Heart Attack Symptoms are Different for Women During American Heart Month, Cape Fear Valley Health specialists say that for women, being tired, having back pain, or feeling nauseated can be signs of a heart attack.

S

ondra Bemesderfer, 53, felt fine the day she had an almost fatal heart attack.

“I felt funny and couldn’t get a breath, a full breath,” Bemesderfer said.

It was out of the blue with no symptoms like chest or arm pain normally associated with heart issues.

She called her son, who worked as a trauma nurse and asked about Covid-19.

“I had gone all day at work feeling fine,” Bemesderfer said. “My best friend who I worked with and I drove through Starbucks after work like we did every day … and when we got on All-American, I started yawning,” Bemesderfer said. “I thought it was odd that I was tired.”

“I was sure I was coming down with something respiratory like bronchitis or pneumonia,” Bemesderfer said. “I took a COVID test and it was negative.”

After she got home, she had planned to take her dogs to Starbucks for a pup cup. But that’s when she started feeling off.

She went into the laundry room to get into more casual clothes and then couldn’t make it further than the bathroom in her hall when she started gasping for breath.

“I yelled at my husband to call 911,” Bemesderfer said.

Her son arrived when the paramedics did and immediately deduced that she was having a STEMI heart attack, according to Bemesderfer. A STEMI, or ST-elevation myocardial infarction, is a heart attack caused by a blockage of blood to the heart, according to the Cleveland Clinic.

Symptoms include shortness of breath, fatigue, and nausea, all of which Bemesderfer was experiencing, although she still did not have any pain. She was confused as to why she was being sent to a cardiac catheterization laboratory, or cath lab, at the hospital and in such a hurry. A cath lab is a hospital room where doctors perform minimally invasive heart procedures.

“It could not have been scripted better though,” Bemesderfer said. “The entire team at Cape Fear Valley was in place when I got there, but it wasn’t until I was on the cath table that I realized this was real. I realized I was having a cardiac event.”

Dr. Mathhar Aldaoud was the cardiologist on call that night and attended to her as part of the team assembled.

“He told me that he was going to go in through my wrist to place the stent and I told him I had to get up to go to the bathroom before I realized it was serious,” Bemesderfer said.

“He said time is tissue and he had to get in there. That the longer it takes to get in there, the more heart damage or more could happen.”

Bemesderfer was awake during the procedure in which they placed a stent—a tiny wire mesh tube used to hold open passages in the body to increase blood flow to the heart—and said her mind was racing while the doctor worked as fast as he could.

“Something was wrong.”

She pushed the panic button on their security system so an ambulance would be automatically dispatched.

“At that point, it was like I was trying to breathe through a cracked straw,” Bemesderfer said. “After the paramedics got there, they put the CPAP mask on me and I could breathe. I felt fine again. I even offered to walk to the ambulance.”

She said the paramedics worked quickly to get her out of the house and to the hospital.

“My son who drives a sports car could not keep up with the ambulance,” Bemesderfer said. “I still didn’t have any pain, but my blood pressure was 260/170 and my oxygen level was 62. I was nauseated, which I assumed was due to my lack of oxygen.”

According to the Mayo Clinic, a healthy level of oxygen is between 75 and 100 and blood pressure over 180/120 is considered a crisis, needing emergency medical help.

Bemesderfer was well over those healthy and in-range numbers.

“I remember thinking that I was just at home, I was just at Starbucks, then I started thinking no one knows how the bills are paid or when they are paid,” Bemesderfer said. “It was an hour from the time at home to where I was on that table and I had just had a normal day at work.”

It turned out she had a pulmonary embolism, where a blood clot had moved from her lung towards her heart and her LAD was 90% blocked. The yawning was the blood clot moving, she said.

The LAD, or the left anterior descending artery, is the largest coronary artery and a blockage of blood flow can cause a heart attack, according to the Cleveland Clinic.

“That blockage in my LAD helped save my life, it gave me time to get to the hospital—I mean God has the ultimate say-so with that,” Bemesderfer said. “The doctor said if I had waited 20 more minutes, I would not have been so lucky.”

She said it was at that moment when she realized she had survived a heart attack and her life had been completely changed in the span of an hour.

