or a 10-year-old, Pares Cuffee has particularly long legs, but they suit him well on the diamond. They give him a steady stance in the batter’s box; the quickness he needs to reach for difficult catches; and power to push off the pitcher’s mound with force.

But Pares’ strong and agile movements on the field are a stark contrast to his form four years ago, when a serious injury destroyed parts of his knee and left him unable to use his right leg at all.

“I was playing on our exercise bench and the heavy weight bar fell on my leg,” Pares says. His mother, Paris Clark, immediately knew that this was not a minor injury. Her son was crying. He couldn’t put any weight on his right leg, and his knee seemed to be stuck in a half-bent position. “Pares is very strong and rarely cries,” says mom. “So I was instantly concerned.”

“Going to CHKD was the only option I even considered,” she says. “That’s where my mother took me and where I always take Pares. The entire place is set up just for kids, and they know how to care for them better than anyone.”

At CHKD’s emergency department, images of Pares’ knee revealed an injury called a tibial spine avulsion fracture – pieces of his tibia, or shin, had broken away from the rest of the bone at the precise spot where the anterior cruciate ligament (ACL) is normally attached. The ACL – one of the four major ligaments in the knee – is located inside the knee joint and connects the bones of the upper and lower leg, providing stability during movement of the leg. “When they showed me the X-ray, it looked like pieces scattered everywhere,” says mom. “It did not look like a puzzle that would fit neatly back together.”

Dr. Allison Tenfelde, a pediatric orthopedic surgeon at CHKD, met with the family to explain that Pares would need an operation to reattach the ligament and repair his broken bone. He went home on crutches, with surgery scheduled for 10 days later.

During surgery, Dr. Tenfelde discovered that Pares’ injury was far more severe than any of the scans had indicated. His ACL was not only detached from his tibia but also torn away from his femur, the large bone of his upper leg. “It was shocking,” SUMMER 2018

On the field, Pares uses his newly regained strength and mobility to play the sport he loves.

says Dr. Tenfelde. “His ACL was completely detached and free-floating. It’s an injury so exceptionally rare that I still have not found another orthopedic surgeon who has ever seen it in a child.”

Instead of a single surgery, Pares’ recovery became a multi-year process that would take more than 100 hours of therapy and multiple surgeries to complete.

The process included intensive physical therapy to re-establish range of motion in his knee, and a preliminary surgery to clean out scar tissue that had formed in the joint.

It would take nearly a year for Pares to make enough progress to undergo the reconstruction procedure. Dr. Tenfelde enlisted Dr. Allison Crepeau, a fellow orthopedic surgeon at CHKD, to assist in the complicated repair.

Dr. Crepeau and Dr. Tenfelde are among a small number of orthopedic surgeons who are dual fellowship-trained in both pediatric orthopedic surgery and orthopedic sports medicine.

The surgeons created a new ACL for Pares using a portion of his ileotibial (IT) band – a ligament that stretches from the outside of the hip to the knee.

They meticulously attached the harvested ligament to the upper and lower bones of his leg, taking great care not to affect the growth area of his femur.

At age 7, Pares still had a lot of growing to do, and damage to that area could result in one leg being shorter than the other.

Dr. Mark Polak, a pediatric anesthesiologist and medical director of anesthesiology at CHKD, also played an important role in the complex surgery.

He is one of a handful of CHKD anesthesiologists who perform nerve blocks – a special way of numbing an area of the body to reduce discomfort after surgery. These nerve blocks can last up to three days and reduce the amount of strong pain medication, like narcotics, children need after an operation.

“As a pediatric anesthesiologist, it’s my job to keep kids safe during a procedure, help them with anxiety before the operation, and ease their pain afterward,” says Dr. Polak. “These families are trusting me with their children.

I take that very seriously.”

“Working with a pediatric anesthesiologist is an incredible benefit,” says Dr. Tenfelde. “They are so comfortable working with kids and incredibly knowledgeable about their special needs. It’s one of the main reasons I choose to work at CHKD.” After surgery, Pares began his recovery in CHKD’s pediatric inpatient rehabilitation unit – the only one of its kind in Virginia – where physical, occupational, and speech therapists help children regain skills and abilities lost after injuries or illnesses.

“Pares was not a typical patient. Most children that come to us are injured in more than one area,” says Dr. Rianna Leazer, medical director of inpatient rehabilitation at CHKD. “But the severity of his injury made it vital for him to start rehabilitation right away. Kids with intensive rehabilitation needs make a much quicker recovery in a hospital setting.”

