Viewpoint Dear Reader,

As the new Director of the Leonard Schanfield Research Institute (LSRI), I am particularly grateful to have this opportunity to introduce you to the current issue of LIFE Magazine. Having recently attended the 10th anniversary of Celebrate CJE, this outstanding event reminded me of how fortunate I am to work for an organization that has helped more than 23,000 older adults and their family members just in the past year. CJE is truly committed to helping people age with dignity and respect. It’s an honor and privilege to work for an organization that has had such an impact on the health and well-being of seniors in our community.

What many people do not know is that I have really come full circle; my career in research actually started 15 years ago at LSRI. This department is where I first began to learn and understand the crucial role research could play in improving the quality of life for older adults. I am happy to be back here and just as eager to apply the skills I have learned since my initial work at CJE to develop a research strategy that will help CJE achieve its overarching mission.

One thing that sets CJE apart from many other eldercare agencies is that it values having a Research Department. Many of our research projects are initiated with the hope that they will develop into programs and services that become integrated into CJE’s comprehensive network of care. Our experience in the field of aging, our local and nationwide partnerships and our proactive work in the aging arena all make the LSRI an important catalyst of social and applied research. With our dedicated staff, innovative direction, strategic alliances and supportive community, our ultimate goal is to conduct applied research and evaluation to 1) improve the quality of life of older adults and their caregivers; and 2) contribute to the development, innovation and dissemination of equitable models of service and healthcare.

This issue of LIFE also features a lively Q & A with Tamar Heller, Ph.D., a respected researcher in the field of mental health disabilities. She touches on some of the special causes that are observed during the winter months: Mental Wellness, Financial Wellness and Jewish Disabilities. We also tip our hat to February’s American Heart Month with a closer look at Lieberman’s Center for Heart Health and the introduction of its new Pulmonary Rehabilitation Program.

WHERE RESEARCH REIGNS

For Dr. Amy Eisenstein, her appointment in 2015 as Director of CJE’s Leonard Schanfield Research Institute (LSRI) was especially meaningful. Over 15 years ago, when LSRI was still nascent, Eisenstein launched her career as a Research Assistant at CJE. Today, CJE is one of the few social service agencies in the country that continues to embrace its own in-house applied research department.

Perhaps it is that early experience at CJE which helped to shape Eisenstein’s longstanding research interests that are primarily related to aging issues, health and disease, with a focus on measuring outcomes associated with long term care and community-based living. Her experience and training have also focused on the use of mixed methodologies, including surveys, secondary data analysis, interviews, focus groups and content analysis.

As the new director of LSRI, Eisenstein first focused her attention on getting a pulse on CJE’s research priorities. In interviews with managers and staff, some common themes surfaced, including: brain health, Parkinson’s disease, population health management and management of multiple chronic conditions. She was excited to learn that staff was forward-thinking about the information and directives that an internal research department can provide to better understand the expectations, outcomes and impact of current and proposed programs.

About the Leonard Schanfield Research Institute The Leonard Schanfield Research Institute, which was established in 1995, is dedicated to collaborative research that addresses the real needs of older adults and their families and the systems that support them.

Many of its research projects develop into programs and services that become integrated into CJE’s comprehensive network of programs and services for older adults and their families. For example, its research on Parkinson’s disease developed into a Parkinson’s training program that was implemented for staff members. This nationally-recognized research institute enjoys partnerships with Northwestern University, Rush University, Rehabilitation Institute of Chicago, University of Illinois at Chicago and many more organizations.

With this information in hand, Eisenstein reexamined the focus of the LSRI and made some changes. The first was to refresh and gain leadership support for a more relevant mission statement: “To conduct applied research and evaluation to improve the quality of life of older adults and their caregivers and to contribute to the development, innovation and dissemination of equitable models of healthcare.” She then refined LSRI’s focus to a three-pronged approach:

Population Health Management—The use of data, analytics, and interventions to maintain and improve people’s health across the full continuum of care – from low-risk, healthy individuals to high-risk individuals with one or more chronic conditions.

Assessment, Planning and Evaluation—Understanding the impacts and the outcomes of the work that’s going on in CJE’s programs and the overarching work that CJE is conducting.

Use of Mixed Methodologies and Accurate Measurement of Outcomes—Employing both qualitative and quantitative methods and measuring patient-reported outcomes such as quality of life.

According to Eisenstein, “My goal for research in general and in long-term care is to bring meaning and hope to the lives of residents and their families.” She also believes it is important that, as an organization, “we should explore and invest in cutting-edge opportunities, a goal which is in alignment with CJE’s value of innovation.”

