Support for carers Carers provide unpaid care by looking after ill, frail or disabled family members, partners or friends who could not manage without their help.

Caring can be very rewarding, but it can also be isolating, physically and emotionally tiring, and trying to find the information you need can sometimes be frustrating.

Warwickshire County Council has commissioned Guideposts Carers Support Service to provide information, advice, quarterly newsletter, training, group and one to one

support. This service also administers the Carers' Emergency Card scheme which provides emergency contact numbers in the event of carers being temporarily unable to continue in their caring role eg due to sudden illness or travel delays preventing their return home.

Financial information and advice for carers Every year an estimated £740 million in carers’ allowances goes unclaimed. The benefit system is complicated and finding out what you are entitled

to can be difficult. Information can be found in the Carers UK website: www.carersuk.org

Employment Making relatively small changes in a carer’s working life, such as later arrival times or reducing hours, can help to make a caring role more manageable.

Information and advice on how to go about requesting flexible working from employers can be found at www.warwickshire.gov.uk/carers.

Young carers A young carer is someone under the age of 18 who helps look after someone who has a physical or learning disability; a serious illness; mental ill health; a dependency on alcohol or drugs or anyone with HIV/Aids. The person cared for may be a parent, grandparent, brother, sister, other relative or a close friend.

The young carers project (www.warwickshireyoungcarers.org.uk) supports children and young people in their caring role through the provision of information, advocacy, practical advice, a range of other support services and emotional support. Young carers can contact

the service themselves or can be referred by their parents or someone who knows them, for example a teacher, social worker or health professional. The project employs experienced, friendly staff who meet with young carers to find out what would best suit their situation. There are regular activity sessions and a chance to talk about any issues or just to have a break and some fun.

Getting out and staying active Day services We understand that our customers want to stay as active and independent as possible for as long as they can. Our aim is to promote independent living by providing a broader mix of services to enable customers to have more choice and control in their lives. If an individual is assessed as needing a day care service, their needs will still be met but they will be supported in a different way. This means using a Personal Budget, which you will control directly, or they can ask Warwickshire County Council, or a provider, to manage their budget. This will give greater flexibility about how, when, and where that care is provided.

Your recovery Being admitted to and leaving hospital If you need to be admitted to hospital the nursing and medical staff will see you shortly after your arrival and ask you questions about your health and home situation. Once a treatment has been agreed you will be told how long you may be in hospital.

However, this will be kept under review as the timing for your discharge from hospital may change depending on your recovery. If you know that you need to go into hospital, it will be important that you make arrangements for the care of your property and pets while you are away. You should also consider: • how your benefits may be affected. The Department of Work and Pensions provide information about how a hospital stay affects benefits (www.dwp.gov.uk) ; • how you can ensure that your essential bills are paid e.g. your gas, electricity, rent, while you are away, so that you do not encounter problems when you return home; and • how to organise your return home.

With your agreement, and where appropriate, a referral will be made to the hospital social work team who can make an assessment of your needs. It is important that you tell them your views and wishes for the future. If you have someone who helps to look after you, for example family or friends, their views, with your permission, will also be considered. They will also have the opportunity to have their own needs assessed through a carer’s assessment.

Sometimes we may need to visit your home. This is to allow us to assess what services and/or equipment will help you when you are discharged from hospital.

Some people may return home from hospital with support. This could be provided by adult social care and/or NHS health teams depending on their needs. You could be referred to the reablement service (explained further on page 8), if you are eligible, where you will be supported to safely and gradually undertake everyday living tasks in your own home, for example, cooking, dressing over a period of up to six weeks. You will then be assessed and, if eligible, and there is an ongoing social care need, your social care practitioner will work with you to produce a support plan.

If you are unable to return home, even with help, you may need to consider your longer term plans.

However, you cannot generally stay in hospital whilst you are doing this, as the hospital will need to be able to provide care to others in need of urgent treatment. By continuing to occupy a bed in a hospital when you no longer need it, you are preventing someone else receiving the treatment they need.

Not everyone will need this detailed assessment of their needs as some people can go home from hospital without any support. Others may have family and friends who can help out for a short time until they feel able to cope on their own again.

Recovery A range of services may be provided that will help you recover when you leave hospital. These could be therapists, care assistants or nurses and they may be provided in your own home, or another supported environment, such as a care home for a short period.

What if I can't go home immediately?

You may not be able to return home immediately because of practical difficulties, or important decisions needing to be made about your future. You will not be able to stay in hospital whilst these things are being sorted out. The team looking after you will explain what arrangements can be made in the meantime.

What if I need a permanent place in a care home?

