The Case for Plastic Surgery
Dr. Thomas J. Balshi
THIS BOOK OFFERS AN OVERVIEW OF COSMETIC SURGERY AND PROFILES A NUMBER OF THE WORLD’S TOP SURGEONS. We have sought to list, alongside the giants of cosmetic and
reconstructive surgery, the bright new stars of today and tomorrow. The profiled surgeons are all highly qualified and skilled professionals and some are legends in their field. The book is not conceived as a directory of surgeons: much of the editorial has been authored by medical doctors and aims to equip the reader with the knowledge necessary to make an informed decision when considering plastic and/or dental surgery. With special thanks to Dr Robert Goldwyn, a legendary name himself, for writing the introduction to the book.
<< Keep young and beautiful, It’s your duty to be beautiful Keep young and beautiful If you want to be loved.>> SUNG FAMOUSLY BY MARLENE DIETRICH AL DUBIN (1933) WRITTEN FOR ROMAN SCANDALS MUSIC HARRY WARREN PERFORMER: EDDIE CANTOR & STUDIO CHORUS
UNLIKE GAUL, WHICH JULIUS CAESAR DESCRIBED AS BEING IN THREE PARTS, PLASTIC SURGERY HAS TWO: RECONSTRUCTIVE AND AESTHETIC. A FURTHER DISSIMILARITY IS THAT THESE PARTS OVERLAP. A BREAST RECONSTRUCTION, AS A PRIME EXAMPLE, WILL SOON BE JUDGED BY THE PATIENT AND THE SURGEON ACCORDING TO AESTHETIC STANDARDS. OF APPROXIMATELY 6,500 CERTIFIED PLASTIC SURGEONS IN THE UNITED STATES, MOST DO A COMBINATION OF AESTHETIC AND RECONSTRUCTIVE SURGERY, AS DID I THROUGHOUT FOUR DECADES OF PRACTICE. Remuneration from cosmetic surgery usually far exceeds that from restorative procedures, the obvious reason being that surgery of appearance falls outside the assignment fee system of third party payers. Not only, therefore, is the speciality of plastic surgery divided into cosmetic and
reconstructive, but so also is the professional life of the individual plastic surgeon as a result of his or her choices – and there are choices. The young plastic surgeon begins a career by covering emergency room and performing reconstructive operations but as the years pass, the aesthetic component grows and takes over. Aside from the fiscal rewards, the associated lifestyle has appeal because of its elective nature. An additional advantage is that the plastic surgeon may choose to do fewer major aesthetic operations but more minor office procedures, such as injecting Botox and fillers, dermabrasion, laser treatments, dispensing cosmetic products, advertising, and even opening a spa – all in the name of beauty, better health, and better income for the plastic surgeon. Cosmetic surgery would not exist without a societal demand for it. The availability of the plastic surgeon, or other surgeons and non-surgeons, who do aesthetic surgery, not only answers the need but reinforces it. Aesthetic surgery has special features, some so obvious that they are frequently overlooked. In contrast to reconstructive surgery, which is performed to improve function and to restore normalcy, aesthetic surgery is done to enhance the patient’s already normal appearance. All aesthetic surgical procedures are directed to areas where the eye can reach under certain conditions. The face, for example, is usually in view in most cultures; the breast, as another example, can be seen only to special observers under certain circumstances. Unlike almost every other surgical situation, many of which are emergencies, operating for aesthetic reason is completely elective, thus giving both patient and surgeon time to make a decision and permitting the
patient to seek more consultations. The patient has gone to the plastic surgeon because he or she wants an operation and may become disappointed and angry if refused. Aesthetic surgery generally involves only a brief physical examination focusing on the body part that concerns the patient. Patients, moreover, are generally healthy. They are willing to risk their physical wellbeing to be happier because of an improved self-image. Happiness, as we all know, is an elusive entity. Aesthetic patients commonly feel guilty and embarrassed because of what they term “vanity”. Adding to this feeling of guilt is the reality of spending money perhaps stretching the family resources in pursuit of something not essential to health. Another distinctive feature of aesthetic surgery is that at least 80 to 85 percent of patients are women. The reasons are not always anatomic but cultural. Our society as well as many others puts great pressure on the female to improve her appearance to the point of subjecting herself to an operation that can produce an unfavourable result or a frank complication. As Freedman observed: “Beauty counts for everyone, but more so for women. From the moment of birth, beauty is sought, perceived and projected onto girls… .” (Freedman, R. Beauty Bound. Lexington, Ma: Heath, 1986, p9) That most plastic surgeons in the world are men is another fact to consider. Male plastic surgeons would not think it unusual for a woman to have a cosmetic operation. The rise of women to greater prominence and powerful positions has not lessened their desire to seek improvement in their appearance. The fact that the surgery of appearance is routinely prepaid is another unique aspect of cosmetic surgery. Aesthetic surgery offers the patient a way of improving the quality of his or her life, the plastic surgeon an opportunity to help that individual and gain satisfaction in doing so but only if he or she acts as a physician committed to whatever is in the best interests of the patient, and not conducting himself or herself as a proprietor of a body shop.
early days and interesting facts of plastic surgery
THE QUEST FOR ENHANCING PHYSICAL BEAUTY IS AS OLD AS MANKIND ITSELF. PLASTIC SURGERY REACHES FURTHER BACK THAN MOST OF US REALISE AND THE FIRST PATIENTS WERE PREDOMINANTLY MALE. FEMALE PATIENTS STARTED OUTNUMBERING MALE PATIENTS TOWARDS THE END OF THE 19TH CENTURY. COSMETIC SURGERY WAS FIRST DEVELOPED IN THE LATE 1400s TO TREAT DISFIGURED VICTIMS OF THE SYPHILIS EPIDEMIC. Cosmetic surgery, as we know it today, was pioneered by German surgeons (some of the most notable names are Carl Ferdinand von Graefe, Eduard Zeis, Hermann Eduard Fritze, Friedrich Dieffenbach and Jacques Joseph) and further developed to deal with the ravages of World War I. When Hollywood introduced the camera close up concept, Louis B. Mayer, made Greta Garbo go for cosmetic dentistry. Marlene Dietrich had rhinoplasty in 1929. Rita Hayworth had her hairline moved. Marilyn Monroe’s dalliances with cosmetic surgery are well documented and obvious when comparing photographs. Hollywood, along with TV and print media, defines our perception of beauty today, although the vast majority of women and men in the public eye would never admit to having resorted to cosmetic surgery. Phyllis Diller was the first to admit to having had surgery and made headlines in 1971 by divulging
details of her operations. There exist on the Internet dozens of websites that delight in highlighting cosmetic surgery gone wrong and informing the public who has had what done. The best and most visited is www.awfulplasticsurgery.com, listing scores of celebrity before-and-after photographs for the delight and entertainment of mere mortals
EARLY DAYS AND INTERESTING FACTS OF PLASTIC SURGERY
timeline of plastic surgery 1597
FIRST DOCUMENTED NOSE JOB
The Bolognese surgeon Gaspare Tagliacozzi performed what is believed to be the first nose job. The operation lasted 20 agonising days as anaesthesia was not available at the time (having only been discovered in 1846).
FIRST TUMMY TUCK
Baltimore surgeon Howard Kelly removed a piece 90cm long, 31cm wide and 7cm thick from the body of Mrs M. a 32-year-old Jewish woman.
FIRST FACE LIFT
German surgeon Eugen Hollander performed the first known face lift on a Polish aristocrat who persuaded him to undertake the surgery by showing him a drawing. The drawing illustrated how, if facial skin were removed at the front of the ear, the corners of the mouth would be tightened.
FIRST EYE LID SURGERY
Chicago surgeon Charles Conrad Miller is credited with performing the first aesthetic eye surgery (rhytidectomy), much copied by his contemporaries.
FIRST SAGGING CHEEKS AND CHINS OPERATION
German surgeon Jacques Joseph developed the procedure for tightening sagging cheeks, leaving virtually no scarring. Jacques Joseph is also the god-father of the modern endo-nasal rhinoplasty. Raymond Passot developed the procedure for removing the double chin in 1919.
FIRST FAT INJECTIONS
Subcutaneous fat injections were developed to give a younger appearance without the scarring.
FIRST TRANSSEXUAL SURGERIES
Berlin surgeons Ludwig Ley-Lenz and Felix Abrahams were the first surgeons to transform male genitalia into simulacra of the external female genitalia.
1930s Sir Archibald McIndoe and Sir Harold Gillies as well as a lot of other notable surgeons, helped patch up veterans of World War I and then World War II, especially the badly burned Spitfire pilots. At the same time, they pursued a very lucrative career in cosmetic plastic surgery in Harley Street. Chemical peels were popular in Europe and then Florida and LA from the Thirties, often practiced by non-medically qualified individuals. Sylvester Stallone’s mother was one of the very early practitioners. Ivo Pitanguy is deservedly famous for popularising plastic surgery in the post-World War II era. Living in Brazil he practices in a country obsessed with looking good.
FIRST BREAST IMPLANTS
Silicone was first used in the form of subcutaneous injections until it proved to carry substantial infection risk. Thomas Cronin and Frank Gerow developed the first silicone gel breast implants in Houston, Texas in 1962. The women in Texas are some of the best groomed and most plastic surgery-loving in the universe.
FIRST BUTTOCK LIFT
Professor Ivo Pitanguy developed the first buttock lift, a procedure which was widely copied by other surgeons. Also in the 1970s Tord Skoog from Uppsala in Sweden, described the SMAS technique for the facelift and made it more than just a skin operation. Claude Lassus from Nice popularised the vertical breast reduction technique and Benelli from Paris the peri-areolar technique (both Frenchmen, obviously, and both passionate about reducing scars on petite French breasts).
DEVELOPMENT OF LIPOSUCTION
Dr. Yves Gerard-Illouz described, against much scepticism, the concept of blunt cannula liposuction using the wet technique to a crowded arena in Honolulu in 1982. Marco Gasparotti from Rome further refined the concept of liposculpture and in the 90s we had the advent of Power Assisted as well as Ultrasound Assisted Liposuction.
For the last 20-30 years various types of laser have removed hair, broken blood vessels and wrinkles and the whole concept of non-surgical surgery with Collagen, followed by synthetic fillers such as Hyaluronic Acid and Hydroxyapatite, has come of age. The most important product of all being Botulinum toxin, or Botox, since the late 1980s. Today elective surgery has become a safe option and the idea of having it done and going home the same day has made it seem even less invasive. Deft surgery with minimal blood loss and using skin glues has reduced bruising and pain and speeded up recovery. Multiple surgeries can be carried out at the same time. New developments are anti-obesity surgery for the fast-food classes and cosmetic surgery to fix the side effects of
has opened yet another avenue of feminising men and vice versa as well as mutating our classic images of male and female facial and body shapes and characteristics. Fashion models are tall, androgynous, hiplesss creatures with broad shoulders and polymer breasts. Men are hairless and have essentially the same shape as the women except for the package between the legs and the absence of mammary glands. The rest is up for artistic grabs.

