Here are three key points to consider in cultivating these enriching connections:

She received her Doctor of Medicine from the University of Maryland. After completing her Internal Medicine Residency training at Wake Forest University, she was appointed Chief Resident. Addressing Chronic Disease in St. Johns County

T

he First Coast YMCA recently conducted a community health analysis and the results were staggering: each of the five counties in northeast Florida (Duval, St. Johns, Clay, Nassau, and Baker) ranked above the state average in at least one chronic disease category, including prediabetes, diabetes, high blood pressure, and youth obesity. All five counties were above the state average for both cancer and arthritis. Translation: chronic diseases are a major issue in our area. Fortunately, the First Coast YMCA is here to help. At many of our locations all over the First Coast, including at the YMCA at Nocatee and the St. Augustine Family YMCA, we offer evidence-based programs that empower the community to live their lives on their own terms. Many of these services are available at little or no cost to members and most are available to non-members as well.

Arthritis is a major chronic disease that’s affecting St. Johns county residents. It’s the most common cause of disability in the country and it impacts more than 50 million Americans every year. If you or someone you know is in that category, we have our EnhanceFitness class that can be a resource to you. The 16-week program is endorsed by the Centers for Disease Control and Prevention and is

a community-delivered exercise class proven to increase strength, boost activity levels, and elevate mood. The certified instructors focus on stretching, flexibility, balance, low impact aerobics, and strength training. High blood pressure is another disease many in our community are facing as heart failure is the number one cause for hospitalization in St. Johns County. Like arthritis, it’s a nationwide issue and many adults don’t know how the manage it properly. The program lasts four months and includes personalized support for every participant, including tips for maintaining cardiovascular health, a monthly educational seminar, and an easy-touse tracking tool. Dental Disease The Silent Killer Periodontal disease is the number one health problem in small animal patients. By 2 years of age, 80% of dogs and 70% of cats have some form of periodontal disease. Small and toy-breed dogs are particularly susceptible. Periodontal disease advances quickly and can be difficult to detect unless your pet is under anesthesia. It can lead to localized problems but can also cause some more widespread problems that can seriously affect your pet’s health and cause pain that can go unnoticed.

Pathogenesis of Periodontal Disease While there are many factors associated with the development of periodontal disease, the inciting etiologic agent is plaque bacteria. Research has shown that inflammation persists while the gingiva is exposed to plaque but that inflammation will resolve after plaque removal.

Gingivitis vs Periodontitis Periodontal disease is described in two stages: gingivitis and periodontitis. Gingivitis is the initial, reversible stage of the disease (inflammation is confined to the gingival tissues). Gingivitis may be reversed with thorough dental prophylaxis and consistent homecare. Periodontitis is the later stage of the disease process and is defined as an inflammatory disease of the periodontium—the tissues that surround and support the teeth.

Plaque -Plaque resides on both the crown of the tooth and below the gumline. Dental calculus is calcified plaque that is also known as tartar. Once a periodontal pocket forms, the effect of the visible plaque & calculus on the crown of the tooth is minimal. Therefore, control of that plaque alone is ineffective in controlling periodontal disease. This is one of the many reasons why “anesthesia-free dental cleanings” are insufficient for periodontal therapy. Plaque under the gumline cannot be addressed adequately without anesthesia

Advancing periodontal disease can have profound consequences such as: Abscess – pocket of purulent material that forms at the root of the tooth which may or may not be seen without anesthesia. One of the most common abscesses we see is an oronasal fistula which is an infection of the upper canines which forms a communication with the nasal sinuses which leads to sneezing and infection in the nasal cavity. Pathologic Fracture – infection in the tooth can lead to fragility of the tooth and subsequent fracture

Blindness – inflammation and infection around the maxillary molars can cause serious effects on the tissue and nerves behind the eye and lead to blindness

Oral Cancer – chronic inflammation in the mouth can lead to increased cell turnover and create a higher incidence of cancer. Also, cancers can be detected more easily during the exam under anesthesia. Osteomyelitis – longstanding chronic infection can lead to infection in the bone which causes bone loss, pain, and pathologic fracture.

Severe systemic consequences – bacteria from the mouth enters the bloodstream and migrates to the liver, heart, and kidneys and lead to serious infections. Based on this information, avoiding professional cleanings in patients who are particularly susceptible to the negative effects of bacteremia, such as pets with diabetes mellitus, kidney/ liver disease, or heart murmurs/ low grade heart disease, is not good medicine. In fact, there is considerable evidence supporting the fact that these patients should be even more strongly encouraged to undergo professional dental therapy, because of the health benefits it provides. Periodontal disease has been called the “silent killer.”50 Periodontal disease is lurking in patients’ mouths and should be addressed when it is evident, but also prophylactic treatment should be routinely performed. Many pet parents have no idea how quickly and seriously periodontal disease can affect not just their pets’ teeth but also their overall health. www.stjohnsmag.com
