Welcome from Worcestershire County Council Welcome to the 2015/2016 Worcestershire Care Services Directory, which provides details about support for adults available in the county including social care. Some of these services are provided directly by the Council. Many others are provided by voluntary, charitable and private organisations. They can be used by people paying for their own care as well as people funded by the Council.

This is the final directory that Worcestershire County Council will be producing. It will be replaced by the new Your Life, Your Choice website. Your Life Your Choice will be launched in autumn 2015 and includes lots of information and advice about how people can stay well and independent and plan for their long-term future.

How might Adult Social Care help you?

Everyone in Worcestershire is entitled to receive information, advice and support about the range of help available to them. This includes the opportunity to have your needs assessed to help you plan and make decisions about your current and future care. You may also be entitled to receive some funding support from Worcestershire County Council to help you.

Adult Social Care is a term that describes the everyday support that you might need due to illness, disability or frailty. It also includes supporting people who care for others. It does not mean the type of healthcare that you would expect to receive from NHS hospitals or your GP. Social care support is focused on keeping you and those who support you as independent as possible, and to enable you to have choice over how you live your life.

If you are eligible for ongoing support, Adult Social Care funding can be received in a range of ways. You can choose to have the Council manage it on your behalf, or, alternatively, payments can be made to you to enable you to be more independent and arrange the care yourself or with the support from your family (this is called a Direct Payment). You can also choose to have a combination of both of these arrangements. For more information on Personal Budgets and Direct Payments, see page 8.

You will be eligible for funded Adult Social Care support if you meet the national eligibility criteria. For more information about eligibility and assessments, see below.

Even if you think you might not be eligible for help from Adult Social Care, you and the person who supports you are entitled to have an assessment. The purpose of the assessment is to work with you to identify what needs you may have and what you or the person who helps you wants to achieve or maintain to improve your life experience and wellbeing. Information, advice and support can be provided to you on a range of different topics and related social care issues.

If you do not feel confident or able to participate fully in any aspect of the Adult Social Care assessment process you can have someone to help you. In cases where there is nobody who can support you, an advocate can be provided to speak on your behalf.

The Council is also able to provide you with the assessment forms so that you can complete these on your own or with someone else you trust to support you. The aim is to make sure that you are as comfortable as possible with how you wish to be supported during and after the assessment.

It is also important to know that Adult Social Care is not provided free of charge. Following your assessment to fully identify your need for care and support, you will also have an assessment of your finances. This will provide you with the information about any contribution that you may need to make towards the costs of your care and support.

It can be found at ylyc.worcestershire. gov.uk/ In addition to providing useful information and advice on a wide range of topics and local services, the site also enables users to register and submit a self-referral for

consideration by a Social Work Team. Who is eligible for council support to meet their care and support needs?

All councils in England must now work to the national minimum threshold for adult care and support needs which was introduced on 01 April 2015. The eligibility threshold is set out in the Care and Support (Eligibility Criteria) Regulations 2014.

In deciding if an adult with care and support needs has eligible needs, the Council must have first completed a needs assessment.

The Council will then use the information gathered in the needs assessment to decide:

• Do the adult’s needs arise from, or are they related to, a physical or mental impairment or illness they have?

• As a result of the adult’s needs, is the adult unable to achieve two or more of the outcomes specified in the eligibility criteria regulations?

• A s a consequence, is there, or is there likely to be, a significant impact on the adult’s wellbeing?

An adult’s needs are only eligible for council support if the answer to all three of these questions is ‘yes’. >>

The Council will put in writing all decisions it makes about meeting eligible needs; this will include information about how to complain if the person or their representative believes the decision is wrong.

What is a needs assessment?

A needs assessment is a discussion between you and a trained person from the Council about your care and support needs and the goals you want to achieve so that you can maintain or improve your wellbeing. The assessment can be done face-to-face, over the telephone or online.

The assessment is the way the Council can understand what care and support needs you have and how these affect your wellbeing. The assessment will include talking to you about any physical needs you have (such as washing and dressing) as well as your mental and emotional wellbeing and what is important to you.

The Council will make sure that you are able to be involved in your assessment. If you wish, your carer, a friend or family member can help and represent you. If you have a lot of difficulty in being involved in the assessment and don’t have anyone you can ask to help, the Council will find someone to be your independent advocate to help you.

If you have complex care and support needs the Council will work closely with other organisations such as the NHS so that you do not have to provide information repeatedly to lots of different people.

