Welcome from Somerset County Council Welcome to the 2015/16 edition of the Somerset Care and Support Directory. I hope you will find it valuable, if you want to know more about what support or care is available for yourself, or for a relative or friend. If you pay for your care and support services yourself or if they are paid for by the local authority (see page 43), this Directory will help you to find the care and support you need, that suits your circumstances Somerset County Council is committed to helping people maintain their quality of life, where possible in their own home. As you know, local authorities such as ours continue to be under significant financial pressures. However, making sure that the most vulnerable members of our communities are safe and well cared for still remains this Council’s highest priority. We know that deciding what care and support is best for you or someone you care for can be difficult.

One in ten people in the UK cares for friends or family members. We support unpaid carers in various ways, for example by providing training, links to support groups and by paying towards short breaks (often called respite).

We work closely with colleagues in the health services, registered care providers and the Care Quality Commission (the independent health and social care regulator) to raise the standards of care and support in Somerset. How can this Directory help?

We know that many people and their carers, perhaps with a little support, can successfully live at home and enjoy their life to the full.

Our key principles are that we want people to remain in control of their lives, to remain independent and to have the opportunity to make their own choices about their care and support arrangements.

Many people, who in the past may have had a package of care to support them, with a little imagination, can often find other, more flexible and suitable ways to help them.

This Directory covers some of these things, called ‘support in the community’ before moving into the more well-known services, such as home care and care homes. These services

too, are developing more ways to be flexible and tailored to what you need. The Directory doesn’t list all the community support available in Somerset, so when you develop your support plan, consider what else is available locally.

Although we have laid out the Directory in a way to make it easy to follow, the things you need may be built up from each section, so that you end up with support that is tailored to you.

Finally, will we be able to help you pay for what you need? As a general rule, if you have over £23,250 in income and savings, no we can’t; if you have less than this and you meet the national eligibility criteria for care, yes we can. This is explained in much more detail starting on page 43.

What is the difference between care and support?

In this Directory, ‘care’ is used to describe any service that helps you with your personal care, for example, helping you to get up, washed and dressed in the morning.

‘Support’ is anything provided to you that doesn’t include personal care, for example, domestic cleaning, shopping and attending community groups. Of course,

some people need a mixture of both of these things; the same person or organisation, or different organisations may provide them.

It may be that you arrange what you need yourself, or that you ask us to help you. The important thing is that you have the right information to make your own choices.

Where do I start?

The information in this Directory is designed so that you can find solutions for yourself, if you wish to, or you can ask us for help and advice if you need it.

The best way to start to understand what care and support you need is by writing a list of the things you are finding more difficult to do. To help you do this, on page 8

there is a blank ‘support plan’ that you can complete if you wish. Firstly, write down the things you are finding difficult to do, then, by reading the Directory you can start to come up with solutions, that you can write in the boxes; things you can do for yourself, things you can ask others to help you with, or things you would like to talk to us about.

Advocacy is when someone helps another person to understand something and to be heard.

Independent advice and support continued be daunting, especially if you have no one to talk things through with. By talking with an advocate they may be able to cover and perhaps explain in better ways what some of the complex processes and possibilities are for you.

An advocate can help you tell other people what your needs or wishes are. They will support you to speak for yourself whenever possible, but they can speak for you if you want them to.

If we think you would benefit from the support an advocate can provide, we will ask an advocate to become involved.

The Care Act 2014 New legislation for social care started in April 2015. The Care Act brings together lots of pieces of Social Care legislation, written between 1948 and 2014, into one document, updated them and makes them much clearer.

We have updated this Directory where relevant. Some

of the new legislation, mainly involving financial changes, doesn’t start until April 2016. We will include these changes in next year’s Directory. But if you would like to find out more please look on the www.gov.uk website and search ‘Care Act’ for some useful factsheets.

Wellbeing A major element in the Care Act talks about wellbeing. If you contact us, we must have your wellbeing at the heart of everything we do, whether you are talking with a call adviser, with a social worker about your care needs or when we help you to write your care and support plan.

What is wellbeing?

‘Wellbeing’ is a broad concept, and relates to a person’s: • personal dignity (including being treated with respect); • physical and mental health, and emotional wellbeing;

• protection from abuse and neglect; • control over day-to-day life (including how care and support is provided); • participation in work, education, training or recreation; • social and economic wellbeing; • domestic, family and personal life; • suitability of living accommodation; and • contribution to society.

Writing your support plan By completing your support plan you may find it helps to identify what you need, and then reading the Directory may help you find solutions. It is very similar to what we would use during an assessment. Obviously, you may prefer to miss this section and just read on.

Help for carers According to Carers UK, one in ten people in the UK cares for friends or family members. We are committed to supporting carers who carry out a valuable role to the person they care for. It is estimated there are over 50,000 carers in Somerset.

Many do not realise that they are a carer, or think of themselves as a carer, and do not know that there is a range of services and information available to them.

Who is a carer?

Somebody who provides necessary support or who looks after a family member, partner or friend who needs help because of their age, physical or mental illness, or disability. This would not usually include someone paid or employed to carry out that role, or someone who is a volunteer.

‘Compass Carers’ – the carers’ support service for Somerset Somerset County Council and NHS Somerset jointly fund a support service with Compass Disability Services, a Somerset based user-led organisation, to deliver a countywide support service for carers. This service is called ‘Compass Carers’. The service is for carers of all ages who care for people with any condition. It doesn’t matter how long they spend in their caring role. The aim of the service is to support carers, to offer individual support so that they can maintain their caring role and also have a life outside of caring.