TIMELINE OF PLASTIC SURGERY
having a skin suit 5 sizes too big. The optionality of gender preference and the sophistication of trans-sexual surgery
THIS CHAPTER OUTLINES THE SURGERY PROCEDURES AVAILABLE TODAY, BUT EVEN AS IT IS BEING WRITTEN, NEW COSMETIC SURGERY TECHNIQUES ARE BEING DEVELOPED IN RESPONSE TO DEMAND FOR REDUCING RECOVERY TIME AND SCARRING. Procedures such as facelifts, which were previously performed with scalpel, will in the future be done entirely with lasers. Tissue engineering is no longer in the realm of science fiction. The creation of living tissues in a laboratory setting for use as a replacement for damaged or diseased body parts will soon be a reality, as will be the growing and reproduction of skin grafts, breast tissue, muscles, stem cells and even nerves. Many of the above
may still be in their infancy stage, but the future of cosmetic and reconstructive surgery will likely have tissue engineering at its core.
<< Tissue engineering is no longer in the realm of science fiction. >> 16
Facial surgery RHYTIDECTOMY (FACE LIFT) Types of facelifts available SMAS lift: Most popular face and neck lift. Relies primarily on lifting the cheek/neck contour. SMAS stands for Superficial Musculo-Aponeurotic System. The operation tightens the face’s subcutaneous layer of muscles connective and fatty tissue. Mini Lift: The most basic lifting operation. The same incisions as for SMAS lift are used but only the skin is tightened. Mid Face Lift: This lifts sagging cheeks below the lower eyelid and corrects the drooping corners of the mouth. Mask lift (or Deep Plain Lift): A special technique is used to dislodge the cheek from the inside of the mouth. Skin, tissue and muscles are detached from the bone to achieve a long lasting affect. Endoscopic Lift: This procedure results in minimum scaring, and the most natural postoperative look. Starting at the hairline, tiny incisions are made to feed the endoscope and other miniature instruments beneath the skin. Every movement is traced on the video monitor. Sagging tissue is pushed back up and the skin realigned. This procedure’s success depends entirely on the surgeon’s qualifications and skills. Endoscopic brow and eyebrow lift: Brow and eyebrow’s appearance can benefit greatly from this surgical procedure, which produces no visible scars. Tiny incisions are made for inserting special surgical instruments under the brow skin. The skin is lifted and some of the brow muscles are removed, in order to deemphasise frown lines. The eyebrows can be lifted during the same procedure. Lip lift: If the upper lip and the nose are too far apart, this can be corrected with an invisible cut at the edge of the upper lift or just underneath the nose. If the problem is a narrow upper lip, one incision is made along the contour of the lip and another several millimetres above it. This second incision defines the new, fuller shape of the lip, and hence needs to be made extremely carefully. The strip of skin between two incisions is removed, the red of the lip is infolded from the inside, and the lip is sown onto the new upper edge. If the problem is an oversized lip, the incision is made on the inside of the lip. Again, a flawless technique is decisive for the success of the operation. Eyelid correction/blepharoplasty: The lifting of the upper or lower eyelid, is one of the most popular medical procedures. It requires minimally invasive surgery and rejuvenates tired looking eyes. Corrections are mainly for ‘hoods’ or ‘bags’ around the eyes to give a more radiant, youthful look. Upper eyelid lift: A CO2 laser is used for this procedure in order to remove the excess skin as well as muscle and fatty tissue underneath it. Lower eyelid lift: Recent techniques have focused on redistributing the fatty tissue instead of removing it. This method is known as the SOOF (Sub-Orbicularis Oculi Fat) lift. This is the only effective procedure for treating skin laxity and fat or fluid deposits in the skin directly under the eye.
Nose: Rhinoplasty, the medical term for a Nose job. The nose can be reduced, augmented, narrowed, straightened, widened, and reshaped by correcting the cartilage and bones. Nose bumps can be reduced, nose tips corrected or saddle noses enlarged. A nose operation needs to fulfil aesthetic as well as functional criteria; it is arguably the surgical procedure that transforms the face the most. The majority of operations last for one to two hours and are performed under full sedation. It is imperative to select a surgeon who specialises in rhinoplasty for the best aesthetic result and also in order to avoid operative risks or post-operative complications. Genioplasty/chin corrections Chin Augmentation: A receding chin is frequently augmented by using a pre-shaped silicon implant. Bones from the pelvis and the skull cap and cartilage from the ears or the ribs can also be used. An implant is inserted through an incision 2-4 millimetres from the lower lip. The cartilage can be removed to augment the chin. Chin Reduction: The bone is shortened or filed down but if an external incision is used stitches are usually removed after a week. Osteotomy: This is a complex procedure in which the surgeon shifts the chin bone forward and it is set using metal plates and screws which are removed after 6-12 months. Ears Ear correction or otoplasty: This is a procedure in which disproportionately large or protruding ears are remodelled and/or pinned back. A surgical cut is made on the back of the ear or in the concha, the cartilage is shortened then remodelled. Then the cartilage is fastened. Correction of the zygomatic arch: This is a procedure to emphasise the cheek bones. To achieve a higher cheek bones effect the surgeon implants autologous material or a silicone pad. Cheek bone operations are mostly performed on Asian patients. For European patients it is more common to augment or reduce the rim of the eye socket.
Body contouring Breast enlargement or augmentation mammaplasty: One of the most common cosmetic enhancement procedures, this is always performed using implants. These can be filled with silicone gel, a physiological saline solution or hydra gel. The most common implant contains silicone gel. The operation is carried out under sedation and last two to three hours. An incision is made, either in the armpit, below the nipple or in the fold under the breast. If the incision is made at the nipple a deflated inlay is implanted and filled once in place. Breast reduction or reduction mammaplasty: This is a more complex procedure than breast enlargement lasting three hours under sedation. It reduces skin, sub-cutaneous fat and gland tissue, after which the breasts are reshaped.
pregnancies and breast feeding. In some cases the breast lift can be combined with implants. Breast reconstruction after cancer or reconstructive mammaplasty: There are two
Breast lift or mastopexy: This procedure aims to correct the effects of gravity, weight loss,
methods for reconstructing the breast. Post-mastectomy, by using silicon implants following
tissue expansion or by remodelling the breast from autologous material. The latter procedure has been developed by Professor Dr. Axel-Mario Feller. The breast is reconstructed with an autologous tissue flap harvested from the buttocks or lower abdomen (this has the added benefit of a tummy tuck). If no excess abdominal tissue is available this could also be harvested from the back muscles although the latter surgical technique is very complex and demanding.
Liposuction/body contouring/lipoaspiration Liposuction is one of the most popular surgical procedures today. It is used to improve the bodies’ contours and requires first class technical skill and aesthetic judgement on the part of the surgeon. It is not suitable for overly obese patients because a maximum of three litres of fat can be removed. Liposuction addresses problematic areas that are immune to low calorie food and fitness training.
Techniques Tumescent technique: Small incisions are made on the fatty tissue and saline solution is pumped through these holes. The fat cells absorb this liquid and start to swell up which makes it easier to dislodge them. The loosened fat cells are removed using a thin suction canular. This procedure can last several hours and requires a considerable physical effort on the part of the surgeon. Power-assisted liposuction (lipolysis by vibration): Developed by Dr. Gerhard Sattler, this technique involves the use of an electrically powered canular containing 24 suction holes. The fat cells are agitated up to four thousand times per minute and are shaken out of the connective tissue. Only local anaesthetic is used with this procedure. Ultrasonic liposuction a.k.a. Ultrasound-Assisted Lipoplasty: This technique projects ultrasound waves into fatty tissue which explodes and liquefies fat cells without damaging the connective tissue. Electrolipolysis: This technique is normally combined with a hormone therapy for cellulite. An acupuncture needle is inserted into the fatty tissue and an electrical current causes it to drain out. After eight to twelve sessions the orange peel skin on the thighs is gone.
Liposuction works with different degrees of success for different problem areas. Once fatty tissue is removed it stays away permanently from the areas treated. Unfortunately, during subsequent weight gain it is deposited on other, untreated areas.
Body contouring with the scalpel If a patient’s skin is not flexible enough to contract around reduced tissue after liposuction a further surgical intervention may be required to contour the body. The downside of this is large visible scars. Abdominoplasty: A surgeon removes excess skin where a flab hangs over the lower abdomen, usually from pregnancy or extreme weight loss. It is a major procedure which can last up to two hours and involves a large loss of blood. The skin is cut right up to the lowest
ribs, sparing out only the navel. The surgeon pulls the undamaged skin downward and removes the excess skin. Brachioplasty (Upper arm lift): This procedure is suitable for patients with sagging skin around the arms when it is lifted. The procedure leaves long scars on the inside of the upper arm. Thigh dermolipectomy or thighplasty (Thigh Lift): Where skin has become lax and wrinkled around the inner thighs, a thigh lift can be performed on the inner and outer thighs or on the buttocks. Incisions (and scarring) run along the right side of the pelvic bones, around the inner thighs to the buttocks. Calf augmentation: The calves can be enlarged with hard or soft silicone gel implants if the calves are overly narrow or one bigger than the other. The surgeon cuts from the thigh, two pockets and implants are inserted. Buttocks augmentation: In this procedure pre-shaped silicone implants are used to round out flat buttocks. The implants are placed between the gluteus muscles.
GENITAL SURGERY Perhaps the most controversial, but increasingly popular branch of cosmetic plastic surgery. Phalloplasty (penis enlargement): An extension of up to 5cm and a widening of up to 2cm can be achieved through surgery under full sedation. Both procedures are challenging and need to be considered seriously. Vaginoplasty: Surgical procedure for reducing and enlarged vagina after giving birth. Labioplasty: Reduction or/and reshaping of the labia Hymen restoration: Where virginity needs to be re-established for cultural or religious reasons.