If it has been agreed that your needs would be best met in a care home (including care homes with nursing) you will need to find a home that your can move to immediately. This may not be your first choice of home and you may want to consider moving again at a later date.

Sometimes it is not possible to find a vacancy in your preferred area immediately and in these circumstances you will be expected to move further afield. This applies to everyone whether they fund their own care or if it is funded by the local

authority. Your social worker or the discharge liaison nurse will provide you with any assistance/ support in finding a home, or you can get information about homes in Warwickshire in this Directory (lists start on page 40) or by contacting the Care Quality Commission (CQC), which has national as well as local information (www.cqc.org.uk).

Will I have to pay?

There is no cost to the individual for any assessment. If we provide a service, or arrange it for you on leaving hospital, you may have to pay for the service in full or make a contribution to the cost of this service. You will need to have a financial assessment if you need or want us to fund all/part of the service and your social worker will be able to explain any charges to you.

We understand that some people may not wish to provide details of their financial situation and in these cases the services will still be offered, but you will asked to pay the maximum charge. If you are likely to require a significant amount of health care on a continuing basis you will receive an assessment and information about your rights and entitlements.

Warwickshire reablement service The reablement service is designed to help people regain the skills and confidence they need to live independently at home, particularly after an illness or after staying in hospital.

It is available for up to six weeks. The aim is to assist you to manage your daily activities like washing, dressing, cooking and shopping. The reablement service will also advise you about safety issues to help you reduce the risks of falls, for example.

Reablement may well reduce your need for ongoing support services by improving your practical abilities so that you can do more for yourself. It has been introduced nationally and aims to keep people away from hospital services and the need for ongoing support, such as residential care services.

At the end of the reablement period, you may be able to live at home without any support. You may need ongoing care, if so, a package of care will be agreed and a support plan will be drawn up. You will be offered a financial assessment at the end of your reablement programme and will be able to discuss

with your care practitioner how your ongoing care and support needs can be met and find out if any financial assistance is available.

This service is only available to people who are eligible. Reablement will not be suitable for everyone as some people may still need a higher level of care.

Occupational therapy Occupational therapists (OTs) work with adults (people over 18 years, or if in full-time education, over 19 years) and older people, alongside other professionals. OTs will assist you as long as you meet the ‘Fair Access to Care Services’ (FACS) criteria and if you are having difficulty with day-today tasks or if you might be at risk of harm, for example, falling, or losing your independence.

Occupational therapists are regulated by the Health Professions Council (www.hpc-uk.org). The Health Professions Council holds a register of all those people who are eligible to practice.

Firstly, they, or one of their team, will talk to you to get full information about your needs. This may result in:

• suggesting alternative ways of completing a task;

• referring you to a more appropriate service;

• for people living in North Warwickshire or Rugby on 02476 390013.

• giving you advice over the telephone; • arranging to visit you at home; or • a combination of these actions.

The occupational therapist or occupational therapy assistant will work with you to agree a plan of action that meets your needs. This may be through:

Independent Advocacy We represent the interests of people who may find it difficult to be heard or speak out for themselves. These can include over 65s, adults with physical or learning disabilities and those with mental health problems.

With the support of trained staff, we provide an advocacy service, which gives a voice to people in need. This enables them to make their own choices and to have their individual needs and views respected and acted upon.

We offer a free professional service to clients referred from Warwickshire County Council, Warwickshire NHS Trust, Coventry City Council (Brokerage) and Solihull Care Trust.

We have over 17 years of expertise and, for those people who do not meet the contracted criteria, our services can be purchased by individuals and other organisations.

• loaning specialist equipment; • recommending minor building adaptations such as grab rails; or • recommending grant support for major adaptations to make your home more suitable for independent living.

Independent Appointee providing Personalisation Support As a Corporate Appointee Service, approved by the Department for Work and Pensions, we provide an Appointee and Money Management Service for anyone receiving benefit who is unable to manage their money appropriately and for whom there is no one else deemed more suitable to take on this role.

This service can also be purchased by individuals or other organisations. It can be tailored to specific client needs where we can purely accept responsibility for claiming and managing benefit entitlement, managing benefits, pensions and pay bills on someone’s behalf. We can arrange support schemes to help clients retain their financial independence wholly or partly. We can support the opening of bank accounts, agreeing budgets and setting up payment plans.

Help to live at home Home care Home care is personal and domestic care that helps keep people safe and comfortable in their own home. This could mean helping customers to get up in the morning and go to bed in the evening. Home care does not provide nursing care, which requires the specialist skills of a qualified nurse.

Who can receive home care?

We will work with you to assess your needs. If you are eligible, an appropriate package of care is put in place.

