Minimum requirements: 2 years of experience, an RN or BSN degree, and an active Colorado nursing license. CNAs needed part-time to full-time to supplement our current staff. Our CNAs provide Home Health and Hospice patient care in the home, in nursing facilities and in assisted living facilities. An active Colorado CNA license is required. Experienced SLP needed part-time to serve our mountain and foothills communities, specifically within our service area along the I-70 corridor. A Master’s degree, 4+ years of experience and an active Colorado license are required. Social Worker needed to be a vital member of our interdisciplinary Palliative Care and Hospice teams based in Evergreen, Colorado (located in the foothills only 20 miles from downtown Denver). This position will provide comprehensive emotional support services for our patients and their caregivers dealing with issues of illness and loss. While this position will primarily work as part of our Palliative Care and Hospice teams, there are also opportunities within the support services team for facilitating group counseling and community education programs. Additionally, you will work with patients and caregivers to develop care plans, advanced directives, and to access community resources. The position is 32 hours a week, with the potential to grow to full-time, is benefits eligible, and is based out of the Evergreen offices, but will require travel to homes throughout the foothills of Clear Creek, Gilpin, Jefferson and Park counties. Requirements: Master of Social Work (MSW) or equivalent experience, ability to demonstrate your competence for supportive counseling and clinical case management, and ability to be an independent team member with strong communication skills; Preferred: A Licensed Clinical Social Worker (LCSW) with experience in healthcare, aging, disability, caregiving, endof-life planning, critical conversations, crisis counseling, and/or grief. Home health, hospice and geriatric experience preferred for all clinical positions. Our service area encompasses the mountain communities within Jefferson, Clear Creek, Gilpin and Park counties (along both the I-70 and 285 corridors). The Community Development Planner performs land use, zoning, and short and long-range planning functions for the City. This professional position shall be the primary point of contact for information flow among developers, engineers, City departments and applicants for all assigned projects to ensure the exchange and presentation of pertinent information to the Planning Commission and City Council. Occasional evening meetings and the ability to work a flexible schedule are required. The Community Development Planner works in the Administrative Department and reports to the City Administrator. Bachelor’s degree in Urban and Regional Planning, Community Development, Public Administration or related field. Master’s degree desirable. Minimum two years progressively responsible experience in municipal planning. AICP credential preferred. This is a full-time, exempt position. Salary range $68,302-$98,462/year plus excellent benefits.
