Australian Gastroenterology Week (AGW) Virtual 4 - 12 September

ENDO-AID uses a sophisticated algorithm via a neural network developed and trained by Olympus to alert endoscopist in real time with an intuitive marker on the screen when a suspected colonic lesion is detected. Among endoscopists in the same health care system and shared patient population, ADR varies from 7% to 53%1,2.

This year AGW was held virtually across 2 weekends in September and despite the ongoing challenges in Australia it was a successful and informative online event. Olympus were delighted to have Professor Krish Ragunath and Dr Sneha John facilitate the Olympus sponsored AGWebinar to present on Endoscopic Imaging in 2021: a technology update. Professor Ragunath and Dr John introduced the latest EVIS X1 technologies and discussed how they have impacted their clinical practice.

Below meta-analysis and pooled analysis of AI CADe in colorectal polyps detection studies in general, five randomised trials shown positive results with pooled adenoma detection rate (ADR) increased from 25% - 37%3.

NZNO NZGNC Annual Chairperson Report 2022 Welcome to April 2022 As we plough on in to 2022, it is showing signs of testing our resilience and team spirit while we see what impact covid brings to our shores again. It is important in these times to show support for one another, keep physically and emotionally healthy, and we will be through it again soon.

Firstly, I would like to thank the outgoing committee members for their time and energy over the past term. Outgoing chairperson, Karen Clarke, has been so generous in sharing her vast experience. Karen is planning to retire this year, after a long and illustrious career, she has seen huge advances over her time, and contributed to many innovations in practice. I’m sure you will join me in wishing her well in her retirement. Wanda Owen finished her second term on the committee, thank you Wanda.

Our focus for 2021-2024 will be around supporting the education of our membership. We did a lot of brainstorming around this last year in order to plan and facilitate this over the coming years. We look forward to sharing developments with you as they come together.

I would like to take the time to introduce you to your 2022 NZgNC committee members, and also thank them for pledging their time to represent, plan and work towards our goals for our Gastroenterology Nursing specialty.

On that note, I hope you enjoy reading your February edition of the Tube. Thank you to the editors, and also our sponsors for their support.

Jessica Southall – Clinical Nurse Specialist for Hepatology in Christchurch. Jess is completing postgraduate study this year to continue on her Masters pathway.

Karen Kempin – Nurse Endoscopist- Hutt Valley DHB, Karen has completed her Nurse Practitioner study, and went before the Nursing Council last week. Congratulations Karen.

Kiran Joseph – Gastroenterology Nurse in Dunedin HospitalKiran is new to the committee, and we look forward to having her help in setting and achieving our goals this term.

Julia Anderson – Professional Nursing Advisor- NZNO- We are very lucky to have Julia continue to advise us in our planning for the college, we have had the support of Julia for a number of years now for which we are grateful.

The Tube April 2022 Editor’s Report Apologies for the late production of the Tube.

COVID under the new name of Omicron has once again moved into our lives creating many more uncertainties than the previous wave of Delta. We are not only now dealing with general living expenses at home but going to work and finding equipment or consumables required to complete our job are on back order with no date of arrival.

Staffing is becoming an issue in the endoscopy suite with stand downs due to sickness or close contacts of Omicron recipients. Rapid Antigen Testing is becoming the new norm along with N95 or surgical masks.

The specialty groups (IBD and Hepatology) are continuing to involve those who are in isolated situations around New Zealand. The IBD committee has linked with their Australian counterparts to promote education. A report on the Hepatology specialty activities is formatted in the journal.

Croft Print, Christchurch has continued to format a great E journal for the college. This is only possible with the posters, articles and reports members supply to me. I am waiting for articles on unit history which is a great way to preserve history. Find forgotten photos and talk to those who are close to retirement or retired.

As nurses, we love to read about how things used to be and also how others are managing.

There is an editor’s prize each year for the best article which is presented at Gastroenterology NZ conference so get pen to paper.

Remember that to collect payment from the education fund you need to have your reports in by due date which is given in the approval of application letter.

A multidisciplinary team (MDT) consisting of Gastroenterology consultants, Gastroenterology clinical nurse specialist (CNS), dietitians, a senior lead pharmacist, Liaison Psychiatry, the charge nurse manager (CNM) of the Gastroenterology Ward was established to look at developing a pathway for patients that frequently represent to hospital with chronic nausea and vomiting.

As the number of patients re-presenting was significant, our aim was to try to reduce the incidence of tube blockage, tube dislodgement or migration.

This cohort of patients often require enteral feeding. In the first instance enteral feeding is in the form of a nasogastric (NGT) or nasojejunal (NJT) feeding tube. Time is taken at the point of tube placement to explain the care and use of the feeding tube.