All our home care customers are regularly reviewed and care packages are updated accordingly.

If you do not qualify for a service funded by us, we will offer advice and information about possible alternative help.

Who pays?

If you have savings of £23,250 or over, or are a couple with £46,500 or over, you will have to pay the full cost of your care.

Independent home care providers Care in your own home is often referred to as either domiciliary or home care.

Given the choice, many people prefer to stay in their own homes surrounded by their possessions and near to their family and friends. This, of course, applies to everyone whether they are elderly and frail, convalescing, have a physical or learning disability, or have dementia. The range of care available is varied and each client should be involved, and agree, with the care provider, a care plan, which states their needs and the expected outcomes.

Domiciliary care is usually arranged in a manner dependent upon needs: Low level domiciliary care – this may be to assist with household chores such as shopping, cleaning and assistance with food preparation. It may also involve a carer accompanying clients on trips out and even holidays. Calls from carers can be flexible and assistance can be given for as little as 15 minutes or when the client needs.

Medium level domiciliary care – in addition to low level care this will normally involve personal care (assistance with washing, dressing etc), and/or one carer to assist with mobility. Clients may wish to have a carer sleep in their home overnight for security and reassurance.

High level care – this involves visits by two or

more carers who assist clients with high dependency needs such as being transferred from a bed to a chair etc. Carers may require specific training for the health needs of a client.

Live-in care – a carer lives in the client’s home, is allowed time off each day and must have a night’s sleep. Live-in care can be arranged as a short respite for the client’s main carer or a permanent arrangement to suit the client’s needs.

ADVERTORIAL

All service providers must be registered and inspected by the Care Quality Commission (CQC).

It is important to be assured that all carers have Criminal Records Disclosures, have received necessary training, are properly referenced, and are eligible to work in the UK prior to them visiting your home. Ask the service provider to show evidence that the carers they introduce are indeed compliant with the legislation, and check their latest CQC report at www.cqc.org.uk or call the CQC on 03000 616161.

Paying for home care Domiciliary care for social needs is often funded in part or whole by the local authority, or alternatively private arrangements can be made with service providers. The majority of social services funded care is contracted out to private companies, rather than the internal local authority service.

Fees for services vary, a good service provider will be able to provide you with a detailed cost breakdown prior to the commencement of any service.

Most service providers will accept clients who pay using Direct Payments (discussed further on page 29). You will still need to have an assessment of your needs, and if you are eligible, you can decide whether you would prefer to arrange the services yourself. If you do not meet your council’s eligibility criteria, you must organise your own care as a ‘self-funder’. Listings of home care providers start on page 14. See page 32 for advice on self-directed support.

24 hour care – this is required when the client

needs assistance or monitoring through the day and night; it differs from live-in care in that carers work on a roster to ensure that a carer is on duty and awake both day and night.

Some agencies are also registered as Nurses’ Agencies, meaning they can provide registered nurses if needed.

Equipment and adaptations Equipment and adaptations can help people live more productive lives. It enables people to carry out essential activities, with the aim of maintaining or

improving independence. In Warwickshire we currently provide:

Assistive Technology (Telecare) Assistive technology is a range of equipment that can help people with a physical, sensory, or mental health disability. It helps people to continue to live at home safely and as independently as possible.

Our assistive technology service, Telecare, can can offer protection from: • • • •

intruders; floods; fire; and falls.

The name Telecare describes the wireless sensors that are linked to a control centre, or to a family member’s mobile phone or pager. The sensors monitor your home and can raise the alarm should an emergency occur.

The service makes a real difference to people’s lives, not only for those using the service to maintain their independence, but also their families and loved ones who really appreciate the peace of mind that the use of this equipment can bring.

Benefits of the Telecare service include: • instant response - the highly skilled operators respond to your call and take the right action to get you the help you need, when you need it;

What types of equipment are there?

• personal service - friendly and dignified service;

Personal trigger - a call for help can be raised from anywhere in the home or garden

• we will not leave you - the operators will stay on the line with you until help arrives; • complete reassurance - 24-hour link to the control centre; and • easy installation - systems are easy to fit and often wireless, so no ugly wiring.

Our aim is to: • assist people to maintain a safe home environment; • assist carers to care for someone at home; • reduce the risk of falls and accidents within the home and reduce the waiting time for assistance following an incident; • prevent avoidable admissions to hospital and speed up discharge; and • prevent or delay admission to a residential home.

The service involves the provision of a telephone link 24 hours a day between your home and the control centre. All that is required is a telephone line and an electrical power point (within six feet of each other).

