ou’ve come down with some kind of bug and your first thought is, “I’m too busy to be sick. I need an antibiotic that will work fast.” If you don’t get the quick fix you hoped your doctor would provide, there’s probably a good reason. Read on as Cedars-Sinai primary care physicians delve into popular antibiotics myths to sort fact from fiction.

Myth #1: Regardless of what ails me, it can’t hurt to give antibiotics a try.

“ It can hurt to use antibiotics inappropriately because there is always a risk of an allergic reaction and side effects, such as nausea and vomiting, as well as potential for kidney injury. By destroying healthy flora in the gut, antibiotics may lead to Clostridium difficile diarrhea, which could be life-threatening.”

STEPHANIE TRAN, MD Culver City Myth #2: It’s OK to stop taking the antibiotic my doctor prescribed when I start feeling better.

“ It’s very important to take antibiotics as directed and finish the entire course of treatment, even if you feel better. If the bacteria aren’t fully destroyed, you may get sick again. And the bacteria that remain can mutate and develop resistance to the antibiotic, so it may not work for you in the future.”

Myth #3: It is OK to use someone else’s leftover antibiotics.

“ Never take medication unless it’s been prescribed for you. It’s a lot harder to help patients who already have taken antibiotics without being diagnosed. It can be difficult to tell which symptoms are from the actual illness and which are side effects from the medication.

When I prescribe antibiotics, I always aim to minimize collateral damage.”

ISAAC J. ADATTO, MD Encino Myth #4: There’s no point in taking antibiotics if I have a viral infection like the flu.

“ You should always see your doctor if you have a high fever (over 101°F) and your symptoms linger for a prolonged period (longer than seven to 10 days), or you start to get better and then suddenly get worse. These can be signs that your symptoms might not be viral and that you have a bacterial infection.”

J

eff Baird has a lot of reasons to love living in Playa del Rey. If he’s not exploring the nearby beach and bike paths, he’s taking in the spectacular view from floorto-ceiling windows in his living room, which frame the Ballona Wetlands and the sparkling blue Marina del Rey harbor.

But perhaps the best perk is that almost everything he likes to do is just steps away from his house.

“It feels like a small town next to a big city,” says Baird, 68, who grew up in New York City and has lived around the world, including stints in Tunisia, Qatar and the San Francisco Bay Area. “The people are friendly, and it’s very community-based.”

That community extends to his doctor, too—Cara Stalzer, MD. In fact, Stalzer’s Cedars-Sinai office is just two and a half miles away in Playa Vista’s new lively central district, Runway. That’s convenient for Baird, but he’d happily drive many more miles than that to see her, and for good reason.

“She saved my life,” he says.

MYSTERIOUS PAIN

The first person Baird credits with saving his life, though, is his wife, Barbara Collins. Last summer, she convinced him to find a new primary care doctor who would take a closer look at his health.

She had good reasons to be concerned. For the past four years, Baird had been having sporadic bouts of chest pain, particularly when walking uphill or carrying jugs of water up the long stairway to their three-level beach house, where they’ve lived for three years.

“It wasn’t a strong pain, but it would get my attention,” Baird says. “I would just sit down for a couple of minutes until it subsided.”

He researched doctors online and chose Stalzer based on her profile. “She seemed personable,” he recalls, and the fact that she was nearby was a bonus. At Baird’s first appointment in September 2017, Stalzer spent nearly an hour with him.

As he’d hoped, she was easy to talk to, but Stalzer didn’t like what she heard about the chest pain.

“It sounded like a cardiac problem,” explains Stalzer, who completed her residency at Cedars-Sinai in 2017.

“I said, ‘OK, we need to look into this.’”

She sent Baird for a “stress test” with Keith Yabumoto, MD, a Cedars-Sinai cardiologist. When that was positive, Yabumoto performed an angiogram—a catheter procedure that shows if heart vessels are clogged.

The news was not good. One of Baird’s coronary arteries was 90 percent blocked and the others were significantly clogged. He urgently needed open-heart surgery, a quadruple coronary artery bypass.

Lying there on the exam table, Baird was stunned.

“I think what hit me for the first time,” he says, “was that sense of not being immortal.”