‘Compass Carers’ have experienced staff, including: • office-based staff (who co-ordinate the team and man the advice/information/support telephone line); • Link workers (who liaise with GP surgeries and Carers’ Champions); • Voice workers (who liaise with, and help establish carer support groups); and • Carer Support Workers (who visit carers to help establish and monitor a support plan).

You can contact them to find out information and get

advice. They can provide you with the emotional and practical support that most carers need to keep them going. Every carer is an individual. This service will listen to you and help you identify the things that are important to you and how to achieve them. Whether it is to do with family, finances, employment, isolation or having a break, they can work with you to help find solutions.

If you are an unpaid carer, then please contact ‘Compass Carers’ for: • emotional support; • information in various formats to suit your needs; • pointing you in the right direction to get help or information; or • access to local carers support groups, or a visit.

Carers’ assessments Carers are entitled to an assessment by social care to find out if, through the provision of additional care and support, it will help you in your caring role. Some support may be paid for by us if you are eligible.

How do I apply for a carer’s assessment?

If you would like more information, ask your social care worker, or phone us on: 0300 123 2224.

When you contact us, we will ask you for information about you and your needs. We may suggest you talk with Compass Carers, or we may be able to give you some advice and information straight away over the phone.

This may be followed up by a member of the social care team coming to see you and, if you wish, the person you care for, to give you more advice. This is called a ‘needs assessment’. If your assessment shows that providing support to either you or the person you care for is needed, then we will tell you how you can arrange this, or help to arrange this if you would like us to.

Do I have to be assessed?

You do not have to have a carer’s assessment. If you choose not to have one, it will not stop the person you care for from receiving services. >>

Carers’ assessments continued >> What happens next?

If we can help, we will write a care and support plan for you, or, if you prefer, a joint support plan for both you and the person you care for, taking into account your needs as a carer. The care and support plan explains what support will be arranged and who will provide it. If your needs are eligible, we will provide you with a personal budget and either we can arrange the support for you or you can decide to have the money so you can buy the support yourself.

What if I am not eligible?

If you are not eligible for services, we can still provide information and advice and put you in touch with other carer support organisations, such as Compass Carers. You

You may be eligible for help if: • your need for support results from providing necessary care for an adult, and • you need support to achieve outcomes in one or more areas of life listed below: • carrying out any caring responsibilities the carer has for a child; • providing care to other people to whom the carer provides care; • maintaining a habitable home environment; • managing and maintaining nutrition; • developing and maintaining family or other personal relationships; • engaging in work, training, education or volunteering; • making use of necessary facilities or services in the local community; • engaging in recreational activities; and • failure to achieve these outcomes would result in a significant impact on your wellbeing (please see the definition of wellbeing on page 7).

Carers’ information, support and counselling Carers’ information On the carers’ page of our website: www.somerset.gov.uk/carers you can find a list of information sheets that may help you. You can view them online, save or print them. They help to explain who a carer is and some of the things carers might have to deal with in their caring role, as well as giving you advice about what support is available and the services we can provide for carers in Somerset. Opportunities for carers There are many learning and training opportunities available to carers. These can be designed to help with your caring situation, for example, basic first aid, looking after yourself, and other opportunities that offer personal development.

Supporting someone with a mental health condition Carers’ Assessment Workers are based in community mental health teams. They specialise in assessing the needs of

carers who care for someone with mental health issues and can advise carers about: • inpatient services; • outpatient services; • day services; • residential care and short breaks; and • contacting social workers and community psychiatric nurses.

Carers’ champions A member of staff in most GP surgeries in Somerset has been appointed as a carers’ champion. They can register you as a carer on the Carers’ Register. This will alert a GP that you are a carer when you attend an appointment and may also help you to access preferential appointment times and carer health checks. The carers’ champion can provide you with information about where to go to find out more about carers’ services.

Carers’ information, support and counselling continued anything from a few hours to a few weeks depending on what is right for you and the person you care for.

The Care at Home Service may be able to provide you and the person you care for with support at home to help with personal care and essential tasks.

Short breaks can be arranged to help meet your needs as a carer as well as those of the person you care for as part of an agreed care and support plan.

The sitting service can help you take a short break with peace of mind. This service provides fully trained staff who can go into a carer’s home or take the person you care for out, and is not part of the Care at Home Service.

Residential short breaks make it possible for the person you care for to spend time away in a care home.

We also have a ‘Take a Break’ scheme. Following an assessment, instead of having your breaks arranged in advance, there is a scheme where you can contact us to arrange your break when you need it.

There may be a charge for some of the short break services. Your social care worker will be able to give you more information, or you can phone us on: 0300 123 2224.

Young carers We have Young Carers Co-ordinators who: • identify and support young people (18 years old and younger) with caring responsibilities in their family; • promote recognition and awareness of their needs and achievements; and • work alongside other agencies to develop schemes and services which support young carers.

They provide information, advice and support, including: • answering questions and giving advice; • carrying out an assessment of young carers’ needs; • speaking up for young carers so that ideas and needs are heard; • introducing young carers to one another; and • organising groups and activities for fun and having a break away from home.

Health and wellbeing In 2013, responsibility for health and wellbeing transferred from the NHS to us, as it is recognised that social care and health and wellbeing are closely linked.

