QUALIFYING CRITERIA
REGISTER FOR THE CLINIC
• Patient must be 5 years old or older • Household income falls within the federal poverty guidelines • Must NOT have Medical Insurance that covers a vision exam and/or glasses, although children MAY have Medicaid or FHCVP • Patient or Parent/Guardian must provide consent to care by completing the Consent Form

INSCRIBIRSE EN LA CLÍNICA
El paciente debe tener 5 años o más. Los ingresos familiares deben estar dentro de los límites de pobreza federales. No debe contar con seguro médico que cubra un examen de la vista ni anteojos, aunque los niños SÍ pueden tener Medicaid o FHCVP. El paciente o su padre/madre/tutor legal debe dar su consentimiento para recibir atención médica completando el Formulario de Consentimiento.
