Welcome to the

2024 BENEFITS OPEN ENROLLMENT The competitive benefits offered to you as an employee at DFA are a valuable part of your overall compensation.

It is important to invest some of your time each year to ensure you get the most out of what is available to you.

Open enrollment provides you the opportunity to review your benefits and adjust coverage for you and your dependents. The benefit elections you make now will become effective Jan. 1, 2024. Your choices can be made online, by mobile device or by telephone. You may also contact your local human resources representative with questions.

Outside of open enrollment, coverage changes are only allowed if you experience a qualifying life event or family status change. When you experience a life event or family status change (marriage, divorce, birth or adoption, death of a dependent, change in your child’s dependent status, or change in your or your spouse’s employment status), you have 31 days from the date of the event to report it and make changes to your coverage.

2024 OPEN ENROLLMENT ANNOUNCEMENTS This is a passive open enrollment, meaning that a majority (including medical, dental, vision and voluntary benefits) of your current elections will roll over for 2024 if you take no action.

Benefits that require you to make an election for 2024 are: • Health savings account (HSA) • Flexible spending accounts (FSAs) — including healthcare FSA, limited purpose FSA and dependent care FSA If your spouse has employer-provided coverage available, he or she must take that coverage as he or she is not eligible for medical benefits from DFA. You have the opportunity to elect voluntary life insurance up to the guaranteed issue amount for yourself or your spouse, even if you previously declined coverage. If you are currently enrolled in voluntary life insurance, you may increase you or your spouse's current election up to the guaranteed issue amount without evidence of insurability!

This opportunity is not guaranteed to be available again. See page 10 for details.

2024 ACTION ITEMS Review your comprehensive open enrollment guide and the enclosed rates and notices before making your 2024 benefit decisions.

Review your 401(k) and life insurance beneficiary designations to confirm the information is current. If you do not have a beneficiary listed, it is important that you add one.

If your spouse and/or dependents have not previously been verified

Make sure to complete any required health coaching calls and/ or tobacco cessation program by Thursday, Nov. 30, 2023, in order to avoid the surcharge in 2024.

MEDICAL AND PRESCRIPTION DRUG PLANS For 2024, you will continue to be offered the choice of two High Deductible Health Plans (HDHPs): the HDHP Enhanced and the HDHP Basic, in addition to the Preferred Provider Organization (PPO) plan. HDHPs have a higher deductible than the PPO plan as described in the summary below, but the HDHPs give you lower premiums and allow you to contribute to an HSA to save for future health care expenses as discussed in more detail on page 7, including details on the automatic one-time HSA contribution from DFA. With the HDHP options, you pay the cost of all services until the deductible is met, with the exception of most preventive care services. This includes the cost of physician services, prescription drugs and all other medical services. Once you meet the applicable in-network or out-of-network deductible, you and the plan share in the cost of services, called coinsurance, until the applicable out-of-pocket maximum is met.

Routine Preventive Care

The plan will limit out-of-network payments to reasonable and customary levels. Amounts over this level will not go toward the deductible or the out-of-pocket maximum. These amounts can be substantial. You may be balance billed the difference between the provider’s charge and Blue Cross Blue Shield’s allowable.

DFA offers plan participants the opportunity to have certain preventive drugs covered at 100%!

If your medication falls on the CVS generic preventive drugs list, it will be covered at 100% if it is obtained through an in-network pharmacy.

In addition to using mail order to fill maintenance medications, you can fill a 90-day maintenance medication at any retail CVS pharmacy. Maintenance medications are commonly used to treat conditions that are considered chronic or long term and usually require regular, daily use. Examples include those used to treat high blood pressure, heart disease, asthma and diabetes.

If you request a brand name when a generic is available, you may be required to pay the difference in the cost of the brand name vs. generic drug (in addition to the applicable coinsurance).

Your specialty prescriptions will come from VIVIO Health. VIVIO will also help you manage your complex or chronic condition with one-on-one case management.

DFA offers voluntary programs that are available to support you or a family member who is a diabetic, has chronic hypertension, or needs to have non-emergency surgery. Details about each of these programs can be found in the ADP document library, which can be accessed online at myADP.com or through the mobile app. You can also access each company’s website by scanning the below QR code with your smartphone camera.

Prevent diabetes, manage your diabetes or manage your hypertension with LIVONGO: DFA offers three programs through LIVONGO which are free of charge to you and your dependents. These programs help you prevent diabetes, manage Type 1 or Type 2 diabetes, or manage your hypertension. Participation in these programs comes with applicable medical supplies at no additional cost.

ACCOUNT ELIGIBILITY

The following must be true to be eligible for a HSA:

Any employee enrolled in either HDHP in 2024 with a HealthEquity HSA will receive an automatic one-time HSA contribution from DFA. If you elect Employee-Only coverage, you will receive $750. If you elect Family coverage, you will receive $1,500.

• Must be covered by one of the HDHPs • Cannot be claimed as a dependent on someone else’s tax return • Cannot have any other coverage such as: • A plan that is not an HDHP • Spouse's plan that is not an HDHP • Medicare or Medicaid • TRICARE coverage (military health care) • Health reimbursement arrangement (HRA) • Health care FSA (does not apply to a dependent care FSA or a limited purpose FSA) It is the account holder’s responsibility to maintain compliance under these regulations.