<< Attention to detail distinguishes the exceptional from the ordinary.>>
Otoplasty Correcting prominent ears (or a distorted ear) is one of the most rewarding procedures in plastic surgery, resulting in a tremendous change in the patient’s self-confidence and self-esteem. Dr. White believes that ear re-shaping, or otoplasty, should involve much more than simply “pinning the ears back.” With an intricate knowledge of ear anatomy and his artistic ability, he sculpts the ears into the desired shape and position. His combination of specific techniques allows him to create natural-appearing ears.
Facial Rejuvenation “Turning back the clock” on the ageing process through facial rejuvenation is now more popular than ever. Whether removing the excess skin and fat around the eyes (eyelid surgery), lifting the brows and smoothing the forehead wrinkles (brow lift), or sculpting the face and neck (face lift), facial rejuvenation helps patients both look and feel younger. A combination of procedures is often indicated so an individualised approach is best. A well-trained surgeon with an artistic sense and a conservative approach is a must. Attention to detail is especially important in facial surgery due to the complicated anatomy. It is for this reason that Dr. White first devoted six years to becoming fully trained in head and neck surgery before proceeding with his plastic surgery training. His approach is to create long-lasting, natural results as inconspicuously as possible.
Nasal Surgery Nose re-shaping, or rhinoplasty, is often considered the most difficult operation in plastic surgery. A poor result cannot be simply “covered up,” since the nose is a focal point of the face. Perhaps this explains many surgeons’ reluctance to perform rhinoplasty. Dr. White’s dual background in plastic surgery and ear, nose, and throat surgery (otolaryngology) has provided him with extensive experience in nasal surgery. Understanding the complex inter-relationships of surgical changes to the skin, bone and cartilages of the nose requires a thorough understanding of nasal anatomy and years of experience with nasal surgery. Frequently, he is asked to operate on patients who are not pleased with the results from other surgeons after one or more nasal operations. Dr. White’s goals in rhinoplasty are not only to create a nose that appears natural and in balance with the patient’s other facial features, but also to improve the patient’s ability to breathe through the nose. He frequently performs septoplasty and turbinoplasty to improve the airway and breathing by straightening the nasal septum and making the turbinates smaller. Turbinoplasty has the added benefit of improving or alleviating nasal allergies.

Breast Lift and Breast Reduction A breast lift, or mastopexy, is performed to restore the youthful appearance of the breast after pregnancy or weight loss. Lifting the breast, elevating the nipple, and decreasing the size of the areola create this more youthful look. Breast reduction is a similar procedure except that the breast is also reduced in size. With both procedures, Dr. White takes great care to sculpt the breast into the most aesthetically pleasing shape. His closure techniques together with his particular postoperative care instructions help to maintain the shape and minimize scars.
Breast Augmentation Dr. Steven J. White offers the latest advance in breast implants: the “Optimized, Rapid Recovery” Breast Augmentation, the culmination of over a decade of experience which allows him to optimise every aspect of the procedure. His goal is to create the most beautiful breasts for each patient. The operation seems deceptively simple — making the breasts bigger by putting in implants. Achieving exceptional results, however, requires detailed pre-op planning, precise surgical technique, and careful follow-up. Dr. White’s “Optimised, Rapid Recovery” Breast Augmentation does just that … in a series of 7 steps. Step 1 involves listening carefully to understand what the patient is hoping to achieve. Before
After surgery by Dr. White
Step 2 entails taking several measurements of the breasts and chest, paying close attention to asymmetries, which are surprisingly common. Selecting the proper size implants is one of the most important aspects of surgery. Dr. White believes this is best achieved by matching the implant to the patient’s tissues and chest wall and then adjusting the size according to the patient’s desires. Step 3 is comprehensive education. Every patient is thoroughly informed about the procedure and what to expect. The next three steps occur during surgery. Step 4 involves precise surgical dissection (not blunt dissection) to create the “pocket” under the muscle for the implant. Dr. White’s specially designed instruments allow him to use the smallest incision to minimise the scar. Step 5 is when he uses “sizers” to carefully match the implant to the patient’s tissues to create the most natural look for each patient. Step 6 is to sculpt or shape the breast tissue (when needed) to achieve the prettiest breasts. Step 7 is the follow-up care. Dr. White has developed a comprehensive post-op program for rapid recovery and exceptional results. His precise surgical technique avoids bruising and minimises pain so his patients can enjoy a quick recovery. They begin range-of-motion arm exercises on the day of surgery. Many patients feel so good they go shopping or out to lunch the next day.

Body contouring Liposuction Liposuction Body sculpting with liposuction has continued to grow in popularity for both men and women. Dr. White believes that success in liposuction is determined by three factors: the amount of fat removed, the improvement in overall body shape and contour, and the smoothness of the skin after liposuction. He optimises each aspect using his advanced Circumferential Micro-Lipo Technique. His specially designed, custom-made cannulas (hollow instruments for fat suction) allow him to create a more natural look with less chance of unevenness. The circumferential approach allows for even greater fat removal and better body sculpting while using his very small diameter cannulas for smoother contours. A tumescent (superwet) technique is used to minimize blood loss and allow larger volumes of fat to be removed more safely. Recovery is faster and most patients are able to return to work after several days.
Male breast reduction Breast reduction for men is performed to correct a condition known as Before

gynecomastia, or male breast enlargement. It is very common, yet seldom discussed. Just as many women Liposuction of chest and torso
in our society are self-conscious about having small breasts, so too are many men concerned about having enlarged breasts. Dr. White has developed a technique to treat gynecomastia that leads to impressive results with fewer, less noticeable scars. He uses special liposuction instruments for greater fat and tissue removal so that direct excision of tissue is rarely necessary. Many men choose to add liposuction of the torso as well to diminish the abdomen and love handles for an even greater physique.
Tummy tuck Dramatic results can be achieved with a tummy tuck since it narrows the waist, tightens the tummy, and removes excess skin and fat. As an added bonus, any stretch marks or unsightly scars below the navel are typically removed at the same time. Although the traditional tummy tuck procedure often results in an “unnatural, operated look,” particularly in the upper abdomen and around the umbilicus (or belly button), Dr. White has perfected several techniques in order to obtain a more sculpted abdomen and a more natural-looking belly button. This takes more time in surgery; however, he believes the final result is worth this extra effort. For most patients, a fine scar is well hidden beneath the bikini line. Due to his refined techniques and his special postop protocol, most patients enjoy a quick recovery and are usually only taking Tylenol for discomfort for just two days after surgery. Combining a tummy tuck with liposuction or a breast enhancement procedure can lead to even more outstanding results.

Steven J. White, MD on Beauty Beauty is a look that pleases the eye. It captures our attention, draws us in, and creates an irresistible urge to gaze and stare. Looking at beauty brings joy. It makes us happy. It can take many forms… Art. Nature. People. Almost anything. Beauty, like art, can speak to each of us in different ways. It is indeed in the eye of the beholder. And yet true beauty is that rare and special quality that we all admire. Its appeal is universal and

undeniable. A beautiful person is someone blessed with the combination of features that come together to form a look that we admire. It is balance and proportion artistically expressed in the human form. Although each individual feature may not be perfect, the collective effect is indeed beautiful. Beauty is a quality we long for. It is our desire, our wish, our goal. It is that which we seek and strive to obtain. It drives us to want to look our best. Looking good makes us feel better about ourselves. This is the essence of plastic surgery – to bring joy to the individual by enhancing their beauty. Seeing how a small change on the outside can lead to a big change on the inside is the true reward for a plastic surgeon.
The most beautiful woman Andie MacDowell personifies beauty, style, and elegance. She has a beautiful face, a gorgeous smile, radiant eyes, and a knock-out figure. She has curves in all the right places. But yet her beauty transcends her appearance. She exudes warmth and charm. She is genuine, sophisticated, and immensely talented. Even her voice is distinctly beautiful and unique. She is glamorous and yet down to earth. She has that special quality that sets her apart. A timeless modern beauty. Captivating, engaging, and truly beautiful.

FEATURE ALTHOUGH THE ATTRIBUTES OF ICONIC BEAUTY REMAIN CONSTANT, it is equally true to say that beauty is in the eye of the beholder. For individuals and entire cultures there are both subjective and objective components to attraction – whether a preference for blonde or brunette here, an appreciation of curves or the slimmer form there, or something more fundamental.
Through TV, film and adverts, the media is a powerful force in conditioning our ideas of contemporary beauty, independent though it remains of timeless, “iconic” factors. A simple comparison of war-era classic films with the modern media makes the point; actresses such as Keira Knightley and Liz Hurley have replaced former visions of beauty like Audrey Hepburn and Ingrid Bergman.
Since the Second World War, as new concerns of health and fitness have gripped developed societies, so our understanding of what is attractive has changed. This trend is no more evident than in the changing face of “beauty” over the last two decades. In earlier eras, when food was often scarce, fat was frequently considered attractive as it could indicate prosperity and a higher chance of survival – hence the cultic statuettes of grossly overweight women found by archaeologists researching primitive cultures. Perhaps as a reaction to our times of plenty, when obesity is reaching epidemic proportions, today's typical beauties tend far more towards the slender than the shapely. The curvaceous models of the 1980s were gradually replaced until, 20 years later, they are now “Size Zeros”. Changing, too, is the image of carefully made-up and maintained beauty common mid-century. Despite being no less deliberate and manufactured, contemporary beauty presents itself in a more natural, “effortless” look: the appeal of Kate Moss and the new cohort of supermodels over the Glamazons of yesteryear.
Fashion is, by nature, ever-changing. Designers and the media both drive this flux and are influenced by it. This is as true of the human body as it is of the clothes that adorn it.

