This year, CHKD continues to expand to meet the healthcare demands of every child who needs our care.

We’ve opened two new health centers. One, on Butler Farm Road in Hampton, offers sports medicine rehabilitation; physical, occupational and speech therapy; lab and X-ray services.

The other, in the Harbour View North area of Suffolk, offers access to pediatric specialists. We opened a new CHKD Medical Group pediatric practice, Premier Pediatrics, in the Hilltop area of Virginia Beach in July.

And, by year’s end, we will open the region’s only urgent care center, just for children and teens. The first CHKD Urgent Care location will open in the Greenbrier area of Chesapeake and operate after hours – meaning afternoons, evenings and weekends when your doctor’s office is closed. Our second location, inside our Oyster Point Health and Surgery Center, will open in 2015.

The Amazing Adventures 0f Claire Bear alerie and Bo Foley were just beginning to share the exciting news of her pregnancy when results from an ultrasound revealed that their baby had a rare birth defect.

Valerie was referred to the maternal fetal medicine program at EVMS to coordinate with CHKD for immediate access to our neonatal intensive care unit and pediatric surgeons. As the reality of their overwhelming diagnosis began to set in, Valerie chronicled her young family’s experiences in a blog she called, “The Amazing Adventures of Claire Bear.”

Meet Claire, a.k.a. Claire Bear, who is set to join our family in May of 2013.

Isn’t she cute?

Most of our friends and family know about Claire’s impending arrival, but until now, we have only told a few about her condition.

On November 6, 2012, we learned that Claire would be born with a condition known as an omphalocele.

The word that took me a while to be able to pronounce, and even longer to spell. An omphalocele is a rare birth defect that arises when the abdominal wall does not close properly during fetal development. As a result, some organs develop outside the abdomen within the umbilical cord.

Claire’s omphalocele most likely contains her bowel and possibly some of her liver. She will need surgery after she’s born, so we were referred to the pediatric surgeons at CHKD. We’ll do our best to keep loved ones up-to-date.

Meeting Claire’s Surgeon March 25

We’ve had MANY appointments since we learned of Claire’s diagnosis, including a recent consultation with the neonatologist, Dr. Kenneth Tiffany, at CHKD. I had no idea what a neonatologist was before our

appointment, but essentially, Dr. Tiffany will be in charge of Claire’s overall care while she’s in the NICU. Both Bo and I really like Dr. Tiffany.

Today was the all-important consultation with CHKD pediatric surgeon, Dr. Frazier Frantz.

Bo and I were extremely comforted by him – from his demeanor to his extensive knowledge and everything in between.

There have been four other babies with omphaloceles born in Hampton Roads in the last two years, and Dr. Frantz was the surgeon for three of them.

So we know Claire will be in extremely experienced hands.

We learned that there are two ways to repair an omphalocele: through surgery to put the organs into the abdomen soon after birth, or a process called “paint and wait” where you keep the omphalocele medicated and wrapped to give the baby’s abdomen a chance to grow before closure a few months later. The general thought is that Claire will not have surgery right away. This could all change but Dr. Frantz has given us tremendous confidence going forward, despite the fact that there are still a lot of unknowns.

She cried within seconds of birth, and it was the best sound EVER.

Seriously, I know that sounds trite, but a lot of omphalocele babies are born with underdeveloped lungs ... so Claire’s wail meant that she was already ahead of the game.

Claire was discharged today, exactly one week after she was born. Every day has been an unparalleled joy as she continues to amaze us at each turn. Dr. Frantz taught us how to apply ointment and gauze to Claire’s omphalocele and an ACE bandage for compression. This will help gently keep her organs in place until surgery.

Claire was born at 9:18 a.m. via C-section. She weighed 8 lbs., 4 oz., and was 21.25 inches long.

Claire demonstrated how feisty she is right from the start.

Claire is pretty much taking care of things on her own - all of her nurses and doctors, including the amazing Dr. Frantz, have been impressed with her progress.

Claire’s omphalocele contains a part of her liver and a part of her bowel.

Although it is considered “giant” due to the presence of the liver, it is actually quite small in comparison to the rest of her abdomen. I think Bo put it best when he compared it to a cue ball. About three-quarters of Claire’s omphalocele was covered with skin at birth, which is GREAT considering that most omphalocele babies just have a membrane covering it -- which is subject to rupture.

Dr. Frantz is the epitome of calm, therefore we were happily surprised when, upon seeing Claire’s abdomen, Dr. Frantz pulled out his cell phone and asked if he could take a picture.

