TRANSPORTATION | BY THURSDAY, APRIL 1, 2021 If your school district provides transportation to Chaminade High School (i.e. yellow school buses), you must register for transportation with them by Thursday, April 1, 2021, for the 2021-2022 School Year. Please contact your school district’s transportation office to confirm how to register. Most districts use either a hard copy form that must be submitted in person, via mail, or online. Please provide the following information to your district: School Name: Chaminade High School • Address: 340 Jackson Avenue, Mineola, NY 11501 Phone Number: 516-742-5555 • Hours: 7:55 A.M. to 3:05P.M First Day of School: Wednesday, September 8, 2021 Any student who is likely to use the Long Island Railroad (LIRR) to travel to and from school (i.e. is from a school district that does not offer transportation) will receive an email from the Assistant Dean’s office in late spring or early summer with instructions about how to sign up for a monthly LIRR ticket with a student discount. New York City residents will receive free student Metro Cards and discounted Nassau Inter-County Express bus cards at the beginning of the 2021-2022 School Year. The Assistant Dean’s office will pass out cards in person during the first few days of school. HEALTH PHYSICALS | AFTER TUESDAY, JUNE 1, 2021 New York State requires Chaminade High School to have on flle an updated immunization record and physical for every freshman. Additionally, Chaminade requires a physical for every student for all four years. Since this physical must cover an entire year, it must be completed after Tuesday, June 1, 2021, in order to be valid until June 30, 2022. Please note: • Appointments held before Tuesday, June 1, 2021, will not be accepted. You should defer your regular insurance-covered appointment until after Tuesday, June 1, 2021. • Insurance carriers only cover one physical per year. • Do not schedule an appointment for late August or early September. Your information may not be ready by the start of school. Chaminade will not allow students to attend classes if their physicals have not been accepted. • If you do have a conflict scheduling an appointment in time, you will need to get a second, out-of-pocket physical from a walk-in clinic. You will also need a copy of your updated immunizations record from your regular doctor.