Conclusion Dr Albrecht remains a world expert on Plastic Aesthetic surgery, skin treatments and antiaging techniques. His belief is that a person’s appearance is a reflection of their inward state, and vice versa. Rejuvenation of the outward appearance is therefore often beneficial for self-esteem and holistic well-being.
Dr. Albrecht on Beauty There is no one beauty. Beauty, in general, makes us feel good. It can be intellectual beauty, sensual beauty, spiritual beauty; it can create intellectual pleasure, a physical sensation of desire, strong attraction, or a warm and positive emotion. It relies on shapes, colors, sounds, movements etc., and on a mysterious perception of vibrations. Plastic beauty talks to our mind or senses, inner beauty talks to our soul. Ideal beauty is the right balance between these two. As a plastic surgeon I work on Plastic beauty. This one has rules and is almost mathematic. It relies on operative techniques as well as the surgeon’s personal touch to produce a mysterious, natural, beautiful look.

beauty and sensuality
IF BEAUTY IS IN THE EYE OF THE BEHOLDER, is the beholder a male or female? Until very recently, the male beholder certainly defined our social version of beauty. But how has this perception arisen? A woman is an object of attention: men look at women and women are aware of it. When a woman walks into a room, she is observed and the observers are men. Consider how the female form has been the subject of so much figurative art. Women, often nude women, have been painted for men to admire. Similarly most pornography caters for male tastes because – to give it a positive spin – the female face and body have always been seen as something to admire. That is an historical truth, and that is why so much art is representational of women. The men pay for it and the women pose: Hugh Hefner and Larry Flint understood this instinctively and didn’t require a lengthy rationale. The reasons men chase good-looking women are rooted deep in our biology. We know attractiveness is a desirable asset, but what is it that defines the physically attractive? Does it have to be supermodel good looks or is it just being well presented? It’s a mixture of both, of course. Even supermodels have their physical imperfections and you can see, in how they dress, that most of them are observing the ‘What not to wear’ rules so as to maximise the good parts and minimise the bad. When men are attracted to women, it is not necessarily for the purpose of reproducing with them, which limits the credibility of the biological conditioning argument. Of course, men are more attracted to sex for its own pleasurable sake than women since, amongst other things, they don’t have the babies. And they are aroused by the obvious parts of the female body that signal sexual pleasure: their lips, their breasts, their buttocks. This was all determined by nature a long time before you and I arrived on the scene but even allowing for the fact that beauty is not all that matters, it does carry some biological weight.
Beauty may not be everything, but it is one of the strongest fixations of the human species.

non-invasive cosmetic interventions as well. His Stevens’ Institute Medical Day Spa is acknowledged as a leader in non-surgical cosmetic solutions with treatments ranging from state-of-the-art lasers to skin care, filler injections and Botox wrinkle reduction. Dr. Stevens was recently inducted into the International Society of Aesthetic Plastic Surgery, an organisation that aims to teach standards of care, to help refine plastic surgery techniques and to evaluate and promote emerging trends and technologies. He continues to be one the world’s leaders in
introducing and developing the use of the laser and other high-tech instruments into the field of aesthetic cosmetic surgery.
Speciality Dr. Grant Stevens’ passion for cosmetic plastic surgery has made him one of the leaders in the specialty. With over 20 years’ experience and over 20,000 cosmetic surgeries on patients from all over the world, he has mastered the techniques allowing him to perform several procedures during a single surgery. In the operating rooms of his Marina del Rey based Marina Plastic Surgery Associates, Dr. Stevens performs many of these trademark operations in addition to numerous non-surgical options to
achieve the ultimate pursuit of beauty and wellness. Dr. Stevens and the Marina Plastic Surgery Associates are internationally recognised for their experience with cutting-edge technology including laser breast surgery, power-assisted liposculpture and minimal incision facelifts. The teams pioneering research with laser surgery and power-assisted liposculpture contributed to the recent major advances in the field of body contouring. Dr. Stevens has perfected a novel technique called the Stevens Laser Bra for Breast Reduction, Breast Lift and Breast Augmentation. The Stevens Laser Bra technique uses a CO2 laser combined with surgery to correct sagging breasts that occur because of aging and childbearing. As the skin loses its elasticity, the breasts lose their firmness and shape leading to a drooping effect on the chest. In the past, plastic surgeons would perform a traditional procedure such as mastoplexy or a breast lift that attempted to surgically correct the issue with varying results. Dr. Stevens noticed the advantages of laser assisted face-lift surgery that shrunk the sagging skin on the faces of his patients and believed that lasers could do the same for breasts. By effectively creating an internal brassiere using the CO2 laser, his new technique produced a beautiful, long lasting lift of the breast. Compared to traditional methods, the laser procedure can decrease the time needed to complete the surgery, making it a safer option as less time is required under anaesthesia. Recovery time is quicker and scarring is minimised by utilising dissolvable sutures placed deep beneath the skin to eliminate the need for painful suture removal. The CO2 laser de-epithelialises the patient’s own skin and creates an internal bra that is attached to the chest wall, holding up the breast tissue. This creates a more aesthetically pleasing shape to the breast maintaining a perky uplifted contour that mimics nature. Over the past several years, Dr. Stevens has performed hundreds of Laser Bra surgeries with impressive postoperative results withstanding the test of time. Dr. Stevens developed the Mommy Makeover after repeatedly hearing “I want my figure back” from women having children. His pioneering research into combined surgery showed that multiple procedure operations were a safe and effective way to achieve maximum benefits with minimal recovery time. Since then, both plastic surgeons and patients from around the world have come to learn as well as undergo these combined surgeries. This surgery generally takes three to four hours and frequently fly-in patients can return home within a week to 10 days. Most patients receive a laser breast lift along with a tummy tuck and body sculpting. Dr. Stevens’ key to successful body sculpting is the Power-Assisted Liposculpture, (PAL), which utilises a vibrating cannula inserted through tiny incisions to gently shake the fat cells loose. This facilitates suction removal of fat with more precision and less trauma than other liposuction techniques. Recent innovations in liposuction make this procedure highly desirable among

healthy, active mothers who just need a little extra help getting rid of the superfluous fat deposits remaining from their pregnancy. The tummy tuck or abdominoplasty is an essential part of the Mommy Makeover. Once skin and muscle tissues have been stretched from pregnancy, they frequently will not retract to their pre-pregnancy form. Stretch marks, belly button deformities and skin laxity are all reasons Dr. Stevens would include the tummy tuck in a Mommy Makeover. The tummy tuck surgically tightens the abdominal muscles and removes excess skin and fat to create a svelte waistline. As with most of Dr. Stevens’ procedures, the tummy tuck can be performed on an outpatient basis under general anesthetic. There are several different types of tummy tucks, but all of them involve the removal of excess skin and fat using a lower abdominal incision. The abdominal muscles are tightened and the remaining skin is then pulled downward. After the procedure, many of Dr. Stevens’ patients have been inspired by their improved shape to lose additional weight. In addition to surgical treatments, The Stevens’ Institute Medical Day Spa, specialises in non-invasive aesthetic therapies such as laser treatments, skin peels and cosmetic makeovers to maintain long lasting, natural results. Dr. Stevens realised early in his career, that excellent surgical results started with healthy skin. He bridged the gap between surgery and skin health, and created one of the first medical day spas. Today, many non-ablative and non-invasive treatments are performed in The Stevens’ Institute Medical Day Spa, to not only prepare the skin for surgery, but also as a maintenance and preventative measure. The Spa gives patients early access to the “The Technology of Beauty”, the cutting edge procedures and products that are developed in the international aesthetic research labs. Clients can enjoy a vast array of services, such as permanent makeup, laser hair removal, ThermaCool non-surgical facelifts, Botox wrinkle reduction and injectable fillers to sculpt the facial contour and replace the loss of fat beneath the skin. Dr. Stevens has numerous lasers at his disposal including Fraxel, Cynosure, Thermage, XEO, DUSA and Titan, each having individual properties to erase even the most difficult scars, red spots, brown spots, vascular lesions and wrinkles. The team of experienced Physician’s Assistants are all experts in the latest injectable products to include Juvederm, Botox, Restylane, Perlane Radiesse, Sculptra, and ArteFill.

Conclusion Dr. Stevens has dedicated his career to women with the stated goal of helping them remain beautiful. His focus on mothers has redefined the field and encouraged the development of new techniques to fit the specific needs of this group. Through the use of laser technology, power-assisted liposculpture and multiple procedure single surgery, Dr. Stevens has revolutionised the way cosmetic surgeons practice. In addition to his surgical innovations, Dr. Stevens’ bedside manner and focus on the individual have gained him an international following. For these specific skills, he is sought out by patients the world over to achieve what was once thought impossible.

Becca’s mom flew with her from Alabama to Los Angeles for the breast reduction procedure. “The moment we met Dr. Stevens, we knew everything would be fine,” Becca remembered. “Dr. Stevens reassured my mom that he would take care of me as if I were his own daughter. And he did.” Dr. Stevens explained his approach to breast reduction in great detail. He said he would separate skin from breast tissue and remove portions of each, but that he would preserve tissue under the nipple without severing the nerves and blood supply. That way, even with her breast size much reduced and her nipple in a new position, sensation would most likely be preserved and Becca would recommended I have his ‘Laser Bra’ – the extra internal support he could give me using a piece of my own tissue inside my breasts. That made sense to me – it sounded like it would help keep my new breasts in position longer.” Becca’s surgery and recovery went without a hitch. She took pain medication for several days and admitted to being irritable occasionally, but she and her mom were out shopping and sightseeing in Southern California within four or five days. “It felt more like a vacation than a trip to have surgery,” Becca said, More than anything about that time, she remembers her feelings about her new body. “Within several days my neck and shoulders started feeling better. When I looked down and saw how much smaller my breasts were, I said, ‘This is so great.’”
<< It felt more like a vacation than a trip to have surgery. >>
Three years later That was three years ago. Today, Becca and her husband, Monty, have an eighteen month old daughter Becca has successfully breastfed. Not only that, having gone through the physical changes of childbearing, her breasts in their internal Laser Bra still look great. Becca has lost weight too, feeling very comfortable with physical activity. “Running around after my daughter is so much easier than it would have been before!”
Christmas 2005 Becca poses with her daughter, Abby, a healthy, breastfed newborn. “Even after breastfeeding, I can honestly say they’re perky!” So, what happened to that young woman who didn’t deserve plastic surgery? She has been transformed in more ways than one. Becca’s reasons for being hesitant for so long were complicated. “I believe it’s different here in the South,” she said. “Though plastic surgery is becoming more and more accepted, most people still don’t talk about rushing out to get it. I was just an average person - healthy, with no other real problems. It seemed like an extreme measure to have surgery, and I thought people would judge me, saying that the reason I had large breasts was because I had gained a little weight. Also my husband and I had other places to spend money, and though he was completely supportive and even encouraging, I didn’t want to cause a big expense. I think I just felt that breast reduction was out of my reach.” “Once I flew to LA, everything changed for me,” Becca said. “I was just so impressed by Dr. Stevens and his practice; I would have done

almost anything to find a way to have the surgery. Even without insurance; I would have saved for years. It was that important to me.” To others dreaming about breast reduction, Becca has much to say. “Educate yourself,” she begins. “Don’t let problems get in the way. And don’t settle for a surgeon you’re not comfortable with. Don’t make flying in and out of Los Angeles for breast reduction a big deal. It’s no harder than getting the procedure done locally. Take a family member and make a good trip out of it.” “Three years later, I still feel elated. I tell anyone who is suffering like I did how happy I am.” No matter what she felt about the surgery and herself before, Becca knows the truth now, and that’s why she wanted others to hear her story. “You know your body best…when you’ve done all you can, don’t feel guilty about taking the next step toward health!”

