TheOntariogovernmentismakingiteasier and faster for people to access the publicly-fundedsurgeriesandprocedures they need by further leveraging community surgical and diagnostic centres to eliminate surgical backlogs and reduce wait times.As the governmentsignificantlyexpandsthenumberof surgeries being done through community surgical and diagnostic centres,itwilldosowith measures in place to protect the stability of health human resources at public hospitals, including requiring new facilities to provide detailedstaffingplansas

part of their application and requiring a number of physicians at these centres to have active privilegesattheirlocalhospital.

“Whenitcomestoyourhealth,thestatus quoisnolongeracceptable,”saidPremierDoug Ford. “Our government is taking bold action to reduce wait times for surgeries, all while ensuringOntariansusetheirOHIPcardtogetthe caretheyneed,nevertheircreditcard.”

Community surgical and diagnostic centres have been valuable partners in responding to the pandemic and addressing the pandemic-related backlog in surgeries. Increasing community delivery of surgeries has proven to increase patient and provider satisfactionandreducestheriskofarescheduled appointment. Surgeries performed at these

centreswillbepublicly-funded.

Ontario has a three-step plan that better integratesandusesthesestate-of-the-artfacilities to speed up how quickly people are able to get surgeriesandproceduresusingtheirhealthcard.

StepOne:Ontarioisurgentlytacklingthe existingbacklogforcataractsurgeries,whichhas one of the longest waits for procedures. New partnerships with community surgical and diagnostic centres in Windsor, KitchenerWaterlooandOttawawilladd14,000additional cataract surgeries that will be performed each year. This number represents up to 25% of the province'scurrentcataractwaitlist,andaccounts for the estimated COVID-related backlog of cataractsurgeries.Thesecentreswillperformthe Cont’donP.2

Ontario is also investing more than $18 million in existing centres to cover care for thousands of patients, including more than 49,000 hours of MRI and CT scans, 4,800 cataract surgeries, 900 other ophthalmic surgeries, 1,000 minimally invasive gynecological surgeries and 2,845 plastic surgeriessuchashandsofttissuerepair.Surgical wait lists are anticipated to return to prepandemic levels by March 2023, barring operationalissues.

Step Two: To further reduce wait times, Ontario is expanding the scope of community surgical and diagnostic centres to address regional needs with a continued focus on cataracts, as well as MRI and CT imaging and colonoscopyandendoscopyprocedures.Tostart as early as 2023, these procedures will be nonurgent, low-risk and minimally invasive and, in addition to shortening wait times, will allow hospitals to focus their efforts and resources on morecomplexandhigh-risksurgeries.

Step Three: Early detection and diagnosis of a health issue has an immense benefit on a patient's quality of life, prognosis and treatment path. As a next step, the government will introduce legislation in February that will, if passed, allow existing community diagnostic centres to conduct more MRIandCTscanningsothatpeoplecanaccess publicly funded diagnostic services faster and closer to home. Starting in 2024, this next step will also expand surgeries for hip and knee replacements. Legislative changes will also, if passed, strengthen oversight of community surgicalsettingssothatpatientscancontinueto expecttoreceivetheworldclasscaretheyknow anddeserveandprovidetheprovincewithmore flexibility to continue to expand access to more surgeries and further reduce wait times. As the provinceexpandstheroleofcommunitysurgical and diagnostic centres, Ontario Health and the Ministry of Health will continue to work with system partners and clinical experts to put in placethehigheststandardsforqualityandsafety.

“Timely and convenient access to surgery and diagnostic imaging is critical to

keeping people healthy,” said Sylvia Jones, Deputy Premier and Minister of Health. “This plan will boost the availability of publicly funded health services in Ontario, ensuring that Ontarians currently waiting for specialized surgeries will have greater access to the world classcaretheyneed,whereandwhentheyneed it.”

As the government shortens wait times using community surgical and diagnostic centres, Ontario Health will ensure that these centres are included in regional health system planning. Funding agreements with new community surgical and diagnostic centres will requirethesefacilitiestoworkwithlocalpublic hospitalstoensurehealthsystemintegrationand linkages, including connection and reporting into the province's wait times information system and participation in regional central intakes, where available. Community surgical anddiagnosticcentreswillalsocoordinatewith local public hospitals to accept patients that are being referred, ensuring people get the surgery theyneedasquicklyaspossible.

Chapleau Moments

As Chapleau High School prepares to celebrateits90thanniversaryin2012,aroundthe same time that it was established, so was the school car that brought education to children along the Canadian Pacific Railway between CartierandWhiteRiverforabout40years.

J.B. McDougall, a school inspector in North Bay, had the idea for the school in 1926 after a survey revealed that "children in the backwoods didn't get any education", according toanarticleinBetterLivingmagazinein1946.

The Ontario Department of Education agreedtotheplanandtheCPR,CNRandONR (asitisnowcalled)allagreedtoparticipate.

Chapleau was selected as the central place for the school cars on the CPR and one, attachedtoafreighttrain,travelledwesttoserve the children as far west as White River, and the other,easttoCartier.Adentalcarwasintroduced in1931.

The school car was not available to children in places such as Nicholson, Amyot, Franz,Localsh,Missanabie,Biscotasing, Sultan where a school was already established

but was for children of miners, trappers, section men,woodsmenandotherswhomaytravelfrom theirisolatedhomesbysnowshoeordogsledto thesidingorspurwheretheschoolcarwouldbe dropped off for five days before moving to its next location. The children would be given enoughhomeworktolastuntilitreturned.

