CONCUSSIONS 101:
What parents
need to know
A

concussion is caused by a direct or indirect blow, trauma or jolt to the head, causing the body and head to accelerate or decelerate, resulting in dysfunction and a chemical imbalance in the brain. Concussions are common in sports, especially in sports involving contact, such as soccer and basketball, or actual collisions, like football and hockey. Children can get a hit, blow or jolt to the head which can cause a concussion and children seem to be more susceptible to concussions than adults. If you suspect your child may have a concussion, immediately remove them from athletic activity and do not let them participate further. Next, seek medical attention quickly. You will need to provide your child’s medical history and his or her cause of injury. It’s important to know there is no single test to diagnose a concussion. A health care provider makes the diagnosis of concussion based on a history of a blow to the head, a report of symptoms, such as those described above, and the absence of signs or symptoms that would indicate a more serious problem, such as bleeding in the head or bruising of the brain. CT and MRI scans can be useful in making sure a child does not have a more serious problem, like bleeding in the head. Patients with concussions have

normal CT and MRI scans. Patients also may be asked to perform tests on a computer (such as CogSport or ImPACT) to help assess how well their brain is working. Rest is ultimately the best way to treat concussions. This includes avoiding all exercise and sport activities until the symptoms are completely gone. Rest also includes “brain rest,” including spending a minimum amount of time on the phone or texting, and may include a reduced homework load for school-age patients (your child’s doctor can write a note asking teachers to reduce the workload while the child recovers). Brain rest also means no gaming or web-surfing. Limited use of social media may be allowed as long as this does not worsen the patient’s symptoms. Rest for a concussion also includes plenty of sleep. Your health care provider will usually recommend or prescribe medicine for the headache, and may also recommend medicine to help with dizziness and nausea, or to help with sleep.

MOST COMMON SPORTS INJURIES By DR. JORGE GOMEZ, Sports Medicine Specialist Texas Children’s Hospital West Campus
U
nfortunately, injuries occur frequently in most sports. It’s important to treat sports injuries correctly in order to prevent further problems. Below is a list of some of the sports injuries we see at Texas Children’s and how they are commonly treated:
Dislocations – shoulder
A dislocated shoulder usually happens when the arm is being held straight out to the side and then forced backward. If the shoulder doesn’t look right, or the normal muscle bulge from the deltoid seems to be missing, this is a sign of a dislocated shoulder. The athlete also usually feels or hears a pop and is in a lot of pain. A dislocated shoulder should be put back in place (reduced) by a health professional (EMT, athletic trainer, nurse, physician, etc.) who has been trained to do this. Because young athletes’ shoulder joints are naturally looser than older athletes,’ they are more prone to these injuries. Most of these athletes can avoid repeat dislocations by strengthening their shoulder muscle through physical therapy.
Anterior cruciate ligament - ACL tears ACL tears are serious knee injuries that can occur with a collision, such as a blow to the outside of the knee from another player, but may also occur when an athlete lands a jump or tries to turn suddenly without any impact. An athlete who has torn the ACL usually feels or hears a pop in the knee and most often are unable to continue their activity. Large swelling of the knee also develops within hours. The athlete usually has a difficult time even walking normally. An athlete who has experienced a popping or tearing feeling in their knee followed by large swelling should see a physician (pediatric orthopedic surgeon or pediatric sports medicine physician) soon. The trained physician can often determine that the ACL
has been torn simply by physical examination. Most young athletes with ACL tears will need to have surgical reconstruction of their ACL in order to return to their sport.
Dehydration
Most athletes who become dehydrated are simply not drinking enough. Other factors that can lead to dehydration include exercising in the heat when they are not accustomed to it, drinking too much caffeine or having a viral illness. Extreme thirst, headache, nausea, abdominal pain and muscle cramping are common signs