He was extremely happy with both the amount of skin coverage and how much the omphalocele has reduced in size.

Claire had her third appointment with Dr. Frantz since her release from the NICU. As always, it was very positive.

The surgery lasted almost exactly 90 minutes, but it felt like forever.

Bo and I walked away feeling extremely optimistic for the coming months.

The omphalocele is now 100 percent covered in skin, and we are looking at closure surgery when Claire is around 6 months old.

Our little Bear is getting bigger and bigger every day! She enjoys playing on her playmat and sitting in her bouncy chair.

Our next appointment with Dr. Frantz is October 9th when we will be scheduling the date for her surgery. While I am excited that Claire will be having her closure surgery, I am nervous all the same.

Claire’s surgery is scheduled for next week. During the surgery, Dr. Frantz will be making an incision right in the middle of Claire’s omphalocele, cutting out existing scar tissue. After pushing her liver and bowels back into her abdominal cavity, he will stretch her rectus abdominus muscle (her “abs”) and close her abdomen.

We should expect Claire to be in the hospital for a couple of days, mainly for pain management. At that point, she should have some good scar tissue forming that will assist with the recovery.

Exactly one year ago today, Bo and I were sitting in the doctor’s office hearing the word “omphalocele” for the very first time and realizing that our world was about to be turned upside down.

If you told me then that we would be where we are now, I would have laughed at you. I am at a loss for words thinking of all the miracles we have experienced over the past 12 months.

I know Claire will be in the wonderful care of Dr. Frantz and the rest of the amazing CHKD staff.

I’m probably not doing a very good job of explaining how I feel, but suffice to say that all my fears are pushed aside whenever I look at her precious face.

Dr. Frantz once again proved how fantastic he is when, instead of sending out a nurse or resident, he came to the waiting room himself.

Not surprisingly, things went better than expected. You’d think we would know by now how much of a fighter our daughter is! Dr. Frantz discovered that Claire actually had extra fascia (connective tissue) tucked under her sternum, which he was able to use to close the hernia, along with her “ab” muscles. Dr. Frantz could not stress enough how beneficial our compression wrapping had been – allowing Claire’s liver and bowels to easily slide into place.

A New Year I hope everyone’s year is off to a wonderful start.

I know that, for us, 2013 started as a year of unknowns and uncertainties, but it ended as a year full of amazement and miracles.

When we first learned about Claire’s omphalocele, I never once imagined that she would already be where she is today. Thank you again to all of you for being on this journey with us, for all your well wishes, prayers, and support.

A World-class

Collaboration W hen Hennglise Dorival first arrived at CHKD in April, she wore a scarf to veil most of her face. When speaking Haitian Creole, her native language, she was barely audible. Yet, just months later, the 16-year-old has a new confidence and new goals for the future. “I want to be a nurse,” she says, her radiant smile a testament to her courage and to the medical community that came to her rescue.

Hennglise’s journey to CHKD began four years ago with a simple toothache. Her mother, Yvrose, took her to the dentist in her home of Bahon, a rural Haitian village with limited electricity. Soon after, a bump the size of a pea developed inside the young girl’s cheek. Within a year, it was the size of two grapefruits. Doctors from Boston tried to remove the growth at a Haitian hospital in 2011, but the tumor returned and continued to expand at an even faster pace.

Staff members from Project HOPE and one of their concerned donors, Larry O’Reilly, heard about Hennglise’s condition and worked to coordinate a second surgery in Haiti. But inadequate intensive care resources and potential power outages in the country raised concerns about the safety of the very delicate and specialized surgery.

Next, with full financial support from O’Reilly, Project HOPE sought assistance from Operation Smile, the renowned Norfolk-based charity that has helped repair children’s facial abnormalities around the world since 1982. Because Hennglise’s condition was too serious to address in her own country, Operation Smile accepted her as a World Care patient for surgery in the United States and reached out to CHKD – its longstanding partner in charitable pediatric craniofacial reconstruction for international patients.

“CHKD has the ability to handle practically any tertiary-care need and has a talented team of medical professionals in pediatric neurosurgery and craniofacial surgery. The hospital was the natural choice for Hennglise’s operation,” says Dr. William Magee, Operation Smile co-founder and the head plastic surgeon for Hennglise’s procedure.

By this spring, the disfiguring tumor had grown to 4 pounds. Stares and comments from classmates had forced Hennglise to drop out of school months earlier, and even her own relatives had stopped visiting. When she and her mother arrived at CHKD for the surgery in April, the mass had pushed Hennglise’s left eye and jaw out of place. “Her abilities to eat and talk were affected, and the tumor’s growth blinded the affected eye,” says CHKD craniofacial surgeon Dr. Jesus Gil B. “Jegit” Inciong. If left untreated, the massive, aggressive tumor could eventually cut off Hennglise’s airway.

