We are excited to present our proposal to join your design team for the Community Smiles new Milwaukee Clinic at Aurora Health Care’s health and wellness hub, The Beacon. We understand that the project scope includes:
1. A leased space dental clinic finish-out of approximately 6,800 rentable square feet. The design process will require close coordination with Aurora Health Care representatives, Chamberlin, LLC, as well as Community Smiles team members.
2. Architectural, interior design, structural, and MEP design services will be provided throughout all design phases, construction, and certificate of occupancy. Monthly visits to the project site are included in our base services.
3. Architectural, structural and MEP requirements will be coordinated with Owner’s selected dental equipment and your preferred equipment vendor(s).
4. An allowance is provided in our fee proposal for coordination of the Owner’s furniture selections with the team’s selected interior design and finishes concept.

AFFORDABLE DENTAL CARE (MERCY DENTAL MISSIONS)

The growing need for additional space and treatment rooms prompted Affordable Dental Care—now Mercy Dental Missions—to relocate to a new tenant suite designed to expand its life-changing, nonprofit services and reach more patients. The new space offers several advantages, including convenient parking, north facing operatories filled with natural light, and close proximity to complementary services.
Patients are welcomed into a bright, inviting reception space that immediately sets the tone for their care appointment. Bold colors and a dynamic custom wall mural reflect the practice’s brand and mission, creating a sense of energy and purpose within the waiting area. The team collaborated on the creation of a dedicated children’s zone, designed to contain activity and sound while preserving a calm, comfortable experience in the main waiting space.
The suite’s long, linear layout aligns operatories along the single window wall, maximizing access to natural light within the patient zone of the practice. Offices and support spaces across the patient corridor feature borrowed lites and full- glazed doors, allowing daylight to extend throughout the entire suite. The patient corridor is articulated with wallcovering accents and false wood beams, breaking down its length while introducing warmth, rhythm, and architectural interest.
Each hygiene and treatment room ceiling is layered with perimeter gypsum board soffits and acoustical ceiling tile, striking a balance between visual texture and acoustic comfort. The soffits form an ideal backdrop for colorful, patient-distracting lighting features that encourage relaxation and ease. Coordinating wallcoverings frame patient monitors at the toe wall, introducing an added layer of visual interest to each operatory using budget-conscious means.
Instrument sterilization, imaging, and laboratory functions are centrally located to streamline workflows and reduce footsteps for the care team. A spacious staff lounge connects directly to a multipurpose room, supporting training, collaboration, and team events.

When the practice welcomed a new Partner, it sparked an opportunity to rethink how the existing space could better support the growing team and patient volume. The transition called for four new operatories—two bright, efficient hygiene bays and two larger, flexible quiet treatment rooms designed for longer, more private procedures. This expansion set the stage for a broader transformation of the patient and care team experience as well.
To make room for a more comfortable waiting area, the team looked to an adjacent former tenant space that had long been used only for storage. By opening it up and weaving it into the existing suite, the waiting room could grow both in size and in function. New amenities—a coffee and beverage bar, a children’s nook, and an internet café—now offer patients and families more choice in how they spend their time before appointments.
Reimagining the arrival experience became just as important. The front door was repositioned to face the street and paired with a welcoming, porch-like entry element that signals a friendly, community-minded practice brand. Inside, the greeting desk and frontof-house operations were expanded to comfortably support the increased operatory throughput and more seamless patient flow.
Behind the scenes, the sterilization area was enlarged to improve access and optimize infection-control workflow via a galley-style layout. The new hygiene operatories take full advantage of the existing vaulted ceiling and skylights, creating an open and airy feel, while the new flexible treatment rooms are tailored for longer procedures and specialty use—ready to adapt as the practice continues to grow.

This newly leased space for a married pair of General Practitioners was designed to replace an outdated office located just 800 feet away within the same retail complex. The goal was to create a modern, streamlined environment that seamlessly integrated advanced dental technology within a highly efficient 2,700-square-foot footprint.
While the suite presented design challenges—including a slightly constrained size, limited natural light at both the front and rear, and low structural beam heights—these limitations became opportunities for creative solutions. A series of layered ceiling planes, accented with faux wood beams and integrated uplighting, enhances the perception of height and openness throughout the space. In both the waiting and clinical areas, soft blue ceiling tones evoke the sky, reinforcing a sense of lightness and the illusion of a larger space.
Personal touches further enrich the patient experience, with curated enlargements of the doctors’ own travel photography displayed along the patient corridor—adding warmth, authenticity, and a sense of connection to the care journey.

Project manager for a single-story addition to an existing clinic. The estimated area of the addition is 8,000 SF. The new steel-framed structure wraps around the western portion of the existing structure on the north, south and west sides and is independent of the existing structure. The roof elevation of the new structure is around the existing structure height and supports a new RTU and screening. The project included a driveup pharmacy canopy is cantilevered from the biulding framing and a mobile MRI structure.
New Middle School, Beaver Dam Unified School District | Beaver Dam, WI
Project manager for anew steel-framed, 166,000 SF, two-story building of Type IB construction. The building includes classrooms, a new gymnasium, graphic arts space, performing arts space that includes an auditorium, a two-story Commons area, library, and other support spaces. The building is partially built into a sloped site, requiring retaining walls..
NorthPointe Birth Center | Beloit, IL
Structural engineer for a new single-story wood framed structure with two glulam canopies. One of the glulam canopies is a drive-up canopy. On the rear of the building grade dropped off, so the foundations were designed to accommodate some amount of earth retention. On the lobby side of the building a significant portion of the wall was glazed so a wind girt and support system was designed out of wood to match the material of the rest of the building structure.
Aspirus Iron River Clinic | Iron River, MI
Structural engineer for a new single-story structure with a walk-out basement. Because the building was on a slope, earth retention was required on the high side. The roof structure was designed to support large mechanical units and associated snow drift. The structure was steel framed with a slab-on-deck over steel beam floor and a steel joist and steel beam roof.
Union Grove High School | Union Grove, WI
Project manager for renovations of an existing high school. Project includes construction of a new covered connection between the agri-science building and the main high school building and a secure entrance; renovation of existing tech ed space and addition of a new tech ed space; construction of a new secure entry on the south side of the school; and renovations to the existing interstitial space to support a new science labs addition.

FEE PROPOSAL
Based on the information provided in the Request for Proposal dated January 19, 2026 and our experience with dental facility planning and design, we are proposing the following percentage-based fees to complete the project:
8.5% of the construction cost (based on an assumed construction cost of $2,000,000) + a $3,600 allowance for interior design coordination of furnishings + a not-to-exceed $4,000 for project reimburseables (plan review fees, travel, printing).
For clarity, our fee includes:

• A Structural Design allowance for confirmation of support for overhead dental equipment, floor penetration coordination for dental chairs, and miscellaneous structural collaboration as required. Major structural rework is not included.

• Design for low voltage, data, card access, cameras and security will be for rough-in only
