Lighting the way, starting today! The IX edition of In4Med aims to “Lighting the way, starting today”, and has as its panels “Extreme Medicine”, “Small people, Big challenges” and “Mysteries of the Brain”. The panels go through the constant challenge of making a difference at the moment, but also through continuous delivery, and especially through the effort for those who have a short life, already affected by so much. Altogether, it involves the way of making a difference in intervention in emergency situations, the challenges of neurology and the mind, as well as in children, which are the first step to go further in the future, starting at the exact moment of our action. It takes place through people who are successful in what they do, however difficult the context of the situation, however difficult the scope of the disease. Success is the result of talent and hard work through true delivery, which allows for magic. People who reveal their light many times in the dark, away from the stage and applause, and who serve as the perfect inspiration for those around them. Passing as persistent, more than insisting, even in fear, because reality or what matters is how we can influence the present moment, seen as opportunities pass. The light comes from within and our willpower is to go further.

Doctor John Gurdon Nobel Prize in Physiology or Medicine 2012

Sir John B. Gurdon, a British development biologist and Nobel Laureate grew up in Frensham, a village in South England. Having been brought up in a home surrounded by ponds and heathland he developed an interest in plants and insects very early in life, especially moths and butterflies, which he collected and studied. At 13 he was sent to study at Eton, where his academic experience was not very happy. After his first term learning Biology, a negative report from his teacher wrote off Gurdon’s pursuit of a scientific career as ridiculous. Thus, young John Gurdon was kept from learning Science for his remaining three years at Eton College. Instead, he was made to study languages. However, with the support of his parents, he was able to spend a year after graduation learning what he had missed during his high school years. After a year of cramming he was able to pass the entrance exams at Oxford and move on to study a degree in Biological Sciences, the area he was truly passionate about. Nowadays, John Gurdon is considered the Godfather of Cloning, and yet, he says the unflattering report made by his Biology Master, which still sits above his desk at the Gurdon Institute in Cambridge, is the only thing he’s ever framed.

SMALL PEOPLE, BIG CHALLENGES Our life journey starts as a fetus, thus we were already alive before we were born. When we stand in front of a child, we learn how fragile, but honest and genuine beings they can be. Indeed, when a child cries, we usually know there is an organic problem or they are in a hostile environment. On the other hand, when a child smiles, the world jumps and advances as a coloured ball. The truth is that hospitalized children lose much of their power and intrinsic nature. That is why we need to make them feel like home by building a sandcastle in their minds and overflowing true happiness. It is with honour and esteem that we present to you experienced speakers on Pediatrics and Fetal Surgery, noble branches of Medicine, living up to the motto “Small people, big challenges”.

Dr. David Grant Can you imagine someone more worthy of having his way enlighted than a child? That herculean task is Doctor David Grant’s daily basis. As a Consultant In Paedriatic Intensive Care, his priority has been increasing the little ones’ safety and the quality of health care delivery through simulations that allow highly specialized teams to improve their skills. Lighting the way of small people of today is a big challenge, which Doctor David Grant is fighting to overcome during his life, day after day, in order to enlighten those who are going to be the grown-ups of tomorrow.

Dr. Luca Mazzone Doctor Luca Mazzone is a board certified pediatric surgeon (FMH). Currently he is Senior Physician at the Department of Visceral, Neonatal, and Fetal Surgery of the University Children’s Hospital Zurich. He is a full member of the Fetal Surgery team since the beginning of the program (2010) with an active participation in over 125 fetal spina bifida repair. He became Co-Head of the Spina Bifida Center Zurich and hold the title of a FEAPU (Fellow of the European Academy of Paediatric Urology) in 2019. He graduated in Medicine from University of Zurich in 2002 and got his M.D. Swiss Federal Diploma in 2002. After 2 years of general surgery residency at the Cantonal Hospital Zug, he began his training in pediatric surgery at the University Children’s Hospital Zurich (Prof. M. Meuli, Department of Pediatric Surgery). He also held a clinical fellowship in pediatric surgery at the Children’s Hospital of Philadelphia, the world’s largest and most important center for fetal surgery from 2008 to 2010 and got Swiss Board certified in Pediatric Surgery in 2011. His research activity in the Tissue Biology Research Unit, Children’s Hospital Zurich focuses on basic research in the area of fetal surgery/fetal tissue engineering. His main research topic is “Tissue engineering of fetal skin for prenatal coverage of myelomeningocele”. He is also a member of the Swiss Society of Pediatric Surgery.

Professor Mark Hannaford A seasoned and keen traveller, Mark Hannaford’s adventures have spanned all of the world’s continents. A Geography graduate from Middlesex University, Hannaford began his work as an assistant field geologist in Australia, having then moved to Operation Raleigh (now Raleigh International) as Project Manager. His childhood dream of venturing into some of this planet’s most exotic places has yielded the most fantastic results - not only is he an award winning photographer, with published photographs in The Telegraph, National Geographic and Sunday Times, but he’s also the founder and current Managing Director of Across the Divide, an organization bent on creating “life changing experiences” in the form of corporate challenges and charity events, even going as far as raising over a hundred million pounds for various non-profits, such as the British Red Cross, OXFAM and Cancer Research UK. Back in 2002, ATD’s sister, World Extreme Medicine or WEM, arose to ensure medical professionals were able to provide the best possible care to patients in the most inhospitable and challenging locations. In fact, Mark’s founding of WEM has allowed for the implementation of the world’s first Msc programme in Extreme Medicine with the University of Exeter Medical School, and earned him the title of Honorary Associate Professor by the same instituition, complimenting his previously obtained Fellowship from both the Royal Geographic Society and the Royal Society of Arts. No doubt thanks to Hannaford’s remarkable efforts, the WEM conference, whose first edition was held in 2012, is a world-renowned event, focusing on Expedition, Endurance and Extreme Sport, Space, Pre-hospital and Disaster & Humanitarian Medicine. Its ultimate goal? To “challenge thinking, build bridges and introduce new ideas about medicine at its most remote and austere.”