Pares’ inpatient physical therapy schedule included as many as three hours per day, six days a week. His treatment focused on improving his range of motion and mobility and was supplemented with occupational therapy to practice basic tasks that would be challenging as he healed, like bathing and putting on his pants and shoes.

During breaks from therapy, Pares worked on new Lego creations and played games in the hospital playroom.

Kristin Radanovic, an occupational therapist at CHKD, helped Pares learn to stand properly on his injured leg without using his upper body for support.

Now cleared to return to full activity, Pares plans to add basketball and football to his future sports calendar.

Completing inpatient rehabilitation was only the beginning of Pares’ recovery process. After discharge from the hospital, he continued physical therapy as an outpatient at the sports medicine gym in CHKD’s Health Center at Oakbrooke in Chesapeake.

“Pares needed so much physical therapy. The convenience of being able to go to a health center near our home was a big help,” says Paris. “And the fact that he could work with his favorite therapist at that location made things even better.”

For most of his three-year rehabilitation journey, Pares worked with Ashley Koto, a pediatric sports medicine physical therapist at CHKD. “When I first met Pares, he had trouble straightening and bending his knee due to the injury,” says Ashley. “He went through a lot of fear and pain to get that range of motion back, but he always showed up laughing, smiling, and raving about his newest Lego creation.”

CHKD’s sports medicine physical therapists are specially trained to care for growing bodies, and they work closely with CHKD’s orthopedic surgeons to ensure successful rehabilitation for kids with injuries of all kinds.

“Ashley was amazing,” says Paris. “She really pushed Pares. Sometimes it was emotional, but he made significant progress when he worked with her.”

Throughout the more than 100 outpatient physical therapy visits Pares completed, he challenged himself to reach a goal of playing basketball, football, and baseball. With his love of sports, Ashley incorporated functional activities, like basketball drills, into his therapy to keep him motivated and inspired about his progress.

“At the beginning, I wasn’t sure it would be possible, but he’s playing baseball now. It’s amazing that he’s come this far,” says Ashley. “After all the ups and downs together, we really developed a relationship, and I think he finally understands why I had to push him so hard.”

“Now, Pares runs around like the injury never even happened. But it has been a long journey,” says mom. “Dr. Tenfelde is so wonderful – we were lucky to have such an amazing surgeon. If it hadn’t been for Dr. Tenfelde, Ashley, and everyone else at CHKD, there’s no way Pares would be where he is today.”

The Right Solution As the home of the region’s most advanced neonatal intensive care unit (NICU), CHKD cares for the area’s smallest and most critically ill infants – many weighing barely more than a pound. When extreme prematurity or medical complications prevent these babies from nursing or eating normally, they often depend on a feeding method called total parenteral nutrition (TPN) to survive.

TPN is a solution of vitamins and nutrients given through an IV infusion, directly into the patient’s bloodstream. The hospital pharmacy creates this customized solution daily for each patient based on their specific nutritional needs. After a period of infusion time, blood tests are done to measure the effect on the baby, and the next order is formulated based on those results.

“We believed there was a way to enhance the nutrition of these fragile babies by streamlining the TPN process,” says Christopher Mangum, manager of quality and performance engineering at CHKD. “So we assembled a team to make it happen.”

That multidisciplinary team included staff members from pharmacy, nursing, neonatology, information services, and quality improvement, as well as NICU family members.

“One of our main goals was to allow babies to receive the TPN solution for a longer period of time before daily lab tests were taken,” says Dr. Jamil Khan, a neonatologist and medical director of the NICU at CHKD. “This would give us a more complete picture of its effects before placing the next day’s order.”

To create more time for infusion, the complex production process would need to happen faster. The team started with the pharmacy space where the TPN is created. “By moving all the necessary components closer together and assembling kits of supplies in advance, the average production time decreased significantly,” says Jim Dice, PharmD, director of pharmacy at CHKD. The ordering process was also streamlined. By establishing an electronic ordering system to communicate directly with the machine that formulates the TPN, a customized recipe is immediately available to the pharmacy staff when the order is entered. “Everything is done online now. What once took a few hours is instantaneous,” says Laura Biava, RN, director of patient care services at CHKD’s NICU. The time saved in the ordering and production process has successfully extended the average time a baby spends on a single recipe of TPN from 8 to nearly 14 hours before labs are drawn. The project was so effective, it has been nationally recognized by the Children’s Hospital Association.

