Fill out the form below and follow the instructions to return it as printed on the form. Please note the deadline on the form. 2. A photo of the volunteer must be submitted to Delta Publications (e-mailed photo preferred) in order for the form to be published. 3. There is no charge for any group, organization, or individual to have their volunteer recognized. 4. We have hundreds of volunteers in each of our communities, and some of our organizations have dozens and dozens of members. In order to make this a manageable undertaking, we are limiting the number of volunteers honored to no more than five per organization.
