GETTING STARTED Connie Provider Directory is a health organization search tool for finding other health organizations or providers.

The directory listings are composite of multiple public and private data sources, including provider name, address, phone number, area of specialty, organization affiliation, and more.

Leap Orbit, creator of the Provider Directory platform, and Convergent, aggregates and algorithmically masters hundreds of national and regional provider datasets to deliver a robust database of provider information, combined with a streamlined, CMS-compliant, FHIR 4.0 API and a broader set of proprietary APIs.

Connie Provider Directory allows for a name misspelled or either first name or last name to be added to the search field.

Searching “Cro” without including any further information will result in a list of possible providers.

Select the 'show' button located next to the provider's name to see more detailed information about the provider.

To use the distance search option, include the following required fields: Street Address City State Zip Code

After selecting 'distance search' users must include the radius distance in the search filled, which defaults to 10 miles, but may be adjusted. Selecting “Search” displays results by distance from the radius in search criteria.

To clear search results and begin a new search, select the “Clear” button.

This will bring users to a new search page
