ACTIVATING THE APP IS AN EASY THREE-STEP PROCESS

Employees and spouses can activate the app with the below instructions. Being registered with a wellness account is required to complete activation for both employees and spouses.

1. Download Asset Health Mobile from the App Store or Google Play

2. Open the app, tap Begin Activation, then follow the instructions to enter the authorization token and the email address associated with your wellness account. (You will need to sign into your wellness account on your desktop or mobile device to complete the authorization process)

3. Set your four-digit PIN. You will use this PIN to access your app from now on Features shown in images may or may not apply based on your employer’s configuration.

Get Started

Log in to assethealth.com/dfa on your desktop or mobile device. If you’re logging in for the first time, your credentials are as follows:

• Username: First initial of first name + Last name + mmddyyyy

• Password: Date of birth (mmddyyyy)

Completing Your Health Assessment

For medically enrolled employees and spouses with a benefits-effective date prior to Friday, June 2, 2023: Log in to assethealth.com/DFA. Sign in to your Asset Health Portal and complete the “Health Assessment” activity. Once you finish the short quiz, this will be marked as complete.

Ways To Complete Your Biometric Screening

Quest Diagnostics Appointment

• Click on the Step 2 tile on the homepage of the new wellness portal. This will direct you to Quest Diagnostics®, where you can schedule your screening at a Quest facility

• Spouses can visit my.questforhealth.com and use registration key: DFA2023

• Visit my.questforhealth.com or call 1-855-623-9355

*Note: Some locations are hosting on-site biometric screening events. Contact your local human resources representative for more information.

Annual Physical With Your Physician

• Click on the Step 2 tile on the homepage and select Physician Form to print off a copy

• Have your physician complete the form in its entirety and sign. You can then upload the form right on the homepage

Note: Annual physicals to have the Physicians Results Form completed are 100% covered by your medical plan and do not require a co-pay on any of our plans. Additional testing and care for conditions found during your exam are subject to your policy. Please allow approximately 14 days for this activity to be marked complete. It will be marked as done once Asset Health receives your completion information from Quest.

Sync Your Apple Watch With The Asset Health Portal

If you have an Apple Watch, please download the Asset Health Mobile app first and activate HealthKit* within the app to sync.

FORGOT YOUR PASSWORD?

Follow the steps to log in to Your Asset Health Portal on the web or app, then when asked for your login credentials, click “Forgot password?”. You will then receive an email to reset your password.

*Apple HealthKit is an app that allows all health and fitness data collected from your iPhone, Apple Watch, etc., to be stored in one place. It can help to simplify your process of syncing activity data for future wellness challenges.