CHALLENGE WHAT YOU BELIEVE

SURROGACY

HOW FAR CAN WE GO? Having a child is something that most people, especially women, dream about their whole lives. Of course, not everyone wants to go down that path – but the majority of people have a special place in their life plans to the thought of giving life to another human being, nourish them and watch them grow. Not everyone has the chance of do so in the most “natural” ways. A variety of issues, such as infertility, can get a couple, a man or a woman to look for alternatives when it comes to conceiving. In the great pool of techniques such IVF and other options, these people often find themselves considering the option of surrogacy. What is, indeed, surrogacy? Surrogacy, is defined as an arrangement made by at least three people, in order for a surrogate or gestational mother to carry a pregnancy for the two intended parents, with the objective of the former party relinquishing all rights to the child, once the child is born. This subject, of course, raises many legal, medical and, maybe the most of all, ethical questions. In the list of countries (and some states) where surrogacy is still illegal, the main players are France, Germany, Austria, Switzerland, China and several states of the USA. In Portugal despite the efforts that have been made in order for surrogacy to be legalized, it still is not permitted by law. In some of these countries, not only is surrogacy held as illegal, there are also severe sanctions that may be applied against doctors who make arrangements for a possible gestational mother to come into contact with future commis-

sioning parents. However, these penalties don’t make surrogacy go away. It is done illegally, sometimes allowing situations like international arrangements, traffic and “baby markets” to take place. In other countries, surrogacy is not directly regulated. Examples of these countries are the Republic of Ireland, Belgium, Hungary, Romania and the Netherlands. It is quite clear that Portugal lies within the group of countries that are against surrogacy arrangements. There is a dangerous void of regulation that leads to surrogacy arrangements being entered into via black market circumstances. Even though gestational surrogacy should be considered as a last resort mechanism due to the fact that the benefits of conceiving naturally cannot be overlooked, this kind of medically assisted procreation has in fact a strong social importance granted that all interests and rights at play are protected. The questions, however, start coming to us like a flood. If it is, indeed, legal, then who can benefit from it – only couples? What are the conditions in which surrogacy won’t compromise any of the party’s rights? How does this affect the baby? And if she changes her mind, who can tell a gestational mother that that baby isn’t hers? The real question is… HOW FAR CAN WE GO? Don’t miss the opportunity to hear an extraordinary debate on the subject of surrogacy, during the IX In4Med, where these excellent professionals will answer some of our bigger questions:

COORDINATION Interview with the Coordination on the IX In4Med For the past eight years, we’ve seen an unbelievable growth take place on this amazing event. What does it mean to you to be the “leading faces” of this years’ event? Joana: It’s an honor. I was a medical student in my 3rd year when they invited me. In4Med is the biggest project of NEM/AAC and thinking about the idea of me coordinating it… I was afraid! I participated in other activities of NEM/AAC and was part of the Organizing Committee of the last edition, VIII, but this is a whole different responsibility. I’m so proud of what we have achieved… In the beginning we had nothing but our work and our motivation. We are in the IX edition, but each year is different and each edition starts on its own. We are just a group of medical students that want to do more for our friends, our colleges, our University, our Coimbra and for the world, even if this is just a simple drop in the ocean. Teresa: Like Joana said, it’s such a great honor to be able to contribute to this amazing event. When Joana invited me last year, I was terrified, since coordinating this project is a huge responsibility. In4Med means months and months of hard work and perseverance and knowing I was going to be part of it was both scary and exciting. I do believe that this year’s edition is going to embody the growth In4Med has been through and I really hope that our participants will feel it too. What are the aspects of this edition that you would like to highlight, not only about the panels but everything that an event like this involves? Joana: Since the beginning of this edition, we wanted to aim for something greater, something that transcends the technical and scientific role of our congress. We wanted to approach the human part, questioning us – What motivates a doctor or a scientist to work? – and

the answer is simple! People. The magic in human connections, the need to help others, the altruism of working hours and hours just for that patient. This year, we wanted to bring those questions to our stage, starting with the choice of our panels, where we’ll talk about mental health in Mysteries of the Brain, about the risks and difficulties of treating someone in extreme conditions in Extreme Medicine and we’ll talk about children, their challenges and the diseases that steal their childhood in Small People, Big Challenges. We also want to question our beliefs and make our participants question theirs in Challenge What You Believe, being this year’s entitled “Surrogacy – How far can we go?”. Having worked so hard for this moment, what are you most excited about? Teresa: That’s a difficult question since at this point every conquest, every novelty that we’re going to be able to give to our participants gets me excited. I’m excited to know their feedback of the scientific program, which I believe is both irreverent and of high quality; I’m excited for them to see our coffee breaks, the amazing improvement in matters of workshops and pre-courses… All in all, I can’t wait for everyone to be able to experience everything this amazing team has been working for. How do you expect the IX In4Med to “light our way”? Joana: Our goal here is is to provide the necessary tools a medical, nursing or dentistry student needs for their future. These tools may be new techniques, new approaches, new discoveries or maybe just a new idea. It may just be an inspiring moment, a life story that made them question the way they think of themselves as future doctors. That’s what we expect In4Med will do, because In4Med only exists not simply because of the people organising it, but also because of the speakers, the companies that support and believe in us, but mainly because of our participants. They are the reason we work for 11 months.