The patient care manager at Cape Fear Valley Health Shonda Ray said women do not have typical symptoms and are less likely to have pain in the center of their chest.

“You see TV shows or in the movies where the man grabs his chest and falls to the floor,” Ray, who manages both the Cardiac Surgery ICU and the Cardiac Surgery Progressive Care Unit, said. “It is not like that in real life. In real life, it can be nausea or vomiting, neck, jaw or back pain or even feelings of anxiety. You might be tired.”

She said sometimes a heart attack can even be confused with a panic attack in women.

“You know your body though and when something is wrong, pay attention,” Ray said.

And when the symptoms are all together—chest pain, shortness of breath, back pain, or jaw pain—she said to call 911 immediately and to take a low dose aspirin if you have it.

“When you call 911, it activates EMS, the emergency medical system, which gets the cardiac-cath team at the hospital ready to go,” Ray said. “The patient bypasses the lobby and registration so that your patient care starts as soon as possible.”

She said their main goal is to get the blockage open.

“Time is muscle and the more time that goes by can cause heart damage,” Ray said. “Think of it like a blocked pipe.”

With 20 years of experience as a nurse on the cardiac wing, Ray said that there are factors that cannot be controlled, such as genetics and family history of heart attacks. In those cases, she said to make sure you are staying on top of blood work with your regular provider.

But there are ways that women can prevent heart attacks that are in their control, like eating healthy food, keeping stress levels down with meditation or yoga, and exercising as a habit.

“Stay away from fat and salt and do not smoke,” Ray said. “The chemicals in cigarettes attack your blood vessels causing blood clots and a buildup of plaque, which cause dangerous blockages and heart attacks.”

Bemesderfer, who has diabetes, said her heart attack was caused primarily by uncontrolled blood sugar.

Since her heart attack in September 2023, her A1C test—which shows how managed a person’s condition is by measuring the percentage of red blood cells that have sugar-coated hemoglobin—has gone from 13.7 to 6.4, putting her in a healthier range. An average below 7.0 is a good blood sugar level for those with diabetes, according to the Centers for Disease Control and Prevention.

She said she now watches everything she eats and has eliminated red meat, fast food, or anything greasy from her diet.

She was not the only one who made a change in their life. Her son Paul also made the decision to go to work for Cape Fear Valley Health.

“My son was so impressed with how Cape Fear Valley Health worked so perfectly for me that night that he decided to change hospitals from where he was working as a nurse in

the Emergency Department,” Bemesderfer said. “He’s now a rapid response nurse at Cape Fear Valley and just celebrated his first year.”

Bemesderfer, who stopped working after the incident to take care of her health, said that she looks at life “a whole lot differently now.”

“You really realize what is important is family and health and that extra stuff, that rotten fruit, will take care of itself,” Bemesderfer. “Women tend to not slow down enough. We take care of everyone else—our husbands, our children, our friends. We have to take care of ourselves, too.”

Expanding Orthopedic Footprint Dr. Daniel DeRosa, an orthopedic surgeon who specializes in minimally invasive procedures, says there are no easy plays when it comes to treating the spine.

Joining Cape Fear Valley Health’s growing ortho team, though, was a simple call.

A

s an orthopedic specialist, Dr. Daniel DeRosa likes to say that nothing in the realm of neck or spine surgery is “a chip shot.”

There are simply no easy plays.

It’s the spine’s complexity and acuity—physicians speak about the intensity and high level of required care—that DeRosa grew to love during his medical training. That passion is evident when he discusses how the spine works and the techniques he uses to address the physiological problems causing pain, discomfort, and immobility for his patients as part of Cape Fear Valley Health’s growing orthopedic team.

“It runs the gamut,” DeRosa says about the kinds of symptoms his patients present. “I have 35-year-olds with advanced arthritis, and I have 90-year-olds with degenerative changes.

The unique thing about my job is I can see someone one day who has one of the worst spines I’ve ever seen, but is relatively asymptomatic, and doesn’t have any nerve pain or leg pain—just some back pain. And then I have some patients with just a whiff of arthritis, but they have pretty debilitating radiculopathy, or nerve pain, in their legs.”

Some orthopedic treatments can be cut and dried—“You know,” he says. “A plus B equals C.” Rare with the spine.