You will be given a written copy of your needs assessment and a copy will be given to anyone that you want to have a copy.

When your assessment is complete the Council will decide if your care and support needs are at a level which means that you are eligible for help from the Council. You could be eligible for help from the Council if your illness or disability means you are not able to do, or have major difficulty in doing a combination of normal day-to-day activities and your wellbeing is seriously affected by this.

The Council will write to you to explain its decision and

give you the reasons for its decision.

If you are eligible for help the Council will explain what help could be available through your Personal Budget (see page 8) for care and support. Your Personal Budget will be calculated based on the information provided to the Council during your assessment and is used to fund care and support arrangements to meet your needs. These arrangements will be agreed with you and described in a Care and Support Plan.

The needs assessment will not ask you about your finances but, if your needs are assessed as eligible for care and support from the Council, you may have to pay towards the cost. So that the Council can work out and tell you how much you would need to pay you would need to disclose details about your savings, assets and income.

Care and Support Plan What is a Care and Support Plan?

The Care and Support Plan is there to help you think about the support you require in the different areas of your life.

It will help you think about the levels of support you will need to be as independent as possible and meet any of the outcomes that were identified from your needs assessment either in the assessment you did yourself or one that you completed with a health or social care professional.

You can use the Care and Support Plan if you are the person who needs help or if you are the carer of someone

who needs help. You can also choose to use the Care and Support Plan jointly as a carer and the ‘cared for’ person if you wish. You can use it even if you are not receiving funding from the Council, as you may find it helpful to think about the support you need now and in the future.

If you require support to fill out your Care and Support Plan, you may ask someone to help you or, alternatively, the Council can help you arrange for an advocate who can help you and speak on your behalf.

A Personal Budget A Personal Budget is the total amount of money you will need to meet the costs of your assessed care, including the amount the Council will pay and the amount you may need to pay towards the cost of your care. In general, the Council charges for social care services. Some people

have to pay the full amount towards their services; some make a contribution and some do not pay anything. It will depend on your individual financial circumstances and will be calculated following the completion of a financial assessment.

What is important to you?

If someone reads your Care and Support Plan they should be able to get a sense of your lifestyle, an understanding of who you are and your interests and aspirations for the future. The Care and Support Plan must show that you have been at the centre of the process.

Your Care and Support Plan needs to contain:

about the support you need. The person who carries out your assessment will discuss these with you and arrange to get the support you require. This person could be:

• What is important to you.

• A health or social care professional can help you.

• Your likes and dislikes.

• An independent person, who may charge you for their services.

• Your hobbies and interests.

• The important people in your life.

• Your hopes and fears for the future.

• What you hope to achieve and by when.

What do you want to achieve or change in your life?

A good way to start to think about your life is to think about how it is at the moment, what is working well and the things that are not working so well. What is your perfect day and what would be your worst day? Sometimes it can be difficult to imagine how life could be better, so this could be a good way to start to think about this.

Your Care and Support Plan must say what you want to change about your lifestyle. This may include changing how you spend your time, changing where you want to live or changing the services you currently receive. It could even include any long-term changes you may want to make.

There are various options available to help you think

• Y ourself – you may want to complete the Care and Support Plan yourself, or with your family or other people in your circle of support.

• An advocate who can complete the Care and Support plan on your behalf.

Your Care and Support Plan should include the type of help and support that you are going to need to ensure you stay safe and well. You may want to think about the help you need at home; this may be cooking, shopping, household chores or personal care. You may need someone to help you get out and about to see friends and family or keep appointments, or you may need help to do more in your local community.

Include things that you have a real chance to achieve and that will make your life better. People who help you to decide will also need to agree with your choices. Don’t include anything that someone else thinks you should do if you don’t agree with it.

If a health or social care professional has carried out your assessment they will have identified any care and support needs the Council will meet, as well as any other care and

What is important to you? continued support needs you may have. It is important to remember that any arrangements you make that are funded by the

Council need to be directly linked to your assessed eligible needs.

How will you arrange your support and pay for it?

As well as using your Personal Budget to meet some of your outcomes, you may want to start thinking about what your natural networks are and whether there are other services that are free and available to you in your own community.

You may also want to use local people or resources, as these can often be easier to arrange than buying from a care agency and may be people that you know and feel more comfortable to have helping you.

Your Care and Support Plan must then show what you intend to spend your Personal Budget on. This must include any money that you have to pay towards your care costs.

