Australian Gastroenterology Week (AGW) Virtual 4 - 12 September

ENDO-AID uses a sophisticated algorithm via a neural network developed and trained by Olympus to alert endoscopist in real time with an intuitive marker on the screen when a suspected colonic lesion is detected. Among endoscopists in the same health care system and shared patient population, ADR varies from 7% to 53%1,2.

This year AGW was held virtually across 2 weekends in September and despite the ongoing challenges in Australia it was a successful and informative online event. Olympus were delighted to have Professor Krish Ragunath and Dr Sneha John facilitate the Olympus sponsored AGWebinar to present on Endoscopic Imaging in 2021: a technology update. Professor Ragunath and Dr John introduced the latest EVIS X1 technologies and discussed how they have impacted their clinical practice.

Below meta-analysis and pooled analysis of AI CADe in colorectal polyps detection studies in general, five randomised trials shown positive results with pooled adenoma detection rate (ADR) increased from 25% - 37%3.

1. One new face joined the committee for the 2020 - 2021 year.

Jessica Southall from Christchurch DHB. Jessica is a Hepatology specialist nurse and is a representative for the Hepatology Specialty Sub group of NZgNC.

3. The annual combined conference in November is to be a hybrid academic programme for both nurses and consultants. This will be beamed out from the Town Hall in Christchurch around the country. Groups are encouraged to register in their home town as well as individuals if you are unable to attend. The conference organisers can provide a group deal if you contact early. If registered, participants can view the meeting a week after the event.

2. Julia Anderson continues on as our NZNO, Professional Nurse Advisor (PNA) and is a valuable resource guiding the committee through the NZNO policies.

3. Committee member and Inflammatory Bowel Disease (IBD) sub-group member Marian O’Connor is on the GENCA Board which means we have presence in Australia. The GENCA board introduced a new membership category last year for nonAustralian residents which has been taken up by many here in New Zealand. The GENCA IBD Foundation school are holding another course in 2022 which is open to the New Zealand IBD nurses or any interested in the specialty. Watch out for the advertisement.

4. With COVID having interrupted our lives in such a dramatic way it was decided to work on a virtual/hybrid meeting once again to bring some normality into our lives. The conference team are working hard to make this another conference to remember.

Communication 1. I am still conscious of the work load and stress levels we in our specialty are dealing with on a day to day basis. COVID has once again created a time of uncertainty for our members and their families. Especially in Auckland. I am attempting to continue the line of communication with emails but am conscious of the fact that I may not be reaching everybody. Please be kind to your colleagues and reach out if you need help.

4. COVID managed to stay away until we had our February and August Face to Face meetings. We scraped in with our Leaders and Managers study day in Wellington before complete lockdown. Zoom meetings are still the norm for our committee and I must admit it was great to catch up with everybody in person the two times we were able to.

2. We have a varied communication plan amongst members which may not suit everyone and this includes the NZNO managed webpage, membership email list, college moderated Facebook group page to widen the information sharing and the Tube quarterly journal. The Tube is now delivered electronically.

5. As a committee we have been updating the policies and documents which have been put aside. As they are updated we are placing them on our website.

6. The IBD Nurses sub group is still active in completing submissions to Pharmac on the many medications required for their specialty.

Membership

7. As chair I am on the committee of NZ Society of Gastroenterology (NZSG) and Endoscopy Guidance Group NZ. (EGGNZ) With gastroenterology moving forward at a fast pace it is important that the college has a voice on these committees.

1. Membership continues to increase as persons joining the specialty become aware of our existence.

My time as chair of the College ends at conference in November and Merrilee Williams has kindly agreed to take on the role of chair.

Forming sub specialties and supporting the Nurse Endoscopists in training has been some of the many milestones that the college can be proud of.

8. The NZSG is active in setting up training for endoscopists providing sponsorship for young investigators and submitting to pharmac on medications required for many gastroenterology conditions. The committee is very supportive of the Nurses College and proof of this is the financial support shown with the annual conference.

I have indicated to my work that I would like to retire end of April 2022 as feel it is now time for me to move aside and let new ideas flourish. I have every confidence that our specialty is in good hands throughout New Zealand.

9. EGGNZ has completed the “Guidelines for Local Credentialing in Adult Endoscopy” and is now working on the PS09 Guideline on procedural sedation.

I look back on my career with fond memories of those who have mentored me and those I have mentored in this ever changing specialty of Gastroenterology. Some sadly have passed away but they left a legacy for many of us. Technology has always fascinated and excited me and I can only envisage many new things happening in the future. Who would have thought 40 years ago that we would be having Artificial Intelligence (AI) in our procedure rooms!!

