7 STEPS EXPLAINED 1. Relationship over task is a critical mindset necessary for everyone involved in our care process, not just our Memory Partners. But why? Because it creates buy -in, which will allow for a successful relationship.

In most cases, we train our caregivers to live in their world and be on their side to be seen as an ally. This builds trusting and endearment which according to science, remains intact in many instances, up to one’s final days of living with Dementia and other impairments.

We need your help, please fill out the Getting to Know You document to help create a partnership plan (step 2).

2. Creating a person centered Partnership plan must be lifestyle, activity, and task based.

A partnership plan helps focus on the remaining abilities and support needed to help do things for themselves.

We are looking to bring value and purpose to a person's life.

Considering preferences, daily routines, schedules, adding activities that bring them joy and helps you connect.

Plans should also include other diagnoses and their requirements for care, Medication schedule, Physical and auditory Abilities and limitations, ADL’s, Home safety and needed housework, and ability to participate, interests, and glimmers.

In order to create a partnership plan, we start with an assessment.

To create a care plan, a person centered care plan, we must first do an assessment.

Our assessments take about 1 hour but can go up to 90 minutes. Our goal of an assessment is to learn your goals for success and each of your concerns... including your most pressing concern.

We use our team that consists of Certified Dementia Consultants, Dementia Trainer, Practitioners to develop a game plan to give everyone the best chance to be successful!

3. Approach is critical, and something we train each of our memory partners on. We understand that our approach will be the difference between a good relationship vs a relationship that causes anxiety, frustration, anger, and confusion. There are many factors to approach that our staff learn here at Pansy Homecare, including visual, auditory, and touching/feeling.

4. Next step is tackling 1 challenge at a time. This is to ensure we don’t overstimulate the person living with dementia by introducing too much all at the same time. Again focus on relationship in order to get incremental buy in.

5. Be a detective is critical.

Behaviors is language.

Think about this when in challenging situations.

Having a detective mindset, which we teach, allows us to explore what is going on and what the person living with Dementia is trying to say.

When in detective mode, we teach: 1. Detect & connect 2. Address Physical Needs first 3. Address Emotional Needs 4. Identify redirect (s) / pivot points (s) 5. Reassess & plan for next time 6. Re-Evaluations Must be done at a minimum every 6 months. As dementia or other cognitive impairments develops, the brain will continue to change, as will abilities. In order to continue to meet the person where they are, currently, we need to re-evaluate and compare notes from the previous assessment.

7. Stack your wins!

This final step is all about your mindset, which should be one on focusing on the positive. Success to us means stacking your wins, and enough wins throughout the day helps keep you be ready and willing to embrace tomorrow. This mindset also helps you to focus on the goods throughout each day.

At Pansy Homecare, we believe that the right caregiver-client match is foundational to delivering exceptional care.

This is why we designed an assessment to facilitate Purposeful Pairing by incorporating concepts from the Myers-Briggs Type Indicator (MBTI) to enhance understanding of both client and caregiver’s strengths, personality, and style.

This intentional matching approach enhances both client engagement and caregiver satisfaction, supporting our Purpose Filled Care model.

Matching Categories We classify caregivers and clients into four categories based on personality traits, interaction styles, and support needs: • Lions – Confident leaders who bring energy and proactive care.

• Owls – Observant and calm, ideal for clients needing a slower, more grounded approach.

• Golden Retrievers – Compassionate and adaptable, perfect for clients with emotional sensitivity.

PURPOSEFUL PAIRING 4 CATEGORIES These four categories take dementia and its associated behavioral types into consideration by addressing the range of needs, behaviors, and personality traits commonly seen in individuals living with dementia.

Here’s how each category aligns with dementia-related behavioral types and needs: 1. Lions (Leadership and Energy) • Behavioral Match: Suitable for clients who may have challenging behaviors such as resistance to care, strong-willed personalities, or those who may become agitated easily. These clients might need caregivers who are assertive, confident, and can gently take the lead while respecting the client’s sense of autonomy.

• Caregiver Strengths: Caregivers who are Lions bring a structured, proactive approach, which can help clients who feel disoriented or require guidance. This category is especially effective for clients who display behaviors that require confidence and a calming but firm presence.

2. Owls (Calm and Observant) • Behavioral Match: Well-suited for clients who are quiet, withdrawn, or anxious and may be sensitive to over stimulation. Clients who experience sundowning, for instance, often need a calm and observant caregiver who can detect subtle cues and prevent escalation.

• Caregiver Strengths: Owl caregivers are ideal for clients needing a steady, gentle presence. They observe without rushing, allowing clients to process at their own pace, which is beneficial for dementia clients who feel overwhelmed by sensory input or experience confusion.

3. Golden Retrievers (Empathy and Adaptability) • Behavioral Match: Ideal for clients who are emotionally sensitive, may display fluctuating moods, or experience distress and need a caregiver who is warm, nurturing, and can offer emotional support. Clients who become anxious, fearful, or agitated benefit from this compassionate, adaptable type.

• Caregiver Strengths: These caregivers excel in showing empathy and can adjust quickly to emotional cues, making them a good match for clients who experience emotional or behavioral swings. They provide reassurance and stability, which can be grounding for dementia clients with fluctuating emotions.

4. Turtles (Steady and Supportive) • Behavioral Match: Best suited for clients who thrive on routine, predictability, and stability and may become agitated or confused by changes in their environment or schedule.

Clients who benefit from repetitive routines or who have developed a strong attachment to daily habits do well with Turtles.