Hand or wrist-fractures/sprains/ tendonitis
Fractures of the wrist usually occur with a fall on the outstretched hand. If swelling at the wrist and pain with movement of the wrist occur, there is likely a sprain or fracture. An X-ray is typically needed to tell whether the wrist is broken. Most wrist fractures can be treated in a cast. If the broken bone is crooked, straightening of the bone under anesthesia may be required. In severe cases, surgery may be required to properly align the broken bone. Young athletes can fracture the growth plates at the wrist, which may result in the bone not growing normally even after the fracture has healed.
Turf toe - hyperextension of the big toe
of dehydration. If an athlete suffers from dehydration, they should stop exercising and drink plenty of water. Young athletes may be more prone to dehydration because their thirst drive is not as well developed.
Concussion
Concussion can occur in many sports and usually occurs from a direct blow to the head. However, a concussion can also occur with a rapid turning of the head. Common symptoms and signs include headache, dizziness, blurry vision, appearing confused and slow to answer questions. The main treatment for a concussion is rest, which for the athlete means no athletic activity, plenty of sleep and brain rest, including minimal use of electronics. Research has shown that young athletes may be more susceptible to concussions and may require a longer period of rest for their brains to return to normal
Turf toe usually happens when the athlete pushes off forcefully going forward on a hard surface with the big toe being forced upward. This injury can happen after one such push-off or may happen after repeated push-offs. If turf toe occurs, there may be some swelling at the base of the big toe, but usually there is just pain, especially with moving the toe up. In order for the injury to heal, the athlete has to avoid fast running or pushing off until the swelling and pain go away. Applying ice can be very helpful. A sports physician or podiatrist may also recommend a stiff shoe or a special shoe insert to keep the toe from bending up. Fortunately, young athletes are usually more flexible and perhaps for this reason don’t often get turf toe.
Meniscus tears
The meniscus, which is c-shaped cartilage in the knee that serves as a shock-absorber, is usually torn with a twisting injury of the knee, sometimes after a deep squat. The athlete often feels or hears a pop. Pain when straightening or bending the knee fully, painful popping and swelling are signs of a meniscus tear. This injury often requires surgery. Certain meniscus tears in young people, especially those younger than 15, may simply heal with rest.

While many minor lacerations may be repaired with or without the use of stitches at urgent care, it may be necessary to send some patients with more complicated lacerations to a pediatric emergency center. These tips should help guide parents as they determine which setting is best suited to treat their child’s ailments and injuries. However, if there is ever any doubt, families can always contact their pediatrician for additional decision-making support. In addition, these decisions just got easier for patient families. The recently introduced Texas Children’s Pediatrics ParentAdvice Center app is designed to help families better understand and make decisions about their children’s health. Available for download on iTunes and Google Play, Texas Children’s ParentAdvice Center helps provide parents the support and information they need to make timely decisions about what type of care their child needs in a medical situation – whether that means providing care for minor illnesses at home or finding a Texas Children’s Pediatrics practice or urgent care facility. With this new app, families can look up a child’s symptoms by body area, keyword or alphabetical list. Each symptom will provide the cause, advice to care for the symptoms, when to call 911, go to the emergency center or to call a pediatrician. l

These are called receptive language or listening skills. They should also speak in complete sentences, talk easily without stuttering, sing songs, name objects and use most speech sounds. These are called expressive language or speaking skills. Each child develops in their own way and it is impossible to tell exactly when they will perfect a certain skill. Simple speech delays are sometimes temporary. It’s important to encourage your child to “talk” to you and for you to spend lots of time playing with, reading to and talking with your child. However, sometimes delays may be a warning sign of a more serious problem that could include hearing loss, developmental delay in other areas or even an autism spectrum disorder (ASD). Language delays in early childhood also could be a sign of a learning problem that may not be diagnosed until the school years. Schedule an appointment with your doctor if your preschooler: w Shows no interest in interactive games w Ignores other children w Doesn’t engage in fantasy play w Doesn’t use sentences of more than three words w Doesn’t use “me” and “you” appropriately w Cannot understand two-part commands using prepositions w Doesn’t talk about his or her daily activities and experiences If your doctor tells you to “wait and see” but you feel uneasy about the advice, the CDC recommends you talk with another doctor to get a second opinion. If your child is younger than 3, your child should be referred to ECI (Early Childhood Intervention). ECI is a statewide program for families with children, birth to 3, to help their children reach their potential through developmental services. You may also contact ECI directly at 1-800-628-5115. If your child is 3 years or older, the referral should be to your local public school. You may also contact your local public school directly and ask to speak with someone who can help you have your child evaluated. If you have problems getting help from the school, ask to speak with the principal. If your child is eligible, the school district staff will, with your input, develop an Individual Education Plan (IEP).

