GETTING STARTED InContext is an embedded application that allows authorized users to receive specific information concerning a patient's clinical history while working in the context of their EHR. Application Program Interfaces (APIs) have been designed and developed which allow EHRs to query the health information exchange for specific patient information.

The queries themselves and the information that is retrieved from the health information exchange are flexible so that the queries can be built logically anywhere into the provider's workflow and the data retrieved can be displayed in a fashion that makes the most sense for both the EHR technology and the person viewing the data.

“As a practitioner, I want a seamless way to view a patient’s chart, including information from other providers that don't use my EHR without leaving my EHR.”​

The Patient List displays all possible matches based on the patient information your EHR has sent to Connie.

Each patient will have a “Match Score” which will tell you how closely the patient matches the information sent to Connie. A patient can either be “Probable” or “Possible”.​

To return to the patient list, click the Back icon.​ Tables can be searched and filtered using the table navigation buttons.

In larger screen sizes, the app menu will appear on the left side.

Use this menu to navigate to the other sections of the app.

This menu collapses once the screen is minimized. ​

The “Clinical Data” section includes Health Records and Prior Visits.​ Encounters displays information regarding the patient’s previous inpatient or outpatient encounters.

This includes date and source and may also include additional information such as Discharge Diagnosis and Admit Reason.​ Health Records displays the patient’s Radiology reports, Laboratory reports, and Clinical Notes received from Connie participants.

Orange flags next to an individual record indicate a value that is abnormal (per the sending organization). A camera icon next to a radiology result indicates that there is an image that can be viewed as well.

Structured Documents displays a table of available Clinical Documents (CCDAs) by Date, Source, Title, Type and Size (a proxy to assist users in estimating amount of content and time to display.) When a user clicks a row the respective CCDA will be displayed.

The “Care Coordination” section includes Care Alerts, Care Team, and Advance Directives.​

Care Alerts are high priority care coordination information meant for complex patients.

Care Alerts include “need to know” info that inform decision making. Click “View” to see more info on the alert.

Care Team displays the organizations and providers that have a relationship to the given patient.

This will also include additional information about the patient’s Care Program and Care Manager.

User feedback provides the ability to submit comments and/or suggestions for the app.

By navigating to the App Menu, a user can click on the 'Feedback' option.