Education 1. The College is actively seeking to support nurses into specialist practice and has expert contacts for those who wish to progress their career in gastroenterology. There is education funding to encourage growth of gastroenterology knowledge and the grants have been increased to reflect increased costs. March and September are the application dates for those who wish to apply.

National action plan for Hep C launched Nancy Carey | Midland Region Hep C Nurse Coordinator World Hepatitis day came to Hamilton this year with the Launch of Mahere Mahi mo te Ate Kaka C- The National Hepatitis C Action Plan for Aotearoa New Zealand.

The Launch was based at Anglesea Community Pharmacy in central Hamilton with around 60 invited guests and attended

by Associate Minister, the Honourable Dr Ayesha Verrall and the Director General of Health Dr Ashley Bloomfield. Also welcoming the new plan was Professor Ed Gane who has been instrumental in fighting the cause for hepatitis C treatment in NZ and Dr Frank Weilert, Hepatologist and clinical advisor for the Te Manawa Taki service.

The day was a celebration of the work already started around hep C elimination with enthusiasm for a new structured work plan going forward.

A new mobile hep C service will soon be available in the Te Manawa Taki Region. Working in collaboration with other services, the mobile service will provide an opportunity to reach people in the community, especially remote areas. The service will offer basic health checks where necessary and it is hoped a partnership with other health providers will enhance the service to improve access and equity.

Work is underway on the implementation of the plan, the first phase covering the next 5 years and then moving forward to reach the elimination timeline goal of 2030.

Introduction

is active and is rapidly replicating, hepatitis B e-antigen (HBeAg) protein is secreted into the bloodstream (Liu et al, 2016). Her HBeAg is positive and she has been in immune-tolerant phase for at least ten years.

Hepatitis B is a silent disease; it often does not present any significant disturbances or disabilities until the condition is advanced; often asymptomatic or mild symptoms.

Physical exam and diagnostics

There is no medical intervention to cure chronic hepatitis B (CHB) (Tu et al, 2018); it increases the risks of developing liver cirrhosis, hepatic failure, and hepatocellular carcinoma (HCC).

People living with CHB are recommended to engage in lifelong liver surveillance (bpac, 2018) to monitor disease progression and provide timely treatment to reduce the risk of HCC and the development of cirrhosis (Terrault et al, 2018).

On examination, her vital signs are within normal parameters, minimal voluntary guarding. Normal JACCOL assessment.

Yan’s presenting complaints are typical signs of cholelithiasis with biliary colic pain (Tanaja et al, 2020). Yan meets the risk profile of high-risk groups being a female, age between 30 to 75 (bpac, 2014), use of oral contraceptives, her recent dietary alteration (overall reduction of intake and dietary cholesterol), along with chronic liver disease with HBeAg positive could be contributing factors to developing gallstone disease (Li et al, 2016).

The role of community hepatitis nurse is expanding to meet the high demand from higher level of screening for positive CHB and the aging population increasing in the risk of developing hepatitis B related liver complications. Engaging with patients and developing a strong rapport is essential in a service that provides lifelong monitoring, patients are empowered to contact community nurses for advice when experiencing gastro symptoms that maybe related to hepatitis flare.

Her high viremia CHB in the past ten years with her alanine transaminase (ALT) gradually elevated; in the context of her previous blood test pattern in relations to her presenting symptoms, it is a possible indication of hepatitis flare related to HBeAg seroconversion or transitioning from immunetolerance CHB to becoming immune active CHB. Viral flare can be asymptomatic; however, fatigue, loss of appetite, weight loss, nausea and vomiting can be present (Li et al, 2016).

In the postgraduate course advanced assessment and clinical reasoning; I have learnt to utilize various structural frameworks to begin differential diagnostic assessment to explore the cause of presenting complaints. Yan is a pseudonym; she has consented to use her event for the case study with omission or alteration to any specific information that may identify her.

In the context of this case, HBV DNA Quantitation combining liver function test provides a high prediction of viral flare with a “sensitivity and a specificity both of 86%” (Chang & Liaw, 2014).

Case study Health history collection using the OLDCART framework:

CHB is a dynamic disease; as the presentation of the complications of CHB is vastly different from one person to another, additionally the transition between inactive to active phases of the disease can be unpredictable (Terrault et al, 2018).

Yan is a 40-year-old Chinese female, afebrile, she was having moderate intermittent right upper quadrant (RUQ) abdominal pain over the past two months. Pain has worsened over time, aggravates after eating, and resolves spontaneously within 2 hours at its longest duration. Two weeks ago, she began having nausea and vomiting with epigastric pain. She feels fatigued, and her weight is down from 68kg to 65kg. She has also noticed her urine colour darkened but no obvious haematuria and dysuria.