• Caregiver Strengths: Turtles offer consistency, reliability, and respect for routines. They are ideal for clients who need regularity, as they provide stability and follow schedules that help clients feel secure, which can prevent behavioral symptoms from arising due to sudden changes.

SOP for Matching Caregivers to Clients – Purposeful Pairing OBJECTIVE

To guide staff in matching caregivers with clients based on the Purposeful Pairing framework, improving care quality and satisfaction for both clients and caregivers.

STEPS

1. Client Assessment Conduct an assessment to identify the client's behavioral and personality traits. Document preferences, routines, and any behaviors impacted by dementia. Classify the client as a Lion, Owl, Golden Retriever, or Turtle based on assessment findings.

2. Caregiver Assessment Evaluate each caregiver’s strengths, personality, and style. Use training records and feedback to assign each caregiver a Purposeful Pairing category: Lion, Owl, Golden Retriever, or Turtle.

3. Initial Matching Process Use the assessment data to match the client and caregiver based on compatible categories.

For clients who are Lions, select caregivers who are calm but confident leaders. For Owls, match with caregivers who are patient and observant. For Golden Retrievers, prioritize caregivers skilled in emotional support. For Turtles, select caregivers with a consistent, routine-driven approach.

4. Trial Period and Monitoring Begin with a trial period to observe the caregiver-client dynamic. Use check-in forms and feedback loops to monitor satisfaction for both client and caregiver. Document observations and be ready to adjust the match if needed.

5. Adjustment and Reassessment Based on feedback, adjust matches if behaviors change or needs evolve. Schedule regular check-ins every 4-6 months or as needed to reassess compatibility.

6. Documentation Maintain updated records in each client’s and caregiver’s file. Document observations, feedback, and adjustments made during the matching process.

PAIN Assessment in Advanced Dementia Scale (PAIN AD) How do you know if your client is in pain?

Too often people living with dementia do not get the support they need as the signs of pain are often missed. Recognizing pain in people with no cognitive impairment is completely different than recognizing pain with people living with cognitive impairment.

For Dementia, using other pain tools like the visual analogue scale or the Faces pain rating scale is not an accurate measure of pain.

During a case study of 333 nursing staff, people suffering from a hip fracture, who had dementia, received just 1/3 of the amount of painkillers as a cognitively intact person, and 1/3 of dementia patients had no assessment of pain documented.

This is why to best understand pain for people living with dementia, we use the PAIN AD scale. This scale takes into consideration the dementia related changes in ability, including changes in communication.

PER SHIFT REQUIREMENTS Badge - with First name, picture, and our logo Badges are mandatory to be worn over the right chest. The reason is because that is also the hand you shake with and wave hello and goodbye with and for people with dementia, that is where they will look and so it will be easier to see the caregivers name with these changes as well.

Bring a change of shirt with them to every shift, that shirt must be a different color from the shirt they have on.

Why, because there will be times a client may disassociate them with someone else, or become upset with them in a moment.

Having a different color shirt allows caregivers to step away from the client, while the client is safe of course, give space, and change into a different color top, then step back in and reintroduce yourself.

This could benefit as the client may no longer association you with the incident or person that was bringing them discomfort before.

Memory Box We place a memory box in the homes of our clients who will be with us long term to help with intentional bonding.

The environment plays a key role - Bright light & Colors Dementia and sight loss are both more common as you get older. However, people with dementia can also have visual difficulties because dementia affects the parts of their brain that handle visual information coming from the eyes.

This means they will have visual problems but have healthy eyes.

We all have a special biological ‘clock’ that is involved in regulating the processes that take place in our bodies each day. This is known as our circadian rhythm. In dementia, the circadian rhythm can be disrupted, which can result in sleep disorders and disruptive behavior during the night. So keeping the areas well lit and bright can be beneficial to ability and day vs night-time recognition.

People with dementia can be less sensitive to differences in colors – including color contrast such as black or white. They may struggle to tell the difference between blue and black. They may also have a reduced ability to detect movement, may have problems with double vision, or struggle to change their gaze.

So, choose colors carefully, not just when it comes to home furnishings but when choosing items. At the dinner table, a white plate on a white table won’t help. A blue plate on a white table is much better. Our green chairs for instance represent some of the last colors to leave the eyes, making it a bit easier to be identified and used correctly.

Patterns can cause confusion, so keep patterns on the wall and floor to a minimum where possible. You may want to remove patterned rugs as the person could view them as obstacles. A person who has Dementia may be confused by black and white square tiles and think the black tiles are holes in the floor and try not to step on them.

It’s critical that your caregiver has been trained specifically for dementia care, no matter their experience. Ask about the topics covered during the trainings, Total hours and frequency caregivers are trained.

Are trainings virtual or in person? And the credentials of the trainer/Training.

What if your caregiver calls out?

Depending on your unique situation, this question is critical. While some have support at home, others might not, and it is important an agency has a plan around how they will handle a call out or planned absence by your caregiver. Ask them what is their process during situations like these, especially if you cannot afford to be without support.

What is your caregiver selection process?

Just like general homecare and dementia care are different, so is being a general caregiver and a dementia specific caregiver. Here you are looking for their recruiting process, and why they select one caregiver over another, specifically for dementia care.

You do not want an agency that “Plugs them and forgets them”. No matter a caregivers experience or training, they will need support and the agency needs to be there during those times. An agency also needs to have a continuous follow up process with a clients and their entire care team, in order to support the clients current needs and abilities.