In addition to CHKD’s pediatric anesthesiologists, intensive care physicians and nurses, Hennglise’s surgical team included Dr. Magee, Dr. Inciong and fellow CHKD craniofacial surgeon Dr. George Hoerr. Dr. Joseph Dilustro, a CHKD pediatric neurosurgeon, and Dr. Eric Crouch, a pediatric ophthalmologist, were also in the operating room to evaluate the need for intracranial and eye surgery. In a surgery that took nearly 10 hours, the team removed Hennglise’s tumor and reconstructed her left eye socket and upper jaw bone.

After surgery, Hennglise recovered in CHKD’s pediatric intensive care unit for several days and was out of the hospital just five days after her operation. In the weeks that followed, her mother returned to Haiti to care for Hennglise’s siblings, and Hennglise continued her recovery at the homes of several host families in Hampton Roads in order to be close to CHKD’s medical resources for follow-up care.

Hennglise wanted to wait until the swelling subsided before taking a look at herself after surgery. Iris Feduniewicz, who hosted Hennglise during her stay, watched as she saw her reflection in their family’s hall mirror. “She stared at herself for a moment, looked at the ground, stared again at her reflection, and a big grin spread over her face,” she says.

Remarkably, six weeks after surgery, Hennglise could eat almost anything, cut in small pieces, and had discovered a new-found favorite food – pizza. She was able to visit several Hampton Roads attractions, like the Virginia Aquarium & Marine Center and the Virginia Zoo. And in June, Hennglise was well enough to join CHKD doctors and Operation Smile caregivers for the one mile walk at The King’s Daughters RunWalk for the Kids at Town Point Park.

Five months after her surgery, Hennglise returned to Haiti with a new smile.

Dr. William Magee (seated) led the surgery, with the help of pediatric plastic surgeons, Drs. Jesus Gil B. Inciong and George Hoerr, pediatric neurosurgeon Joseph Dilustro and pediatric ophthalmologist Eric Crouch.

“She has recovered well. She is talking well in Creole and is happy,” says Dr. Inciong. Still, he adds, the teen’s medical journey is not quite over. Removing the tumor was just the first step.

“As her face heals and the tissues settle down, more surgery will be required,” Inciong says. Hennglise required an eye prosthetic and dentures. She remained in the United States on a medical visa until September, and CHKD and Operation Smile continued to provide follow-up care up until her return to Haiti this fall.

“Operation Smile and CHKD have had decades of cooperation caring for children with craniofacial abnormalities,” says Dr. Inciong, whose work as an Operation Smile volunteer spans more than two decades. “We are fortunate that both complementary organizations are local here in Hampton Roads, but capable of serving the needs of children worldwide.”

Hennglise, who traveled more than 1,000 miles from home, is grateful for that cooperation and the team that came together to change her life. And her new confidence, new energy and new smile – a grin full of gratitude – say it all.

W

hen Eileen Boyle dances, her moves are breathtaking. Yet just a few years ago, Eileen felt winded and was losing stamina, even during routine dance practices. In fact, just climbing the stairs in her home seemed to take her breath away.

In her sophomore year in high school, Eileen began having trouble keeping up with her dance classmates. Her concerned parents, Linda and Chuck, took her to the pediatrician who suspected that her breathing difficulties were caused by pectus excavatum (PE), an abnormality of the chest wall that caused Eileen’s chest to be depressed in the center between her ribs. Like many teens with PE, Eileen had always had a slight indentation in the mid-chest, but it became more pronounced as she grew, compressing the lungs and heart and making it harder to breathe, especially in dance class.

“I didn’t make a connection with the shape of my ribs and the fact that I had lots of difficulty breathing when I danced and always got more out of breath than everyone else in my class,” the teen said.

The Boyles, who live in Albemarle County, looked to CHKD and its renowned center for chest wall repair for help. The hospital is the home of the Nuss Procedure – a minimally invasive surgery for PE pioneered by Dr. Donald Nuss in the late 1980s – and is recognized nationally and internationally as the premier center for treatment of chest-wall abnormalities. The

center’s surgical team performs an average of 130 chest wall repairs annually with patients coming from across the state, the country and even the world.