Major growth for Cape Fear Valley Health For DeRosa, who joined Cape Fear Valley Health in July 2024 and works out of Valley Orthopedics & Sports Medicine on Walter Reed Road in Fayetteville, it’s hard to overstate the importance of the spine’s role in the body’s overall function.

The spine supports structure. It keeps us standing tall, facilitating our movement. But it also houses the nervous system, enabling communication between the brain and the rest of the body. Signals originating in the brain need the spine as a delivery channel, telling muscles and organs what to do; signals from the body use the same wiring, the same spinal pathway, to the brain in reverse.

It’s the original information superhighway. Disruption, then, through injury, disease, or degeneration, can mean trouble.

DeRosa’s addition is part of a significant period of growth of Cape Fear Valley Health’s orthopedic footprint. Over the past three years, it has grown from two clinics to nine. Through acquisition and expansion, there are now more than 30 service providers within Cape Fear Valley Health’s ortho team in Cumberland County, Harnett, Moore, Bladen, and Sampson counties. Combined, they serve an average of 600 patients per day.

What makes DeRosa unique—aside from being Cape Fear Valley Health’s sole orthopedic spine specialist—is that he’s one of the 30% of spinal surgeons who performs advanced minimally invasive anterior column surgery through the side of the body to treat spinal disorders and reduce long term back and leg pain, he said, resulting in shorter (or no) hospital stays, less blood loss, and faster recovery time. Smaller incisions also mean muscles and tissues in the surgical area suffer less stress, particularly advantageous for older patients or those with other negative health conditions.

“So there’s less demand on health care systems, better outcomes both short and long term,” he said.

An example: DeRosa can perform a single-level lumbar surgery through a patient’s side in about 20 minutes. A surgeon without his training and expertise would take a more traditional approach, going through the posterior spine with “big, open incisions that involve a lot of electrocautery and burning of the muscles,” DeRosa said.

Methodologies he employs are catching on among spinal surgeons, DeRosa said, but it’s still a niche. For him, that expertise means being able to treat more patients, and solve problems, more effectively.

Many of his patients present challenges tied to achieving simple desires, like the retired military officer he recently treated who wanted nothing more than to play with his two grandchildren, but couldn’t because of severe leg and back pain. Or the patient who loved golf and saw it as a stress reliever and respite from work, but suffered from neurogenic claudication—a compression of nerves that made walking distances or swinging a club result in leg cramping, pain, and numbness.

technique that resulted in the grandfather playing “everything he wanted” on the floor with his grandchildren just six weeks post-op. For the latter, a lumbar spine procedure—again, minimally invasive—had the patient back on the fairways in just three months.

“With the spine, there’s a lot of nuance,” he said, “and there’s a lot of context, so there’s diagnostic testing and making sure you’re doing the right thing by the patient.”

Overlap compounds complexity. Arm pain, for example, can originate from a problem in the neck. Or degenerative changes combine with carpal tunnel syndrome or acute elbow pain— requiring nuance, investigation, and multiple levels of treatment protocols. Sometimes, DeRosa adds, it’s about “knocking out” one problem to make a patient functional again before working to heal other symptomatic issues.

“I tell patients, ‘Spine surgery is different than other musculoskeletal procedures/intervention.’ There are a lot of 18 | 2026 Medical & Dental Directory

moving parts, so a lot of it is, ‘What are our goals? And how do we accomplish that in the most effective manner?’”

‘Time to go civilian’ DeRosa earned his DO (Doctor of Osteopathic Medicine) from the Kentucky College of Osteopathic Medicine before completing internships and residencies at Tripler Army Medical Center in Hawaii and a fellowship at Duke University. He accepted an Army health professional scholarship; a portion of his more than 13 years in the U.S. Army was spent as a Flight Surgeon in an aviation regiment in Central America and deployed to combat zones in the middle east as an Orthopaedic Surgeon with a Forward Surgical Team.

Along the way, his professional focus switched from orthopedic and related trauma surgery to the spine, and he was serving as a spine surgeon at Womack Army Medical Center when that active duty obligation came to an end. His wife, Raffaella, also a doctor—she’s a urologist at Womack—had transitioned out

T

wo years ago in May, Rachel Thurnher was sitting bedside with her father, George Caley, at his home as he quietly slipped out of this world, surrounded by love, comfort, and peace.

