Brenda Gramann (2001-2017). The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by residents to represent the community’s best interests. The charge of the Board is to set policies that will guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait - a commitment to serving the park and recreational needs of our community.

Board Meetings

Regularly scheduled meetings of the Board are held on the second and/ or the fourth Monday of each month according to a schedule adopted by the Board yearly. Please check our website for our current meeting schedule. Regular Board Meetings begin at 7:00 pm.

Write and star in a musical! Kids work as a team to choose some of their favorite Broadway tunes and write a unique story about them. British Soccer Camp

Campers will learn individual foot skills, technical drills, tactical practices, small-sided games and scrimmages. Campers will receive a personal player evaluation. Campers should bring water, shin guards and athletic shoes or cleats recommended. Fee: 37142

Kids First Sports Flag Football Camp We’ll focus on skill-enhancing drills combined with playing flag football games. A championship game will be on the last day.

White Sox Pitching Camp

The White Sox Pitching Club will give you a firsthand look at the pitching mechanics that all great pitchers use! Even if pitching isn’t in your plans, throwing correctly is critical to becoming a good player. Pitching with accuracy, velocity and minimal arm strain is no accident. There is a process that should be followed from Little League to the Big Leagues. The White Sox Pitching Camp will teach you that process. Location: Evergreen School Fee: 37230 37231

Skyhawks Beginning Golf Camp

Skyhawks Golf focuses on building the confidence of young athletes by teaching proper technique to refine essential skills of the game. Skyhawks has adopted the SNAG (Starting New At Golf) system to its curriculum. SNAG utilizes a set of equipment designed for beginning golfers, including oversized plastic club heads and “mini tennis balls” to help build confidence and to have fun. Developed by PGA professionals, SNAG is specifically designed for the entry-level player. No need to bring your own clubs-all equipment is provided. The Fountain View Recreation Center Gymnasium will be open for youth to play basketball during weekday afternoons. Participants can pay a daily entrance or, if a high school student, purchase a fitness center membership in advance. YOUTH SPORTS YOUTH SPORTS FAQs How do I register? Register online at csparks.org or use the registration form in this guide. By registering your child you agree to comply with the Parent Code of Conduct which can be accessed on the home web page of each youth sports league and is discussed by the coach at their pre-season parent meeting.

How do I register for tee ball, soccer, and high school leagues? Registration is by grade level. Players are to be registered for the grade the player is currently in. Preschoolers must be entering kindergarten for the 2014/15 school year.

How do I register for youth baseball?

Can I request for my child to be placed on a specific team? No, due to the nature of the program, we cannot accept any other coach, player, or carpool requests. However one mutual friendship request will be honored for players in an instructional league, (8U) baseball and softball, or grade 1 and 2 soccer. A park district request form must be used for these requests and the guidelines listed on the form must be followed. These forms are available at Simkus Recreation Center and on our web site. Request deadlines are the same as registration deadlines (not early bird).

How do I volunteer to help? Volunteers are used for the following position:

Can my child play up? Playing up is not allowed for instructional soccer or tee ball.

For all other leagues playing up is allowed by one age or grade level but the following process must be adhered to:

Applicants must be at least 16 years of age.

Contracts are available at the Simkus Recreation Center and online at www.csparks.org.

1. The player must register at his or her age or grade. 2. A request to play up must be made to the program supervisor. 3. The player must attend the evaluation day for the sport and must attend at the time designated for the requested league/ level (i.e. if a player should be playing in the Mustang league but wants to play in the Bronco league, player must attend the Bronco player evaluation).

What happens if I register after the deadline? Players placed on a wait list are not guaranteed placement on a team. If you register before team formation, the player will be moved off the wait list if there are enough to form an additional team. However, if you register after team formation, or if another team cannot be formed, placement will only be done when a team or teams are below the established maximum number of team participants. Players will not be added to any team one week prior to the first scheduled game.

Private Swim Lesson

Where are lessons held? Swim lessons are held at Fountain View Recreation Center, 910 N. Gary Ave., Carol Stream, in the new 25yd indoor pool and warm water pool. How do I register? • New swimmers - select a swim class level based on your child’s age and previous lesson history or call the lesson coordinator at 630-784-6195. • Returning swimmers - select a swim class level based on your child’s previous swim lesson report card. Class levels may be combined if deemed appropriate. Classes fill quickly; transferring from one level to another is not always possible. If you have questions, please call our Swim Lessons Coordinator at 630-784-6100 ext. 195. How does my child progress through levels? Swimmers will receive a report card at the end of the session indicating which level to register for in the next session. Refunds and make-ups will not be given for classes missed or forgotten. What happens on the first day? Arrive 15 minutes early for check-in. Please stay for the entire swimmer’s class, in case of emergency where consent is required. Please bring: Towel, swim suit already on child; non-potty trained participants must wear a swim diaper with tightly fitting plastic pants under swim suit; report card from previous session. Starfish Swimmers Level 3 (5-12yrs.)

Starfish Babies (6-18mo.)

Parents will assist in teaching their little swimmers with water adjustment and exploration, blowing bubbles, supported front and back kicks, and assisted floating.

Starfish Tots (19-36mo.)

Parents will assist with supported front and back kicks, floating with assistance, swimming readiness, putting face in water, blowing bubbles, jump in with assistance. Skills will be completed with instructional aides such as noodles.

Starfish Paddlers Level 1 (3-5yrs.)

Swimmers will learn to enter and exit the water, alternation of arms, front and back kicks, back glide, back glide with kicks, and water safety skills.

Starfish Paddlers Level 2 (3-5yrs.)