“These babies are now getting better nutrition, faster,” says Dr. Sandip Godambe, vice president of quality and clinical integration at CHKD. “It doesn’t always take lots of big steps to create meaningful change. Sometimes, several little steps can make a big difference.”

n a move he described as tectonic, the president of Children’s Hospital of The King’s Daughters said the health system will expand its mental-health footprint in the community with a proposal for a new psychiatric center.

The hospital must get state approval before moving forward, but president and CEO Jim Dahling said CHKD is committed to embracing what its community surveys have shown to be the No. 1 health concern: mental health services for children.

Three to five children a day arrive in the hospital’s emergency room in need of a mental health evaluation, a number that has increased by 40 percent during the past four years, according to CHKD. If the children have both a medical and psychiatric condition, they can be admitted for inpatient care. But if their sole diagnosis is psychiatric, they need to be referred to a psychiatric facility. In some situations, such as eating disorders, that means traveling out of state.

The hospital submitted paperwork to the Virginia Department of Health on March 1 to build a new medical tower, to be located just off Brambleton Avenue, on the same medical campus as CHKD, Eastern Virginia Medical School and Sentara Norfolk General Hospital. The tower will include 48 inpatient mental health beds, all private rooms, day treatment programming and outpatient mental health clinics. Additional space will be used for the expansion of other CHKD services, with a five-story parking garage to accommodate all visitors to the building.

The expanded mental health program would employ about 250 health care providers and support staff, and treat children 2 to 18 years of age.

The request for a “certificate of public need” would also include 12 beds in the current hospital for inpatient psychiatric treatment. The proposal will be reviewed by the state health officials to make sure the need for services is justified. A public hearing also will be conducted, likely in May or June. The state health commissioner is expected to make a final decision by early August.

“Everyone from parents to social workers to government agencies to school systems are telling us that mental health services for children are the No. 1 priority,” Dahling said in an interview.

National statistics are showing that 1-in-5 children need mental health services.

“The tipping point was the frequency of children coming into the ER in crisis,” Dahling said. “Parents are saying, ‘Please help.’ There are simply not enough localities to place them.

They stay in the ER sometimes for a day. You roll all of that together, and we cannot walk away.”

A five-day snapshot of 14 mental health patients seen in the ER included: • A 5-year-old with a history of setting fires, and being violent to animals and adults.

• A 16-year-old who took his parent’s diabetes medication after experiencing suicidal thoughts. His brother died by suicide at 13.

• A 13-year-old who experienced sexual assault and later overdosed on Benadryl.

• An 8-year-old who expressed suicidal and homicidal thoughts and tried to jump out of a second-story window.

Currently, the system receives 15 to 20 mental health referrals a day from primary and specialty care providers.

with waits as long as six weeks.

The hospital has one psychiatrist and is now recruiting for more, but a national shortage has made that challenging.

Stephanie Osler, director of mental health services at CHKD, said there are four local inpatient units they can refer children to: Kempsville Center for Behavioral Health in Norfolk, Maryview Behavioral Medicine Center in Portsmouth, Riverside Behavioral Health Center in Hampton, and Newport News Behavioral Health Center in Newport News.

But there have been times when those facilities have been full or the situation would not be good for the child.

For instance, if it’s a young child or someone with autism, or a child who is gay, lesbian or transgender, sharing a room, or even a unit, would be difficult. Parents and

caregivers often have to travel to Staunton, which has a state children’s mental health facility, or to Northern Virginia, or even out of state.

Osler said it’s particularly hard to find appropriate programs for children with eating disorders. The system has also struggled to find treatment for children as young as 3 or 4 getting kicked out of day-care and preschool settings because of behavior problems.

Courtney Boone, president of the Virginia Beach chapter of National Alliance on Mental Health, said parents with children who need mental health services often go outside the region, creating hardships for the children and parents. She shared an email from a local mother: “When you are at the point of needing acute care for your child, neither of you are in a place you want to be. If you have done it more than once, a bad experience at a

previous placement can also cause anxiety for both of you, and sometimes as a parent, you can delay getting needed help because of the options available.”

Another woman, a 59-year-old Norfolk mother of a 13-year-old daughter and a 10-year-old son with mental health issues, said she’s had to take her son to Johns Hopkins University in Baltimore for treatment for brain injury and psychiatric problems, and her daughter to Northern Virginia.