Having a research staff embedded in the organization, working on an abundance of on-going and emerging projects, means that CJE will be assured of having well-constructed, outcomes-driven programs. In today’s healthcare climate that demands greater accountability and scrutiny, Dr. Eisenstein and her team are the foundation for CJE’s vision to be a model for health and human services providers throughout the country.

Amy Eisenstein received her Ph.D. from the University of Illinois at Chicago School of Public Health. She is also Adjunct Assistant Professor at Northwestern University’s Feinberg School of Medicine, Department of Medical Social Sciences. She resides in Chicago with her husband and two children.

CURRENT LSRI GRANTS AND SUBCONTRACTS The Population Health Project—This is an assessment project to help us understand the diverse aspects of our client population’s health status, including identification of chronic health conditions, healthcare needs and healthcare providers. This kind of information will allow us to better maintain our relevance in meeting the needs of older adults in our community.

Motivational Interviewing and Physical Activity in Parkinson’s Disease—A subcontract from the Rehabilitation Institute of Chicago. The goal of this project is to test the efficacy of a telephone-based motivational interviewing and coaching intervention and a smartphone application for self-monitoring to improve physical activity in persons with Parkinson’s disease.

Medication Use Questionnaire— This is one of the first questionnaires of its kind to be used as a tool to evaluate how medications are being used, whether prescriptions are understood correctly, if medications are causing financial concerns and many other important factors.

Evaluation of Illinois’ Dementia-Capable State Grant— A subcontract from the Illinois Department on Aging. To evaluate a systems change process taken on by the State of Illinois to provide a foundation for building a more dementiacapable state. A dementia-capable state is described as one that provides the highest possible quality of life for all state residents (and their families and caregivers) with dementia.

Gauging Health and Wellness of Older Adults in Lincoln Park— A joint project with the Jewish Community Centers of Chicago. As the name suggests, this is a study to learn about the changing health and wellness needs of adults as they grow and age in an urban environment.

Community Engagement for Early Recognition and Immediate Action in Stroke (CEERIAS)—A subcontract from Northwestern University. Certain minorities do not call their city’s 911 emergency service response system when experiencing a stroke. This is a study to understand why there are such wide population discrepancies in utilizing this community service for an acute health crisis and to create a city-wide intervention to reduce these discrepancies.

WINTER 2016

The Illinois Geriatric Workforce Enhancement Program (GWEP)— A subcontract from Rush University Medical Center. The GWEP is focused on educating older adults, families, caregivers, students, and professionals about person-centered, culturally competent management of Multiple Chronic Conditions, and to transform existing primary care systems to meet the needs of older adults with Multiple Chronic Conditions.

Caring Together, Living Better—A subcontract from AgeOptions.

LSRI is evaluating this grassroots interfaith partnership to build a regional network of support for caregivers of older adults living in the Western Suburbs of Chicago.

LIEBERMAN CENTER— CJE’S HEALTHCARE HUB

Healthcare excellence has always been a priority at Lieberman Center for Health and Rehabilitation. The range and quality of regular and specialized care that Lieberman Center provides across the healthcare continuum exceeds the standard of care. L ieberman C enter P ulmonary R ehabilitation P rogram L aunched Lieberman Center for Health and Rehabilitation recently launched its new Pulmonary Rehabilitation Program. The Program is designed to provide individualized care to patients who have diseases of the lungs.

The Lieberman Center Pulmonary Rehab Program is led by Dr. Maher Najjar, a board-certified Pulmonologist who works in close collaboration with an interdisciplinary team that includes highlyexperienced specialists from the following disciplines: Internal Medicine, Physiatry (a Physical Medicine and Rehabilitation specialty), Infectious Disease, Cardiology and Physical and Occupational Therapy.

Patients with the following acute and chronic conditions will be treated in the Program, which utilizes outcomes-based care that keeps close track of patient progress: • Pneumonia

• COPD (Chronic Obstructive Pulmonary Disease such as Asthma and Emphysema) • Lung Cancer

• Sleep Apnea

• Other diseases of the respiratory system

The Pulmonary Rehabilitation Program staff consists of specially-trained nurses and skilled technicians. Our team of Registered Nurses, supported by a Registered Respiratory Therapist, has been trained in special procedures such as: • Inhalation Therapy (Nebulizer Treatments) • Infusion Therapy • C-PAP

• Bi-PAP with night-time telemetry monitoring to evaluate therapeutic effects of the device.

Another important feature of the Program is bedside chest X-rays and bedside laboratory testing that allows patients to remain in bed, avoiding disruptive and stressful transports. These valuable diagnostic and evaluative tools are available 24-hours a day, seven days a week for constant monitoring of patients’ conditions.