Health is influenced by many factors which become more complex as we get older, and we recognise how important it is to make sure you can access support for emotional and mental wellbeing as well as physical health issues.

Things that can impact on health and wellbeing include: • spotting signs and symptoms of illnesses; • staying active; • keeping a healthy weight; • getting out and about; • keeping your mind stimulated; • staying warm;

• having an adequate level of income; • having a healthy diet; and • staying safe from falls.

Keeping active as we get older is really important, so the Zing website: www.zingsomerset.co.uk has lots of information about activities across Somerset, including activities for those who are less mobile.

Memory loss may affect people as they get older, however if you are concerned about your memory, there is lots of information and support available. To start with, visit your GP to discuss your concerns. If you are diagnosed with dementia, the Dementia Somerset website: www.dementiasomerset.org.uk provides useful information and lists support for carers and various services available.

Staying safe from falls Staying steady on your feet becomes more important as you get older and we have already talked about the benefits of remaining physically active, however being active also reduces your risk of falls.

As part of ageing you may develop health conditions that can lead to falls. Muscles can become weaker and

eyesight and hearing may decline; people can also lose confidence, which affects how active we are. We may suffer from chronic illnesses and have to take medication which can all contribute to the risk of falling. Also our bones become weaker and we may develop osteoporosis (where our bones become more fragile). To keep bones strong

Staying safe from falls continued include lots of calcium and vitamin D in your diet and aim to have 15 minutes of sun a day on your skin during the summer months, without sunscreen and taking care not to burn, as sunlight is the main source of vitamin D. If you are concerned talk to your GP, you may benefit from medication to make your bones stronger.

Other factors, such as wearing badly fitting or worn shoes, foot problems and things in the home such as loose rugs, trailing wires and other trip hazards can all make falls more likely.

If you are worried about your balance or have had a fall contact us on 0300 123 2224 to be referred to an Independent Living Team or speak to your GP. They can identify the reasons why you fall and help address the issues, this might include a falls assessment and recommendation to attend a Balance and Safety class at your local Community Hospital. If you are unable to get to your local Community Hospital, a physiotherapist may be able to set up an activity plan with you at home. Also consider a community alarm (Lifeline), see page 17.

If you do fall and break a bone, you may find that you won’t be able to cope with living in your own home. So it makes sense that you reduce your risk now.

Independent Living Teams are in place across the county and can support you to stay in your own home and help you to remain independent as we know how important this is.

The Silver Line The Silver Line is the only free confidential helpline providing information, friendship and advice to older people, open 24 hours a day, every day of the year.

Launched by Dame Esther Rantzen, The Silver Line aims to combat loneliness in the over-65s by providing friendship, information and advice through calls to trained volunteers.

Specially trained helpline staff can: • offer information, friendship and advice;

• link callers to local groups and services; • offer regular befriending calls; and • protect and support those who are suffering abuse and neglect.

Support in the community The following section lists community support that you can contact directly to talk about what you need and make arrangements for them to help you.

There are lots of things going on in your local community that could help you to remain living independently in your own home. These include: • various groups that meet to provide companionship; they may even arrange for someone to collect you to take you to group meetings; • pubs that provide meals, usually at great value;

• organisations that can help you with some domestic tasks, such as cleaning, shopping, gardening, or simple repairs to your property.

• befriending schemes, where volunteers can come and visit you at home or accompany you if you would like to go out; and

These are simple but important things that can help you feel safe and give you peace of mind.

The ‘Somerset You Can Do’ Service The ‘Somerset You Can Do’ Service is funded by us.

If you would like to find out about what community support is available in your area, you can contact the free information service, which can: • put you in touch with organisations and groups in your community that are there to help you; • provide you with information on local community organisations you might like to join; • help you to get the information you need if you have sensory loss or other disabilities; • help you with your skills, development and learning; • put you in touch with Village Agents, who are recruited by the Community Council for Somerset to provide people in Somerset’s rural communities with easier access to information services;

• give you information about equipment you could hire or buy to help you live independently at home.

As well as the Information Service, the ‘Somerset You Can Do’ also provides a volunteering service that supports and recruits people who want to volunteer to support others. There is also a community development service.

This supports groups and organisations to become established and helps to improve the community support they offer by making it more accessible.

There is a thriving Active Living network with over 120 community groups, clubs and organisations across Somerset. They offer the opportunity for people to come together, enjoy the company of others and develop new friendships and social circles.

Each centre is unique and offers a varied programme

of activities to suit most needs, such as light exercise, developing and learning new skills and knowledge, access to advice, information on looking after your health and wellbeing whilst having fun at the same time.

Home Improvement Agencies Aster Care and Repair Somerset, part of Aster Living, is the preferred provider of Home Improvement Agency services for Somerset. They are funded by us, the District Councils and NHS Somerset to help you arrange a major adaptation to your home if you need it.

They can provide technical services, such as drawing plans and getting quotations for work. They work with occupational therapists in Adult Social Services and the district councils to make sure the work progresses smoothly. They keep lists of reliable contractors, who can provide a quality service and make sure that everything is as it should be when the work is finished. They can help you to complete the works privately if you are not eligible for funding under the Disabled Facilities Grant (DFG) means test.

