DEEKSHA AGRAWAL The OhioHealth neurologist is a newcomer to the list. What drew you to neurology? While going through the basic science curriculum and rotations in medical school, I found the subject matter in neurology most interesting. At that time, I also had the opportunity to interact with some very interesting patients with rare diseases. I liked the challenge of “localizing the lesion” and figuring out what part of the nervous system may be affected. What is a common ailment you’re seeing these days? Due to the social, financial and family stressors from the COVID pandemic, many of our patients are experiencing worsening headaches, seizures or functional debility. It has been important to discuss stress-relieving techniques as part of the management of these conditions. When should someone be concerned about a headache or frequent headaches? A patient should be concerned especially if there has been a change in the characteristic of the headache. This may be the location, severity or other features of the headache. Some other “red flags” can include headaches with fever, positional changes, exertional headache or those headaches that are new, especially in the elderly. Do you recall a particularly gratifying case—and what made it gratifying? It’s difficult to pinpoint one specific case, but in general it would have to be any case where the patient had a drastic improvement in his or her quality of life. It is gratifying to see that with treatment, a patient is now able to actively participate with his or her family or perform his or her job well due to an improvement in his or her neurological condition.

TANYA GURE The Ohio State University geriatrician has been on the list continuously since 2015. What did the COVID-19 pandemic reveal about the care of older people in this country? Our nation faced painful realizations about nursing home and congregate living spaces, where the highest proportions of deaths occurred. We also saw disparities in mortality outcomes between high- and low-resourced nursing homes. In addition, we’ve learned painful lessons about the impact of prolonged isolation on all of us, but especially among older adults.
What is the proudest moment of your career? My Ohio State University Wexner Medical Center colleagues extended hands and resources to the nursing home community where our geriatric team works, offering much-needed support at a time of great desperation during a COVID-19 outbreak. We quickly assembled an on-site monoclonal antibody infusion service for over 100 assisted living and nursing home residents with acute COVID-19 infections, through collaboration of OSU infusion services, nursing and the divisions of pulmonary and infectious disease. How has geriatrics changed since you started practicing? The biggest change I’ve witnessed over my 15 years of practice is the broadening of telemedicine and home monitoring services. Prior to the pandemic, there was low receptivity among older adults to telemedicine visits. Therefore, most geriatric specialist care occurred through face-to-face visits in the home, clinic or another community setting. The public-health-mandated home restrictions during the pandemic facilitated a fast uptake of remote and virtual visits by older adults. In-person visits are still highly valued by patients and providers, but now there is a greater appreciation of how telemedicine care can be effectively utilized.

PHILIP SANTA-EMMA The Mount Carmel Health System physician is the only hospice and palliative medicine specialist on the list. How do palliative and hospice care differ? Both hospice and palliative care work to palliate—or lesson—symptoms, and both offer supportive care and treatment, but they differ in eligibility and focus. Palliative care is a medical subspecialty that focuses on the care and support of patients, along with their families, who have serious medical illnesses. It is unique in medicine in that it is not a specialty focused on a specific body part, but rather on the whole patient who is ill. Hospice care is the management of patients who are in the late phase of incurable diseases. The primary role of hospice is to relieve suffering and improve a patient’s quality of life. What is the biggest challenge you face in palliative care? One of the biggest challenges stems from the misunderstanding that palliative care is the same as hospice care. There has been confusion, since a lot of palliative principles are shared with hospice—the relief of suffering, the focus on what makes your own life unique, what quality care means to you as an individual, etc. Palliative care is appropriate anywhere and anytime across a patient’s disease—from the initial diagnosis throughout treatment and into recovery. What types of patients do you typically treat in palliative care? Patients and families often think that palliative care and hospice care are only for cancer patients, but I see patients with advanced heart failure, renal failure, emphysema, stroke, as well as advanced cancer. Palliative medicine and hospice care are not focused on the disease, but rather on the patient who has the disease, their family, their support system and their own individual goals of care.
