Patient Centered Medical Home In July 2013, Central North Alabama Health Services Inc.’s (CNAHSI) Family Health Centers received Primary Care Medical Home (PCMH) certification by the Joint Comission. A Patient-Centered Medical Home is a system of care in which a team of health professionals works together to provide all of your healthcare needs. We use a team approach to communicate and coordinate your care and provide the best possible outcomes for you.

YOU, the patient, is the most important part of the Patient-Centered Medical Home. When you take an active role in your health and work closely with us, you can be sure that you are getting the care you need.

After choosing your primary car eprovider, you will be assigned a team of dedicated halthcare professional who work to meet all of your individiual needs.

YOUR CARE TEAM Primary Care Provider (PCP)

Nurses/Medical Assistants

Your PCP will explain your medical care, listen to your feelings and questions, and arrange for additional care.

Your support clinical staff will gather information for visits, provide patient education, and coordinate delivery of test results.

Receptionists/ Records Staff Your support clerical staff will update demographics and information, provide patient reception, and answer questions.

Case Managers/ Specialists

Case managers offer case management, coordination of preventive care, coordination of inernal/external care, and referrals.