Should a sensor be activated, a message is sent to the control centre where trained operators take the most appropriate action, such as contacting a family member, neighbour, doctor or the emergency services.

Examples of the type of equipment used in Warwickshire are:

Bogus caller button - a discreet button placed near a door, which can be used to call for help from the 24hour control centre when a stranger asks for entry into your home Smoke/carbon monoxide detector - a life-saving device, the detector provides additional protection by raising an instant alarm call to the control centre if anything is detected Fall detector - automatically detects a serious fall and raises an alert with the 24-hour control centre Flood detector - this small radio sensor gives an early warning by alerting the control centre of potential flood situations in the home Movement detector - this allows the 24-hour control centre to detect for inactivity and raise an alert, or alternatively to audibly verify and record the presence of an intruder.

How do I get Telecare?

If you are already supported by a social worker or an occupational therapist you can simply speak to them about the Telecare service.

Integrated Community Equipment Service (ICES) The Integrated Community Equipment Service helps people to live in their own homes more easily by providing a range of equipment. This can range from a simple walking stick, to a more complex item like a mobile hoist. To receive this service you must be assessed and eligible for social care support (explained on page 28).

There is no charge for equipment provided by ICES. What equipment is available?

The equipment service has a range of equipment available in stock. The assessor has to ensure that you meet the criteria for that piece of equipment before being able to issue it. If the assessor recommends a non-stock item of equipment, for example, a specialist shower chair, this will have to be individually ordered and may take longer to provide How do I get my equipment?

The equipment service will make an appointment with you to deliver and fit the equipment. The assessor will demonstrate and instruct you in the safe use of the equipment.

ICES currently do not provide: • wheelchair services; • school equipment;

How do I return my equipment?

• sensory equipment;

To return equipment not being used telephone 0800 915 7744 and they will arrange a time to collect it from you.

• communication equipment; or • telecare.

How long can I keep the equipment?

The equipment will remain with you for as long as you need it. You will be contacted to see if you are still using the equipment at regular intervals.

If your needs change you can ask for a reassessment, which may mean your equipment is exchanged for a different piece of equipment.

How do I get my equipment repaired?

You are expected to routinely check, clean, and keep equipment in good working order. If the equipment breaks or becomes faulty do not use it please contact us immediately on 0800 915 7744.

Smartcare - assess yourself on-line for equipment We want to reduce the risks of falls and accidents in the home and enable people to live more independently. We are currently working with an organisation called ADL Smartcare to provide an on-line, risk-free assessment service. Here you will be able to find out what equipment is available to help you such as grab rails, raised toilet seats, bath boards, or tap turners.

Warwickshire residents will also be entitled to a discount on equipment ordered through this service. There are lots of items of equipment that could help you - for all parts of your home.

Meals service The meals service in Warwickshire is delivered by County Enterprise Foods who produce and deliver ready-made meals, direct to your home. The service is for older and vulnerable people and aims to help you to live as independently as possible.

The meals are: • tasty and nutritious;

• available on the days of your choice up to 7 days a week; and • breakfast and tea time packs can also be delivered with your main meal.

We can supply meals to suit any dietary need including: • vegetarian;

• varied; and

• African Caribbean;

• made using fresh ingredients and seasonal vegetables.

Frozen meals These meals are delivered together weekly, fortnightly or monthly. You can then choose which meal you want to eat on which day, at a time to suit you. The frozen service offers a choice of over fifty meals a day.

The meals are for any elderly person and also for adults who have:

• Halal; • Kosher; • spicy meals; • specific medical conditions;and • soft food diets.

To receive community meals, telephone County Enterprise Foods or visit one of our Customer Service Points. We can talk to you about what meals we can provide and what is the best kind of delivery for you.

If you prefer, someone else (such as a friend, relative, neighbour, health or housing worker) can get in touch with us on your behalf.

• a deteriorating or life-threatening condition or are nutritionally at risk.

• plating meals, getting cutlery out and pouring a cold drink;

Health and wellbeing Safeguarding adults Many adults, because of illness or disability, may be unable to protect themselves from abuse. Vulnerable adults may be certain older people, people with learning disabilities, physically disabled people, people with mental ill-health or those with a short or long term illness.

Abuse can be defined in many ways however the terms most commonly used are: • physical; • emotional; • neglect;

• institutional; and • verbal.

Abuse happens in situations where one person in a relationship has more power than the other. In adult abuse cases this could be a vulnerable adult's paid or unpaid carer, a worker, relative, in fact almost anyone.

Mental health It is estimated that one in four people living in the UK will be affected by problems regarding their mental wellbeing at some point in their lives. If you have any concerns about your mental health you should visit your GP who will be able to offer you advice and support.