According to the international guidelines for CHB infection, normal ALT for healthy females is ranged between 19-25U/L. This value is lower than the normal enzyme reference range in New Zealand local laboratory for ALT of normal range <40 U/L. The fundamental determinant of the threshold for treatment is based on changes in ALT, with values more or equal to two times upper limit of normal (ULN) (Terrault et al, 2018).

Yan has presented with classic symptoms of gallstone disease; she also is living with CHB infection, which a hepatitis flare could also have similar presentations. When the hepatitis B virus (HBV) 8

If Alanine Aminotransferase (ALT) results, gamma-glutamyl trans peptidase (GGT), aspartate aminotransferase (AST), and HBV DNA are elevated, it could rule in hepatitis flare (ESAL, 2017).

However, if the hypothesis of gallstone disease is also present, the ALT and GGT could also be raised. However, in a pure hepatitis flare, alkaline phosphatase (ALP) should not rise, which could rule in cholelithiasis (bpac, 2014).

The gold standard for diagnosing a liver injury and cholelithiasis is via ultrasound of the abdomen; it is an inexpensive, high specificity, and non-invasive procedure (Tazuma et al, 2016). It can identify any calcification and free-fluid in the abdomen.

The degree of elevated ALT, AST, and GGT with a high level of HBV DNA cannot rule out hepatitis flare. In Yan’s condition, the acute phase of cholelithiasis can obscure the degree of hepatitis flare. A further blood test will be required to evaluate whether hepatitis flare has occurred or is continuing, and whether HBeAg seroconversion has been achieved.

Conclusion Yan’s acute presentation occurred in a close time frame of a likely episode of a hepatitis flare, in which some of the symptoms overlapped, making differential diagnosis more difficult.

It highlights the importance of having a good knowledge base of medical understanding, a thorough history taking, and physical examination skills to begin the differentiation process, and bearing in mind that more than one disorder could be happening simultaneously. This case study is not a straightforward investigation but a real-life example of more than one diagnosis co-exists.

Leaders and Managers Day – 12th August Committee Report Our yearly get together at the Leaders and Managers Day this year was focused on wellness and selfcare. It has been a tough year for everyone and the committee wanted to pay particular attention to caring for the leaders so that they in turn can better help and support their teams.

The Rongoa Collective gave us an inspirational talk about Te reo Māori perspective of holistic health and wellbeing, and I certainly learnt a lot about the history of Māori healing and the techniques used. We tried some tea made from Hebe to clear our vessels, as well as a spray aimed to induce a calmer state (a must in any endoscopy department at the moment).

One inspiring talk was given by Karen Kempin, who has moved into a new role as a nurse endoscopist. She showcased her journey in gastroenterology nursing applying the Knowledge and Skills Framework to each step of her career. This tied in nicely with a brain storming session about what education can be provided that is specific to gastroenterology; and whether this takes the shape of postgrad study or a modular format that can be transferred between endoscopy units in NZ (similar to the DOPs). This discussion came about after the overwhelming response from the recent questionnaire asking for education in some format. Thank you to all that completed the questionnaire.

For me the most beneficial part of the day were the talks about wellness. Karyn Chalk had been experiencing wakefulness at night and increased anxiety. This meant she found herself unable to focus on her workload, and struggled to prioritise her day: an overwhelming feeling that I am sure most of us have experienced at some point in our lives. Karyn decided to make a change and undertook some research into havening techniques. Havening, is an alternative therapy developed by brothers Ronald and Steve Ruden; it relies on “amygdala depotentiation” that purportedly can help people with psychological problems, particularly those related to phobias, post-traumatic stress and anxiety. Karyn showed us a few techniques, and these certainly made you feel calmer as a result. The other talk by Claire Pearce, who works for RAISE (previously EAP) gave insightful techniques to help reduce stress levels. These included; going for a walk, engaging with friends, and enjoying laughter every day.

The Lifetime team spoke to us about planning for retirement, which for some was a real eye opener! Better start saving! Overall, it was a very interesting day and an excellent opportunity to network with other leaders in gastroenterology; a valued experience for all new leaders coming through.

EDUCATION NZ IBD Nurses Meeting 2022 – Planned for 18th March 2022, Copthorne Hotel, Wellington

The full results which included data from Australia is currently being collated for publication in the GENCA Journal (late 2021).

Marian is working with Lydia White, (ex-committee NZIBDNG member) in completing this paper for publication at this present time.

The committee have been working hard on the 2nd NZIBDNG annual meeting which has been booked for 18th March 2022, in the Copthorne hotel in Wellington. This meeting will be limited to the membership of the group, which is specialist of clinical nurse specialists working within inflammatory bowel disease from across the country, which includes 25 nurses.

