INSTRUCTIONS
Log in to your Submittable account and click on the submission number next to your Last Dollar Grant application.
Click on the “FORMS” section. Scroll down until you see the “Additional Forms” section, then click “Open” to start filling out the required form.
If you need to save your progress to complete later, you can do so. To access your draft, you will follow these same steps. The blue button will say “Continue”