A small amount of people may experience mental health problems to such a degree that they are diagnosed as having a mental illness, requiring the involvement of specialist services and support.

The majority of people will not experience mental illness, but will experience mental health problems at different times in their lives.

If people are given support and information, those experiencing mental health problems can make positive changes and improvements. Only a small minority of people do not respond to appropriate help and need more specialist involvement.

Dementia services The term ‘dementia’ is used to describe the symptoms that occur when the brain is affected by specific diseases and conditions.

Dementia is a common condition and there are over 100 different types. In England alone, there are currently estimated to be around 750,000 people living with dementia (source: Alzheimer’s Society).

In Warwickshire we are proud to be taking the lead on a partnership approach to dementia services and information and have been instrumental in

The End of Life Care Strategy ‘How we care for the dying is an indicator of how we care for all sick and vulnerable people. It is a measure of society as a whole and it is a litmus test for health and social care services.1’ Although the subject of dying is often painful to contemplate and as a society we do not discuss death and dying openly, the way care professionals approach the process is incredibly important for the client, their family and carers. The Department of Health has published Strategies for organisations involved in providing end of life care which aim to promote high quality care for all adults at the end of life, whether this be at home, in a care home, in hospital, in a hospice or elsewhere.

A care pathway approach for both the commissioning and delivery of services is strongly recommended by the Department of Health, and should encompass : • Identifying people who are approaching the end of life and initiating discussions regarding their preferences for end of life care.

• Care planning: assessing needs and preferences, agreeing a care plan to reflect these needs and preferences and reviewing these regularly.

• The coordination of care across all sectors (health, public and private).

• The delivery of high quality services in all locations.

‘Tell Us Once’ service When someone dies, the last thing the loved ones left behind want to be landed with is extra paperwork and red tape. But from now on, thanks to a new ‘Tell Us Once’ service being rolled out across the country, it will be easier for families to notify government authorities about the changes in their circumstances.

‘Tell Us Once’ has been launched by the Department for Work and Pensions in partnership with local authorities across England, Wales and Scotland. It enables key local and central government departments and services to be notified simply through one single appointment with your local registrar.

When you make an appointment to register the death in Warwickshire, you can extend your appointment and take up the ‘Tell Us Once’ service.

This will mean that the registrar can send your information to other government organisations that need to know about the death.

Choices if you cannot live at home Many older people can end up living where getting about is difficult or they are not happy to be because they want to please others, or not be a burden to their family. Moving after an accident or an illness can mean there is not enough time to think about what you really want. Having the

opportunity to think about the sort of place one might find desirable and suitable before a decision needs to be made is important.

• you are living at home - not in a residential care home;

Each scheme is different and what is available varies from site to site. However, each scheme will have:

• you have your own front door - you decide who comes in; • couples and friends can stay together; • there is a mix of able and less able people; • 24-hour care services are available - if you need them; • you get support to keep your independence; • you can join in social activities - or you can be private; • you have control over your finances; and • you have security - a buzzer entry system and a locked front door.

Extra care housing offers a new way of supporting you to live independently for as long as you possibly can. It gives you the security and privacy of a home of your own, a range of facilities on the premises, with 24-hour care services available if you need them.

Extra care housing is sometimes called ‘very sheltered housing’. The look of the accommodation varies enormously: some are new purpose-built retirement villages, others are modernised sheltered housing schemes with extra services on site. Many offer a choice of accommodation such as flats or bungalows, for sale or rent. What all residents of extra care housing have in common is the security of owning or renting your own home, control over your finances and the peace of mind that goes with having 24-hour care and support available on site.

You may be thinking of moving or having to leave your old home because of life changes or

• Housing - a range of modern and affordable one or two bedroom homes available to buy, rent or part own which are purpose-built or adapted for your care and support needs • Communal areas - these might include a cafe, hairdressers, library, guest accommodation for your visitors, a day centre with organised activities and outings.

• Neighbourhood facilities - extra care schemes are based within local communities so the usual range of neighbourhood facilities are close by such as leisure centres, shops, GP surgeries and public transport.

What support can I get?

Home care staff can offer support with your personal care, including: • help with washing, dressing and getting to and from the toilet; • reminders to take medication; • preparing snacks and heating up meals; • shopping; • laundry; and • some domestic tasks.

There is access 24-hours a day to support with practical care tasks. Some extra care sites have dementia units which provide specialist care in a community setting.

Extra care housing continued What extra care doesn't cover People who need specialist nursing care (such as dementia) frequently or throughout the 24-hour period, beyond the level available from the Community Nursing Service, may need specialist nursing home care (explained further below).