Although scheduling a back-to-school physical and vaccines is an important way for parents to prepare their child for this exciting moment, as a pediatrician I know the preparation for your child’s success in school starts long before that first day. Many people believe children learn to read and write in kindergarten or first grade. However, the foundation for these skills is built years before your child enters school. You are your child’s first teacher and can help ensure your child’s ultimate success in school by engaging with them, starting at birth. Children are born ready to learn and they learn best though the everyday experiences with the people they love and trust. The American Academy of Pediatrics recommends including the 5 Rs of Early Education in your family’s daily activities to help build the skills necessary for learning in school: w Read together as a daily, fun, family activity. Include reading as part of a regular bedtime routine. This is a wonderful way to have special time with your child and help them quiet down and settle more easily to sleep. w Rhyme, play, talk, sing and cuddle together often throughout the day. Talk about the things you see in your home to your infant, sing silly songs and play pretend with your toddler. Find colors, letters and numbers as you explore the world around you with your young child. w Build routines for meals, play and sleep, which help children know what to expect and what is expected of them. w Give rewards for everyday successes (especially for effort toward goals like helping), understanding that praise from those closest to a child is a very potent reward. Develop relationships that are nurturing, reciprocal, purposeful and lasting, which are the foundations of healthy early brain and child development. w Once in preschool, your child will be immersed in a world of learning. Having acquired early language skills will help ensure they

A
s a dad to three pre-teen sons (11 year-old triplets), the thought of them dating in the not-so-distant future is slightly terrifying. From a young age, my wife and I have been very clear about our expectations about how they treat their peers (both boys and girls) and about bullying. As they get older, we know there are things we can teach them now to prepare them to have healthy dating relationships in the future. As our boys watch how we interact with each other, we hope they learn how to behave in a healthy intimate relationship. Just as when they were younger and parroted the words they hear, our children learn about relationships from those they see around them. By valuing and respecting each other’s needs, demonstrating equality in decision making, and discussing our disagreements in a respectful, open way, my wife and I strive to model a relationship built on respect. Even though it was only recently that I explicitly talked with them about the “birds and the bees,” we know it is important for us to talk with our sons about healthy dating relationships from an early age. There are many teachable moments we use to discuss teenage dating. Television, movies and music provide many examples of both healthy and unhealthy relationships. Taking time to use these as opportunities to reflect on what they hear and see allows them to explore issues such as self-respect, personal boundaries, healthy dating relationships and recognizing abuse. As a pediatrician focusing on vulnerable children and families, I have seen first-hand the deep and lasting consequences of abuse and violence in relationships. According to the CDC, among adult victims of sexual and physical
violence, 22 percent of women and 15 percent of men first experienced some form of intimate partner violence between the ages of 11 and 17 (CDC Understanding Teen Dating Violence Fact Sheet, 2014). It is important as a parent to educate yourself about dating violence. You can help your teen identify unhealthy behaviors and patterns and provide guidance on healthy relationships. Some warning signs of abuse to watch out for in your teen include isolation from family and friends, erratic changes in mood – anxiety or depression, increased levels of aggression, sudden failing grades, decreased involvement in school or extracurricular activities, and engaging in unhealthy behaviors, like using tobacco, drugs or alcohol. If you suspect your teen might be in an abusive relationship, listen and given them support. Do not isolate your teen by giving ultimatums. (“If you don’t break up with them right away, you’re grounded.”) Help find additional support through your pedi-