A LOT TO LIVE FOR

On December 12, Baird underwent open-heart surgery with a Cedars-Sinai surgeon at the Smidt Heart Institute.

Just one week later, he was home.

Since then, he’s been slowly but surely regaining his strength. He works from home, as a city-planning consultant, and he and a partner have a firm near San Francisco that helps local governments address affordable housing issues. Soon, he plans to resume his twice-monthly trips to the Bay Area.

Even though Baird likens open-heart surgery to “getting hit by a truck,” he seems unruffled by the experience.

Relaxing at his kitchen table in dark, stonewashed jeans and a casual blue collared shirt, he’s quick to smile and laugh, and his easy, friendly nature melds perfectly with the nearby beach. But as much as he loves the simplicity of Playa, he’s also an adventurer.

After college, he served three years in the Peace Corps, teaching physical education in the African countries of Tunisia and Togo. Later, he lived in Qatar, working with the government there on a large development project.

His destination of choice these days: Italy. He and his

wife—who between them have three adult children and one 18-year-old grandson—got married in Italy and hope to eventually retire there.

HEALTHY CHANGES

For now, though, Baird is perfectly content walking their two dogs, Lola and Zooey—an excitable pair of ChihuahuaItalian greyhound sisters—along nearby Ballona Creek.

He can’t wait to dust off his beach cruiser and resume his regular bike rides along the coast.

He’s lost 35 pounds since that first appointment with Stalzer, mostly through drastically cutting down on sugar, white bread and unhealthy fats. The lifestyle changes have also helped other aspects of his health: He is no longer prediabetic, and his doctor couldn’t be prouder.

“He’s really shown that healthy lifestyle changes can go a long way, and he’s lowered his risk of further heart disease,” says Stalzer, who also lives in Playa del Rey and loves being close to her job at Cedars-Sinai’s brand-new medical offices. Her colleagues include other family physicians, pediatricians and an urgent care staff who serve the growing needs of the corner of Los Angeles nestled between Santa Monica and the South Bay. “I feel a real sense of community here,” she adds.

“Many of my patients can walk to the clinic or work nearby, or I’ll run into them at Whole Foods. I like how local everyone is.”

Baird is grateful to Stalzer and all his doctors, including the multitalented cardiology team she recommended.

Standing at his living room windows, looking out at the view and his life ahead of him—as Lola and Zooey exchange happy, high-pitched yaps and circle excitedly around his feet—Baird can think of only one word to describe how he feels.

“Lucky,” he says. “I’m just really, really lucky.”

S

itting in her home near Larchmont Village, Cheryl Dunnett, MD, contemplates her backyard. Though charming, it isn’t very large and it’s mostly paved. Yet, every summer, she transforms this hardscape into a tomato-vine jungle. All it takes is 30 pots, 90 cubic feet of soil and a trip to Tomatomania.

The festival happens in March, and it’s Dunnett’s favorite event—the week she chooses her seedlings for the coming season.

“I don’t make plans during Tomatomania. I don’t return phone calls. I’m off the grid,” says the green-thumbed internist, half-joking about the nature of her obsession.

A physician with Cedars-Sinai for 28 years, her passion for Solanum lycopersicum is legendary at her practice.

Each year, she harvests more than 25 varieties: purples, yellows, reds and greens of all shapes and sizes.

In her kitchen, she turns this bounty into omelets, rustic tarts, gazpacho and a cheesy fresh tomato sauce that she spoons over steaming noodles for a signature dish she calls “Summer Pasta” (see recipe below).

Dunnett took up vegetable gardening in the mid-1990s when she was working hard as medical director of the growing Cedars-Sinai Medical Group. Communing with her tomato vines brought instant tranquility.

“I just loved going out there at the end of a busy day and tinkering, seeing what was growing, what had turned yellow,” Dunnett says. She’s especially vigilant in detecting bugs and caterpillars, the organic grower’s nemeses.

She picks them off by hand and “relocates” them.

“The secret,” she says, “is you have to really watch. If you start to see something that seems wrong, you have to diagnose the problem.” She’s still talking about tomatoes, but she might as well be talking about her practice.

Dunnett is the kind of doctor who watches over her patients like the constant gardener she is.