They also provide a subsidised handyperson services called ‘Handihelp’ which can help you with small repairs, maintenance and minor adaptations (not funded through Adult Social Services). A charge is made for this service which is subsidised by the local authority and, therefore, keeps costs reasonable. They will provide a quotation if you ask them. They also carry out free home safety checks when they visit you.

The ‘Handihelp’ service is also available as a home from hospital discharge service if you need minor repairs and adaptations to your home to make sure that your move back is safe. Some parts of this service may be provided at no cost to you.

Leaving hospital If you, your carer, family or hospital staff feel that you will need extra support when you leave hospital, you can ask us to carry out an assessment while you are in hospital or before you go into hospital if you are waiting for an operation. See page 42 for an explanation of what an assessment is.

A social care worker will agree a discharge or care plan with you, which tells you what help and support will be

arranged for you when you return home. This makes sure that you can leave hospital safely, as soon as you no longer need hospital care. Someone from the local social care team will contact you after you have returned home to check that the care plan is still right for you. The help may be increased or reduced depending on how you are.

The aim is to provide the right short-term help to allow you to recover your independence.

Red Cross Home from Hospital service The Home from Hospital Service helps many people each year to go back home after a stay in hospital. The service can give short-term practical and emotional support to help you get your confidence back and live independently.

Help can be provided by: • taking you home and helping you to settle in; • shopping for you; • collecting prescriptions and pensions; and • sitting with you if your carer has to go out.

Trained volunteers from the British Red Cross provide this free short-term service.

Aster home from hospital service Aster also offers a home from hospital service. If possible someone will visit you in hospital before you go home.

But, they can come to see you at your home if you prefer or if there isn’t enough time before you go home. They’ll talk to you and find out what your needs are and agree with you how they can help. They can do lots of things to help you. Here are a few examples; they can: • arrange for basic food supplies ready for your return home, or help you get a food parcel if you’re short of money; • provide a visiting service to reassure you;

• give advice or carry out checks to see if you are receiving all the benefits or grants you might be entitled to, and they’ll help you fill in the forms if needed; • carry out home safety checks; • give advice and help to find things like care, cleaning, gardening, shopping or dog walking; and • provide lots of information about local groups or organisations that offer help in your area.

Day time care and support Day time care and support can be a made up of a variety of activities, designed to help people who live in their own home but who need help to get out and about.

Activities are often run in the community, from a local residential or nursing care home, to an activity group in a local hall or housing scheme. Care and support is available in a way that encourages and enables people to remain as independent as possible by helping them to maintain

and improve their wellbeing.

There are many day time activities that can also allow carers to have a break.

Check with your local care homes and care agencies to see if they run this service, or, if you qualify, your social care worker can book the service for you, or you can book it yourself if you have a Direct Payment (explained on page 45). This service does not include an overnight stay.

Meals There are organisations in Somerset that, for a fee, offer a variety of different meals. Often this is by providing you with a frozen meal that you can warm up and eat when you choose.

Somerset Community Meals service Somerset Community Meals service is for older people and people with disabilities who we agree need help to make sure they have regular cooked meals. This is provided at a discounted rate from a menu by Wiltshire Farm Foods.

There are also some more expensive meals that you can buy if you want to.

The Meals at Home service delivers frozen meals to you every two weeks. The frozen meals must be kept in a freezer and can be reheated in a microwave, oven or special steamer. This means that you can choose what you eat and when you eat it and can have a cooked meal every day.

Your dietary, religious or ethnic needs can be catered for.

If you need to borrow any of this equipment from Wiltshire Farm Foods there is no charge for this, as long as you place the minimum order of ten meals every two weeks.

If you need help to get your meal ready or to eat it, this can be arranged after an assessment of your needs. You may have to pay for this; how much will depend on your circumstances.

You can still have frozen meals without being assessed by us. Wiltshire Farm Foods is a private company that provides a service to many parts of the country. You can buy meals from them privately but this won’t be at a discounted rate.

There are many other products available to support more vulnerable people to live as independently as possible in the community for longer. The aim is to give people more choice and control over how they live their lives and the risks they are able to take. These products are often known as ‘Telecare’.

Telecare can prompt, remind and inform people and their carers about things in daily life which need to be done, such as taking their medication on time. They can alert a family member or carer that a person has got out of bed at night and needs assistance. Or they may alert a monitoring centre that something has happened and appropriate action should be taken.

Community Alarm Services In Somerset, Community Alarm services are available through district councils. See page 63 for contact details. They can provide a device that sits between the phone and its socket point, and an alert button that you wear. By pressing the button, an alert is directed to a call centre that will respond instantly. They can speak to you even if you are not near the phone, but this will only happen when the button is pressed.

Blue Badges A Blue Badge helps people with severe walking difficulties who travel as drivers or passengers, to park close to their destination in on-street parking and some car parks. The scheme is also for people who are severely sight impaired.

Care and support at home Home care Before you decide you need home care you should consider and seek advice about equipment and adaptations. This may reduce, or even eliminate your need for home care.

Home care services provide someone to help people who are struggling to cope in their own homes with daily activities, such as, cleaning and shopping and personal care, including, getting in and out of bed, washing, dressing and using the toilet. For people with additional needs, specially trained staff are available. With the right help many people are able to recover their independence after a crisis.

If you are frail, have been ill, or have just come out of hospital, the short-term support of a home care worker could be welcome. Also known as domiciliary care or care at home, it is available to anyone, but is particularly aimed at older people or people of any age with a physical or learning disability, or a mental health condition. Agencies can also help parents and carers of children with disabilities and provide support to your own carer if you have one.