How much does extra care cost?

There are three parts to extra care costs: • housing - the cost of renting, buying or shared ownership of your home; • service charge - home maintenance and communal facilities; and • care and support costs - these vary depending on your needs. You may be eligible for support with this following an assessment by a social worker.

The amount you pay will depend on your individual circumstances. We will conduct a financial assessment and tell you exactly how much it will cost.

For some older people residential care is the best option.

Care homes - definitions If you decide that a residential care home is the best option for you we will advise you on the type of care home that will best meet your needs - this could be a residential home, or a home that provides nursing care. The home might also need to provide a specialist service, for example, dementia care. We will help you find out about homes, vacancies and prices.

All residents have to pay for residential care, but the amount will be assessed according to your income and savings.

All care providers in the country must be registered with the Care Quality Commission (CQC). Homes are inspected by the CQC, who report on their findings.

These inspection reports are available at the home or from the CQC (www.cqc.org.uk).

• Care homes with nursing If you think you may need nursing care in a home, you will need to be visited by a social worker or a care manager to work out what care you will need.

This visit might be in your own home, or in hospital if you’ve been ill, or in a home. You will be fully involved in planning your care needs.

If a care home providing nursing care is the best solution for you, the social worker will give you information to help you find a home which meets your care requirements. The cost of the nursing care part of your fees is paid by the NHS to the home directly: the current amount is £108.70 per week.

You can contact the Nurse Care Management Teams for your area at your local Primary Care Trusts (PCTs).

There are two types of residential home: • Care homes (personal care only) If you are reasonably active, but would like greater security and care, subject to a needs assessment, a care home offering only personal care may be the best option. Personal care includes bathing, feeding, dressing and help with moving but it must be paid for if your capital/savings exceed £23,250.

What is a Social Care Assessment?

People who are thought to be eligible for support have a social care assessment, this is how we find out what people’s needs are and what outcomes they want to achieve with their support.

To achieve this we will need to ask you questions about your needs or problems and how they are affecting your day-to-day life. We will only ask questions which help us to understand your needs or problems. This will include basic information such as your name and address, date of birth, next of kin, and GP. We may also need to discuss your current health, medical history, family, housing and financial situation, and personal care.

The assessment is an opportunity for you to say what you think and to ask us questions. We will listen carefully to your views and take your wishes and feelings into account. If you need an interpreter because your first language is not English, or you use sign language, we will arrange this.

You will only need to give your personal details and information about your needs or problems once

and, if you agree, we will then share this information with other people involved in your care. We may have to ask other people for information about you.

For example, we may need to ask your doctor to tell us something about your health problems. We will not normally collect or record any information about you, or share it with anyone else, without your permission. We call this ‘consent to share and record information’ - the person who does your assessment will explain this to you. If you contact us by telephone on 01926 410410, you will initially speak to a customer service advisor based in the customer service centre, who will be able to advise you of your eligibility for further assessment for services.

If you are eligible for help we will talk to you, and your family and carers with your permission, about the options available to you. We will then write a plan that sets out the support we have agreed with you. If you are caring for someone on a regular basis and feel you would like to be assessed in your own right as a carer you can request a carer’s assessment.

What is the Support Plan?

After your assessment is completed we will discuss with you how you wish to be supported, this can include your family, friends or carers if you want it

to. The Support Plan will give the detail of when you will get support, the type of support, how often you have it and who will provide it to you.

What is a Personal Budget?

Warwickshire now give people who are eligible for long term support a Personal Budget, this is the money to buy the support identified in the Support Plan.

The Personal Budget can be used in two different ways, you can chose to take the money and arrange your own support, this is called a Direct Payment which just means that we pay you the money directly

rather than paying the person or organisation that supports you. This gives you a lot of control over how your support is provided and makes it simple for you to change the arrangements.

What is a Personal Budget? continued family or friends if they are willing to help you.

The other way of receiving your Personal Budget is for the Council to arrange the support for you this would be done by the person who completes your assessment and support plan. We will still involve you in deciding

how to arrange your support but you will need to come back to us if you want to change anything.

You are also able to have a Direct Payment for some aspects of the support plan and have the Council arrange support for other parts of your plan.

Your financial assessment We are not able to subsidise the whole cost of care packages in Warwickshire and so we are required to carry out a financial assessment to find out whether you have to make a contribution towards your support.

To find out if you need to contribute to the cost of your care, you will be visited from one of our specialist finance visiting officers who will look at your income and any capital or savings you have, and will make sure you are receiving, or have claimed, all the benefits you are entitled to.