You may want to take your Personal Budget as a Direct Payment that you either manage yourself or you ask a family member or someone else to manage on your behalf. (If you have someone else acting on your behalf

the Council will need to agree this.) Direct Payments can offer you a lot of flexibility in how you can use your Personal Budget. If you are thinking about taking a Direct Payment, the Council can help you to understand how it operates and some of the things that you might need to think about, such as what you might need to do if you want to employ people to help you.

If you do not want to have a Direct Payment you can ask someone from the Council, for example a Social care worker, to arrange the services on your behalf. Or you can arrange some help yourself and ask the Council to help you arrange some other services for you.

It is very important that you consider a contingency plan for what will happen if things go wrong or don’t turn out the way you wanted. It is wise to keep an amount of your budget to one side to cover the contingencies such as arranging care at a short notice.

How will you ensure you stay in control of your life?

Your Care and Support Plan must say what decisions you will make and what decisions other people will make if you lack capacity.

If your Care and Support Plan relies on someone else making decisions on your behalf it must say how they will help you and how they will know that you are in agreement.

Support Plan agreement Once your Care and Support Plan is completed your health or social care professional must see the plan and agree it with you before you can receive your budget, if you are eligible for one. The Council has a duty to ensure your support arrangements will meet your assessed support needs effectively and that any risks in the way you have chosen to meet them are identified and contingency plans made. The Council will also tell you in writing if it is unable to provide services for some of your needs and why.

The health or social care professional will be looking for a number of details in the plan, making sure that: • The plan will keep you healthy, safe and well.

• The plan makes the most of things that you can use and do in your community and takes account of your natural networks of support.

• The plan meets the needs and outcomes that have been identified in your assessment.

• The plan is within the budget amount that you were given to meet your needs.

• The plan includes information about how you will manage any risks, and what support you need to stay safe and well.

• If someone else has helped you to complete your plan, the decisions that have been made are your own or in your best interests.

• If someone is in control of your budget, that they are the right person.

• Appropriate plans are in place if your planned care and support doesn’t happen/turn up.

• Your care and support is best value for both yourself and the Council.

• The costs of the services in your Care and Support Plan are realistic and affordable and meet your eligible needs appropriately.

• The support you want to meet your needs is lawful.

• The length of time that you think you will need your different types of support for is included.

The Council will want to know that your Care and Support Plan is working well for you. A health or social care professional will agree a review date of your plan with you to ensure that: • Your care and support services are satisfactory.

• You are still eligible for support from the Council.

• Your eligible needs are still being met.

The Council may discuss this over the telephone, faceto-face or ask you to review your Care and Support Plan yourself. This will depend on your particular circumstance.

Your Personal Budget may go up or down depending on your eligible needs and the costs of services.

If the Council does not agree with your Care and Support Plan, it will give you the reasons in writing. If you are not happy with the Council’s response you may wish to use the Council’s complaints process to have your case heard by an independent person.

Adult safeguarding What is ‘adult safeguarding’?

Safeguarding is aimed at adults who, as a result of their care and support needs, are unable to protect themselves from either the risk, or the experience, of abuse or neglect.

In these cases, local services must work together to safeguard adults in a way that supports them in making choices and having control over how they want to live.

What is abuse and neglect?

Abuse or neglect may be deliberate, or the result of negligence or ignorance. Unintentional abuse or neglect arises, for example, because pressures have built up and/ or because the behaviour someone with care and support needs displays is not being properly addressed.

Abuse and neglect can take many forms. The Care Act 2014, which sets out the safeguarding responsibilities of local services, identifies the following areas but advises that this is not an exhaustive list as abuse or neglect can take many forms and the circumstances of each case must always be considered: • P hysical abuse – including assault, hitting, slapping, pushing, misuse of medication, restraint or inappropriate physical sanctions.

• D omestic violence – including psychological, physical, sexual, financial, emotional abuse and so called ‘honour’ based violence.

• S exual abuse – including rape, indecent exposure, sexual harassment, inappropriate looking or touching, sexual teasing or innuendo, sexual photography, subjection to pornography or witnessing sexual acts, indecent exposure and sexual assault or sexual acts to which the adult has not consented or was pressured into consenting.

• P sychological abuse – including emotional abuse, threats of harm or abandonment, deprivation of contact, humiliation, blaming, controlling, intimidation, coercion, harassment, verbal abuse, cyberbullying, isolation or unreasonable and unjustified withdrawal of services or supportive networks.

• F inancial or material abuse – including theft, fraud, internet scamming, coercion in relation to an adult’s financial affairs or arrangements, including in connection with wills, property, inheritance or financial transactions, or the misuse or misappropriation of

property, possessions or benefits.