Care at home can be provided by private or voluntary agencies or individuals. If you choose agency care, it will be delivered by a small team of care workers. They must be specially trained in areas such as manual handling, first aid and food hygiene. Care workers will also have had background checks with the Disclosure and Barring Service (which used to be called the Criminal Records Bureau (CRB)).

Not all agencies provide the same services and charges vary. Some agencies offer a night-sitting or even a live-in service and some are also registered as nurses’ agencies, meaning they can provide registered nurses, if needed.

All home care organisations that provide personal care must register with, and be inspected by, the Care Quality Commission (CQC), which makes sure that quality standards are met by the service and that staff are appropriately trained to do the job. You can ask the CQC for their last inspection report regarding any agency you are considering. For more information on CQC see page 37.

What if I move house?

If you have a personal budget from us and plan to move home, please contact us so we can make sure that your care arrangements continue when you move. This could be for you and your carer if you have one.

If you are moving to another local authority area, for example from Somerset to another county, with your permission, we will let your new social services offices know.

They may wish to contact you to talk about your care

needs before you move. We will provide them with your care and financial assessment information, personal budget allocation, and care and support plan so that they can make sure the same level of service is ready for you when you arrive in your new home.

They will probably contact you soon after you arrive to make sure you are happy with the arrangements and to see if anything needs to be changed.

Housing with support, or care and support Sheltered and retirement housing Sheltered and retirement housing is suitable for people who can live independently but provides the peace of mind that there are some on-site support services available that can be called on to provide low level support for short or longer term needs.

There are many different types of sheltered schemes and retirement housing, available both to rent and to buy. They are provided by local councils and housing associations. Schemes usually consist of between 15 and 40 dwellings. These may be bed-sitting rooms, selfcontained flats, bungalows or luxury apartments. The complex is often built around a garden or communal facility and is linked to a central control centre for security.

Some schemes are simply housing for older people and linked to a community alarm. Most schemes have a scheme manager or support co-ordinator/worker and

a community alarm service. There are often communal facilities, such as a lounge, laundry, guest flat and garden.

Meals are not normally provided, but a few schemes include a restaurant and some can arrange hot meals.

Private developers also build retirement housing for older people, who are usually over 55 years, to buy or to rent, or for shared ownership. They set their own entry criteria for people with care and support needs.

Retirement housing is aimed at people with very minimal support needs, while sheltered housing is for people with higher level support needs, for example, who can live reasonably independently but may need some support with daily tasks. Often, people who move into sheltered housing, through the enabling type of service, find they can do more easily the things they were struggling to do before.

Extra Care housing Extra Care housing is a form of housing, similar to sheltered schemes, with self-contained accommodation together with some communal facilities.

In an Extra Care scheme, a higher level of care and support package is available on site, so is suitable for people who have care and support needs.

Extra Care allows you to live in your own self-contained accommodation and have regular care and support needs

met by a designated team of care and support workers including a night time on-site sleep-in member of staff who can be available in an emergency.

The complex is often built around a garden or communal facility and is also linked to a central control centre for security and emergencies. There are also communal facilities, such as a lounge, craft and day rooms, laundry, garden, guest rooms and meals are available at most schemes at lunch time.

Housing-related support is now called ‘Pathways’ We commission ‘integrated support’ using two Pathways.

1. The Somerset Pathways to Independence (P2I) is a multiagency, integrated housing-related support service for young people aged 16 to 25. It includes care leavers who live in or have a local connection with, the Somerset area.

The service allows young people with housing-related needs to progress along a pathway of personal goals until they can live independently. But first and foremost, it will try to prevent the need for a young person to step on to the pathway at all, through targeted prevention measures.

The P2I Services will prevent youth homelessness and enable the young people who are accommodated to progress from support and supervision to independent living by providing:

• targeted prevention and homelessness awareness through education, information and advice; • floating support that includes mediation and reconciliation; • emergency accommodation; • supported accommodation; • move on accommodation; and • resettlement support.

2. The Somerset Pathway for Adults (P4A) provides an integrated range of prevention, accommodation and resettlement services for vulnerable people who are considered to be ‘socially excluded’. These include people with a criminal background, or who have been identified as having poor mental health, or are sleeping rough,

Housing-related support is now called ‘Pathways’ continued single and homeless and who need accommodation or support to prevent homelessness so that they can be independent long-term.

There is evidence that many people can be thought of as temporarily socially excluded at different times in their lives.

Most people can deal with these situations through their own resilience and resources.

The overall aim of the P4A is to provide vulnerable people with a targeted support service. The service is tailored to the person and focuses on them getting and

keeping somewhere appropriate to live. It also helps them integrate into the community with support to identify sustainable income routes, reducing a reliance on benefits, with access to education, training and work related activities, including volunteering. It is a priority for these services to support people who do not have the resilience or resources, either through themselves, or their family or community, to overcome problems of exclusion and enable them to find stability.

Care homes Types of homes Sometimes people decide to move to a care home when they are in a crisis and cannot work out how to get the help they need to stay at home. Often people who are helped through the crisis can keep living at home with the right support.