The agenda has been generated with local nursing colleagues, Lisa Griffiths and Megan Mackay, and the committee is grateful to their support and acquisition of local speakers.

The committee has been focused also on ensuring that this meeting is fully funded to its membership. To this end, our application for an education grant to NZgNC has been approved which provides funds of $5,000 (July 2021). In addition, we have submitted separate educational grants applications to three pharmaceutical companies which are in process at present. If some of these grant applications are not successful, then the committee may look at a small registration fee per nurse attending to support some of our costs. Membership will be advised of this in due course.

NZ IBD NURSE KSF

We are also grateful to Christine Ho, Sarah Cook and Lucy Mills whom have agreed to review the KSF document which is due for its 2-yearly review. The committee has approached NZgNC and put forward for a proposal to support costs for this small group to meet face-to-face once in order to complete this work. Karen Murdoch is currently linking with this group also.

GENCA IBD Nursing Foundation School a hit!

Within New Zealand there are currently no official nursing Inflammatory Bowel Disease (IBD) education papers or courses.

Most of the education for the IBD nurses around the country comes from on the job training either from the consultants they work with, other IBD nurses already in the service or from Pharma sponsored meetings. Whilst this has been acceptable to a point, it does not provide a good foundation for IBD nurses with a basic knowledge of IBD, and what is required for ongoing monitoring, assessment and treatment. An NZ IBD Nursing Audit (pending publication) also confirmed that more than 80% of nurses in New Zealand had no access to specific IBD education. This is despite the continuing increase in the number of people with IBD in New Zealand (BPAC, 2021).

The course was compromised of 9 different modules including: The Role of the IBD nurse, Anatomy, Physiology and Pathophysiology, Assessment Tools in IBD, 5-ASA and Corticosteroids, Immunomodulators in IBD, Biologics in IBD, Preventative Care in IBD, Blood, Stool and Opportunistic Infection Screening and The IBD Helpline. Each module had a small quiz following the module, and at the end of the course each participant was required to complete a case study to obtain their certificate of attendance.

The IBD nurses around the country have wished for a formal IBD course to provide a foundation for their knowledge and to provide a formal certificate to show they are “qualified” to do their role.

The response from NZ nurses was excellent with 15 of the 100 participants hailing from New Zealand. A few people who attended the course were able to provide some feedback, here are a few snippets from their feedback:

The NZ IBD Nurse Committee has made formal links with the Gastroenterological Nurses College of Australia (GENCA) which provides the IBD Nursing Foundation School and IBD Advanced Nursing School courses to Australian nurses, either working within the IBD field or with patients with IBD in a ward or infusion setting.

This formal arrangement between both groups has allowed for a reduced fee for non-Australian membership requests, this was negotiated between Marian O’Connor (CNS Crohn’s & Colitis, Taranaki) who sits on the GENCA board since 2019.

I think this course gave us important skills and will help assessing our IBD patients better, using the various scores. It will also help understanding the implications of these scores to their care and monitoring needs. Irit – MacMurray Centre, Auckland As I am part of the IBD Nurses Australia (IBDNA) Education sub-committee I was tasked with teaching and marking the assignments on this course as part of the foundation schools planning. I enjoyed teaching on the course, I presented the section on Biologics in IBD and the monitoring of patients on Biologics. This was great for my knowledge as teaching others consolidates our own knowledge (Jarrett, 2018). As there are only two biologics available in New Zealand, but five in Australia, I had to research the three other biologics in depth, and this was

useful as I’m hoping these drugs may be available in New Zealand in the near future. I will then have the confidence to educate my patients about these drugs if they do become available, and I will understand what these drugs do in more depth. Australian practice is slightly different to New Zealand practice, so I had to find the right balance in teaching nurses from both countries to ensure both parties understood my presentation, and could infer meaning as required for their own practice.

It is great that this opportunity has opened up with GENCA to provide education in IBD to New Zealand nurses, particularly as we have had some new nurse’s start within the IBD group, but it is also great for those who have been in the role for a while and just want a refresher for the fundamentals in IBD. Hopefully there will be further IBD Nurse Foundation Schools as well as the IBD Nurse Advanced School, which I think will provide further education for the more complex situations we often find ourselves in within IBD. The NZIBDNG aim to continue our formal links with GENCA, with both myself and Marian O’Connor, in our current roles focused on maintaining these links, moving forward. I look forward to continuing my time on the IBDNA Education Sub-Committee and the NZ IBD Nurse Committee as the Education Lead and hopefully finding myself part of more great opportunities like these.