Onestoryrelatedisthatonestudentbuilt aleantoagainsttheschoolcarandlivediniteach weekthatheattendedclasses.

When the school at Nicholson closed in 1936, the school car stopped there until 1956. It stoppedoperatingin1967.

Better Living described it as "the little red schoolhouse being magically turned into a railwaycoach."

Thecarwascompletewithstudentdesks, blackboards and a heater and also had living quarters for the teacher. All textbooks, pens, pencils and scribblers were provided by the departmentofeducation.

Often the teacher was called upon to provide assistance to parents as translater, adviser,letterwriterandhostatsocialgatherings held in the evenings on the car. Adult night classeswereoften held.

All teachers were qualified under department of education regulations, and accordingtoa1951reports:"Teachersarechosen because of their understanding of and adaptability to pioneering life. In addition to teachingintheclassroom,theydomuchtoenrich the life of students and parents, many of whom are not familiar with Canadian habits and customs."

AsIwasresearchingthispiece,I was intrigued by some of the words I came across such as "backwoods" and "pioneer life".

Growing up in Chapleau in the 1940s,itneverstruckmethatwe lived in the backwoods, or close toit,orwereexperiencingpioneer life.

Infact,wehadacampatHealey, built by my grandparents, Lil (Mulligan)andHarryMorris,and my mother Muriel E (Hunt) Morris, 17 CPR miles west of ChapleauonLakeWindermere.

I guess I really was in the backwoodsofpioneerlifeduring the wonderful summers I spent there. Had we lived there year round I may have attended the school car but somehow would have had to make my way to Musk or Pardee as there was no sidingorspurlineatHealey.

Interestingly,inaguidebookpreparedfor King George VI and Queen Elizabeth for their RoyalTourofCanadain1939,referenceismade toNorthernOntario. Itnotedthatthetrainwould pass "though a land of great rock hills and tall pines, the railway wanders ...on the main transcontinentalline."

Althoughtheguidedoes notmentionthe school cars, it does say that Cartier was the eastern terminal for dental cars. The cars, "donated and hauled by the railways, contain a dental clinic and living quarters for a dentist in charge."EquippedbytheRosedalechapterofthe Imperial Order of the Daughters of the Empire, they had been maintained and operated by the department of health of the provincial governmentsince1931.

The program provided for free dental treatment to children in Northern Ontario who might otherwise not have received any care. I recallthedentalcarebeingmadeavailabletous after the death of Dr. William Young, "the dentist", the brother of Dr. G.E. Young, "the doctor."Chapleauwaswithoutaresidentdentist formanyyears.

ThetrainpassedthroughGeneva,Benny, Pogosaming,Metagamaandotherplaces,andthe next comments were made about Biscotasing. The guide noted that Biscotasing, meaning " narrow filled with waterlilies, connecting two lakes" had been a Hudson's Bay Company tradingpostpriortothearrivaloftheCPR.

Biscotasing was the "jumping off place forthecanoeroutetoFlyingPost,JamesBayand HudsonBay --arouteusedsinceearliesttimes by the Indians on primitive expeditons, then on journeys to scene of Canadian Pacific construction -- where they made their first contact with civilization outside the trading posts."

Again an interesting use of language by theguidewritersin1939whotoldtheRoyalsthat the canoe routes had been used by aboriginal peoples since "earliest times" on "primitive expeditions". I can only surmise that the writers were sent out from England in advance of the Royal tour to write an interesting story for their Majesties who were paying their first visit to Canada.

However, by my calculation, the King and Queen were likely asleep as their train travelled from Sudbury to Chapleau. The Royal Train arrived in Chapleau at two a.m.and there werenoplansforapublicappearance.However, Reeve George Fife of Chapleau had sent a message advising that he wanted to meet them whenthetrainwasinChapleau.Theygotupand metMrFife.

Many factors can affect it, and they aren’t all within our control. But we can take some actions to care for ourselves and help protect our mental health overall. Here are some ideas to help you boost your mental healthonaday-to-dayandlonger-termbasis.

Get some regular exercise: Whether it’s playing a team sport, going for a walk or doing yoga, and whether it lasts 5 minutes or 50 minutes, getting your body moving can haveapositiveimpactonyourmentalhealth

Reachouttoothers:Feelinglonelyand socially isolated can harm anyone’s mental health.Connectwithfamilyandfriendsorjoin activities like recreational sports or a choir where you can meet new people. Even if it feelsintimidatingtoreachout,positivesocial connectionsareacrucialpartofwell-being.

Findwhat’smeaningfultoyou:Weall need something fulfilling in our life to help give it meaning. Experiment to see what activities or practices bring you joy or satisfaction and make them part of your routine. Re-evaluating your goals can be an importantpartofthisprocesstoo.

Learn to recognize your patterns: Do you binge on junk food when you’re stressed and then feel guilty about it? Or forget to eat when you’re overwhelmed? Maybe you tend togetsnappywhenyou’refeelingtired.Once you recognize common patterns in your own thoughts or behaviours you can try to plan ahead to prevent them, like stocking up on healthymealsaheadofabusyseasonatwork ordoingyourbesttogetenoughsleep.

Remember what you can and can’t control:Takingafewmomentstobemindful, thinking about the big picture, and recognizing the things you can and cannot change can be valuable ways to shift perspective and get you thinking more