Before considering moving into a care home, we recommend you talk to us first to make sure that all other options have been exhausted that could help you to remain living independently at home, or in other suitable accommodation. If this is no longer possible, moving to a good quality care home may be the best option. These are the types of care homes: Residential care homes provide personal care, such as washing, dressing, bathing and assistance with toilet needs, if required. They do not offer nursing care. In some homes some staff may only be ‘on call’ and not ‘on duty’ at night. Homes with fifteen residents or more must have at least one member of staff awake ‘on duty’ at night. Some provide specialist care for people with mental health difficulties or physical disabilities. If you have a health problem that needs a qualified nurse, this care will be provided by the District Nursing Service on the same basis as for people living in their own homes.

Care homes with nursing provide nursing care 24-hours a day. Usually people living in these homes will be very mentally and/or physically frail or immobile, and have health care needs that can only be met by a registered nurse. Some offer

general nursing care for older people, while others specialise in mental health or physical disability. Care homes with nursing are often dual registered to provide both ‘residential personal care’ and ‘nursing’ care. If you choose one of these homes you won’t need to move if your condition changes.

Also, this type of home helps couples who may have different needs to live together in the same home.

Care homes that specialise in supporting people who have severe physical disabilities, learning difficulties or autism.

These care homes do not normally provide nursing care.

Specialised residential care homes are residential homes, only available in Somerset, that specialise in assisting people with a high level of dementia but who do not have nursing needs.

Somerset Partnership NHS and Social Care Trust employ Specialist Care Development Nurses (SCDNs) who support residents in these specialised care homes and provide advice and support to the care staff. They are involved in discussing the needs of possible new residents to make sure that the home is suitable for them, and also that the overall dependency levels in the home can be managed by the staff who work there. They will agree whether the home is suitable for all new residents.

The Care Quality Commission (CQC) registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others.

Inspectors carry out a mixture of announced and unannounced inspections looking at the quality and safety of the care provided. They look at whether the service is: Safe; Effective; Caring; Responsive to people’s needs; and Well-led.

Inspection teams are led by an experienced CQC manager and often include experts in their field. The team may also include professional or clinical staff; Experts by Experience; people who use services and carers.

Following an inspection, each care home and home

care agency is given a report of how it rates against national essential standards of quality and safety. CQC also rates the services as ‘Outstanding’, ‘Good’, ‘Requires Improvement’ and ‘Inadequate’. By March 2016, CQC expects to have rated every adult social care service in England.

Care homes contracts – independent advice It is advisable to consult a solicitor before signing any contract, to make sure that the terms are fair and you understand your rights and obligations.

You may also wish to take independent financial advice

from an organisation like Age UK or a Citizens Advice Bureau.

Choosing a care home To decide which type of home would be right for you, you should ask for a needs assessment. See page 42 for an explanation of what an assessment is. A social worker will carry out the assessment. Other professionals, such as an occupational therapist, a district nurse or your doctor will also be involved, with your permission. This will help make sure you choose the type of home that would best meet your needs.

If you are moving to a care home with financial help from us you have the right to choose a home anywhere in England, Scotland or Wales, as long as we agree that it meets the following requirements: • The home is suitable for your assessed needs. This means we must be sure that the home can give you the help your assessment shows you need. For example, a residential care home will not be suitable if you require regular nursing care. If we consider a home to be unsuitable we will tell you why, and ask you to choose another.

• There is a place available. We may be able to arrange for you to move to another home while you wait for a place to become available in the home of your choice.

• The home owner agrees to our usual contract conditions. If they do not, you will be asked to choose again.

• The home does not cost more than we usually pay for the type of care that you need. If you wish to move to a care home that charges more than this someone will have to make up the difference – this is called a ‘top-up’ or ‘third party’ payment. See page 48 for more details.

At your assessment we can tell you about homes in Somerset that provide the type of care that you need, and which ones accept the amount that we usually pay.

Comprehensive lists of care homes and care homes with nursing begin on page 64 of this Directory. In most cases we will leave it to you, or someone else you have asked to help you, to visit the homes you are interested in and decide which one you like best. Your social worker can help to arrange visits to homes if you do not have anyone to take you.

If you do not want to choose, and you have no one else who you would like to do it, you can ask your social worker to arrange a place in a care home for you.

Things you need to think about Every home should produce a brochure or service users’ guide to tell people about the service it provides and the facilities offered to its residents.

Choosing the right care home is important. If you are able to do so, visit more than one home before you make up your mind. You may be able to spend the day, have a meal and talk to people who live in the home and meet some of the staff. This will help you make up your mind.

You may be able to have a trial stay before you finally decide.

You should ask to see the Care Quality Commission report before you visit and think about the things that are important to you and what questions you want to ask.

How can we help you?

Independent Living Teams (Reablement service) Somerset County Council and the NHS have ‘integrated’ some of their staff into single teams, so that they can share their different skills and provide a co-ordinated approach, to help you regain your independence.

A team includes: a physiotherapist, a social care worker, an occupational therapist, care workers and support workers.

Other support, for example from a district nurse, carers support or support from staff skilled in mental health issues, can also be arranged if you need it.

You may be recovering from an illness or surgery, or just need help to do some of the things you used to be able to do.

Your GP or hospital doctor may refer you to this service if they think you require the support the team can offer, or you can

contact them directly by phoning us on: 0300 123 2224.

The very experienced staff will talk with you and your carer, if you have one, to understand you and what you need, and then help to design and deliver a support plan (see ‘Understanding you – assessments’ in the next section).