These external credentialed specialists, Lieberman’s own core of physicians, skilled nurses and multidisciplinary staff members all work as a team to enable Lieberman to offer the following specialty programs:

• Center for Heart Health—A model program that provides multidisciplinary care and rehab for congestive heart failure and its underlying conditions (heart attack, coronary heart disease, high blood pressure, etc.) with advanced technology to monitor a patient’s sodium, weight and medication under the direction of a board-certified cardiologist.

• Pulmonary Rehabilitation Program (see facing page).

• Dialysis Program—Hemodialysis and peritoneal dialysis, led by a board-certified nephrologist, provides less stressful on-site dialysis that speeds recovery. A dialysis nurse, dietician and social worker help with patient education and training.

• Infusion Program—Nursing staff and specialists provide medicines and fluids requiring delivery through infusion, such as intravenous medications, antibiotics and nutrition therapy, while pharmacists monitor infusion prescriptions.

• Wound Care Program—New and treatment-resistant wounds, such as pressure sores, infectious wounds and post-operative wounds, are treated under the direction of board-certified wound and foot surgeons, as well as a physiatrist.

• Lieberman +30 Transitional Care Program—Patients discharged from Lieberman receive a detailed discharge plan and a Transitional Care Nurse visits patients within 72 hours after they return home to provide sustained monitoring for 30 days which may reduce the chances of re-hospitalization.

With all of its resources, Lieberman Center also offers longterm care for those with complex medical needs. Examples of Lieberman Center’s advanced healthcare offerings are: Alzheimer’s Special Care, a Parkinson’s Disease Program and Hospice and Palliative Care.

A RESEARCHER’S ADVICE TO FAMILIES OF ADULTS WITH DISABILITIES: PLAN . . . PLAN . . . PLAN

Your Eldercare Consultants, CJE’s team of geriatric care managers, are often called by family caregivers to help navigate the evolving needs of their older parents. But sometimes, the situation can be reversed. With February’s Jewish Disability Awareness Month on the horizon, we’re reminded of a startling statistic: Over 71% of adults with intellectual and/or developmental disabilities (IDD) live with their families, and of those roughly 3.5 million individuals, 24% live with caregivers aged 60 or older*.

In light of policy changes directly affecting long-term and community supports for this vulnerable population, we touched base with Tamar Heller, Ph.D., who is well-known for her expertise on this issue. She has been an invaluable resource to CJE’s Linkages program, which assists older adults caring for adult children with disabilities.

We asked Heller to address a few questions for families at the initial planning stage.

Q: Why is planning for the future so important for families of individuals with disabilities?

A: There are many reasons why planning for the future of an individual with disabilities is so important, and why families with aging caregivers are so anxious about it. Most people don’t really want to think about their own mortality, or about what it’s going to be like when they become more frail. Certainly, there are also many fears that no one will love their child like they do, and they don’t want to burden their other children or family

members. However, siblings often become the primary caregivers, and communicating intentions for financial, healthcare, residential, and social arrangements with other family members as well as the individual with disabilities is important.

Q: Planning for the future can be a long and challenging process. As a family, what are some things we can do to make the transition easier?

A: Meet as a family to initiate a conversation about the future even if you might not be ready to make decisions yet. If possible, speak with the person with a disability first to understand what he or she wants in life. Also, ask all family members what they envision for the future. Many family caregivers report that they are the sole providers of social and recreational support to their family member with disabilities. Planning ahead can expand the “network of care” so that the person with disabilities is not only dependent on family members, particularly aging parents.

She also directs the Rehabilitation Research and Training Center (RRTC) on Developmental Disabilities and Health, is CoPrincipal Investigator of the RRTC on Community Living Disability Policy and the Family Support RTC, and co-founded the national Sibling Leadership Network. Q: Is there a checklist of items we should discuss together?

A: Limited access to financial and legal information and a lack of understanding about local health and human services can make planning ahead a challenging undertaking for many families. Legalities can vary from state to state, but a general guideline is to:

• Ask your family member with a disability what he or she would like for the future.

• Ask other members of the family (parents, siblings, aunts or uncles, etc.) what they envision for the future.

• Identify a health care proxy agent and have a health care proxy form signed for your family member with a disability.

• Appoint a successor caregiver or multiple caregivers sharing the responsibility.

• Identify the eligibility requirements for services your family member with a disability currently receives.

• Explore residential options and secure a future residence for your family member with a disability.

• Create a will, stipulating a Special Needs Trust, if applicable. Tell all family and friends in the care network about the trust so they are aware of assets to be left behind.

• Discuss supported decision-making options, including power of attorney, partial or full guardianship, or financial counseling for your family member with a disability.