The visiting officer will also discuss with you any expenses you have as a result of your care needs or disabilities. We call these ‘disability related expenses’ and can include things like exceptional laundry costs or paying for certain items of equipment. The visiting officer will discuss these items with you and agree weekly amounts that can be included as expenses that relate to your disability or care needs.

Once this information has been collected and agreed, the visiting officer will then be able to calculate whether or not you will need to make a contribution towards your support. If you are happy to proceed with the visit it would be helpful to have your financial information available when the visiting officer calls. This will include things like: • benefit award letters including state pension details and housing benefit award and occupational pension details; • rent/mortgage and council tax details; • details of all bank/building society statements, savings accounts, PEPs, ISAs etc; and • Trust funds, bonds and any other income.

Any recent receipts you may have for the following would also be useful: • fuel bills; • recently purchased disability equipment; • invoices for privately arranged care purchased directly by you; and/or

• essential purchases that enable care to be provided in your own home.

How is the amount I need to pay calculated?

The total amounts of your income, capital and savings will be added together and from this we will deduct your housing costs such as mortgage/rent payments and council tax. In line with Government guidance we will also deduct an amount equivalent to the basic level of Income Support or Pension Credit that the Government says that you need to live on, plus 25% and also deduct the amount we have agreed with you as ‘disability related expenses’.

How much will I have to pay for my social care services?

If you have savings of £23,250 or over, or are a couple with £46,500 or over, you will have to pay the full cost of your care.

You will be sent a letter from the financial assessments and benefits advice team advising when the visiting officer would like to visit.

The result of your financial assessment will be sent to you either by your Personal Budget administrator or the finance assessments and benefits advice team.

Charges for social care By law we can ask people who receive adult social care services to contribute to the cost of the support they receive. These contributions help us to continue to provide adult social care services to the growing number of people who need them.

Everyone will be offered a financial assessment to work out how much they can afford to pay towards the cost of their care.

As part of your financial assessment, we will take account any savings you have, as follows: • less than £14,250 - we will not take these into account; • between £14,250 and £23,250 - we will apply a tariff income of £1.00 for each £250 (or part of £250) and add this to your income used in the assessment; and

• above £23,250 we will assess you as being able to pay the maximum contribution, no matter what other income and disability costs you may have.

The financial assessment will make sure you keep a basic level of income to include basic housing costs and agreed disability-related expenses. As part of the assessment we will also give you advice on welfare benefits so you may be able to increase your income by claiming all the benefits you may be entitled to.

What happens if I do not want a financial assessment?

You can choose not to have a financial assessment, but this means that you will need to make the maximum contribution for your care.

Local charges The following are an indication of our current charges from April 2012. We suggest you check our website for any changes.

Your care, your choice Self-directed support and personal budgets Self-directed support is about you being in control of the assistance you need to live the life you choose. If you have been assessed as being eligible for social

care support, we want to put you at the centre of this support and give you control over that support.

Steps to self-directed support There are seven main steps to helping you be in control of your support:

1. Completing a self assessment This helps us find out: • if you are eligible for social care support; and • about your day-to-day life; and • your social care needs.

We will arrange to visit you to help you complete the assessment. If you want, you can involve a family member, carer, friend or someone else who is important in your life to help with this too.

We also want to support carers. We will also assess someone who regularly cares for you, for example, a family member, friend, neighbour etc. if you would like us to, provided they are not being paid for caring for you.

You can tell us about the changes you would like to make, the things you would like to stay the same, and what you would like to achieve - we call these outcomes.

It may be that you are able to make some choices about how you want to be supported. If you are unable to make your own decisions about your support, we will work in line with the Mental Capacity Act 2005 to make sure we meet your best interests.

After we have completed the assessment we will look at what you have told us and work out an estimated amount of money you may need to pay for your support (we call this your ‘estimated Personal Budget’). This will help you as a guide to plan your support.

Developing ‘My Support Plan’ How you want to use this money (‘estimated personal budget’) will be written in a plan called ‘My Support Plan.’ This describes the support you want to meet your social care needs and how you plan to achieve your outcomes. This must also keep you safe, well and be legal.

A social care practitioner will meet with you to help you with your plan. You can also have a friend, family member or a trustee to help you as well.

The final amount of money is only confirmed when your support plan is agreed. This is called your ‘Personal Budget’.

3. Agreeing ‘My Support Plan’ Your personal budget must be spent on meeting your eligible social care support needs and the outcomes in your support plan. You can use your personal budget on many different things within the amount of budget available to you, for example: • employing someone to support you in day-to-day life (we call this person a personal assistant); • buying support from a care agency; • meeting other people, perhaps at a local club;

• keeping well by having regular exercise, perhaps swimming, or being part of a local walking group; • having help to run your home; • having a break so that your family carer can have some time off; or • learning a new skill or maintaining an existing skill.