This may be from simply explaining how you can get further support, for example housing advice, through to providing you with equipment, physiotherapy support or care if they feel it is needed. This is a short-term service, with the aim that you will regain your independence in that time. Some people may need some care and support for longer, the team will talk to you about this and help you to make the arrangements you need.

Understanding you – assessments If you contact us, and we cannot provide the information you need over the phone, we will ask someone to visit you and talk to you, and the people who matter to you, so that we can help you decide what you want to do and how we can help. This process is called a ‘needs assessment’ and is something everyone is entitled to.

We assess the need for adults of working age and older people who have disabilities, mental health problems, a sensory loss or general frailty, and can provide support to people who are most in need. The aim is to actively promote independence and choice and help people to be self-reliant and live in their own homes for as long as possible. When this is no longer possible we help people to choose good quality supported housing or care homes.

Anyone can ask for a needs assessment, regardless of their financial situation. The assessment and advice is free.

Some people may have to make a financial contribution towards any longer-term care and support; for people who can afford to pay this may be the full cost charged by your chosen provider.

How to get an assessment You, or a friend, relative or a health professional like your GP, can ask for an assessment by phoning us on: 0300 123 2224. A social care worker may come to see you or the assessment may be carried out by phone first and then followed up by a visit up to four weeks later, depending how urgent your situation is. If you are in hospital, the nursing staff can arrange for a social worker to contact you, either during your stay in hospital, or on your return home.

A needs assessment is a discussion to make sure that we understand you, and are clear about what you need. It includes finding out: • what difficulties you have; • what you can do for yourself; • what things you cannot manage at all; • what things you can’t manage now, but with some support may be able to do in the future; • who helps you now; • what you think you need or would like to achieve; and • the best way of meeting your needs.

With your permission we will also talk to people closely involved with you, for example, your carer, family and friends. Other professionals, such as an occupational therapist, physiotherapist or district nurse may be included as part of your assessment. This will make sure you are given the right information to make choices

about your future.

At the end of your assessment you will help to write your support plan. This will list all the things you need and who will help you to do them. We may provide some of this help, if you are eligible (see page 43).

If you are a carer (you provide regular and substantial care to a relative or friend, and are not paid for it), you are entitled to an assessment, even if the person you look after doesn’t have help or funding from us. Services that support you as a carer may be included in the care and support plan of the person you care for or separately in your own support plan. For more information on carers’ assessments, see page 9.

All the information you provide will be kept securely.

We will share relevant information with professionals like a doctor or nurse unless you ask us not to.

Throughout your assessment you can always have your carer, a relative, an advocate (see page 6) or a friend present to support you.

The assessment will take account of your religious beliefs and cultural practices.

What happens after the assessment?

There are many different ways you can receive help and support. We will discuss the best way of doing this with you. If you are eligible for our help towards paying for your care and support (see page 43) you may want us: • to buy services from other organisations for you; or • to give you (or someone on your behalf ) money so that you can arrange the help for yourself - this is called a ‘Direct Payment’ (see page 45).

If we pay for all or some of your care, we will regularly review the help you get. If your eligible needs have increased, we will consider additional help. Equally, we will consider changing, reducing or stopping help from us if you no longer need it or no longer meet the eligibility criteria.

Occupational therapy assessment If your assessment shows that you need specialist equipment, for example a hoist and sling, or minor works such as stair rails, then, following an occupational therapy assessment, the team may provide the equipment free of charge. Having the right equipment can often mean you don’t need further care and support or it can reduce the amount of support you need.

The occupational therapist will also be able to give you some advice and information about the various housing options available to you. This may include moving to alternative housing or you may need major alterations to your current home, such as putting in a stairlift or wet-floor shower. If you need major alterations, the OT team will ask your district council to consider you for a Disabled Facilities Grant. The district council will ask you for information about your finances and you may have to

pay towards the cost of the work. Adaptations that can be considered for a Disabled Facilities Grant include: • access - widening doorways for wheelchairs; • bathroom adaptations; • fixed overhead hoists; • lifts – stairlift or through-floor lifts; and • general alterations including permanent ramps.

If you are not eligible for a grant, the occupational therapist will give you information and advice so that you can make you own arrangements to adapt your home.

It is important that you do not start any work before you have received grant approval from your district council - you will not normally be entitled to any help for work already undertaken.

Telehealth These are devices that can support people with particular long-term conditions, such as chronic obstructive pulmonary disease, heart failure or diabetes. They provide them with the ability to collect vital measurements including weight, blood pressure and heart function. This reduces the need for outpatient appointments and alerts

nurses and doctors to manage your health needs.

These devices can be used in people’s homes or in GP surgeries. They rely on a control box collecting information about the person’s condition from sensors, such as blood pressure cuffs or scales. They are only provided following a health care assessment of need.

Coping with a sensory loss We have lots of information if you, or someone you know, has a sensory loss. You can find this on our website: www.somerset.gov.uk (search for ‘hearing and sight loss’).

We also have specialist sensory loss workers who can talk with you and give you information and advice. Please phone us on: 0300 123 2224.

Paying for care Will we contribute towards your care costs?

Following your care assessment, we use national rules to work out if you are eligible for us to help pay for any care and support that you need. There are two sets of rules: Care eligibility and financial eligibility.