• Modern slavery – encompasses slavery, human trafficking, forced labour and domestic servitude.

Traffickers and slave masters use whatever means they have at their disposal to coerce, deceive and force individuals into a life of abuse, servitude and inhumane treatment.

• Discriminatory abuse – including forms of harassment, slurs or similar treatment; because of race, gender and gender identity, age, disability, sexual orientation or religion.

• Organisational abuse – including neglect and poor care practice within an institution or specific care setting, such as a hospital or care home, or in relation to care provided in one’s own home. This may range from oneoff incidents to ongoing ill-treatment. It can be through neglect or poor professional practice as a result of the structure, policies, processes and practices within an organisation.

• Neglect and acts of omission – including ignoring medical, emotional or physical care needs, failure to provide access to appropriate health, care and support or educational services, the withholding of the necessities of life, such as medication, adequate nutrition and heating.

• Self-neglect – this covers a wide range of behaviour; neglecting to care for one’s personal hygiene, health or surroundings and includes behaviour such as hoarding.

Many people are able to stay in their own homes for most or all of their lives by getting the correct help and support at home.

Food and drink Eating and drinking Many people prefer to be as independent as possible when it comes to eating and drinking.

If you are having difficulty swallowing food or drink, you should speak to a Speech and Language Therapist who will advise you on the correct techniques and equipment.

It could be that you are unable to drive and can’t carry your shopping bags, or it might be that you get tired walking around the shop or can’t see the food labels due to a visual impairment. The Ask Sara website can help you find out if there is any equipment that might help you do your food shopping, but there are some other things you can do as well: • Consider using online shopping with a supermarket.

• Ask the customer service desk at the supermarket for assistance.

• Some supermarkets offer a delivery service for people who shop in store. Check with your local supermarket if they offer this as there may be minimum spends or delivery charges.

• If travelling to the supermarket is difficult, see if a family member or friend is able to take you with them when they go.

• Use your Attendance Allowance to pay for a taxi.

• Check whether there is a food van in your area.

• Use Shopmobility schemes to help you get around your local shops.

Preparing Food There is lots of equipment available to help you prepare food. To find out what might be suitable for you, please visit the Ask Sara website for more advice, or, see the healthy eating page on Your Life Your Choice at ylyc.worcestershire.gov.uk Food banks Food banks are non-profit organisations that provide food parcels to those in crisis who have insufficient money to buy food. Trussell Trust food banks provide emergency food, and support to address a client’s underlying problem.

Each food parcel contains a minimum of three days’ nutritionally balanced, non-perishable food. Schools, churches, businesses and individuals donate nonperishable, in-date food to a foodbank. Food is also collected at supermarkets where shoppers can donate items from their shopping basket.

Every referral to a Trussell Trust food bank is made by frontline care professionals (such as doctors, health visitors, social workers, welfare officers, the police and probation officers) who will identify that a person is in crisis and will issue a food voucher. Residents need to have a referral from one of these agencies to be able to access the food bank.

In Worcestershire, there are a number of Trussell Trust food banks and other independent services that support residents in need of food.

Home maintenance Keeping your home and garden well-maintained can sometimes be difficult, especially if you have a health or physical condition. A well-maintained home can help you to stay in better health and avoid accidents, whilst the garden is often a sanctuary for many people and can help with mental wellbeing.

If you are struggling to keep up with smaller jobs that need doing around the home and garden, you may be able to get help from voluntary organisations. There may be a small charge or you might be asked for a donation.

For larger jobs you may need to hire a tradesman.

Always check previous work from testimonials, ratings on the internet or personal recommendations from family and friends. If you are worried about hiring someone you don’t know, you can get further advice at the Worcestershire Regulatory Services website at www.worcsregservices.gov.uk If you live in a Housing Association home or rent from a private landlord, then they are responsible for keeping the home maintained and you should contact them directly for anything that needs doing.

Housework Being able to clean your home and launder clothes can become difficult if you are living with a physical or health condition. Living in an unclean environment can not only have an impact on health, but can also affect your mental wellbeing.

This can be for many reasons such as ill-health, a disability or bereavement.

Tips to help you: • Get rid of junk mail.

• Decide which letters or bills are most important and need to be dealt with first. These may include rent, mortgage or health appointments.

• Use a calendar to make a list of when letters or bills need to dealt with.

• Arrange to pay your bills by direct debit if you can.