“Breasts are so significant in our culture,” Christy Carlton said, “in fact in any culture. Breasts are about nurturing; they give love and life. My breasts are central to my feelings about being a woman.” To Christy, breasts are meaningful in form as well as function. “I’m a visual person,” she said. “I have an appreciation for the human body. Beautiful breasts are important to me.” For Christy, therefore, it was natural to pursue her idea of “the perfect breast.” “It’s not that I didn’t feel attractive before,” she explained. “It’s that after three children, my breasts weren’t as full as they used to be.” After breastfeeding, Christy said, “They just never came back.” For the Los Angeles area resident, breast enlargement was a natural step. Though Christy says she knew from her early 20s that she wanted to take steps to fine tune her looks, everything fell into place for her two years ago. Just reaching her mid-40s, she was a few years out of her marriage and she felt free to pursue goals that were important to her as an individual. And her 25-year high school reunion was just around the corner; she wanted to look her best. “Charlie gets you on your way to creating the perfect breasts. Dr. Stevens is the man with the
Christy called Charlie Sheridan, Director at Marina Plastic Surgery, to talk and get set up with Dr. Stevens. “Charlie is an amazing woman,” Christy said “She makes you feel completely at ease.”
Christy had done quite a bit of research about breast enhancement in the Los Angeles area before her consultation with Dr. Stevens. “I knew I wanted gummy bear implants,” she said. “I had read all about them and knew they were for me. I also knew what ‘profile’ means and knew which profile would probably be best for me.” Nonetheless, Christy and Dr. Stevens had a lot to talk about during the appointment. “We reviewed photos together so I could show him the look I had in mind. And of course I didn’t know what size implant would give me that look. But Dr. Stevens did”. Christy says he gives a lot of his energy and attention to creating the perfect breast for you. “Dr. Stevens

is the man with the magic hands.” A combination breast augmentation/liposuction/tummy tuck procedure, plus ThermaCool Face Lift, was scheduled for Christy several months before her high school reunion. She went to the event in a “fabulous” dress, an outift she could not have carried off with the same impact before surgery. “An old classmate of mine commented that I’m just like a fine wine,” she laughed. “He said I just keep getting better.” “It’s great to have the opportunity to reflect on all that’s happened
two years after surgery,” she said. “Today it’s hard to remember how I was before the procedure. But I know that after two years I am still excited with the results. I feel very beautiful.” Christy has her perfect breasts. They are a full size 36C and they’re firm and shapely. Depending on what she’s wearing, she doesn’t always have to wear a bra. “Yes, they’re perky!” Christy says. “Even my nipples look great.” Discussing what perfect breasts might mean to other women, Christy says everyone should have her own idea about that, with emphasis on “her own.” “Even though I had started dating again,” she commented, “the surgery was for me and no one else. Make sure you do this for yourself.”

I believe from the bottom of my heart I had the BEST plastic surgeon in the world! I will definitely DEFINITELY be going back for future “improvements” on my body.
Pat A. (on her breast augmentation experience with Dr. Grant Stevens) Dear Dr. Stevens, Just wanted to follow up with you after 21 years. You gave me a breast implant surgery years ago in your last month as chief resident at Washington University. I was a nurse there and got a great deal financially having ‘students’ do surgery and anesthesia. To make a long story short, I saw you interviewed recently on some celebrity related program my daughter had on. I thought of meeting you in 1984 or 85 and how you told me that your dream was to work in California - helping beautiful women stay that way! Congratulations on living your dream. Your bio and web site are great. My implants are GREAT, still. I am nearly 50 years old and have soft, shapely, healthy 36C breasts. Not only that, but I have breast fed babies since the surgery without any problems. Just thought you might be interested in some of your first and finest (!) work. Good luck and continued success!
WHY DO CELEBRITIES LOOK GREAT? THIS MAY SURPRISE A FEW PEOPLE – BUT THE SINGLE GREATEST QUALITY THAT SEPARATES US FROM THE VAST MAJORITY OF PEOPLE IN THE PUBLIC EYE (ESPECIALLY THOSE WHOSE JOBS DEPEND ON THEIR LOOKS) IS THAT THEY’VE HAD A LOT OF SURGERY, AND THAT THEY HAVE IT DONE ALL THE TIME. It is not unusual for celebrities, if that is what we must call them, to have treatments every 6 months to a year – facelifts, nose jobs, eyes tightened, cheek implants, Botox and so on. They have to, otherwise they would simply not look anywhere near as beautiful or young as they do. The question of whether people in the public eye have had cosmetic surgery is thus obsolete. It is rather a question of when and what, rather than if.

Dr. Rubinstein on Beauty What defines beauty? Beauty is the subtle combination of soft lines, volume, and proportion. There is a quality of softness to any beautiful feature. Hard angles and sharp lines are generally not perceived as beautiful. Volume is another component of beauty. A youthful face has volume and youth is attractive in itself. With age, one begins to lose volume and acquire a hollow look. A breast would not be beautiful without appropriate volume. Plastic surgery can rectify the loss or absence of volume but never at the expense of proportion. Too much volume is equally unattractive. Furthermore, each feature (whether body or face) must be in balance with the rest. Indeed, there are many beautiful people that don’t have any distinctive individual features but this is compensated by excellent proportions. Ultimately, beauty is in the eye of the beholder and the most important “beholder” is oneself. Each person is the best judge of their own beauty. It is my mission to understand each patient’s vision and aspirations and help them achieve the desired improvement. My favorite example of beauty is Kim Basinger. She has a beautiful face and body with excellent proportions.
The operating room
THE DEFIANCE OF AGEING ITSELF IS SUCH A RELATIVE TERM. WE CAN AGE OUR BODY BY WILD SWINGS IN WEIGHT AND LIFESTYLE AS WELL AS EXCESSIVE SUNBATHING, SMOKING, DRINKING, OR EVEN EXERCISE AND DIETING. IN GENERAL, EXERCISE AND DIET WILL HELP KEEP US IN SHAPE AND DEFY AGEING TO SOME EXTENT. COSMETIC SURGERY CAN ADDRESS ISSUES THAT EXERCISE AND DIET ALONE CANNOT REMEDY. As we get older we usually lose some of the subcutaneous fat on our face and look more skeletal. While it is trendy to look skeletal, this is not necessarily a good thing – the image of youth is to have a nice smooth look with a fuller face. This is derived from the layer of fat beneath the skin, which covers the underlying muscle. As one gets older, this layer is difficult to recreate – excessive weight loss merely compounds the difficulty. Cosmetic surgery cannot do everything… but we are getting there. There is a section of society that seeks to retain and perfect their youthful looks. Their obsessive drive towards perfection has created something approaching a new breed of people: the super-enhanced who, while growing older, appear not to age. A necessary condition of joining this breed is relatively unlimited funds, and the desire to belong – a kind of ‘celebrité oblige’– but what truly marks out the super-enhanced from the rest of us is the quality of eternal youth. Of course, when this is mentioned, the critics begin reaching for their Dorian Gray parallels. Unlike Dorian Gray, who aged completely when he saw his portrait, there need not be some sudden cataclysmic ageing event when you come due for your next facelift – providing you maintain the look regularly. The startling thing is that women in their sixties, ten years on from their first facelift in their fifties, still look ten years younger than their peers. Until now the give-away has been the hands and décolleté which reveal the true age of the canvas. Yet even these can be corrected. Remember: a continuing relationship with your plastic surgeon is not some Faustian pact with the devil. Agelessness is not bought by selling your soul but by paying your surgeon’s fees – although the ancient family heirlooms might have to be sold to achieve it.

Speciality Dr. Pitanguy has influenced all aspects of plastic surgery, but has had the most impact on facial procedures. Having performed over 8000 “face-lifts”, his techniques perfected the art by developing ways to minimise scarring and achieve a completely natural look. He single-handedly made Brazil the vanguard of aesthetic and cosmetic surgery and the most popular destination for plastic surgery outside of the United States. Last year alone, over 50,000 foreigners came to Brazil seeking health treatments. Dr. Pitanguy’s luxurious private clinic on Rue Dona Mariana in Rio de Janeiro caters to VIPs and wealthy clients willing to pay a premium for the best surgeons. However, compared to United States and European standards, the costs are remarkably reasonable. The medical and surgical staff consists of highly qualified professionals who practice under the guidance of Dr. Pitanguy utilising the latest state-of-the-art equipment. Dr. Pitanguy himself operates several times a week on carefully selected patients. The facility is equipped with four advanced operating rooms designed to meet the highest international specifications and 14 private suites outfitted to lodge post-operative patients and their families in premium comfort.