a crisis de obesidad en Estados Unidos está incitando una campaña de educación sobre nutrición destinada a ayudar a los niños y a las familias a aprender a comer de modo más saludable y a mantenerse en forma. Con el aumento de la obesidad infantil, es indispensable que los niños aprendan desde pequeños la importancia de la dieta equilibrada y de la actividad física. Para Kimberly y Chris Cunningham la alimentación saludable es una prioridad familiar. Los pacientes con síndrome metabólico tuvieron habilidades matemáticas, habilidades de ortografía, lapsos de atención y flexibilidad mental significativamente inferiores. n traumatismo es la consecuencia de un golpe, contusión o impacto directo o indirecto en la cabeza, que provoca que el cuerpo y la cabeza se aceleren o desaceleren, causando un mal funcionamiento y un desequilibrio químico en el cerebro. Los traumatismos son comunes en los deportes, especialmente en los deportes que incluyen contacto, como por ejemplo el fútbol y el basquetbol, o choques reales, como el fútbol americano y el hockey. Los niños pueden recibir un golpe, choque o contusión en la cabeza que puede causar un traumatismo y los niños parecen ser más susceptibles a los traumatismos que los adultos. amentablemente se producen lesiones con frecuencia en la mayoría de los deportes. Es importante tratar las lesiones de los deportes de forma correcta a fin de prevenir más problemas. A continuación presentamos una lista de algunas lesiones deportivas que atendemos en Texas Children’s y cómo se suelen tratar:
Dislocaciones – hombro
Un hombro dislocado en general se produce cuando el brazo que está extendido al costado del cuerpo es forzado hacia atrás. Si el hombro no luce derecho, o parece faltar la protuberancia normal del músculo del deltoide, esto es una señal de un hombro dislocado. Además el atleta suele sentir u oír un chasquido y tiene mucho dolor. Un hombro dislocado debería ser colocado nuevamente en su lugar (reducción) por un profesional de la salud (EMT, entrenador atlético, enfermera, médico, etc.) que está capacitado para hacerlo. Los atletas jóvenes son más propensos a sufrir estas lesiones porque las articulaciones del hombro están naturalmente más flojas que las de los atletas de más edad. La mayoría de estos atletas puede evitar las luxaciones reiteradas si fortalece la musculatura de los hombros a través de la fisioterapia.
Desgarros del ligamento cruzado anterior LCA
Los desgarros del LCA son lesiones graves de las rodillas que se pueden producir con una colisión, como por ejemplo un golpe en la parte externa de la rodilla dado por otro jugador, pero también se pueden producir cuando un atleta aterriza de un salto o intenta un giro brusco sin ningún impacto. Un atleta que se desgarró el LCA suele sentir u oír un chasquido en la rodilla y la mayoría de las veces no puede seguir con su actividad. También se produce una gran inflamación de la rodilla en horas. El atleta suele tener dificultad incluso para caminar normalmente. Un atleta que sintió el chasquido o la sensación de desgarro en la rodilla seguida de una gran inflamación debería ver a un médico (cirujano ortopédico pediátrico o médico deportólogo pediátrico) con prontitud. El médico capacitado a menudo puede determinar que se desgarró el LCA
simplemente con un examen físico. La mayoría de los atletas jóvenes con desgarros del LCA necesitará una reconstrucción quirúrgica de su LCA para poder volver a practicar su deporte.
Deshidratación
La mayoría de los atletas que se deshidratan sencillamente no están bebiendo lo suficiente. Otros factores que pueden desencadenar en deshidratación incluyen hacer ejercicios en el calor cuando no están acostumbrados, beber demasiada cafeína o tener una enfermedad viral. Sed extrema, jaquecas, náuseas, dolor abdominal y calambres musculares son indicios comunes de deshidratación. Si un atleta sufre de deshidratación, debería dejar de ejercitar y beber mucha agua.
(Fracturas/torceduras/tendonitis) de mano o muñeca
Las fracturas de la muñeca suelen suceder por una caída sobre la mano extendida. Si se producen inflamación de la muñeca y dolor al moverla, es probable que haya una torcedura o fractura. Se suele necesitar una radiografía para saber si la muñeca está quebrada. La mayoría de las fracturas de muñeca se pueden tratar con una escayola (yeso). Si el hueso quebrado se torció, se podría requerir enderezarlo bajo anestesia. En los casos graves, se puede requerir cirugía para alinear el hueso quebrado como corresponde. Los atletas jóvenes pueden fracturarse las placas de crecimiento en la muñeca, lo que puede hacer que el hueso no crezca normalmente incluso después de sanar la fractura.

Traumatismo en la cabeza

El principal tratamiento para un traumatismo es descansar, lo que para el atleta significa nada de actividad atlética, muchas horas de sueño y descanso mental, incluso el uso mínimo de aparatos electrónicos. Las investigaciones han mostrado que los atletas jóvenes pueden ser más susceptibles a los traumatismos y pueden requerir de un periodo más largo de descanso para que sus cerebros regresen al funcionamiento normal.
El dedo del césped suele aparecer cuando el atleta se eleva con fuerza al desplazarse hacia adelante sobre una superficie dura con el dedo gordo del pie forzado hacia arriba. Esta lesión puede producirse después de un despegue de ese tipo o puede producirse después de muchos. Si se produce el dedo del césped, quizá haya algo de inflamación en la base del dedo gordo, pero en general sólo aparece dolor, especialmente cuando se mueve el dedo hacia arriba. Para que la lesión se cure, el atleta debe evitar correr rápido o despegar de esa forma hasta que la inflamación y el dolor desaparezcan. Aplicar hielo puede ser muy útil. Un médico deportólogo o un podólogo también pueden recomendar un calzado rígido o una plantilla especial para evitar que el dedo se doble hacia arriba. Afortunadamente, los atletas jóvenes son en general más flexibles y quizá por este motivo no suelen desarrollar el dedo del césped.
Desgarros de los meniscos
Los meniscos, que son cartílagos con forma de C en la rodilla que sirven como amortiguadores, suelen desgarrarse con una lesión de torsión de la rodilla, a veces después de una sentadilla pronunciada. El atleta suele sentir u oír un chasquido. El dolor cuando se extiende o flexiona la rodilla por completo, chasquidos e inflamación con dolor son indicios de un desgarro de menisco. Esta lesión suele requerir de cirugía. Algunos desgarros de meniscos en jóvenes, particularmente los menores de 15 años, quizá se curen simplemente con descanso.