Personal care Dressing and undressing If you find dressing difficult, there are a number of things you could try to remain independent. Choosing appropriate styles and fabrics, adapted clothing as well as using special techniques or equipment can all help to make dressing easier. Visit ylyc.worcestershire.gov.uk for ideas.

Equipment is also available to assist with a variety of dressing tasks. The Council recommends that you seek advice from an occupational therapist before buying or using any of this equipment.

Assistive Technology Assistive Technology is the name given to devices and equipment that can be used to support someone with care needs or their carer, either as a standalone solution to their needs or when used as part of a broader package of care. This equipment can be as simple as a device to remind you to take your medication or can be more sophisticated by linking with other equipment, connecting you to a service that can monitor your safety and wellbeing. There are often options available to you that will allow you to make changes, to your home or to how you do things, that will make your home more suitable for your changing needs.

However, sometimes people find that making changes to their home is not enough and it is necessary to look for somewhere else to live.

You can speak to an occupational therapist who may be able to help you change your home.

Locate your nearest Occupational Therapy team by accessing the Worcestershire Health and Care Trust website on www.hacw.nhs.uk They may suggest adaptations such as: • A stair lift.

• Access ramps.

• Grab rails around the home.

You may want to consider undertaking significant

building works to change the layout of your property. This could include: • Widening doorways so that you can use a wheelchair.

• Replacing your bath or shower unit with a level-access shower.

• Adding a downstairs toilet, bathroom or bedroom so that you don’t have to keep using the stairs.

• Installing low level kitchen units.

To make these changes you will need advice from qualified builders on what to do.

These questions may be useful when considering using the services of a home care (domiciliary) provider to help you build up a picture of how your care needs will be met in your own home.

If you are arranging and funding your own care either privately or through Direct Payments or a Personal Budget, you are able to choose your own worker or care agency.

For care packages which have been arranged by us, contracts and care plans will have been completed by your care manager and the agencies used will have been accredited to work for us.

Agencies • What experience does the agency have in your particular field of need? Can they supply references for you to check?

• Home care agencies providing personal care, must be registered and inspected by the Care Quality Commission. Ask to see a copy of their registration certificate.

• How long has the agency been operating?

• How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick, will you be notified in advance that a different carer will be attending?

• How can you contact the agency in an emergency or outside office hours?

• How hard or easy would it be to make a complaint and how are things then put right?

• If this is a private contract ask for a copy of the agency’s contract terms and read these carefully with someone you trust. Get as much help as you can going over it and ask any questions you may have before signing anything.

Carers • You should expect your personal preferences, dignity and privacy to be respected. Discuss this with the agency and ask how the most suitable carers for your particular care are chosen. Can you talk to them before deciding?

• Carers should be fully trained or be in ongoing training.

Ask the agency about their policies on this.

• You will have a care plan drawn up by the agency which the carers will work to. Ask how often this plan will be reviewed by the agency.

• Carers must be checked with the Disclosure and Barring Service and have a criminal records disclosure – make sure this is the case.

Other forms of support at home Intermediate care Intermediate care is provided by a range of services to promote faster recovery from illness, prevent unnecessary acute hospital admissions and support timely discharge from hospital. It is a programme of therapy, care and treatment, usually lasting no more than two weeks, designed to help you to become as independent as possible.

The structured programme of support may mean that you do not have to go into hospital at all, or may enable you to get home faster following an admission to hospital.

Intermediate care is designed to rehabilitate you and to help

you manage by yourself.

Community intermediate care provided by the NHS, is free and is provided in your own home.

The Directorate of Adult Services and Health and Worcestershire NHS also work together to support you to recover from illness at home rather than being admitted into hospital. Services can be provided in nursing/care homes when 24 hour care is necessary to enable you to be monitored effectively and usually lasts for no more than two weeks to allow for recovery and return home.

Promoting Independence (PI) The PI service provides short-term assessment and reablement to enable you to live as safely and independently as possible within your own home and to reduce the need for ongoing care. The service is available between 7am and 10pm, seven days a week, covers all geographical areas within Worcestershire and is free of charge to service users. The service supports independence with activities such as washing, dressing, managing medication, preparing drinks and meals and getting around your home safely.

The PI service may support you to return home from hospital or from a temporary residential/nursing home placement or after a key event such as a fall. The team consists of team leaders, occupational therapists, physiotherapists, co-ordinators and PI assistants who work closely together to help service users achieve their desired outcomes. If it is identified during your time with the PI service that you need some continuing support, the team will, as appropriate, liaise with health and social care services on your behalf.