Patients even have the option to recover in luxury on Dr. Pitanguy’s private island, Angra dos Reis Bay, off the coast of Rio de Janeiro. In addition to surgery, the clinic performs aesthetic treatments geared towards the health and beauty of the body to achieve complete well being and sustained results. This concept of integrated medicine gave rise to the Ivo Pitanguy Clinic Health and Beauty Center. A team of beauty professionals including physiotherapists, psychologists and massage therapists provide pre- and post-operative patient support to round off the full spectrum of care. The clinic is headed by Dr. Pitanguy’s daughter, Dr. Gisela Pitanguy who is continuing in her fathers footsteps. Dr. Pitanguy’s legacy of philanthropy and service to the community is demonstrated through the long-running Ivo Pitanguy Institute which renovated a ward at the public Santa Casa da Misericórdia Hospital. Over the past forty years, Dr. Pitanguy and his team of 40 surgeons have operated on impoverished patients free of charge. trapped under a crane. She went on to walk again,
helping to solidify the reputation of the New York University Microsurgery Service, which he helped to establish. His experience in trauma and facial nerve damage has made him a leading authority The rooftop greenhouse
on facial reconstruction. This expertise has carried over into his cosmetic work making him one of the most sought after plastic surgeons in the world. Despite his popularity, Dr. Baker still finds time to operate on non-cosmetic patients who require complicated surgeries.
Credentials, Publications and Awards Dr. Baker is a board certified Diplomate of the American Board of Plastic Surgery and a Professor at the New York University School of Medicine. He received his medical degree from the Columbia College of Physicians and Surgeons, where his father was a professor and chairman of the department of otolaryngology. In recognition of his outstanding work, Dr. Baker received the Farleigh Dickinson University Distinguished Achievement Award and The American Academy of Achievement Award after saving the severed hand of a music student. His success has made him one of the most sought after plastics professors and he has taught his techniques in hundreds of conferences around the world. He has made 15 videos on Facial Paralysis, Nasal Reconstruction and Face-Lift Techniques and has performed numerous live surgical demonstrations throughout the United States, Canada, Europe and South America. He is consistently listed in America’s Top Doctors, the national guide identifying the top 1% of doctors practicing in the United States, and is regularly featured in New York publications.
Speciality Dr. Baker developed and popularised the minimally invasive short scar face-lift utilising smaller incisions than traditional facial procedures. Designed to conceal scarring and creating a more natural result with a buoyant appearance, the short scar approach is useful in younger patients or those with previous facelifts. The hidden incision line allows patients to wear a “pony tail” pulled back and decreases the risk of hair loss at the site of surgery. The short scar lift is performed with mini-incisions along a deep section of the facial plane, allowing the simultaneous removal of skin, fatty tissue and muscle to provide longer lasting results. Unique to Dr. Baker’s short scar process is an improved facial lift and redefinition leading to a more vertical appearance that helps define the face and cheek bones. As not all patients will have the desired effect, the short scar lift also allows Dr. Baker to extend the incision during the procedure and modify the surgery into a traditional face-lift. The surgery can be performed in half the time as traditional methods with quicker healing periods allowing patients to return to socialising and work within weeks of the procedure. Although famous for his facial surgeries, his repertoire includes liposuction, abdominoplasties, breast augmentation, reductions, full body lifts and numerous other procedures to complete the total cosmetic package. Dr. Baker’s famous bedside manner is known throughout New York and many of his patients are repeat customers.
Conclusion Dr. Baker’s expertise and knowledge of facial anatomy has gained him the reputation as the best face lift surgeon practicing today. His innovative approach and pioneering techniques have revolutionised the way other plastic surgeons perform facial aesthetic surgery. Due to his continued research and reconstructive surgeries on post-traumatic and cancer patients, Dr. Baker continues to refine his techniques and procedures. Those who lunch on the upper east side of New York can be confident many of the women in the surrounding streets have the “Baker Touch”.
Dan Baker on Beauty: The primary requirements for human beauty are facial and physical characteristics which are exceptional and have universal appeal to our eye. There are of course physical variations in different races which represent beauty. In addition beautiful people possess an elegance and charisma and often sexuality which sets them apart from the overwhelming majority.
Who is the most beautiful woman in the public eye? Why? and How? There is no "Most Beautiful Woman", but many from different cultures and races. Our lives are global today and beautiful women come from around the world. The majority who are considered "Most Beautiful" do not sustain that label because beauty requires more than just physical attributes. Some of the "Most Beautiful" women who have withstood the test of time include Cleopatra, Marlene Dietrich, Audrey Hepburn, Princess Diana, Marilyn Monroe, Elizabeth Taylor, Queen Raina of Jordan.

Speciality Dr. Aston’s chief areas of research are facial anatomy in aesthetic surgery and developing techniques to gain the best possible surgical results. He is perhaps best known for pioneering FAME (finger-assisted malar elevation), a face-lift technique that manipulates the soft tissue foundations as well as the skin of the face, achieving rejuvenation without an “operated” appearance.
<< I treat every one of my patients as if he or she was the most important person in the world. >> 86
Dr. Aston’s clinic is famous for its staff of dedicated nurses and assistants (known as ‘Aston’s Angels’, some of whom are ex-models), meaning that hospital visits are brief and patients can rely on unparalleled round-the-clock aftercare for as long as they need it. Men account for an increasing percentage of his patients - around one in six nose, eye and liposuction operations and one in ten face lifts.
Conclusion ‘I treat every one of my patients as if he or she was the most important person in the world. A surgeon deserves fame only if his latest operation deserves it.’
Dr. Aston on beauty: “It is impossible to define beauty but you know it when you see it; whether it is a rose or a woman. From an anatomical and mathematical standpoint one can define various angles and proportions that make a balanced and beautiful face but beauty is in the ‘eye’ not in the ruler. For example, few would argue that Grace Kelly was one of the most beautiful women in the world. She not only had an aesthetic harmony to her facial features but also a look of softness and elegance.”
<< She not only had an aesthetic harmony to her facial features but also a look of softness and elegance. >>
THROUGHOUT THE AGES AND ACROSS VASTLY DISPARATE COUNTRIES AND CIVILISATIONS, human appreciation of beauty has remained remarkably constant. For all the natural variation in what is considered “beautiful”, as different qualities and attributes are emphasised by individuals and whole cultures – not to mention carefully directed by the media – the consistency evident across so many human boundaries is all the more striking.
Many of these qualities are markers for fertility, ingrained into our collective consciousness by thousands of generations of evolutionary reinforcement. Signals of youth and health, such as good teeth and a clear complexion, are obvious examples. Others are more subtle. The classic hour-glass figure, appreciated by so many men, is a case in point. Women with a waist-to-hip ratio of 0.7 are repeatedly judged to have the most attractive figures, and there is evidence that these measurements are also the healthiest. The preference for these proportions holds true for iconic beauties from the Venus de Milo to Marilyn Monroe: Size Zero may be a current obsession, but curves are never going out of fashion.
Whilst the elusive quality of “beauty” cannot entirely be reduced to a clinical formula, there are mathematical principles underlying the construction of the face and body, the perfection of which are often exemplified in the most attractive people. The Golden or Divine Ratio, a proportion of approximately 1 to 1.618, appears time and again in nature in both plants and animals and has, unconsciously and deliberately, been mirrored in art and architecture through the centuries. Leonardo da Vinci's drawing Vitruvian Man famously used it to emphasise the proportions of the ideal human body.
Spirals based on the Divine Ratio are found in patterns of seeds on flower heads, and the shells of snails, as well as the shape of the human ear. Faces of iconic beauties, from the Mona Lisa to almost every top model of the present day, turn out time and again to be governed by the divine ratio, with the size and spacing of eyes, nose, ears and mouth – even the widths of the teeth themselves – following this same precise mathematical formula.
Beauty, it seems, is neither skin deep nor fleeting.

Dr. Hober believes in enhancing patients’ natural attributes and achieving a subtle, natural appearance in the final result. Patients’ needs and wishes are paramount as each is treated on an individual basis, having not only different requirements, but also different expectations. Dr. Hober places high value on understanding these expectations before planning the operation, and most importantly customising every procedure.
Dr. Magnus Hober on beauty: ”To qualify beauty is difficult. ‘What’s in a name? That which we call a rose by any other name would smell as sweet,’ as William Shakespeare put it. In my mind, beauty is something that attracts the different senses of the human mind at different times and on different occasions. Also, the perception of beauty changes with time and age. The emotional experiences we have when encountering people also colour our perception of beauty in various ways.”
On who is the most beautiful woman: ”I believe that one of the most beautiful women in the public eye is Ann-Sophie Mutter. She is not only beautiful in terms of physical appearance, but also in the magnificent way she expresses feelings through playing the violin. This combination of body, soul and mind is vital for my appreciation of beauty.”

Dr. Balshi has integrated the academic aspects of his career into his clinical practice to make cutting-edge treatment modalities available to the general public. He is the founder of Prosthodontics Intermedica (Pi) and the Institute For Facial Esthetics, Inc. in Fort Washington, Pennsylvania. These facilities are among the first dental implant treatment and training centers in the United States, using

NobelBiocare’s Brånemark System of osseointegrated implants. The center focuses on advanced clinical treatment, especially for patients who have little or no bone available for implants. The entire team does extensive research with osseointegrated dental implants. In conjunction with this practice, he continues to develop patient educational material to ensure optimal pre-and-post operative care, including instructional videography and even a cookbook for implant patients.
The Prosthodontics Intermedica (PI) Clinic Prosthodontics is the specialty of implant, esthetic and reconstructive dentistry dedicated to the restoration of oral function while achieving an esthetically pleasing cosmetic result. The field was revolutionised by the discovery of the unique biological properties of titanium and its ability to osseointegrate or combine with bone to form a permanent anchor. This was discovered by Dr. Brånemark, who inserted the first titanium dental implant into a human patient, and later became Dr. Balshi’s mentor. The original and historic implant process entailed an initial visit to undergo extraction, bone preparation and the insertion of numerous individual titanium screws followed by several months of healing, while waiting for the anchor to take hold. This period often proved to be a painful and embarrassing experience for patients as they were often restricted to wearing uncomfortable removable dentures with undesirable cosmetic results. The delayed placement of a tooth often resulted in shrinking of the surrounding gums and bone creating an unnatural-looking result.