“I love the continuity of seeing somebody year after year,” she says. “Over time, I build relationships. I take care of my patients physically, but I also know what’s going on with them emotionally and socially. I let them know they can trust me and talk to me.”

Long Beach. Her lawyer mother was the family gardener; her aerospace engineer father was in charge of irrigation.

After graduating from the University of California, San Diego, Dunnett studied medicine at Harvard.

A Southern Californian to the bone, she couldn’t wait to leave Boston. She returned to Cedars-Sinai for her residency and never left.

Why would she? She loves what she does.

“It’s more than a job,” Dunnett says. “I try to advise my patients and to be a cheerleader.” To that end, she prepares motivational handouts on topics like changing unhealthy habits through goal-setting and the healing power of meditation—something she knows about firsthand thanks to her own spiritual explorations.

After so many years, her medical practice includes multigenerational families, from kids to their grandparents. Asked to name her oldest patient, Dunnett beams— it’s a woman who lives near Lafayette Square, not quite two miles from Cedars-Sinai’s main campus.

“She’s 97. I saw her just today. She came in with her daughter and grandson. She’s an amazing woman, the matriarch of her family, a pillar of her church and an amazing cook.”

Dunnett’s relationships with patients can be a two-way street. Sometimes those she gives tomatoes to reciprocate with gifts of their own homegrown produce or fresh eggs from backyard hens. It’s a joyous exchange.

Tomatoes. And taking care of her patients. “These are the things that make me happy,” she says.

Dr. Dunnett’s Summer Pasta INGREDIENTS

6–8 large ripe tomatoes, chopped into 1/2-inch cubes 16 oz. fresh mozzarella, cut into small cubes 1 cup basil leaves, cut into fine strips 1 cup olive oil (mild extra virgin or regular) 1 tsp. freshly ground pepper (more to taste) 1/2 cup grated Parmesan cheese 1 lb. linguini pasta 10 oz. plain goat cheese, crumbled 1/2 cup pine nuts, toasted

INSTRUCTIONS

• About eight hours before serving (minimum of two hours), combine the first six ingredients in a large bowl and set aside at room temperature for the flavors to get “married.”

• Cook pasta al dente. (Use diluted chicken broth for the cooking liquid for more flavor, but not for vegetarian diners!)

• While cooking pasta, crumble up the goat cheese and toast the pine nuts.

• Drain the pasta and, while it’s still very hot, immediately toss with the tomato mixture, which will mostly melt the mozzarella.

• Put in warmed pasta bowls to serve, topping with a generous amount of crumbled goat cheese, pine nuts and freshly ground pepper.

• Let the cheeses melt over the hot pasta.

It’s just so fresh and wonderful!

A SPRING GUIDE TO POLLEN IN SOUTHERN CALIFORNIA

Seasonal allergies leave many of us red-eyed and sniffling. Pollen from grasses, trees and weeds is often the culprit behind allergies. These plants release their pollen from the male part of a flower or cone. Wind, animals and insects carry the powdery grains to fertilize other plants.

Here are the most common sources of pollen in Southern California.

GRASSES

Most types of grass release pollen only when they grow tall. A few types, including Bermuda, notoriously release pollen even when kept short.

Perennial ryegrass and tall fescue don’t usually flower and release pollen until they’re 12 inches or taller. Grass pollen is usually too small to be seen.

Researchers found that 1 in 3 hospitalized patients shows symptoms of depression.

The condition can lead to poor outcomes for several medical conditions, as patients who are depressed are less likely to take their medications and return for follow-up appointments. Cedars-Sinai routinely checks all hospitalized adult patients for signs of depression to help them in their recovery.

MEMORIES ARE MADE OF THIS

A large-scale study led by Cedars-Sinai will explore the brain’s process of forming and retrieving memories, and how those memories influence everyday decisions. Scientists will record the activity of individual neurons in the brains of patients who are undergoing surgical treatment for drug-resistant epilepsy.

The long-term goal is to help develop new treatments that combat memory-destroying conditions like Alzheimer’s disease.

Hitting that mark, however, got tougher last fall when new guidelines were announced by the American College of Cardiology and the American Heart Association.
