Social Determinates of Health Supports Program
RESOURCE PACKET
This packet intends to provide individuals and families enrolled in our Social Determinants of Health (SDOH) Supports programs such as Food Engagement through Efficient Delivery Services (increasing food access), Mobility Access through Supplemental Services (increasing access to transportation), and Technology Equipment Access (providing a Chromebook to allow access to technological equipment) with existing community resources. As the above programs near their ends, we encourage participants to enroll in existing community resources described below providing food, transportation, and technology. If you need assistance enrolling, please reach out to identified supports such as case managers, therapists, peer supports, and others. If you need peer supports we encourage you to contact CBH Member Services 888-545-2600.
Table of Contents Community Resource Guide – DBHIDS has created a one-page resource guide, complete with key numbers and information. For a list of additional key documents and resource information visit HealthyMindsPhilly.org/Boost. Food Access Flyer – Attached is a flyer detailing the places and dates where you can receive free meals. The phila.gov/food free food and meals finder also offers up-to-date information on distribution sites throughout the city. Or take advantage of the Amazon campaign allowing those that receive EBT benefits to utilize Amazon Fresh to get their groceries delivered for free. Increased Mobility with Transportation Flyers – Depending on your Medicaid or other insurance coverage, you can receive transportation to and from your medical and behavioral health appointments through Modivcare. Their informational document provides more details on the Medical Assistance Transportation Program, alongside the TripCare Flyer. We hope these will provide you a better understanding of the transportation services within the city. Digital Access Through Technology Flyer – Contact a Digital Navigator through the Office of Innovation and Technology to help determine whether you qualify for a free device and free broadband internet services. We have also included their flyer on connecting to the Affordable Connectivity Program for those who are interested in looking through the information without the assistance of a digital navigator. TechOWL Flyer – Contact TechOWL for assistance connecting with devices to assist those with vision and hearing loss. They also offer training in how one can best utilize these devices, as well as assistance via their helpline. BenePhilly Flyer – BenePhilly can assist you in signing up for the benefits you qualify for and assist you in the application process.