THOMAS J BALSHI
When Dr. Balshi launched his Prosthodontics Intermedica (Pi) clinic, he sought to put his experience and research into practice. Dr. Balshi works closely with his partner, Glenn Wolfinger, who is a Harvard University board certified Prosthodontist and director of clinical protocols at Pi. The clinic was developed to offer exceptional, cutting-edge dental solutions guided by a team of dedicated doctors, researchers, and clinical assistants with experience treating patients of all ages, anxiety levels, and esthetic requirements. Dr. Balshi introduced his Teeth In A Day™ protocol in 1994 and has since treated countless patients with exceptional results. The typical initial consultation involves non-invasive scanning of the patient’s oral anatomy utilising the center’s in-house i-CAT cone beam scanner. The results are then used to create an anatomically perfect prosthesis to be implanted at a later date. During the actual surgical procedure, extraction and implantation

occurs in coordination with specific pre-planned, computer directed coordinates to match the insertion point of the prosthesis. Rather than placing traditional individual tooth implants, a temporary prosthesis is attached as a single unit, similar to a fixed bridge to a series of implants,
which actually serves to anchor the teeth in place for greater stability. Once complete healing has occurred, a permanent prosthesis is positioned to create a natural, hassle-free, enduring result that was not achievable in the past. In Dr. Balshi’s own words, there is a “dramatic reduction in treatment time and the number of appointments, immediate full function of the new teeth, a natural cosmetic look and very little discomfort. In fact, some people have reported almost no pain at all!”
Fort Washington Dental Laboratory Inc., Dr. Balshi’s Pi clinic operates alongside Robert Winkelman’s Dental Laboratory in Fort Washington. Bob is currently the national instructor for Nobel Biocare in laboratory procedures. As the first technician in the world to be trained in Sweden on the breakthrough Teeth In An Hour™ protocol. returned in the same day. Working under the auspices of this prestigious organisation, Dr. Balshi’s Pi Dental Center at the Institute for Facial Esthetics has become the model upon which prosthodontists worldwide are designing their treatment centers. The facility offers all forms of restorative dentistry and has a team of prosthodontists, a resident endodontist for root canal treatment, a periodontist specializing in tissues surrounding the root of the tooth, dental hygienists, and a myriad of surgical nurses, clinical assistants, and administrative professionals. The clinic provides all forms of treatment from simple single-tooth restoration to the replacement of complete upper and lower dentures with permanent teeth, as well as gentle root canal therapy. The Institute also includes top-notch craniofacial surgeons who can move the jaws for better facial balance. It is also a center for adult orthodontics using very advanced technology such as tongue side invisible braces and the Invisalign system. Pi clinical services are all provided in-house by the Pi team, with the i-CAT scanner used for the twenty-second upright CT scan, and skilled laboratory technicians at the Institute’s Fort Washington Dental Lab, standing by to fabricate and fine tune each new dentition. As would be expected, all surgical procedures are performed with meticulous attention to sterility and patient comfort. day—with a perfect smile. Pi treats patients who are left without treatment options. Their results are often available exclusively with Pi technology. Teeth In An Hour™ is a new and still-developing protocol for replacing missing teeth with computer guided surgery. It involves the CAD/CAM technology and computer robotics for building the new teeth, most of which takes place outside of the patient’s mouth. Once the implant guide and the teeth have been fabricated, the actual placing of the implants and the attachment of the teeth usually takes less than one hour. Dr. Balshi was among the first in America to work with this technology and has been actively involved in its development for more comprehensive and widespread use. On December 14, 2005, he was viewed by millions of people around the world when he performed a live Teeth In An Hour™ surgery on ABC Television’s Good Morning America. Over the years, the Pi clinic has evolved from a dental care center to a place of education. Doctors from around the globe can now observe procedures from Pi’s state of the art teaching observatory detailing the step-by-step clinical delivery of Pi’s Teeth In A Day™ and the new Teeth In An Hour™ procedures. Dr. Balshi’s educational legacy of research and innovation are guaranteed to continue with the addition of his youngest son, Stephen, to the Pi staff. Stephen Balshi holds a Masters Degree in Biomedical Stephen Balshi
Engineering from Drexel University and plays a key role in the success of Prosthodontics Intermedica. He is director of the research programs, a faculty member for the continuing education clinical training programs and the center’s authority on computer guided implant surgery. He is also the COO of CM Ceramics, Mahwah, N.J., an innovative CAD/CAM dental ceramics laboratory. In addition to the benefits of restorative surgery, all patients of the Pi clinic are made aware that it
is the implementation of good dental hygiene and routine care that are essential in preserving a beautiful smile. The clinic’s ultimate goal as a health care center is to eliminate and prevent dental disease and the loss of teeth. Their team of oral hygienists are experts in preserving both natural and synthetic smiles and educating patients on techniques for dental hygiene home care to ensure long lasting results.
Conclusion Dr. Balshi’s contribution to the science of cosmetic and restorative dentistry is unparalleled. His ability to combine clinical practice with innovative research has allowed the general public access to treatments that were once confined to the experimental laboratory. His quest for knowledge and passion for perfection combined with his consummate bedside manner, clearly classifies him as one of the best cosmetic rehabilitation dentists in the world. As Mario Andretti succinctly stated, “There was no question of whom I would go to. In racing, you can’t exist or be successful unless you surround yourself with the best people and technology. It’s the same when it comes to your own well being or your family. You are cheating yourself if you don’t go for the best”
Dr. Balshi on Beauty “I like to think that my patients have the best smiles in the world! Among the most engaging smiles belong to racing hero, Mario Andretti and jazz singer, Cassandra Wilson. I also find the smiles of Christie Brinkley and Faith Hill noteworthy and the smile of Donovan McNabb particularly strong. There is no beauty in the human face without a smile. The smile is that property which magically connects the sheer goodness of the internal spirit with a perfect complement of designer “pearls” inside the mouth. When the placement of chiseled lines and hues accurately reflects one’s inner charisma, the result is a magnetism known as BEAUTY. It makes the beholder look again, and then again, and yet again…with wonder.” Christie Brinkley

DR. PER HEDÉN IS A MASTER OF THE
MULTIDISCIPLINARY APPROACH TO AESTHETIC SURGERY. IN 1981, HE DISTINGUISHED HIMSELF BY
LEADING THE FIRST MICROSURGICAL OPERATION (SURGERY USING A MICROSCOPE) IN STOCKHOLM.
THE OPERATION RAPIDLY BECAME A CATALYST FOR ESTABLISHING MICROSURGERY IN THE REGION.
DR HEDÉN IS THE HEAD OF AKADEMIKLINIKEN, SCANDINAVIA’S LARGEST PRIVATE PLASTIC SURGERY
HOSPITAL, WHICH HE FOUNDED WITH HIS COLLEAGUE DR. JAN JERNBECK. Beginning in 1990, Dr. Hedén
headed up the microsurgery unit in plastic surgery at Sabbatsberg, earning renown for his microsurgical
repair of facial paralyses. Today, patients throughout the world seek out Dr. Hedén for this type of surgery.
He began aesthetic surgery for private patients in 1984, culminating in the 1991 opening of Akademikliniken.
Per Hedén’s medical studies began at the Karolinska Institute in Stockholm in 1971, where his interest in microsurgery developed while studying under the legendary microsurgeon Marko Godina in Ljubljana, Slovenia. During his illustrious academic career, he studied plastic surgery among other elite surgeons at Harvard University. He also continued his scientific research at the Karolinska Institute, where he became an associate professor at the Institute in 1990.
Speciality At Akademikliniken, Per Hedén offers the best of what the medical world has to offer in anatomical cohesive silicone breast implants to his patients. Implants that have a texture similar to body tissue are moulded into the precise shape of a woman’s breast and come in approximately 130 variations in width and height. The implants are specially customised to fit to the patient’s particular glandular tissue, thoracic shape, and most importantly, the patient’s personal preference. According to Dr. Hedén, these silicone-based implants last twice as long as previously developed implants, which usually need to be replaced within eight to 10 years. In the last decade, Per Hedén has performed over 5,000 procedures using his novel implants. Many of his patients are Americans who travel abroad to find techniques and implants that are not currently available in the U.S. Per Hedén has been invited to perform operations in countries around the world to demonstrate his techniques in breast augmentation. In 2006 alone, his clinic educated more than 100 international plastic surgeons in the latest concept of breast augmentation surgery.
Credentials and Associations Per Hedén is a member of the Swedish Plastic Surgery Association (where he has served on the board). He regularly reviews papers for publication in the Scandinavian Journal of Plastic Surgery, and serves on the board of reviewers for international abstracts of the American Journal of Plastic and Reconstructive Surgery. Over the years he has participated as a lecturer and moderator at numerous national and international plastic surgery conferences, and has also visited and studied at various internationally renown clinics. He is a member of and participates regularly in the meetings of the American Society of Plastic Surgeons (ASPS) and the International Confederation of Plastic, Reconstructive and Aesthetic

co-authored a book on bloodflow measurement using
the laser-doppler method. He authored the plastic
sugery book, Plastikkirurgiboken, published in Swedish in 1996. It has been translated and published in Russia and is being used as the basis for a second book. Plastikkirurgiboken has sold 5,000 copies in Sweden, and features photographs of a selection of patients Per Hedén has made more beautiful over the years.
Per Hedén successfully gave a series of instructional plastic surgery courses on body contouring at the American Society for Aesthetic Plastic Surgery meetings. He has guest lectured at several international meetings, and has organised and hosted the annual Alpine Plastic Surgery Courses for general practitioners.
About the Clinic As noted above, Per Hedén is a founder and head of Akademikliniken. The clinic boasts eight operating rooms, 37 overnight beds, and includes the Skin Science Center, as well as a recently incorporated department for aesthetic dentistry. More than 100 staff members and specialists in plastic surgery sub-specialities work in the clinic. Akademikliniken draws patients from the world over and has out-patient facilities in London, Copenhagen and Oslo. More than 2,000 patients have operations at Akademikliniken in Stockholm every year. The clinic recently formed an international organisation with a clinic in London, gaining greater international prestige. An important element of Per Hedén’s practice is the newly opened Skin Science Center, with skin therapists and dermatologists who incorporate skin care alongside aesthetic surgery. The Skin Science Center uses the latest laser technology for treatments ranging from permanent hair removal to the removal of vascular malformations and tattoos. The center is well known for its expertise in the use of Botox® and fillers such as Restylane®. Several non-surgical body contouring treatments are also performed with the use of different per-cutaneous ultrasound techniques. With the Alpha Tech method, developed in Israel, fat cells are vibrated loose so that they can be easily broken down and processed by the body naturally. The LPG Endermologie is another technique used to treat cellulite, excess fat, circulation and weak skin through deep massage that changes the disposition of fatty tissue.
Conclusion Per Hedén’s unique place in the world of plastic surgery is about more than creating surface beauty. His professional stature, years of experience, specialties and dedication to excellence make him one of the most sought-after surgeons in the world. His name has become synonymous with beauty—his services are the Jaguar of plastic surgery.
Per Hedén on Beauty It is impossible to establish a universal definition of human beauty since this varies from culture to culture, and from time period to time period. However, there are several globally universal well established measures and proportions for the human body and face that are regarded as signs of beauty and attractiveness. These signs run like a red thread throughout history and different cultures and are related to biological quality and fertility. For the human body this is exemplified with the so-called WHR (waist-hip ratio), which in all cultures is a sign of fertility and is thus regarded as attractive. Regarding facial features considered to be beautiful, the features are often greatly symmetrical with average distribution, but also encompass certain characteristic deviations from normal distribution. These deviations are related to good biological qualities documented scientifically for the female face and include a delicate lower jaw, larger eyes, fuller lips, and high cheek bones. Examples of beauty icons in the middle of the last century as well as the present moment

