmonary function test or PFT).

This common and painless test can show you how well your lungs are working. It’s important to understand the condition of your lungs so you and your doctor can determine the best course of care.

Prevent infection.

Flu and pneumonia are the top causes of breathing difficulty and hospitalization for people with NetworkHealthMedicare.com

COPD. Get vaccinated to help reduce your risk of infection.

Take your medications.

Make sure you understand your doctor’s medication instructions.

Closely following your treatment plan can help prevent COPD flare-ups (or a worsening of symptoms).

Avoid your triggers.

Know what makes your COPD symptoms worse. Common factors

include cold weather, strong fumes and smoking or secondhand smoke.

Consider keeping a journal of your flare-ups to help you identify your triggers and how to avoid them.

Develop a COPD action plan.

It’s not always possible to avoid a COPD flare-up. But, you can identify the early warning signs and take action to keep breathing difficulty from becoming an emergency. Talk to your doctor about developing a personal plan.

Keeping active and involved helps Lois and Bob Pflum live well

ois Pflum has always had a passion for others. If you were to ask her what her hobbies are, she’d tell you “reading, writing and people.” Which makes sense, when you look at how she is spending her retirement—volunteering and freelance writing.

Lois and her husband Bob both retired in 1995. As they got older, they realized how important it was to stay healthy, and being active was a big part of that. Lois decided to pursue her interests and began freelance writing.

She’s been published in area magazines and newspapers and enjoys writing the monthly cover story for Fond Du Lac County, a Gannett publication. She also began volunteering more for many organizations, including the Network Health Medicare Ambassadors.

Although retirement is supposed to be a time to slow down, Lois and Bob believe in keeping their calendars full. “We know how important it is to keep our brain and our bodies active.” So, it’s no wonder that when she was diagnosed with a health condition, she wasted no time in figuring out the best way to manage it.

Last year, Lois was struggling with stomach pain. She was experiencing “awful, awful cramping.” She quickly made an appointment with her doctor who initially

believed it was an infection. Lois was hopeful that after a round of antibiotics, her health problem would clear up.

Unfortunately, it wasn’t quite that simple.

Lois found out her stomach pain was caused by a condition called diverticulitis. Diverticulitis occurs when small pouches (called diverticula) develop along the walls of the intestines and become inflamed. Some people may not even be aware they have the condition, yet others experience sharp cramping. Unfortunately for Lois, she was in the second group.

As you get older, you’re increasingly likely to develop a chronic condition. According to the Centers for Disease Control and Prevention (CDC), 80 percent of older Americans have a chronic condition and about 50 percent have two. That’s why it’s important to do what you can to manage conditions through education and lifestyle choices.

Lois decided to attend a Living Well with Chronic Conditions Workshop put on by the Aging and Disability Resource Center in Fond Du Lac. “At first, I wasn’t even Nancy in her sure I belonged there.” She explained she felt a little out of place, because others in the class were learning to manage conditions that were more severe, such as cancer and heart disease. But, the instructor assured her she would benefit

from the helpful techniques taught in class.

The workshop was a six-week course that met once a week for two and a half hours. It taught Lois practical ways to deal with pain and fatigue. It also gave her tips for better nutrition, weight management and how to communicate with doctors. Lois was glad she stayed, “I picked up so many great hints. It was well worth the time.”

By working with her doctor and actively managing her condition,

Lois was able to stop the concern from becoming a larger problem.

Unfortunately, that’s not always the case for everyone. It can be hard to deal with ongoing health problems, as many as one in three people with chronic conditions will also struggle with depression. But, Lois took a different approach. By keeping active, through her writing, clubs, exercise and volunteer work she kept her spirits high. She says simply, “It didn’t cramp my lifestyle.”

Designed specifically for anyone with a chronic condition, such as arthritis, high blood pressure, heart disease, chronic pain or anxiety, this workshop teaches self-management skills and develops a step-by-step plan to achieve specific goals. The six-week class meets once a week for two and a half hours.

Bob and Lois celebrated their 55th wedding anniversary on February 21, 2014. They joke the reason their marriage has lasted is because they both call each other “the boss.” And, Lois also refers to Bob as her best friend. Together they have two sons, five grandchildren and eight great grandchildren.

ormerly, a physician’s job description was simple: cure sometimes, relieve often, comfort always. Before anesthesia and antibiotics, there wasn’t much a doctor could do.

According to the Education for Physicians on End-of-Life Care course, in 1900, the average life expectancy was just 47 years. 90 percent of people died suddenly.

Things have changed. Average life expectancy is now almost 80 years and only 10 percent of people die suddenly. 90 percent die of one or more chronic illnesses.

Doctors have many treatment choices to offer. All agree what should be done is what the patient would choose. Unfortunately, more than 50 percent of the time when people age 65 or older come into the hospital, they are unable to make their own health care decisions, as cited in The Journal of the American Medical Association Internal Medicine.

What is the doctor to do? What would the patient have wanted? Who should decide?

To answer this dilemma, Congress passed the Patient Self-Determination Act in 1991. All hospitals are instructed to ask if patients have an advance directive. If patients have this document, it must be on the hospital chart and its wishes must be honored.