Often talking about this with someone else can help you think about the different possibilities.

4. How do I use my personal budget?

There are a number of ways you can use your Personal Budget:

• you cannot buy a service from us, as the council is not allowed to sell their services this way;

Direct payments

• you cannot use it for health related services i.e. dentist;

Direct payments provide greater choice and flexibility in the way you arrange the support you want, and you can include things we (the council) do not organise and directly pay for.

Choosing direct payments is the only time you will use your personal budget as money.

You will receive the money every four weeks paid into your designated bank account to pay for your support.

If you have capacity to agree to direct payments and you do not want to manage it alone, you may wish to nominate a person you know, or a trust, to open a bank account and receive the money on your behalf. They could also arrange the support following your support plan. You will still be accountable for the way the direct payment is managed.

Trust - this is usually a small group of people who manage your direct payment on your behalf and can include anyone you agree to, such as members of your family.

If you support someone who does not have the capacity to agree to direct payments you may be able to act as a Suitable Person. If you are appointed as a Suitable Person you will be accountable for receiving and managing the direct payment. The law tells us who can be appointed as a Suitable Person and we must follow a process to understand who can act as this person.

There are only a few things you cannot use your direct payment for: • you cannot purchase long term residential care but you can take short breaks;

• you cannot use it for your own personal bills i.e. personal clothes, food, household bills, rent, and mortgage; • you cannot use it for gambling, buying alcohol or cigarettes; • the law tells us you cannot pay someone who lives in the same household unless we agree; and • you cannot use it for anything that is illegal or does not keep you safe and well.

Sometimes people under certain mental health legislation cannot be offered direct payments.

In Warwickshire we have a direct payment support service. Here you can access support, information and advice. Please ask us for the direct payment support service contact details.

A personal managed budget You may decide that you prefer not to receive your personal budget as money, and instead you want us (the council) to make your arrangements for you.

If you choose to do this, we will use the money to pay for your support. Your support will be limited to the organisations that we contract with, and the services we provide.

If we agree with you that residential care is the best way to meet your support needs, this will be arranged as a managed budget for legal reasons.

A combination budget (a mixture of a direct payment and a managed budget).

4. How do I use my personal budget? continued for some of your support, but you want your social care practitioner to arrange some things for you - we call this a combination budget.

Individual service funds Individual service funds are an option for people when their personal budget is known. We will work with you to choose a provider that will help you meet the needs

and outcomes agreed in your support plan.

If you agree, we will give your Personal Budget (some, or all of it, depending on what has been agreed) to the provider on your behalf. The provider can only use your budget for your support and must account to you. This gives you some flexibility and control as you agree with the provider the exact detail of your support.

5. How much will it cost me?

We will carry out a financial assessment with you to see how much money you can afford to pay towards your support. Depending on the outcome of the financial assessment you may be asked to pay

towards your personal budget to buy the support you need.

You will be told how much you need to pay before being given your personal budget.

6. Keeping records and spending the personal budget If you are going to receive direct payments, we advise you to open a separate bank account in your name, or the name of the person you have agreed will help you with managing the money.

We will advise you how to keep simple records and how to get the types of help and support to make things easier for you.

At first, we will need to look at your bank statements for the separate account for direct payments every three months to help you manage and keep on top of things and we will write and request these from you.

If you find that you are spending more money each month than you have coming in, you must tell us. We can help you to find out why. You may need

to spend your money in a different way to meet your agreed outcomes. However, we cannot give you more money if you run out.

We will look at how much you have in your personal budget. If you do not spend the entire personal budget, we will ask you to pay back anything above the agreed reserve amount.

We support the flexibility of personal budgets and we will allow you to build up an amount of money to help you to plan for such things as your employer responsibilities and achieving your agreed outcomes. Usually around eight weeks of funding would be acceptable. Anything saved above this must be agreed during your support plan or review.

7. Reviewing We arrange review meetings with you to see if you need a different amount of money and support.

However, you can tell us at any time if you feel your needs have changed.

The purpose of these review meetings is to look at your support plan and see if your support is still helping you to achieve your outcomes. We will look at what you want to change, or keep the same, and check that the allocated funding is meeting your needs. A large underspend may trigger a review to ensure the resource relates to your support need.

outcomes. We will work with you to stay within the rules and discuss alternative arrangements to maximise the choice available to you.

You will only continue to receive a personal budget if you are eligible for social care support. If your circumstances change and you are no longer eligible you will be supported to close down your personal budget responsibilities.