London. He completed the Royal College of Surgeons accreditation process in plastic surgery in 1984 and was appointed Consultant Plastic Surgeon to The Great Ormond Street Hospital for Children NHS Trust, and University College Hospital London in March 1985. He directed the supraregional Craniofacial Unit at The Great Ormond Street Hospital for Children NHS Trust from 1987 to 2006 and continues as Emeritus Consultant Plastic & Craniofacial Surgeon. Barry Jones is a member of numerous prestigious organisations in his field, including the British Association of Plastic Surgeons, the British Association of Aesthetic Plastic Surgeons, the European Association of Plastic Surgeons, the American Society for Plastic Surgery, The International and European Societies for Craniofacial Surgery and many others.
Publications and Awards Barry Jones has authored over 150 journal articles in surgical and plastic surgery fields over the past three decades. He was honoured by The Royal College of Surgeons of England with a Hunterian Professorship for his work in facial aesthetic surgery in 2005. Mr. Jones’s Hunterian oration, “Facial Aesthetic Surgery: The use of scientific principles to optimise outcome and limit risk”, was delivered at the College in November, 2005 to an international audience. It marked the first time that a Hunterian Professorship was awarded for an aesthetic surgical subject. Barry Jones has also been the recipient of many sought-after honorary and visiting professorships at leading universities around the world.
About the Clinic Barry Jones’s consulting rooms are rather discreetly based at the corner of Devonshire Street and Upper Wimpole Street where he receives patients in an elegant upper floor room with a sitting area and a sophisticated system of medical photography. Suture removal and injection treatments, such as
Botox and dermal fillers, are all done on site by a resident senior nurse for the comfort and convenience of the patient. Major surgery and any surgery requiring general anaesthesia or intravenous sedation is done at the King Edward VII’s, Sister Agnes Hospital close by Barry Jones’s practice. King Edward VII’s is a wellestablished facility with an unparalleled reputation for patient care. Among private hospitals in the U.K., it leads the way with the highest ratio of nursing staff to patients. There is also a ward dedicated to plastic surgery with operating and ward nursing staff trained in plastic surgical techniques.
Testimonials Barry Jones is unique among plastic surgeons in that he does not believe in or harvest testimonials. For all that, his name will consistently come on top of the recommendations list when searching for the best in London. He is also many GPs recommended plastic surgeon, which says a great deal about his professional reputation.
Conclusion Barry Jones has long reached a point in his career where his reputation precedes him. He is a quintessential English surgeon, formal and well-spoken, the customary bow tie adding a touch of sartorial eccentricity to his persona. He will discuss the possibilities that surgery can offer to each individual patient honestly pointing out the limitations and relevant potential risks and complications as well as benefits. Barry Jones does not offer promises he can’t keep and his patients value the no-nonsense approach and candid advice. Patients’ decisions often evolve in the course of the initial consultation and are meticulously and professionally adhered to.
Barry M. Jones on the most beautiful woman: The most beautiful woman in the public eye, living or not, after much thought and on balance, has to be Audrey Hepburn as having an innate, un-enhanced natural beauty which would be appreciated by most.
On beauty: To define beauty is effectively impossible. This is such a broad concept which may be applied either to something animate or to something inanimate but there is no short, snappy “sound-bite” definition. As far as humans are concerned, beauty has as much to do with personality and “the soul” of the individual as it does with their physical attractiveness. The latter probably has more to do with an evolutionary drive to pick a partner who is likely to be genetically sound and fertile than it does to any sophisticated analysis. Most people would see a “beautiful face” or human form as conforming to broad normatives of symmetry and proportion. The idea of “golden proportion” is real and even individuals who have little appreciation of aesthetics in general will have an innate appreciation of any individual or structure which looks right. This is often conforming to the golden proportion. In summary, we may all interpret beauty differently (“beauty is in the eye of the beholder”) but attributes of symmetry, proportion and skin clarity will be interpreted by most as “beautiful”. This relates only to an image. True appreciation of beauty requires insight into the personality and behaviour of an individual.

the case for plastic surgery
I’VE NEVER UNDERSTOOD WHY SOME PEOPLE GET SO PO-FACED WHEN THE MATTER OF PLASTIC SURGERY – OR COME TO THAT ANY OF THE NEW PROPS THAT ARE THERE TO MAKE THOSE OF US WHO ARE NOT SO YOUNG FEEL BETTER ABOUT OURSELVES – ARE UP FOR DISCUSSION. IT’S ALMOST AS IF IT WERE A MATTER OF DEEP MORAL IMPORT AND THAT THERE IS SOMETHING INHERENTLY SUPERIOR ABOUT ALLOWING THE WRINKLES TO WRINKLE AND THE FROWNS TO FURROW AND THE DROOPY BITS TO GET DROOPIER. Plastic surgery seems to me to be fundamentally a physical issue and providing you’re not proposing to pay for it by swiping the bread from your children’s mouths I cannot see that it is a moral issue at all. What good plastic surgery does is to cut away the droopy bits of skin. It removes bags from under the eyes, the droopiness on the top eye-lids (not a good look bloodhound eyelids), sagginess from the chin, and generally tightens up the skin. That’s it. Now where’s the problem with all of that? Of course if it’s badly done it’s a hideously expensive mistake but these days it seems to me surgeons are getting better and better at it – heaven knows they’re getting more practice – but the general notion that it would be better to put up with the brown spots, red veins, droops and sags when there is something that can be done about them seems to me just plain daft. Nobody who looks at the before and after pictures of such cosmetic success stories as Mary Archer, Julie Christie, Sharon Osbourne could surely still take the view that in their shoes they wouldn’t have done the same? All three – and there are thousands of others like them – look a million times better and happier now that the damage the years can do has been held back a little longer. Sharon Osbourne has spoken out loudly and frequently about how happy she is with the work that has been done and quite right, too – whatever it is (and I’m not privy to the details though I know it includes a face-lift and some body treatment) it was worth it every time. It speaks of vanity is the usual charge by those who get all sniffy at the very idea. Well, firstly I see nothing wrong with vanity – vanity makes us try harder, vanity makes us strive in many areas of our lives, vanity makes us look better which adds considerably to the gaiety of the nation. But there is more than vanity at stake. Quite often these days it is often a matter of job survival. As we all live longer and longer (hurrah!) we want to go on living full and busy lives and if we want to do that it is no use denying that looks count. Whether you’re behind the counter in a shop, working in a lawyer’s office, trying to hold your own in the world of fashion or running a small business of your own, how you are seen by the outside world will affect your prospects. Middle-aged women – think of Lady Thatcher in her hey-day, or Madeleine Albright to name but two extremely high-flyers – are these days doing extraordinary things.
Whereas their mothers and grandmothers were winding down (if they weren’t dead) many are just coming to the peak of their careers. They may have ten, twenty, thirty more productive years ahead. Of course they want to look their best. Until fairly recent times they only had the beauty companies, the fashion designers, the therapists and nutritionists to help them on their way. These days there is a bewildering array of often very technical options for them to choose from, ranging from the surgeons’ knife to lasers, light pulses, fillers, acid peels, and lots, lots more. Anything from sun-damaged skin to brown spots, red veins, dubious moles, too much hair and many other afflictions can now be treated. And if the treatments are there that will deal with the things that indubitably make women feel less confident about themselves I cannot for the life of me believe it is anything other than a good idea to have them attended to. “But it’s dishonest” is the cry one sometimes hears. It is only dishonest, it seems to me, if those who try to look their best try to pass themselves off as something that they are not. A foolish ploy it seems to me and not one that most of the women I know have in mind. Most simply want to look as good as they can and want the unattractive signs of ageing (the droops, the brown spots, the whole sorry lot) removed. Apart from anything else, as Joan Bakewell said so eloquently on the radio the other day, most people feel most comfortable with people who have largely lived through the same span of years - the music, the books, the theatres, the history that have been shared are a bond that is too precious to ignore. There are, of course, those who are frightened of the surgeons’ knife – and not without reason. Mistakes are sometimes made (it must be almost impossible to estimate the damage that Mrs. Wildenstein alone must have done to the world of cosmetic surgery) and the problem is that once made, they are hard to put right. And common observation does reveal that we do only have one face apiece (and only one body come to that). But the answer to that isn’t no cosmetic surgery – it’s better cosmetic surgery, more finely tuned lasers, more skilled technicians. The key has to be to take time and trouble to find surgeons with a good track record. As more and more women who have had “work” done begin to speak out and tell their friends the good news, it will become easier and easier for those who are nervous to hear at first-hand about who is good and who is not. As to those who claim that it is undignified and that they themselves would never do anything quite so drastic. Take a good look at them and they nearly always turn out to be twenty-something beauties who haven’t the faintest idea of what it feels like to look in the mirror day after day and see things gradually going to pot. Just wait, I long to say to them, and see if you’re quite so adamant when your own looks begin to fade. Read Nora Ephron’s rail against the trials and tribulations of growing older “I feel bad about my neck” and those
Gore Vidal once put it, “isn’t for cissies.” But praise be these days there are things - highly technical, very skilful, very successful - things, that can be done and for that I think we should all be duly grateful.