She and her husband were foster parents of the two children, who were severely abused when they were young, and they have since adopted them. “She was 4 years old when she was traumatized, but it’s like it was yesterday,” said the mother, who asked not to have their names used to keep private their background of abuse and mental illness.

They are both in and out of the hospital. At times, her daughter has had to wait more than a day at CHKD before a psychiatric bed was found for her.

“It’s a shame to have to go so far to get help,” she said.

“Northern Virginia? That’s ridiculous. That’s shameful. But for so many people, that is what we have to do.”

Amy Sampson, vice president over mental health services for CHKD, said that in the past, the health system has typically focused on medical and surgical needs of children and handed off acute psychiatric care to other facilities. But so many medical conditions have a psychiatric component, and the rate of children experiencing mental health problems is on the rise.

One pediatrician made the observation that diagnoses were shifting from typical ear infections to attention deficits, depression and obsessive-compulsive disorders: “I am stymied here and have little to no assistance. It is like fighting a battle and I am not armed.”

In 2014, the health system assigned six licensed clinical social workers to do mental health assessments and interventions. The first year, the system’s mental health team did 3,500 visits. Since then the number of mental 16

health therapists assigned to that duty has grown to 25 and they are on track for 18,000 visits at nine different CHKD locations this year.

That was the first step in bolstering mental health services, along with getting mental health training for pediatricians.

“We want to give children a gliding path, postdischarge, so they don’t end up back in the ER two weeks later,” Dahling said.

If the system gets state approval, it would take two years to get the 12 beds in use in the current hospital and four years for the new facility to be built.

The King’s Daughters, founding organization of CHKD, has pledged $1 million to the mental health initiative. Dahling said the health system will be talking with government officials, community leaders and philanthropic organizations to raise funding for the effort.

Dahling said the move will eclipse the most recent shift in strategy, which was when they began building medical centers and urgent care clinics in other parts of the region 15 years ago.

“It’s extremely exciting,” he said. “And a little terrifying.”

She was given a new life – now she’s giving back

Christina Goode’s personal story drives her passion to support CHKD’s mental health program and the community where she’s raising her 5-year-old twin boys, Owen (left) and David (right).

N

orfolk mom Christina Goode learned the value of philanthropy from her family. “My parents always stressed the importance of taking care of your community,” says Christina. “I know I’m lucky to have the life I do, and I feel it is my responsibility to give back.”

The Goode family has been supporting CHKD since 2003, and recently made a very personal and meaningful gift to CHKD’s mental health program.

“I spent more than 20 years of my life battling addiction and thoughts of wanting to harm myself,” says Christina. “Back then, my family didn’t know where to go to find help. If access to a local resource like CHKD had been available, I could’ve gotten the care I needed much sooner than I did.”

Now a mother of 5-year-old twin boys, Owen and David, Christina has been in recovery for 10 years, and she is dedicated to helping young people who face mental health and addiction issues.

“The early years of your life are so important in shaping your future,” says Christina. “The community already trusts CHKD, so when it comes to caring for children with mental health issues, it’s a perfect fit.”

The psychiatrist, psychologists, and counselors in CHKD’s mental health program are trained specifically to treat children, which is something that resonates with Christina. “For me, it took finding the right treatment for both my addiction and my PTSD,” says Christina. “Care that is designed just for children and teens makes such a difference.”

The Goode Family Foundation’s gift to CHKD’s mental health program will help facilitate much-needed growth and give more young people throughout the community the chance to receive the care and services they need.

“Mental health issues affect everyone,” says Christina.

“Everyone should be motivated to help CHKD address this growing need in our community.”

Carrying on a legacy of giving

R

ichard Olivieri had a talent for recognizing the needs of his community, and a true passion for finding ways to meet those needs. His professional success and personal achievements only increased his philanthropic drive to help others.

“Richard felt this community had done so much for him that it was his responsibility to give back wherever he could,” says his wife of 53 years, Maureen. “We began supporting CHKD many years ago because we saw what a valuable resource it was to so many families. I’m proud to continue that support today in Richard’s memory.”

New York City natives Richard and Maureen moved to Virginia Beach in 1964. They immediately embraced their new home and began a family, which today includes five adult children, their spouses, and seven grandchildren.

For more than five decades, Richard helped to shape the landscape of Hampton Roads as a partner in the development firm of Pembroke Enterprises, an active member of his church parish, and a quiet contributor to several local charities.

“As a father, Richard was especially motivated to help children,” says Maureen. “He was in awe of the services CHKD provides to all of the children in our region and was inspired to do whatever he could to help.”