“It was everything he wanted,” Thurnher said. “He had my brother on one side, me on the other, and was not in any pain. All his wishes were honored, and it was a good death.”

Thurnher said her father was the inspiration for bringing the No One Dies Alone (NODA) program to Cape Fear Valley Health in January 2025. The program brings in volunteers to provide comfort to those dying who have no loved ones or family to be at their bedside during the last moments of their lives.

“He started my hometown's hospice program in Virginia and 40 years later became the patient,” Thurnher said of her father, who was the president of the Winchester Medical Center.

Comfort care means that the hospital will not do anything further to prolong life, according to Thurnher. Feeding tubes are taken out, IV fluids are stopped, and her team of volunteers can step in to help the patient pass peacefully.

“We are truly just there to be a positive presence,” Thurnher said. “We pray, we meditate, we read, we provide a comforting touch, we play music according to their preference. We do anything a loved one would normally do.”

Thurnher keeps a comfort cart stocked with fuzzy blankets, flowers, flameless candles, lotion, lip balm, a CD player with Bluetooth to play anything the patients want, cards if they would like to write to loved ones, and all sorts of books, both religious and secular.

— Rachel Thurnher When he was diagnosed with stage four pancreatic cancer, Caley was told that he would only have a few months left to live. He had time to plan and he was able to have all of his wishes upheld for the end, which sparked the drive for others to have the same, even if they did not have anyone available to be at their side.

“We try to have it all in that cart and provide whatever the patient would like as we sit vigil, talking softly with them in their final moments,” Thurnher said.

For Thurnher, the education coordinator for Cardiac Diagnostics and the Cardiovascular Recovery Unit and now also the program coordinator for NODA, it is an honor to be with someone at the end of their life to help them achieve what she calls “a good death.”

“I always enjoyed cardiac nursing throughout school and in my professional career,” Thurnher said, “but I also knew after I was with my dad that I wanted to volunteer with end-of-life patients.”

When she started researching how to help those who might not have support in the area, she found the NODA program and knew it would be a perfect fit for Cape Fear Valley Health.

The program was created on the other side of the country, according to a Cape Fear Valley Health press release. The Sacred Heart Medical Center in Oregon started the NODA program in 2001, eventually becoming an international program, where volunteers, or “Compassionate Companions,” help ensure every patient has someone by their side in their final moments.

“The founder of No One Dies Alone, Sandra Clarke, has even written a program guide to help other hospitals start their own chapter of NODA,” Thurnher said.

Thurnher held the first interest meeting with pastoral care and volunteer services departments in May 2024. When the hospital introduced the program in January 2025, over 100 24 | 2026 Medical & Dental Directory

volunteers raised their hands to help within two days of the program’s announcement.

She attributes some of the interest in the program to Fort Bragg’s military families who pass through duty stations and learn to help those around them.

“As a military spouse in a military town, we take care of each other,” Thurnher said. “If you live here, it’s integrated in what we do. We live in a special place.”

Thurnher said she has been blown away by the level of compassion that is being shown by the volunteers.

Shortly after the program launched in January, the team had their first patient. The man, on his deathbed, might have been alone without Thurnher and her volunteers who took shifts with him until he ultimately completed his life’s journey.

“Our first patient was really special for everyone,” Thurnher said. “He had no family readily nearby so we had someone sit

with him the entire time. We facilitated a good death without pain. He was clean and comfortable. It was really meaningful for our volunteers.”

She said she was honored and proud that the program was in place for him and that it functioned exactly how she’d hoped.

“It's as much of a gift to the volunteer as it is for the patients and their families. It’s really just a win-win-win,” Thurnher said. “It’s an opportunity to be with someone at the end. It’s an honor. It’s uplifting for your soul and your well-being to be present, to face humanity and give this gift to someone.”

She said it is as much of a gift to the volunteer as it is for the patient and their families.

Bereavement training for volunteers is on the third Saturday of every month from 8 a.m. to noon at the Center for Medical Education at 1638 Owen Drive in Fayetteville. “Volunteers are given specialized training in compassionate communication, comfort care, death and dying, health care ethics, and spiritual care,” Thurnher said.

