Health Forums and Social Prescribing Networks We facilitate three forums Alton, Fareham & Gosport and East Hampshire and Havant and are usually held 4 times a year, 3 virtual meetings and one face to face.

•

Our health forums are a meeting point for the health services and more than 50 community groups that support people with health conditions.

•

The Forums allows patients to influence how health services meet their needs and become more effective.

Our Social Prescribing networks enable the sharing of best practice and learning across Hampshire and Isle of Wight.

•

To develop effective working relationships and provide mutual support for providers including further improving understanding and relationships between health and voluntary, community and social enterprise sector partners.

•

To highlight and find solutions to common issues including promoting local activities which have opportunities and potential for replication and expansion.

•

To identify and undertake joint future working opportunities where appropriate