Urgent Response The Urgent Response service provides short-term homecare that is free of charge for up to 72 hours. The service supports people who are in crisis and need support with daily activities such as washing, dressing, preparing drinks and meals and getting around their

home safely. The service also provides a ‘Carer’s Back Me Up Service’ which supports people whose main carers have been unable to support them because they have been admitted to hospital, for example. A health/social care worker will help identify your longer term needs.

Support for carers Who is a carer?

Thousands of people provide unpaid care or support to an adult family member or friend, either in their own home or somewhere else. Caring for someone covers lots of different things, like helping with their washing, dressing, or eating, or taking them to regular appointments.

Are You Carer Aware?

The online Carer Aware Tool explains who a carer is, their legal rights and what services are available in Worcestershire to support them. You can find this by

typing ‘carer aware’ into the search feature on the Your Life, Your Choice website.

They focus on providing information, useful hints and tips, practical knowledge and skills, and confidence to help support your caring role. What is a carer’s assessment?

A carer’s assessment is your opportunity to talk with a social or health care worker about your needs and feelings as a carer. The purpose of the carer’s assessment is to consider: • H ow caring impacts on your life and to give you a chance to talk about your needs.

• T he support needed to help you to continue to provide care or to decide who should provide this if you are unable to continue.

• Y our own health and family commitments as well as your work, education and training needs, and the impact that your caring role might have on these.

The carer’s assessment is not a test and there are no wrong answers.

Download the Department of Health Support for Carers leaflet from ylyc.worcestershire.gov.uk You can choose to have your needs as a carer assessed separately or jointly alongside the person that you provide support for. If the person you support does not get any help from the Council you are still able to access a carer’s assessment to look at your own needs.

your own health and wellbeing and to refer yourself for a full carer’s assessment.

Eligibility as a carer In April 2015 the Care Act introduced new national rules for deciding who is eligible for care and support.

What is a Carer’s Support Plan?

The Carer’s Support Plan is a plan of action for the carer, outlining what the carer can expect to happen following on from their carer’s assessment.

Through the assessment process, the carer’s needs

and outcomes they want to achieve should have been identified. The Carer’s Support Plan will look at how these outcomes can be achieved to help the carer get the most out of their daily life.

What support is available for carers?

Carers’ Personal Budgets The Care Act 2014 enables carers to have their eligible needs met by the local authority in the form of a Carer’s Personal Budget taken as a one off Direct Payment. This

enables the carer to meet their identified needs and to look after their own wellbeing as discussed at the assessment and support planning stages. This may be, for example, relaxation courses, gym membership, studying

What support is available for carers? continued fees or equipment, such as a laptop to stay in touch with friends and family or a camera to pursue a hobby.

The aim of the Carer’s Personal Budget is to promote a carer’s sense of wellbeing and to support the carer to continue in their caring role whilst achieving the outcomes that are important to them.

Short breaks for carers To help to fulfil caring commitments without carers being put under unnecessary pressure or stress the Council supports carers to take short breaks (sometimes referred to as ‘respite care’). Short breaks can be provided in a number of different ways: • A few hours’ break on a regular basis through the Council’s Flexible Breaks Service.

• T he person being cared for staying for a few days in a care home with the advantage of staff available day and night.

• S upport in the home, where helpers come to the home of the person being cared for. This will depend on how long the carer will be away and the needs of the person they care for.

• With local families in their home offered by the Council’s

Shared Lives Service.

• A Direct Payment so that the carer can organise and purchase their own short break services, tailored to the needs of the person they care for.

How is a short break or respite care arranged?

Carers can contact the Health and Social Care Access Centre and request a carer’s assessment for themselves and a personal assessment for the person they care for.

These assessments will look at whether the carer and the person they care for are eligible for services and take into account the needs of both people. It will look at what support is needed. Looking at the need for short breaks in order to support a carer to continue to care will be one element of these assessments.

Is there a charge for short breaks services?

There is no charge for the flexible breaks service but some short break services may be subject to a charge.

A financial assessor will visit to discuss how much the carer or person they care for will need to contribute towards the cost of their services, and to ensure that all the benefits that both people are entitled to are being claimed.

What is a Carer’s Review?

A Carer’s Review offers an opportunity for the carer to consider whether the outcomes and support identified during their carer’s assessment are still meeting their needs and to highlight any changes in circumstances.

