Who we are.
What are Senior Outreach Services?
The Center for Youth and Family Solutions (CYFS) serves seniors through In-Home Counseling and the Friendly Visitor Program, offering services to seniors to help improve their quality of life and prevent or delay nursing home placement. CYFS is now happy to add Senior Outreach Services to seniors in Peoria, Tazewell and Fulton counties. We provide information on how to get needed services and benefits such as:
Adult Protective Services
Who we Serve. Our Process. Fees.
We provide services to seniors ages 60 and older residing in Peoria, Tazewell and Fulton Counties.
A Senior Outreach worker will provide an inhome or community visit to identify individual needs and discuss available services and benefits. By utilizing our software programs, the Outreach Worker will identify qualifying services, making referrals and following up to determine if needs have been met. We are also there to answer any questions that you may have.
There are no fees for our Outreach Services, all participants have the opportunity to make a voluntary contribution if they choose.
Senior Outreach services and activities are made possible, in part, through a grant from Central Illinois Agency on Aging, Inc.
