Dear New Wildcats and Parents: We are so excited to welcome you to the Organization. We are looking forward to a new and exciting season. We look forward to interacting with you and your children in the months to come. Our goal this year is to make this season a positive and fun experience for your child. REGISTRATION PROCESS Each youth participant and parents are responsible for completing, signing, and returning all forms required. These forms must be completed and turned in by the first day of the start of the season. Parents need to read all information carefully and sign forms indicating their understanding of the expectations set forth by MWC. All members must have all debts paid to MWC prior to current season to be eligible for registration of current season. Required Documentation: The following documents are required to complete the registration process: ▪ A current physical for 2015. ▪ A copy of the birth certificate must be submitted. We will need to see the

original before accepting the copy. ▪ Acknowledgement page from Handbook.

FINANCIAL OBLIGATIONS Before you consider this or any program, please carefully consider the financial commitment involved. At MWC, we are as up-front with our costs as possible and try to overestimate costs at the beginning of the season to make financial planning easier for our families. Please see the attached Program Package Information Sheet. Payments are due on the date specified. After that, a $10 late fee will be charged. The Michigan Wildcats Parent Information Guide © 2015


ATTENDANCE REQUIREMENTS: Specific guidelines for excused absences ▪ Parent must contact the coach and/or coordinator prior to practice or event. ▪ A doctor’s note must be provided for missing more than 3 practices and/or events in a row because of sickness. Injuries: If an athlete is injured, he or she must provide a doctor’s note explaining the restrictions and a date that an athlete may resume normal activities. Injured athletes are to attend practices. If it is a severe injury and an extended leave is required, MWC will allow an athlete to take a leave from the team without losing their position on the team. Examples of excused absences: include illness, school events that earn a grade, injuries, vacations, illness or death in the family. PRACTICE RULES: It’s important that athletes attend all practices. Absences hinder the performance of the team goals. Athletes should stay with their team when dropped off for practice. No athlete should leave their practice area without the permission knowledge of the team coach. Athletes should wear identified practice clothes or uniforms required to perform their sport! Further details will be given by the Athletic Director and/or Cheerleading Coordinator.

PRACTICE RULES: Athletes must bring their own bottle of water to practice. It can get very hot at practice and athletes need to drink water at every break. Do not leave empty water bottles on the field area. Have pride and keep the environment clean! A FEW REMINDERS FOR OUR PARENTS:

It is important that athletes arrive to practice on time and are picked u promptly when practice is over. Parents are expected to retrieve their child/children no later than 10 minutes after the end of practice. There will be a late fee assessed for late pick-ups: $5 for the first ten minutes, then $5 for every ten minutes after that. Please slow down when entering or exiting the practice field parking lot. Please do not allow small children to be left at the practice field unsupervised. This can be a safety concern. Therefore all rules would be greatly appreciated as follows: ▪

If it is raining when arriving for practice, stay in the area. If it is a light rain, we will practice.

If the rain gets heavy or there is lightning, athletes may be sent home. Always check with the team coach.

Please do not approach the coaches during practice time. Ask questions before or after practice. Their attention needs to be on the athletes at all times.

Parents are responsible for their child/children when athletes are released on game days.

COMMUNICATION: WEBSITE & E-MAIL: Ongoing Communication will be made via email, text, website, and social media tools. Our marketing package is underway! If there are cancellations due to weather, we will try to send a text message at least one hour prior to the start of practice. We must have an updated cell phone number and/or e-mail address at all times. If you do not have internet access, please call the coach directly. If you prefer not to receive e-mails from us, please notify us. It will be your responsibility to contact someone from the team for all information. FUNDRAISING ACTIVITIES: All athletes and parents will be required to participate in all fundraising projects throughout the year. Parents are also asked to provide volunteer hours to Michigan Wildcats Youth Club, we can’t be successful without your help, and there are many activities that will be coming up. Please help our organization experience success through active support. ORGANZATION REGISTRATION FEE: $125.00 (Includes: Rental of Uniform, Banquet, Trophies, Insurance and Miscellaneous Items)

FAMILY PARTICIPATION Parents must participate in the club for it to function well. We need every family to volunteer no less than 5 hours. If you do not complete your volunteer hours, you will not receive a refund of your $ 50 volunteer fee. This is a per family fee and is paid at equipment pick up. Your refund will be given at year-end banquet or applied to next year’s registration. DYFF MICHIGAN WILDCATS YOUTH CLUB Programs & Services Consent, Waiver and Release of Liability, Photo and video release On behalf of my minor child _______________________, I hereby give permission for my child to participate in DYFF’s Michigan Wildcats Youth Club Programs. I hereby warrant that both I and my child are familiar with the risks associated with participation in activities. I acknowledge that my child’s participation in this program is wholly voluntary.

I do hereby agree and consent to my child’s participation in MWC and do assume all risks and hazards which are part of the conduct of the associated activities. I hereby release, absolve, indemnify, and hold blameless Michigan Wildcats Youth Club, Detroit PAL, its officers, directors, employees, contracted employees, independent contractors, instructors, agents, organizers, and volunteers of any and all liability for damage, injury, or expense of any kind arising out of or connected with my child’s participation in this organization. I understand that in case of a medical emergency, my own personal medical plan will be used. As a condition of participation in MWC the youth named in this application, I acknowledge that I have read this consent form, and knowingly, on behalf of my child, assume all of the risks associated with participating in any way with MWC programs. As consideration for being permitted to participate in MWC programs , I hereby agree that I, my assignees, heirs, distributes, guardians, and legal representatives will not make claim against, sue or attach the property of Detroit Public School District, or their Affiliates or the supplier of any of the equipment used in these activities for injury or damage resulting from negligence, failure of care, omission or other acts, howsoever caused by an employee, agent or contractor of DPS incidental to my child participation in these activities. I hereby release, to the fullest extent permitted by law, the DWC, Detroit PAL, DPS and the affiliates, from any and all actions, claims, demands, or liabilities that I, my assignees, heirs, distributes, guardians, and legal representatives now have or may hereafter have for injury or damage resulting from my son/daughter’s participation in these activities irrespective of whether the same is based on breach of warranty, negligence, strict liability or any other theory or recovery. LOCATION OF POTENTIAL PRATICE & GAME SITES DYFF’s Michigan Wildcats Youth Club would like consideration to use one of the following sites as a practice field: Paul Roberson, Malcom X Academy, 2585 Grove Street, Detroit, MI. 48221 Johnsons Recreation center, 8640 Chippewa Ave. , Detroit, MI. 48235 Palmer Park, 19013 Woodward Avenue, Detroit, MI. 48203 Tindal Recreation Center, 10301 W. Seven Mile, Detroit, MI. 48221 The DYFF’s MWC hopes to have coordinated efforts with Detroit PAL and the City of Detroit Parks & Recreation on practice home game fields. We would like to play our games at Central High School, 11600 Linwood, Detroit, MI 48206, as our home field. Interior DYFF’s Michigan Wildcats Youth Club intends to eventually purchase a Youth Complex to offer full time programing to youth. [TBD] Hours of Operation DYFF’s Michigan Wildcats Youth Club will program year around in schools, after school and summer programing for athletic, educational, and mentoring services to youth.

We anticipate the acquisition of property to conduct its business and ongoing programing for youth. Outdoor sports will be played on public school and park property. Indoor sports location have yet to be determined.