Where it is considered necessary the review should be carried out at least annually; it is necessary to ensure that the caring role is still sustainable.

Young Carers Are you Carer Aware?

Many young people do not recognise themselves as a carer. For them they are simply helping a family member, friend or neighbour, undertaking tasks that they consider to be part of their relationship with that person.

Everyone is encouraged to use the Carer Aware training tool on the Your Life Your Choice website. It explains who is a young carer, their legal rights and what services are available in Worcestershire to support them.

Worcestershire Young Carers transitions service The Young Carers service includes those young carers in transition aged between 16 and 24.

The transitions service offers one-to-one support, with education, employment and training being the focus.

It provides support with a range of needs including emotional support, help with referrals to other services/ agencies and support to access these services. It also

holds a six weekly consultation group.

Referrals are received from many sources including the Access Centre, GPs, colleges and other health professionals; also the service is happy to receive selfreferrals, which can be made to the Transitions Key Worker.

NHS-funded care Continuing care is provided by healthcare services from the NHS and/or social and community care services from your local authority.

Health services are provided through NHS organisations, such as clinical commissioning groups and

the Health and Care Trust.

Continuing care services may be provided by the NHS or Worcestershire County Council in various places, for example, in hospital, in a care home or care home with nursing, in a hospice, or in your own home.

What is NHS continuing healthcare?

NHS continuing healthcare is the name given to a package of care which is arranged and funded solely by the NHS for individuals outside hospital who have ongoing healthcare needs. If you need NHS continuing healthcare, your health and social care needs will be complex, substantial and ongoing, caused by a disability or chronic illness, or following hospital treatment.

Continuing healthcare is also commonly known as ‘continuing care’.

You can receive NHS continuing healthcare in any setting, including your own home or in a care home. NHS continuing healthcare is free, unlike support provided by local authorities for which a financial charge may be made depending on your income and savings.

If you are found to be eligible for NHS continuing healthcare in your own home, this means that the NHS will pay for healthcare (e.g. services from a community nurse or specialist therapist) and associated social care needs (e.g. personal care and domestic tasks, help with bathing, dressing, food preparation and shopping). In a

care home, the NHS also pays for your care home fees, including board and accommodation.

Who is eligible for NHS continuing healthcare?

Anyone over 18 years of age assessed as having a certain level of health needs may be entitled to NHS continuing healthcare. It is not dependent on a particular disease, diagnosis or condition, or on who provides the care or where that care is provided. If your overall assessment of needs shows that you have a ‘primary health need’ you should be eligible for NHS continuing healthcare. Once eligible for NHS continuing healthcare, your care will be funded by the NHS, this is however, subject to regular reviews, and should your care needs change, the funding arrangements may also change.

Whether or not someone has a ‘primary health need’ is assessed by looking at all of their care needs and relating them to four key indicators. Visit www.nhs.uk for more information and to obtain an NHS continuing healthcare checklist.

NHS-funded Nursing Care If you qualify for NHS-funded Nursing Care, the NHS is responsible for arranging and funding nursing care from a registered nurse in a care home which is registered to provide nursing care. This nursing care cannot be provided by local authorities. The NHS funds this nursing care through a payment called the Funded Nursing

Contribution (sometimes known as Registered Nursing Care Contribution). Currently this amount is £112.00 per week and is paid directly to the home.

Your needs will be assessed to decide if you’re eligible for NHS-funded Nursing Care.

Accommodation People who have a long-term, severe health or physical conditions can sometimes find they are no longer able to live independently, even with assistance from others.

If adapting your current home isn’t appropriate and you are looking to move to a care environment then there are several options available. These include: • Supported living.

• Adult placements and supported lodgings.

• Buying your own home.

• Residential care.

• Cluster flats.

• Supported living networks.

Shared Lives Shared Lives schemes enable adults who need support to take part in family and community life, which promotes the strengths and abilities of each individual. Support ranges from personal care to practical support.

In every Shared Lives home you are helped and supported by a carer who will share their home with you and include you as part of their family.

Shared Lives carers are located throughout Worcestershire. Some are in towns, others in the countryside. All are family homes and can offer long and short stays. You will have your own bedroom and the use of other rooms, for example the bathroom, living room, kitchen etc.

• Whoever you choose to live with you can bring your own things for your room, if you wish.

• Your friends and family can visit you when you want to see them.

• Your carers will be happy to support you to do the things you like to do.

• Your Shared Lives carer can support you to learn new skills and help you to live more independently.

