Crossroads at Lakewood is a five-year old, dedicated Memory Care community, purchased by Crossroads Senior Living, a non-profit organization, in 2020. We are the only full-service, allinclusive, dedicated dementia care community in Lakewood.

Crossroads Senior Living is very different from most other entities providing senior care. The purpose of Crossroads is to take care of seniors and their families, and the profits are granted to one of the best charities in the United States, Maranatha Volunteers International

As a specialist in providing memory support for those with dementia, Crossroads at Lakewood understands the importance of a scheduled rhythm to each day. However, we also believe in providing personal care to each resident.

This means embracing each residents’ unique personality and paying attention to their specific needs. We care for your loved one as you would at home—with love, care, patience, and dignity.

Crossroads Senior Living creates a residential, homelike environment designed for seniors who need some assistance with daily activities such as bathing, dressing, medication reminders, and specialized dementia care programs. Our facilities preserve the rural culture so important to our residents, yet provide the comprehensive assistance desired. We focus on wellness and keeping residents as independent as possible by providing a tailored plan of assistance.

The Crossroads staff is made up of compassionate people who truly enjoy and understand seniors. We pride ourselves on providing exemplary one-onone assistance for every individual. Our residents and their families can rest assured knowing they receive the most attentive care possible.

With exercise classes, a whirlpool spa, 24-hour staffing and scheduled transportation, you and your family will rest assured knowing that your new life at Crossroads is easier and safer.

Affordability

There is a range of comfortable and affordable options in senior living at Crossroads. We have various apartment layouts and floor plans to fit your needs. The good news is that you don’t have to invest your life savings to become part of the community, and there are no buy-in fees. You pay only a monthly rent.

RATES

Crossroads at Lakewood offers a simple month-to-month rental plan.

Semi-private apartment: starting at $6,135 per month

Private apartment: starting at $6,835 per month

One-time Community Fee: $2,500

Second person fee (room and board only, care is extra): $1,500 per month

EXTENDED CARE RATES

Crossroads at Lakewood is an all-inclusive community. Your rent covers all your care needs.

AMENITIES

• A fully supervised, success-oriented activities program

• Regularly scheduled social events with family involvement welcome

• Medication management

• Routine assistance with daily living activities, such as bathing, dressing, and hygiene

• Comfortable and attractively decorated living rooms and activity rooms

• Electronically monitored security system

• Paid utilities

• Support groups and educational programs

SUPPORT SERVICES

Our residents receive a comprehensive health assessment to determine the type and amount of care and social services needed, including the following:

• Coordination of care with home and hospice providers

• Rounding physicians who visit residents regularly in the privacy of his or her apartment

• Resident-specific activity programming, including in-house activities, group activities, and outings

• Therapeutic diets

RESPITE CARE

Respite care: $250 per day

Our community provides an option for those needing memory support services. You may consider using respite services for family vacations, caregiver unavailability, or trial stays. Daily fees for respite care are collected in advance and range from 14-30 days.

OTHER

• A reservation deposit of $1,000 is required and will hold a room for two weeks.

• A late fee of $100 if rent is not paid by the 10th of the month.

•Day care - $150 per day