• Develop a Letter of Intent detailing your family member with a disability’s wishes for the future.

Q: What is a Letter of Intent?

A: While it’s not a will, a Letter of Intent details the optimal and acceptable conditions of living for the family member with a disability and can be a road map for families as they plan for the future. It should be updated yearly, and can include such information as: • Emergency information.

• Expressing wishes for the future.

• Deciding where to live.

• Financing the future.

• Employment, retirement and daily activities.

• Supporting daily and major life decisions.

• Making social connections.

• Healthcare and wellness.

“MY GRANDFATHER WAS MY BEST FRIEND”

by Carole Klein-Alexander This simple yet powerful statement is how Russell Karlins began our interview about why he and his wife Laura are longstanding donors to CJE SeniorLife. His tender memories of his grandfather, Hyman Garvin, peppered the conversation and provided insight into why the Karlins are so dedicated to CJE’s mission to enhance the lives of older adults.

“Laura and I just want to do our little part to try and make sure that seniors have a comfortable and safe place to live. My grandfather had such a positive experience at CJE’s Robineau Residence that choosing to donate to such a wonderful organization seemed a logical choice to us. We donate to many amazing causes, but when you have actually experienced an organization’s services, it makes a bigger impression on you.”

Mr. Karlins’ connection with CJE began in the late 1980s when his widowed grandfather moved at the age of 85 from an apartment in Rogers Park into CJE’s Robineau Residence in Skokie. “From day one, Robineau was a nurturing environment. He fit right in and got involved in the community. He loved participating in all the activities and was especially proud to be recognized for his role in the children’s reading program.”

In memory of his grandfather, the Karlins family dedicated a bench that graces the front lawn of Robineau. Looking at current Robineau residents sitting on the bench, chatting with friends while watching neighborhood children run down the sidewalk, it is easy to imagine how Hyman would have been honored by this gift.

Robineau Residence has been a very special home for seniors since opening in 1982. Nestled in a quiet residential neighborhood in Skokie, it was specifically designed to foster social interaction among 24 older adults living independently. The thriving, single floor community has six suites, each comprised of four private bedrooms and baths that converge around a small living room/kitchen combination… a perfect arrangement to encourage socializing among its residents who enjoy group activities, meals, transportation, excursions and 24-hour staff.

amount of social interaction which is critical to the older person.” Indeed, the Karlins reminisced about Hyman’s enthusiastic conversation about all his activities from the preceding week, such as his latest arts and crafts creation, when they would take him out for Sunday dinner. Mr. Karlins will never forget how his grandfather’s last years at Robineau were incredibly rich and fulfilling.

Deeply committed to CJE’s mission and values, the Karlins are ongoing donors to CJE and have also supported CJE’s innovative programming that is meeting the evolving needs of today’s seniors. Their most recent contribution went toward the launch of the Virtual Senior Center, an Internet-based way for seniors to connect with family and friends throughout the country for lifelong learning opportunities and socialization.

But the most fundamental reason for their enduring connection to CJE still goes back to the unique bond between a beloved grandfather and his grandson… a covenant that is deeply rooted to Jewish values.

As Russell Karlins reflects, “I don’t think I would have changed anything about my grandfather’s experience at CJE…and that is why we have decided to do something. We are very blessed and want to give back to an organization that really understands how to respect and honor older adults. Actions truly do speak louder than words.”

According to Dorothy Levant, Manager of Robineau Residence, “Although Robineau was designed at a time before there were so many residential options for seniors, it is still an incredible model…communal living with great programming. There is a tremendous

Dorothy Levant, Manager, Robineau Residence. Dorothy, known to all of us as “Dotty,” has been with CJE since 2001. She coordinates occupancy, maintenance, housekeeping, volunteers, Life Enrichment activities as well as dining and security at Robineau. Dotty makes sure that all the pieces of the puzzle fit well together.

TRANSITION TO WELLNESS (TTW ) DRAWS RECORD CROWDS The annual educational series was expanded this year, providing six presentations at three locations: Lieberman Center, Weinberg Community and Adult Day Services-Evanston. Also, the attendance set a record, with over 275 attendees. C.E.U.s were offered to social workers and nurses.

2015 CELEBRATE CJE A RESOUNDING SUCCESS With nearly 700 attendees, the 10th annual dinner and reception was the most well-attended one in recent years. Held at the Hyatt Regency Chicago, this year’s event raised over $700,000 to support CJE’s comprehensive network of programs and services.

Artwork created by CJE residents and clients was showcased for the first time at this year’s event. A variety of works in all media was available for sale. CJE’s talented art therapy and enrichment staff explained the art therapy process and the many art activities that abound throughout CJE’s many locations.