Swimmers will continue to refine strokes learned in Starfish Swimmers Level 1 and 2. Swimmers will be introduced to deep water skill-building and focus on techniques and endurance training for distance swimming. Lessons will be conducted in an average water depth of 4-12 feet.

Starfish Swimmers Level 4 (5-12yrs.)

Swimmers will learn advanced skill strokes such as breaststroke and elementary back stroke, open wall turns. Deep-water skills such as basic diving, retrieval of underwater objects and treading water will be taught and water safety skills. Swimmers will be grouped based on skill level. Lessons will be conducted in an average water depth of 9-12 feet.

Starfish Swimmers Level 5 (5-12yrs.)

Swimmers will learn unsupported front and back float, fully submerged face and head underwater, blowing bubbles, alternation of arms, front and back kick, back glide, back glide with kick and safety skills.

Swimmers will continue to refine advanced strokes such as breast stroke and butterfly techniques while also learning flip-turns. Intro to Competition

The Dolphins Swim Team of Carol Stream Park District is a volunteer based program for youth who want to join the world of competitive swimming. The Dolphins compete in the DuPage Swim and Dive Conference along with 12 other park district swim teams. Team swimsuit fee is not included in registration fee. Team swimsuit is not required to participate in the program. Swim suit sizing will be held at the team Parent and Swimmer meeting May 5. Swim meets are held Wednesday evenings and Saturday mornings.

Can I watch a gymnastics class? Spring 2014 classes are 8 weeks long. Parent viewing opportunities are May 27-June 2 and June 7. How do I choose which class level to register for? Assessments are required for Grasshopper, Beginner, Beginner II, Advanced Beginner, and Novice levels. Assessments are also required for any of these levels if more than one session has been skipped. Color Guard

Color Guard is a sport that works within a marching band to add a visual aspect to the music. Learn the basics behind Color Guard with an introductory class in flag spinning. You will learn how to do basic moves like drop spins and flutters, and put them together into a musical routine. Registrants need to purchase their own 6’ silver aluminum pole and practice flag or bring their own to class. Registrants purchasing flag through program registration pay an additional $25 equipment fee with registration. Fee: 36648

What is recital dance?

Recital dance classes run September-May and perform in the annual spring dance performance.

How do I register?

Classes begin in September, however, registration for recital classes will still be accepted up to November 23. Dancers are to be registered by age by the start date of the class.

How can I pay for recital classes?

Payment for recital classes can be made in full at the time of registration or monthly EZ pay payments can be arranged. A $47 recital costume fee for each dancer is also required upon registration. Recital costumes are ordered on Dec 1 and are non-refundable after that date.

When is the dance recital held?

The annual recital is held in May. The 2014 event will be held on May 9 at Wheaton Academy. Tickets will go on sale on May 2. Dance Recital Picture Day is May 3 at Simkus Recreation Center.

Are there any dates recital classes don’t meet? Classes will not meet Mar 12, or Mar 31-Apr 5, 2014.

Leading a parade, being in your high school band, or even twirling on the Miss America stage are all possibilities as a baton twirler. Join this fun baton twirling session and learn the basics of twirling, dance and twirl choreography and teamwork with Silver Knights. Batons are provided for class instruction. Into The Woods Jr.

Into The Woods Jr. is a whimsical fairy tale with all of your favorite characters—Cinderella, Little Red Riding Hood, Jack (and his beanstalk), and the Witch. The musical centers on a baker and his wife who wish to have a child; Cinderella, who wishes to attend the king’s festival; and Jack, who wishes his cow would give milk. When the baker and his wife learn that they cannot have a child because of a witch’s curse, the two set off on a journey to break the curse and wind up changed forever. With our new nature exploration scouting program, you can create a custom schedule for your troop! Leave the work to us! No more searching for curriculum to help your scouts earn a badge. ActivKids Before & After School 2013-14

Your kids will have a blast learning new games, sports or creative activities at ActivKids, our before and after school recreation program for kids in grades K-5. Your kids will love ActivKids for the fun and friends. You’ll love ActivKids for knowing your children are safe, supervised and feel at home in the familiar setting of their own school. ActivKids is convenient, too. Flexible morning drop-off and evening pick-up times mean no rushing to beat the clock. Register for the whole year to match your schedule, or purchase a punch pass to use on a occasional basis.

All ActivKids forms are available at Simkus Recreation Center or at csparks.org/activkids. To register for these programs, complete the ActivKids registration forms and submit with payment. Forms are available at your school’s ActivKids site, at csparks.org/activkids, at the Simkus Recreation Center or at Fountain View Recreation Center. Please register 48 hours in advance. Two weeks advance notice is required if your child needs special assistance from an WDSRA aide (1:1 support staff) - go to page 44 for more information, If you want a set schedule: • Complete and sign the ActivKids Registration Packet. • Sign up for EZ Pay for Automatic Withdrawal of payments via credit card or bank account on the 1st of every month or • Pay all program fees in Full at time of registration. • Print and mail/fax the forms, together with your full payment or EZ Pay form.

WAIVER REGISTRATION/BROCHURE WAIVER & RELEASE IMPORTANT INFORMATION The Carol Stream Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for the above listed programs/activities must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity. WARNING OF RISK Recreational activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers and injuries due to inclement weather, slip and falls, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities exist. In this regard, it must be recognized that it is impossible for the Carol Stream Park District to guarantee absolute safety. WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK Please read this form carefully and be aware that in signing up and participating in the programs listed, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a Result of participating in any and all activities connected with and associated with these programs (including transportation services and vehicle operations, when provided). I recognize and acknowledge that there are certain risks of physical injury to participants in these programs, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation.