Home to Home Shared lives can be part of the circle of support for service

users and their families, whether they are living in their own homes for a few hours a week and/or having respite stays. This can support individuals to increase or retain independence, learn new life skills and also offers respite and support to family carers.

Supported living Supported living is where a person with a disability gets the support they need to live in a home they own or are a

tenant in. They can live alone or with others. The support people receive is personalised and centred on their needs.

Sheltered housing Sheltered housing may suit you best. You can expect to live independently as tenants or leaseholders, safe in the knowledge that there is help nearby if you need it. Sheltered housing will suit people who would like to remain independent but prefer the added security and

reassurance of a scheme manager and an alarm call service.

A scheme manager is either based at the site or visits regularly. Many sheltered housing schemes also have communal lounges, laundry facilities, lifts, door entry systems and specially adapted facilities.

Extra care housing Extra care housing combines living in a home of your own with personal care. These schemes are usually aimed at older people. Extra care housing is similar to sheltered housing, but provides additional care services

Paying for care How much will I have to pay for care?

Use the confidential, online financial assessment calculator on ylyc.worcestershire.gov.uk to get a rough idea of how much you’ll pay for the services Worcestershire Council supplies to you.

This covers: • Care and support in a care home setting.

• Care and support in a non-care home setting, for example, in your own home.

When someone has been assessed as needing care and/

or support they will need to be financially assessed. Social care, unlike healthcare, is not a free service.

A financial assessment is needed to decide if you are eligible for any public funding and what you can afford to contribute towards your care and/or support.

In order to do this, Worcestershire County Council will need to complete a financial assessment, in accordance with the Care Act 2014. Please visit Your Life, Your Choice for more information about paying for care.

Seeking financial advice It’s important to know that you are making the right decisions when it comes to planning and paying for care and support. The Council advises everyone to seek financial advice from a professional adviser before making important decisions about their social care.

Feedback Adult Social Care – make a compliment, comment or complaint You may make a compliment, a comment or complaint about Worcestershire Adult Social Care Services. This will help the department to achieve its goal of providing an effective, high-quality service. Use the feedback form to say what you think, or contact your local advocacy service if you require help to make your views known.

A form is available on the Worcestershire County Council website at www.worcestershire.gov.uk/ Type in ‘complaints’ on the search facility. Alternatively you can telephone 01905 766365.

You can comment: • On any part of the Council’s service which you are not happy with.

• If there is a service or assessment you think you need that the Council has not offered you.

• If any action, attitude or behaviour of a member of staff has affected you, or someone close to you.

• On any other matter reasonably connected with the exercise of the Council’s social services function.

What happens when the Council receives your comments?

Your comments will be acknowledged within three working days. You will then be contacted to discuss your comments, what outcomes you want to achieve, the best method of dealing with any concerns, and timescales.

There are a number of methods that can be used to try to resolve any complaints, including referring them to the team manager responsible for the service, holding a meeting to discuss your concerns and seek a solution, conciliation, or investigation.

If you have made a complaint and the Council has agreed a method of dealing with it, but it becomes

apparent that your complaint is more complex or serious, then a better method might be suggested. For example, if your complaint is referred to the team manager for a quick resolution, but it is found that there are some more serious aspects to the complaint, then the Council may want to conduct an investigation instead to ensure that it deals with your concerns thoroughly, and try to achieve an appropriate resolution.

Once your comments have been dealt with a response letter will be sent to you within the agreed timescales. If an investigation is carried out then you will also receive a copy of the report.

If, after this, you are dissatisfied with the Council’s response to a complaint you can go to the Local Government Ombudsman. However, the Ombudsman will expect you to have given the Adult Social Care Services a chance to deal with your concerns first.

You can also speak to the Ombudsman if you have a complaint about services you have received that you pay for yourself, although it’s best to speak to the registered service provider in the first instance.

Concerns about inspected and regulated care services If you are concerned that the services you or someone you know are receiving do not meet national standards, you can contact the Care Quality Commission (CQC).

All care providers must be registered under a system which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety.

The CQC registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others.

Inspectors carry out a mixture of announced and unannounced inspections looking at the quality and safety of the care provided. They look at whether the service is: Safe; Effective; Caring; Responsive to people’s needs; and Well-led.

Following an inspection, each care home and home care agency is given a report of how it rates against national essential standards of quality and safety. CQC has also started rating services as ‘Outstanding’, ‘Good’, ‘Requires Improvement’ and ‘Inadequate’. By March 2016, CQC expects to have rated every adult social care service in England.