EXPANDING CARE The Growth of Cape Fear Valley’s Medical Group The hospital network helps attract new physicians and specialties, growing their clinics in the past five years to now have more than 100 in total.

hen Elizabeth Sawyer, M.D., transitioned from the U.S. Army to private practice, she had two clear goals: she didn’t want to work for a hospital, and she didn’t want to teach residents.

Mark Cobb, president of Cape Fear Valley Medical Group, said that, within the health system, there are more than 90 primary and specialty care clinics in Cumberland County and the seven surrounding counties, including Moore, Hoke, Harnett, Bladen, Columbus, Robeson, and Sampson.

But just a year and a half into her dream role, everything changed. Her clinic became Cape Fear Valley General Surgery — Village Surgical, and with that shift, so did her perspective.

Mark Eaton, M.D., with Cape Fear Valley Podiatry — Foot and Ankle Specialists — Fayetteville, said their clinic was the largest that was brought into the medical group, and the growth has been a tremendous benefit to the clinic and the community.

Now, Dr. Sawyer embraces both opportunities—thriving as part of a collaborative hospital team and finding fulfillment in mentoring the next generation of surgeons.

“I have found tremendous support and infrastructure from the health system and have been able to build the breast program to the degree it is now,” Dr. Sawyer said. “We have grown the program by 40 percent. It’s been a tremendous value to be a part of the network.”

Her clinic was acquired by Cape Fear Valley Health in an effort to increase patient access to specialized services and expand into rural, underserved areas.

“We wanted to improve our practice and expand so we looked at it as a partnership,” Dr. Eaton said. “In the six years we have been a part of the health system, we have doubled our patients seen from 33,000 to 65,000 per year and gone from seven physicians to 16.”

Dr. Eaton said it makes them the largest hospital-based podiatry clinic in the United States. With the backing of the health system, the podiatry group was also able to create the first Podiatry Resident Clinic in the country, where resident doctors treat patients under the guidance of an experienced podiatrist.

“Being a part of this system allows us to expand our services within our seven locations, which allows more access for our patients,” Dr. Eaton said. “It gives our patients access to specialty doctors and care.”

It is also a benefit to the community doctors, some of whom might be aging or want to see their operations become more efficient.

It is also another reason that Dr. Eaton said their practice is now gladly part of the network.

“We wanted to create a legacy and continue to guide training for younger doctors,” Dr. Eaton said. “It’s another way to give back to the community for us.”

“When a clinic becomes part of Cape Fear Valley Health, it allows doctors to just be doctors,” Cobb said. “They can practice medicine without worrying about the business operations part of it. Being part of a larger network means they don’t have to deal with staffing issues or supply embargoes.”

He said it helps save smaller clinics in private practice from operational issues and challenges that can significantly impact patient care, including shortages and delays.

Cobb said the main driver of acquiring more clinics is the patient and their access to care. It also helps financial 30 | 2026 Medical & Dental Directory

stewardship across the board between the hospital, clinics, and those needing the care.

Another benefit is the ability to more easily transfer information from the clinic to the hospital, following the patients wherever they go within the health system.

“It’s a much easier path to travel,” Cobb said. “Patients have one medical record, and that record is shared among Cape Fear Valley Health, accessible by providers at any time.”

Johnnie Moultrie, M.D., who is with Hope Mills Family Care, recognizes this benefit daily with his patients.

“There are so many different specialties that our patients can get in to see if they need something specific, like interventional radiology or women’s health,” Dr. Moultrie said. “We can expedite referrals, and it makes our healthcare system very efficient.”

With specialties ranging from primary care and orthopedics to neurosurgery and pulmonary medicine, Cape Fear Valley Health is growing and will be adding even more specialists to the network in 2026.

Community Gains Faster Care: Same-Day Clinic Launches Ahead of Peak Flu Season Cape Fear Valley Health in November opened SameDay Medical, a new medical clinic that bridges the gap between primary care visits and emergency room visits.

ayetteville’s newest Cape Fear Valley Health medical clinic is open for business and is welcoming patients.

On November 3, SameDay Medical began offering local residents a new option for convenient access to health care. The facility held its official ribbon cutting on October 30, and its staff could not be more thrilled.