The Olivieris have focused their giving on helping CHKD serve local children who have experienced abuse or neglect.

In 2006, the couple created an endowment fund that supports the CHKD child abuse program. Over the years, their generosity has helped the program establish a presence in Virginia Beach and supported the child abuse pediatrics fellowship program, which trains pediatricians in the subspecialty of child abuse pediatrics.

“Richard realized the importance of the work that was being done at the CHKD child abuse program right away,” says Maureen, a longtime member of CHKD’s Child Abuse Development Board. “For us, it was difficult to think about what would happen to the children and families that need these services if this program didn’t exist. We wanted to do what we could to help ensure CHKD was able to provide this resource to our community.”

Since his passing in December 2017, the family has carried on Richard’s legacy of support, even asking for memorial gifts to be given to CHKD in his honor. “Richard never missed an opportunity to give back,” says Maureen. “Helping CHKD care for more children in need is something he would want – giving to CHKD doesn’t just help children heal, it changes lives.”

Bringing CHKD closer to home

H

igh school sweethearts, R. Carroll Smith and his wife, Jacqueline, have been married for more than 57 years. As their family has grown over the past five decades, so has their support for CHKD. Today, the Smiths have two sons, four grandchildren, and one great-grandchild.

The couple made their first gift to CHKD nearly 25 years ago. “We decided to support CHKD for many reasons, but we were especially motivated by the experience of having a sick child. Our youngest son, Roland, had an unexpected and very serious illness when he was 13. As a parent, you never forget how that feels,” says Jacqueline.

The Smiths have been particularly interested in supporting CHKD’s efforts to improve community access to key outpatient services. “We always wanted to help our community,” says Carroll, “and knowing how important pediatric healthcare is, we realized that giving to CHKD would benefit many local families.”

In 2005, the couple made a gift to CHKD’s Health and Surgery Center at Concert Drive in Virginia Beach. “Seeing what that multi-service pediatric facility was able to offer the community motivated us to help make this level of care

more easily accessible to families all over Hampton Roads,” says Carroll.

In keeping with that desire, when the opportunity arose to support a CHKD health center in their own city of Chesapeake, Carroll and Jacqueline were eager to contribute. Today, the CHKD Health Center at Oakbrooke is a state-of-the-art pediatric facility that houses a variety of services, including primary care pediatrics, sports medicine physical therapy, and occupational and speech therapy. It also offers laboratory and radiology services with a family waiting room that has been dedicated to Carroll and Jacqueline’s grandchildren.

“CHKD is special because it’s just for kids, and we feel fortunate to have a facility nearby for our family,” says Carroll.

“We’ve visited several CHKD locations over the years and have always been impressed – not just by the facilities, but also by the professionalism and care of the staff. It’s what made us want to get more involved.”

The couple continues to give to CHKD today, and they are looking forward to helping the health system care for many future generations of local children. “We love CHKD because it is always all about the children,” says Jacqueline, “as they should be.”

Philanthropy Matters | Making a Difference

Making a Difference Individuals, business, and organizations throughout the community hold campaigns to support the children at CHKD. Here’s a look at some of our all-star supporters.

Nearly 400 supporters came together for an evening of fun and games at the eighth annual David Wright Vegas Night to benefit CHKD. This year’s event raised $177,000 for the kids, bringing the eight-year cumulative total to more than $1.3 million. The Tidewater Children’s Foundation made a $100,000 investment in the CHKD emergency department’s safe space for children battling mental health issues. Our friends at Tidewater Children’s Foundation have supported the children at CHKD for more than 40 years.

More than 800 women attended the third annual Hampton Roads Mom Prom held in February. This year’s sold-out event raised a record $55,000 for CHKD. Next year’s prom is scheduled for February 9, 2019 – save the date!

At this year’s Howard Hanna annual meeting, Hoddy Hanna (far right) with Dick Thurmond (far left) and Kim Georges (center left), presented a $70,000 check from the Howard Hanna Free Care Fund to Larissa Trinder of CHKD. Howard Hanna has donated more than $250,000 to CHKD.

Katie Gaston and Gregg Nigro of Dunkin’ Donuts (pictured center), along with their mascot Cuppy, presented members of CHKD’s child life staff with a check for $10,000 from the Dunkin’ Donuts Joy in Childhood Foundation. The foundation also donated dozens of superhero-themed “brave gowns” for hospitalized children to wear.
