A Message From the CEO

Your health matters, and in 2024, we are increasing our Wellness Benefit so we all focus on our health in the coming year. These things are changing in our Wellness Passport Program: 1. Our Wellness benefit is increasing to $750, over the longstanding benefit of $520! All employees who demonstrate their commitment to their own health by earning 100 passport points can earn a deposit into a tax-advantaged spending plan (Health Savings Account/HSA or Flexible Spending Account/FSA) in January 2025. You don’t need to be enrolled in our medical plan to earn your benefit—just take action to improve your own health in 2024.

2. Our Wellness benefit for spouses enrolled in our health plan coverage is increasing to $250!

Enrolled spouses who get their annual physical and then complete a health risk assessment will earn a reward for 2025.

3. Earning the Wellness Benefit is easier than ever! Completing your annual physical and then taking a health risk assessment online gets you across the finish line to 100 points and earns you the $750 benefit. You can still earn an additional 300+ points through other preventive actions like twice annual dental cleanings, meeting biometric health range goals, participating in company-sponsored community service events or completing Health Coaching programs. We will be holding drawings throughout the year to reward our employees who get healthier by taking action and earning more points.

What is Open Enrollment?

This is your time to ELECT, DECLINE, or CHANGE your Medical, Dental, and Vision coverage and/or ENROLL in or CHANGE your election in a tax-advantaged Health Savings Account (HSA) or Flexible Spending Account (FSA).

The Open Enrollment online portal is available Monday, October 30th (7:00am) through Wednesday, November 13th (11:45 pm) for the January 1—December 31, 2024 plan year.

After Open Enrollment, you can ONLY change your coverage or add/remove dependents if you have a Qualifying Life Event (marriage, birth or adoption, change in employment status, divorce, death, gain or loss of other coverage, new student status). If a Qualifying Event occurs, you have 30 days from the date of the event to make the change.

Your Guide to Online Enrollment Setting up your Portal Access

Full-time employees are eligible for ALL of the benefit plans offered.

You may also choose to enroll your eligible dependents in any of our health benefits.

SmartBen is our online enrollment tool. The site is accessible 24 hours a day, 7 days a week.

The following tips will help you prepare and complete the online enrollment process.

What You’ll Need to Enroll: • •

SSN and Date of Birth for any spouse or dependents you currently (or plan) to cover If you enroll in the health savings account, you will need beneficiary contact information

Steps to Complete Your Enrollment:

1. Log in to crowder.smartben.net (Google Chrome or Microsoft Edge) or log into the SmartBen NOW mobile app.

Username: Crowder + Employee ID (Ex. Crowder12345) Password: DOB (MMDDYYYY) Keep your password confidential.

If you do not know your employee ID, Login to your ESS portal and click on profile. You can also contact anyone in Employee Services by calling 1.800.849.2966 (ask for Employee Services) or emailing employeeservices@crowderusa.com

Although your first enrollment is your DOB, all new employees enrolling in the SmartBen portal will be required to change their password. If you don’t know your password, you can click on “I forgot my password” or contact anyone in Employee Services.

2. Once logged in, you will see a Benefits Enrollment box containing a countdown of the number of days remaining in the Open Enrollment window. a. Select Begin Enrollment b. Select Open Enrollment under available enrollments 3. Review and Elect Benefits. a. You will enter the Enrollment process at the Benefit Manager Page b. Use this guide to review your options and make your benefit elections c. To enroll or make changes, click on a benefit name If you are enrolling a spouse or dependent in coverge, select Manage People to add dependent records for enrollment.

• Employee: Review your personal information and make updates if necessary • Spouse/Dependent: Select Add a Spouse or Dependent to add your spouse and/or dependent’s information if you are enrolling them into a benefit plan. Click Save when you are finished.

NOTE: Adding spouse/dependent/beneficiary records to this section does NOT assign them to applicable coverage. Once all records have been added, select Continue. Official assignments to coverage will occur within the benefits as you are making your desired elections.

4. Once within the benefit, select the desired level of coverage under Who’s Being Covered. a. This will update the plan options to the appropriate level of coverage b. Once you’ve decided on your desired plan and level of coverage, click Select under the applicable plan c. The option will turn green and you can select Continue d. Once all requirements for the benefit have been updated, select Continue 4

5. Once all elections are complete, each benefit will have a green light. To finalize your elections, select Continue from the Benefit Management page. a. Review your Elected Benefits to confirm each is listed as expected i. If changes need to be made, select Return to Lights ii. Health Savings Account (HSA) What is a Health Savings Account?