“I'm super excited to bring this clinic to our community and to start taking care of patients,” said Brittany Bailey, service line director for SameDay Medical clinics and primary care clinics in Hoke and Fayetteville.

And this is just the beginning. Over time, SameDay Medical will expand to 12 locations across the Sandhills area and into southeastern North Carolina, Bailey added. The clinics, which are part of Cape Fear Valley Health, are standalone facilities with their own recognizable brand.

adults, and for those without primary care physicians who rely on walk-in clinics, SameDay Medical is a convenient and affordable option.

At SameDay Medical, billing and payment terms are patient friendly. The clinic accepts most insurance plans and charges the usual primary care copays, which cost less than emergency department visits.

And if a patient has no insurance, a healthcare provider will still see them.

“We charge a $50 fee when they come in and then bill the rest,” Bailey said. “We don’t turn people away.”

SameDay Medical is also tailor-made to alleviate the pressure on emergency departments that are often crowded with long wait times by treating patients whose illnesses and injuries may not be true emergencies.

Over time, SameDay Medical will expand to 12 locations across the Sandhills area and into southeastern North Carolina.

“We’re making history right now with this concept,” Bailey said.

LaTisha Thomas, manager of operations at SameDay Medical, serves dual roles with other Cape Fear Valley Health clinical providers.

“I oversee the SameDay Medical staff, ensure the clinic runs smoothly, and everyone has a smile on their face,” she said.

SameDay Medical bridges the gap between patients’ primary care physicians and the hospital emergency room, Bailey said.

“If you can't schedule an appointment with your regular doctor right away because appointments are a couple of weeks or months out, but your illness is not urgent enough to go to the emergency department, then just walk into SameDay Medical, and we'll take care of you,” she said.

“It saves everyone, from patients to providers and hospital personnel, time and money if they can stay out of the emergency department,” Bailey said.

The Journal of Healthcare Management published a research article in 2025 that found “lengthy wait times for appointments with clinicians are a major source of frustration for patients, and difficulties with access” to health care represent a serious public health concern. According to the research article, same-day medical clinics are effective in alleviating the problem and are “a patient-centric means of improving healthcare delivery” by reducing the amount of time people spend seeking treatment.

Open from 8 a.m. until 8 p.m. seven days a week, and typically, SameDay Medical’s most intense rush hours are early morning, when people are heading to work, and after 5 p.m. as they return home.

Appointments are also available as a convenient option for busy patients on the go.

The clinic is also a go-to facility for vaccines to stave off the flu, shingles, COVID-19, and other infectious diseases.

“You don't have to worry about taking much time out of work,” Bailey said. “You can absolutely schedule your appointment early, late, or around your lunch hour.”

For patients with serious illnesses or injuries to bones and joints, the healthcare team will refer them to specialists.

The spacious facility on Raeford Road, with its distinctive royal blue and white exterior, is warm and inviting on the inside with a roomy and comfortable waiting area. The office features seven patient rooms and offers on-site lab and X-ray services and point-of-care testing.

“If you come in and tell us your throat's really sore, we'll swab it, perform a strep test, and it takes 15 or 20 minutes for the result to come back,” Bailey said. “So, whether patients have COVID, the flu, RSV, mono, or any other illness, we can find out while they’re here.”

From allergic reactions to headaches, colds, sprains, and lacerations, SameDay Medical offers treatment for a variety of issues, whether it’s lung ailments like asthma, bronchitis, and flu, infections, insect or animal bites, minor burns and exposure to chemicals, and issues involving workers' compensation claims.

“If a patient is experiencing a true emergency, we’ll get them stable in a patient procedure room, call an ambulance, and have them transported to the emergency department,” Bailey said.

As SameDay Medical attracts more patients, Bailey expects the clinic to have two full-time providers available for every shift and predicts that as word about the clinic spreads, the patient load will grow to at least 80 a day.

“We’re a convenient service in a high-traffic location, so I anticipate we’ll stay busy,” she said.

The facility is still new and not well-known yet, but feedback has been positive so far, Bailey said.

“So far, so good,” she said. “Our patients have been very happy with their wait times, the amount of time they spend with our providers, and the care we're giving them.”