A
lgunas veces las familias necesitan ir a un centro de emergencias después del horario de atención, los fines de semana o quizá incluso durante el día, cuando se produce algún acontecimiento significativo con su hijo o hija. Aunque el centro de emergencias es el lugar correcto para algunos incidentes o padecimientos, la mayoría de las veces, las dolencias menores pueden y deberían ser tratadas en una instalación de atención urgente pediátrica cuando el consultorio de su médico está cerrado y no se siente cómodo esperando hasta que abra. Una de las inquietudes más comunes en los padres es cuando un niño tiene fiebre. En el caso de bebés de 4 semanas de edad o menos que tienen fiebre de 100.4 grados Fahrenheit o más, los padres deberían llevarlo de inmediato a un centro de emergencias pediátricas. Si el niño tiene más de 8 semanas y tiene fiebre de 100.4 grados Fahrenheit o más, los padres deberían comunicarse con el pediatra o buscar atención en una clínica de atención urgente pediátrica. Los bebés entre 4 y 8 semanas de edad con fiebre deberían ser trasladados a un centro de emergencias pediátricas si el bebé parece enfermo; si no es el caso, comuníquese con el consultorio de su pediatra para recibir indicaciones. Desde que eran pequeños, mi esposa y yo hemos sido muy claros acerca de nuestras expectativas sobre cómo deben tratar a sus pares (tanto niños como niñas) y acerca del acoso escolar. A medida que crecen, sabemos que hay cosas que podemos enseñarles ahora para prepararlos para mantener relaciones sanas de noviazgo en el futuro. Cuando nuestros hijos nos ven interactuar a nosotros, esperamos que aprendan a comportarse en una relación íntima sana. Al igual que cuando eran más pequeños y repetían como pericos las palabras que oían, nuestros hijos aprenden acerca de las relaciones a través de aquellos que los rodean. Al valorar y respetar las necesidades mutuas, demostrar igualdad en la toma de decisiones y hablar sobre nuestros desacuerdos de forma sincera y respetuosa, mi esposa y yo procuramos modelar una relación construida sobre el respeto. A pesar de que fue reciente mi plática explícita sobre “los tortolitos”, sabemos que es importante que nosotros hablemos con nuestros hijos desde una temprana edad sobre las relaciones sanas de noviazgo.Hay muchas ocasiones que nos sirven para enseñar y conversar sobre el noviazgo adolescente. La televisión, las películas y la música proveen muchos ejemplos de relaciones sanas y de dañinas. Tomarse el tiempo para usarlas como oportunidades para reflexionar sobre lo que oyen y ven les permite explorar temas tales como el respeto por sí mismos, los límites personales, los noviazgos sanos y el reconocimiento del abuso. Como un pediatra que se concentra en niños y familias vulnerables, he visto con mis propios ojos las consecuencias profundas y duraderas del abuso y de la violencia en las relaciones.Según los CDC, entre las víctimas adultas de violencia sexual y física, el 22 por ciento de mujeres y el 15 por ciento de hombres sufrieron por primera
vez alguna forma de violencia con la pareja íntima entre las edades de 11 y 17 años (Hoja informativa de 2014 de los CDC para entender la violencia en el noviazgo adolescente). Como padres, es importante educarnos a nosotros mismos acerca de la violencia en el noviazgo. Usted puede ayudar a sus hijos adolescentes a identificar conductas y patrones nocivos y puede proveerles orientación sobre las relaciones sanas. Algunas señales de advertencia del abuso que debe vigilar en sus hijos adolescentes incluyen aislarse de la familia y de los amigos, cambios erráticos de estado de ánimo: angustia o depresión, aumento de los niveles de agresividad, calificaciones desaprobadas repentinas, menos participación en actividades extracurriculares o escolares, y participación en conductas perjudiciales, como usar tabaco, drogas o alcohol. Si sospecha que su hijo o hija adolescente está en una relación abusiva, escúchelo y bríndele apoyo. No aísle a su hijo o hija con ultimátums.