For many this is a new concept and the amount of information required to learn can be quite overwhelming, especially when at time of placement, the patient is unwell and maybe less receptive.

Cost was multifactorial. Not limited to, but including emotional, psychological and financial cost to: • The patient and their family members or carers

• To clinical resources in Emergency and Gastroenterology Departments, including Nurses, Senior Medical Officers, Central sterile services and recovery services • And also to hospital resources including: Bed stay, acute endoscopy, the cost of sedation medication and replacement tubes and not to forget, the ambulance service

An in-house audit revealed over a 14 month period, 40% of patients with NJT’s, re-presented to hospital with avoidable tube issues such as blockage from incorrect flushing or incorrect medication administration.

At our meeting we discussed the best way to address the problem.

It was clear due to repeated re presentation with feeding tube issues, that our existing generic leaflet was inadequate, so we decided to design a more expansive, individualised patient information booklet. With valued help from one of our senior lead pharmacists, we looked at similar information in New Zealand and overseas, medicine datasheets and how to safely crush medications that came only in tablet form.

The result is a patient information booklet, written to safely manage enteral feeding tubes, incorporating advice on the correct administration of medicines via the tube.

The new booklet includes:

• how to care for skin at nares, checking the measurement to nares and signs of tube dislodgement,

• managing and preventing a blocked tube including flushing, how to flush and when to flush, and These incidents with feeding tubes often lead to avoidable interruption of much needed nutritional intake and delayed administration of vital medications.

• how to manage and administer medicines via the tubes

With blockage being one of the main issues, the booklet contains basic advice on flushing the tube, including frequency and technique.

The booklet also serves as a good resource when visiting with health care professionals. We are at the point where the booklet has been reviewed by our District Health Board health literacy group and signed off by our Pharmacy and Therapeutics Committee and is currently waiting to be printed so that we can start handing it out to our patients.

We were aware of this not being “yet another handout” for the patient, so rather than be too wordy and for better understanding, we incorporated pictures and infographics to reinforce the information.

We hope that this booklet will help to reduce the number of unnecessary representations to hospital. We have plans to translate it into other languages and to make a video to enhance the booklet, which will help reinforce the information, and will include some frequently asked questions and answers.

We are looking forward to comparing the hospital presentations over the six months following the booklet distribution to see if the presentation to hospital with feeding tube issues decreases.

We thought that a “Do’s and Don’ts” pages may simplify instructions. There were many “Do’s”! We were aware of the danger of information overload so picked the points that we thought were the most important in the hope that they would be adhered to. On brain storming there were even more “Don’ts”.

We applied the same logic, cutting it down to a more manageable amount of information.

A table was included to personalise the booklet to the individual patient where there is opportunity to add details on the type, make and size of the enteral feeding tube that the patient has in situ. There is a grid for medications to be entered with dose, day, times and method of administration. The table provides a reliable reference for the patient, their family or carers, removing possible anguish from memory loss or confusion, until familiarity with feeding tube use to administer medications is established.

Idea Generation Workshop – Rectal Cancer We are delighted to host an Idea Generation Workshop in collaboration with TROG Cancer Research with Convenors A/Prof Sam Ngan and Dr Deborah Wright.

Gaps in current knowledge or embryonic ideas for future AGITG-led clinical trials in rectal cancer with a focus on neoadjuvant treatment are encouraged for submission.

New Look NZ Hepatology Nurses Group The NZNO Gastroenterology Nurses College Hepatology Nurses Group is proud and pleased to share our new logo.

The koru shape is an acknowledgement of being part of the wider NZ Nursing Group.

Green is the colour of awareness, advocacy, and education concerning liver disease.

The shape in the middle of the bow is the shape of the liver.

The Māori translation is Rōpu meaning group and Tāpuhi meaning nurse, or to tend in sickness or distress.

The group acknowledges it has taken a long time to get established however, it is good to finally have an identity within the Gastroenterology Nurses College.

It is hoped that when we are able to travel and meet without restrictions in the future, members of the Hepatology Nurses Group will be able to review and contribute to these guidelines that will guide our practice as hepatology nursing in Aotearoa New Zealand continues to develop and evolve.

We welcome articles that will be of interest to nurses working in Gastroenterology and related. Our aim is to publish a high quality, professional and educational journal for nurses working within the specialty of Gastroenterology.

Structure of Article for submission

All manuscripts received by the editor will be acknowledged, however, reports, area news or letters to the Editor will not. If you have not received confirmation of receipt within six weeks, please contact the Editor.