This tax-advantaged savings account is offered through HSA Bank to help you save for medical expenses (now and in the future) with tax-free dollars. It is available only if you are enrolled in a qualified high deductible health plan like the Crowder Advantage Health Savings Plan. The IRS significantly increased the maximum allowable for individuals and families in 2024. Make sure to check your annual commitment and weekly contribution rate to ensure you are getting the full advantage of the savings available to you and your family.

How do I use a Health Savings Account?

Once you enroll in an account through HSABank, you setup a pre-tax contribution to be deducted weekly from your gross pay. You can adjust your weekly contribution rate at any time, but you are limited to an Annual Maximum Contribution by the IRS, based on your coverage tier in the qualified plan. If you have single coverage, the individual max applies. If you have coverage for your spouse or family, you will be limited by the family max.

Only the balance of your account is available for use to pay expenses.

Employer contributions are allowed, BUT the amount contributed reduces the maximum allowable amount for the plan year. You can choose to transfer funds to investment accounts (no minimum balance required), and the interest/investment income accrues tax-free.

Your account balance “rolls forward” every year, and because it is “individually owned” you take it with you if you change companies or retire. In addition, you can use your HSA to pay COBRA or Medicare premiums.

Individual Maximum: (Employee Only Coverage) Annual Contribution = $4,150

Family Maximum: (Employee + 1 or Family Coverage) Annual Contribution = $8,300

Catch-up Provision: Employees 55 and older can contribute an additional $1,000

Flexible Spending Account (FSA) What is a Flexible Spending Account?

Crowder partners with Flores & Associates to offer our employees TWO Flexible Spending Account (FSA) options: The Medical Reimbursement FSA is a perfect partner for the Advantage Health Spending Plan, and helps you pay for out-of-pocket health care expenses such as medical, dental, prescription drugs, eyeglasses, contact lenses, and hearing aids. You can contribute up to $3,200 in 2024, and the funds are available through the Flores Benefits Card or by reimbursement. Employees may carry-over up to $640 from 2023 if enrolled for 2024. The full amount committed for the plan year is available on your effective date of insurance.

The Dependent Care FSA allows you to get a tax-advantage on your dependent care expenses so that you (and your spouse, if you are married) can work or go to school. You can contribute up to $2,500 (single) and $5,000 (married) and the care must be for a dependent child under age 13 or a dependent of any age that lives in your household and is incapable of care.

To manage your medical reimbursement or dependent care FSA account, go to www.flores247.com. You can sign up for text messages to alert you to activity and balance information, and download the Flores e-Receipt app to digitally capture and transmit receipts.

Review Your Beneficiaries on File!

When you sign up for benefits such as life insurance, 401(k) or a health savings account, you are asked to name a beneficiary to receive the proceeds should you pass away. Oftentimes, employees fill out the forms and move on, maybe even forgetting about which beneficiary is on which account. However, it is good practice to periodically take the time to review your beneficiaries, especially when you have a significant change in your life such as a marriage, birth of a child, or a divorce. These life events do not automatically change a beneficiary election on file. Your original designation remains in force whether it still reflects your wishes or not.

2024 Wellness Passports

All employees (regardless of coverage under Crowder’s insurance) and covered spouses can participate in the wellness passport to earn passport points towards an annual wellness benefit. Once you have met your wellness passport requirements and submitted all documentation to wellness@crowderusa.com, you are eligible to receive $750 deposited into your Health Savings Account (HSA) or Flexible Spending Account (FSA) in January 2025.

Spouses enrolled in Crowder’s insurance who meet wellness passport requirements and submit all documentation to wellness@crowderusa.com will receive an additional $250. All passports and documentation must be submitted by October 1, 2024.

Remember, all annual well visits and vaccines are covered at 100% under Crowder insurance!

The Employee Assistance Program (EAP) is a FREE benefit to you and any immediate family member in your household. This benefit gives you and your family members FREE access to a counselor for up to 6 sessions (per issue) per calendar year. At Crowder, you and your mental health and well-being are important to us! Did you know that the Construction Industry has the second highest suicide rate in the United States. With 123 suicides per day, which represents 1 death every 11 minutes, we want to share the resources we have available to each employee and their families including the EAP, 988 Suicide and Crisis Lifeline and our Cigna mental health support services.