QUALIFIED EXPENSES IRS Publication 502 provides a complete list of eligible expenses and can be found at irs.gov. Funds can be used to pay for medical expenses for yourself, your spouse and tax dependents even if they are not covered by your health plan.

Employer contributions count toward the IRS annual limits, so plan your contributions carefully to avoid exceeding those annual limits.

Employer and employee contributions are state taxable for California and New Jersey employees.

YOUR RESPONSIBILITIES AS THE HSA ACCOUNT HOLDER It is your responsibility as the HSA account holder to keep your itemized receipts. HSAs are monitored by the IRS, and non-qualified expenses are subject to your normal income tax rate — if you are under the age of 65, there is an additional penalty of 20%.

DENTAL PLANS Dental coverage continues to be administered by Delta Dental of Kansas, and you have access to the nationwide Delta Dental networks. You can find an in-network dentist by using the dentist search online at deltadentalks.com or by downloading the Delta Dental mobile app.

FLEXIBLE SPENDING ACCOUNTS Stretch your health care and dependent care dollars by using pretax dollars for qualified costs by participating in the flexible spending account (FSA) program. Pretax means the dollars used for eligible expenses are not subject to social security tax, federal income tax, and in most cases, state and local income tax.

Participation in the FSA program must be elected each year during open enrollment for the following calendar year. The health care and limited purpose health care FSA allow for up to $610 of your unused 2023 FSA balance to be carried into the 2024 FSA plan year.

If you are not eligible to open or contribute to an HSA (see page 7 for details), you may participate in the health care FSA. If you are eligible to contribute to the HSA, you are only eligible to enroll in the limited purpose health care FSA and/or the dependent care FSA.

VOLUNTARY BENEFITS DFA offers employees an opportunity to choose benefits that suit their personal circumstances and lifestyle by purchasing additional protection through these voluntary benefit offerings. You are not required to be enrolled in any other company benefits to elect these coverages.

LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D) INSURANCE BENEFITS DFA provides full-time employees with a basic life insurance benefit and an accidental death and dismemberment benefit through New York Life Group Benefit Solutions.

In addition to your basic life insurance benefit, full-time employees may also elect supplemental life insurance in $10,000 increments from $10,000 to $500,000. Spouses are able to elect up to the lesser of $250,000 or 100% of the employee’s supplemental coverage in $5,000 increments. A $10,000 or $20,000 dependent child option is available (the benefit is $2,500 for dependents seven days to six months of age).

Any amounts elected above the guaranteed issue amount will require evidence of insurability. If required, an evidence of insurability form will be mailed to you after the open enrollment period ends. Evidence of insurability is not needed for the dependent child coverage.

You may purchase additional AD&D insurance through New York Life Group Benefit Solutions: • Yourself: AD&D coverage is available to a maximum of $500,000.

• Your family: If you elect family coverage and no dependent children are covered, your spouse coverage amount will be 50% of your coverage amount. If one or more dependent children are insured, your spouse coverage amount will be 40% of your coverage amount. Each of your covered children’s benefit amount will be 10% of yours, subject to a maximum benefit of $20,000.

For 2024 only, even if you previously declined coverage, you will be able to elect up to $250,000 of coverage without completing an evidence of insurability form. Employees who currently have between $10,000 and $240,000 of supplemental life insurance may increase their coverage to $250,000 without proof of good health during open enrollment. You may also elect up to $50,000 of supplemental life coverage for your spouse without submitting evidence of insurability. Any spouse life elections for amounts above $50,000 will require evidence of insurability.

ACCIDENT INSURANCE

IDENTITY THEFT

AFLAC | Voluntary accident insurance provides a range of fixed, lump-sum benefits for injuries resulting from a covered accident or for accidental death and dismemberment. These benefits are paid directly to the insured and may be used for any reason, from deductibles and prescriptions to transportation and child care. Two plans are being offered, a low plan and a high plan.

ALLSTATE IDENTITY PROTECTION | Your identity is made up of more than your social security number and credit score. That’s why Allstate Identity Protection does more than monitor your credit reports. They help you look after your online activity, from financial transactions to what you share on social media — so you can protect the trail of data you leave behind.

CRITICAL ILLNESS INSURANCE AFLAC | Voluntary critical illness insurance provides a fixed, lump-sum benefit upon diagnosis of a critical illness, which can include heart attack, stroke, paralysis and more. These benefits are paid directly to the insured and may be used for any reason, from deductibles and prescriptions to transportation and child care. This coverage also includes a $50 annual wellness (health screening) benefit.

HOSPITAL INDEMNITY INSURANCE

LEGAL PLAN METLIFE | Legal matters, both planned and unplanned, are part of life. Enrolling in the MetLife Legal Plan gives you the financial and emotional peace of mind to know you will be covered for many expected and unexpected legal events.

The MetLife Legal Plan provides you, your spouse and dependents with fully covered legal services from attorneys experienced in estate planning documents, civil suits, adoption, identity theft issues and much more.

AFLAC | Voluntary hospital indemnity insurance provides a range of fixed, lump-sum daily benefits to help cover costs associated with a hospital admission, including room and board costs. These benefits are paid directly to the insured following a hospitalization that meets the criteria for benefit payment.