Eileen – diagnosed with progressive, severe pectus excavatum – was a good candidate for the Nuss Procedure, explains Dr. Michele Lombardo, a pediatric surgeon at CHKD. Tests confirmed that Eileen’s PE was putting pressure on her heart and restricting her lung expansion.

“Eileen had just completed a growth spurt when I met her, and her symptoms had worsened.

Her dance performances were becoming increasingly more difficult,” says Dr. Lombardo.

Unlike open surgical techniques, the Nuss Procedure uses just two small incisions on the side of the patient’s chest in order to insert a curved metal bar under the sternum. The bar is then rotated, propping the sternum into the correct position. The bar is secured to the patient’s ribs and typically remains in place for two to three years to push out the rib cage and restore normal chest wall anatomy.

The Nuss bar is ultimately removed in an outpatient procedure.

are improved and similar to patients without PE. This results in improvements in breathing and exercise tolerance,” Dr. Lombardo says.

Her family made the decision to schedule surgery during the summer before Eileen’s senior year of high school. Dr. Lombardo performed the Nuss Procedure on Eileen in July of 2013. The procedure went smoothly, and after recovering in the hospital for a few days, Eileen returned to her home in Charlottesville with plans to make regular follow-up visits to CHKD every few months.

Eileen experienced a dramatic transformation right away, with the absence of what she described as a dent in her chest the size of a baseball. “I now understand what it’s like to have a normal chest,” she says.

More important, though, are the physical and health benefits Eileen will have for the rest of her life. “In normal day-to-day exercise, I’ve definitely noticed an improvement in my breathing,” says Eileen.

She’ll depend on that stamina to make her dance aspirations come true. And she’s well on her way. In fact, just months after the Nuss Procedure, Eileen felt well enough to audition for a prestigious dance training program with the Joffrey Ballet in New York City – an opportunity that could set her career in motion. Newly transformed with her Nuss bar in place, Eileen’s performance won her a spot in the esteemed program.

“I’m so excited about training in New York,” says Eileen, who left this summer to study contemporary dance. She says she’s prepared for the rigors of training eight hours a day, five days a week and, eventually, launching a professional dance career.

As they watch their daughter make this incredible leap toward her dreams, the Boyles know they made the right choice by going to CHKD for Eileen’s life-changing Nuss surgery. “Dance is everything to her,” says Linda, “And now she can pursue the career that she’s always wanted.”

CHKD’s International Center for Chest Wall Repair As the clinical home of the Nuss Procedure, CHKD has become an international treatment and research center for chest wall abnormalities. Nearly 1,800 patients have undergone surgical correction for chest wall defects at CHKD, and CHKD surgeons perform an average of 130 repairs a year. CHKD’s center also offers non-invasive bracing treatment options for patients with less severe conditions, including the Vacuum Bell for PE. As a leader in chest wall corrections, CHKD has hosted the annual Nuss Thoracic Deformity Lecture Series, an international conference for pediatric and cardiothoracic surgeons and their clinical staff featuring live demonstrations since 2001.

Leave a legacy for the children.

Mary Arnold, along with her late husband, Morris, lived a quiet life in the Green Run neighborhood of Virginia Beach. They did not have children or grandchildren. They did not have siblings, nieces or nephews. Still, they recognized the importance of CHKD to their community and made contributions over the years to support the hospital.

Mary passed away recently after designating CHKD in her will as the major beneficiary of her estate. Her legacy gift will provide a perpetual stream of income to forward the mission of CHKD. Generations of children will be cared for thanks to her thoughtful act of generosity.

Anyone can leave a meaningful gift to the kids at CHKD through a will and ensure that the children in our community continue to receive the specialized pediatric healthcare they need to grow healthy and strong.

The annual CHKD employee phone-a-thon raised $58,900. It was a two-night event with 132 employees participating as team captains, callers and volunteers. “The Cat in the Hat” team pictured here included employees from the social work, chaplaincy services and finance departments and raised the most for CHKD during the event.

Ware River Circle 25th Annual Holiday House Tour

Tuesday, December 9, noon – 5:30 p.m.

A beloved tradition continues this year as the Ware River Circle celebrates the 25th annual holiday house tour to benefit CHKD. This year’s event will be held in the elegantly festive home of Marian and Herbert Fitzgerald at Wilson Creek Farm in Gloucester. Built in the 1870s, the historic venue offers a beautiful setting for a holiday tour. Expansive porches, 10-foot ceilings, antique furniture and restored architectural details throughout the estate are the ideal backdrop for a seasonal gathering. Tickets for the open house tour are $25 each and may be purchased by emailing margarettery1@cox.net.