Care eligibility The criteria are based on identifying how your needs affect your wellbeing. If you meet all the following criteria you are eligible:

• managing toilet needs;

1. Your care needs are due to a physical impairment or mental impairment or illness.

• developing and maintaining family or other personal relationships;

• managing and maintaining nutrition; • maintaining personal hygiene; • being appropriately clothed; • being able to make use of your home safely; • maintaining a habitable environment;

Will we contribute towards your care costs? continued • accessing and engaging in work, training, education or volunteering; • making use of necessary facilities or services in the local community including public transport and recreational facilities or services; and • carrying out any caring responsibilities an adult has for a child.

3. As a result of not being able to achieve these things, it has a significant impact on your wellbeing (please see page 7 for a definition of wellbeing).

Financial eligibility Please note financial eligibility criteria will change in April 2016. The following information is accurate until then.

If you are eligible following a care and support assessment, we then need to complete a financial

assessment. If you have more than £23,250 in savings and income you are not eligible. You will usually be asked to make your own care arrangements and pay the full cost of your care. Or you can ask us to make the arrangements for you. We will charge you an arrangement fee for the time it takes us to complete this for you.

If we can help you with the cost of care, you may be asked to make a contribution - see below. There may be things you want to do that we can’t pay for.

If you have care and support at home and are eligible for financial support, we will work out how much it will cost and allocate this money to you. This is your personal budget. You will then have the opportunity to have this money as a Direct Payment (minus your contribution) to spend on meeting agreed objectives, or you can ask us to manage your money for you (see page 45).

If you live in a care home please see page 46.

If you don’t agree with the decision, you can ask for it to be looked at again.

Whether we contribute to the costs of your care or not, we will always provide help and advice or direct you to someone else who can help.

Working out your contribution The Financial Assessment and Benefits Team The person who assesses your care and support needs may ask for someone from the Financial Assessment and Benefits (FAB) team to arrange to phone or visit you.

This person will talk to you in confidence about your income and capital. They will complete a benefits check to make sure that you are receiving all the benefits that you are entitled to. They also help people by completing benefit claim forms, if they would like them to, and follow up claims to check that payments have been made.

If you have any money, care or support provided by us, they will work out how much, if anything, you should contribute towards this.

This service is available to anyone who is receiving support through us.

Will you have to make a financial contribution towards care at home?

If you have over £23,250 in savings and income or choose not to give details of your finances, you will need to buy your care privately. We can help you to do this if you need us to.

If you have under £23,250 in savings, a member of the Financial Assessment and Benefit (FAB) team will visit you to work out how much you can afford to contribute. They will look at your savings, income and expenditure relating to any illness or disability you may have. You may not have to pay anything.

Attendance Allowance This is a non-means-tested, non-taxable benefit from the Department for Work and Pensions (DWP) paid at the lower rate of £55.10 each week for people who need care by day or night, and at the higher rate of £82.30 each

week for people who need care both during the day and night. Everyone who needs care should claim this benefit.

However, if we are helping to pay the cost of your care, this will affect your claim.

Personal budgets Your personal support plan When we visit you we will talk about the things you would like to achieve to help you to be as independent as possible. These are written into your care and support plan.

The plan lists the things you will arrange yourself, or ask someone to do for you. It also lists any money, services or equipment we have agreed to provide for you.

Option one We can give you a contribution to your personal budget to manage yourself, so that you can be flexible to make your own support arrangements; but it must be used to achieve the things written in your care and support plan.

If you choose this option, the money will be given to you as a Direct Payment.

Your personal budget A personal budget is a weekly amount of money allocated to you to meet your assessed needs and objectives written in your care and support plan. We currently have two ways that your personal budget can be managed:

Option two You can ask us to hold your personal budget for you and we will organise your support for you. This is called a Local Authority Managed Account.

You may also decide to have a mixture of both options.

How is my personal budget calculated?

When we visit you to talk about your care and support needs we will also be able to roughly work out how much any care and support will cost. This is called your ‘indicative budget’. Knowing this will help you to start planning your care and support plan.

We then work out how much you should contribute (see page 44).

When your care and support plan has been completed, we deduct what you have been assessed to pay to work out our contribution.

Our contribution, added to your assessed contribution, is your final personal budget.

Direct Payments Direct Payments are money paid to people by the Council who have assessed care and support needs so that they can choose, arrange and pay for their own care and support. Direct Payments make it possible to have more choice and control over the type of help and support needed, rather than using our contracted services.

If you qualify for help, the social care worker who completes your needs assessment will explain Direct Payments and how they work and give you information to read. You can then take time to decide what you want you to do. You must want to use the scheme and be able to manage your care and support arrangements, and keep information about how the money is used. You can ask someone else to help you manage your Direct Payment for you, such as a friend, family member or volunteer. You may be thinking of employing someone to help you, this means you will take on the responsibility of an employer so you need to know and understand the rules that employers must follow. We have organisations that can help you with this (see below).

If you use the Direct Payments Scheme you will have money paid into a separate bank account that is only used for your Direct Payment. The money can be used to help you with what you want to achieve. For example, this could be personal care, support to use daytime activities or respite care. You can buy help from an agency or employ your own carers.

You cannot use your Direct Payment • to buy equipment; or • to pay for food, gas, electricity or other utilities.

You can receive a Direct Payment as well as benefits. Your Direct Payment will not affect the benefits you receive. It is not the same as Direct Payments from the Department for Work and Pensions.

The Direct Payment Advisory Scheme Independent advisory services are available to support people who have Direct Payments. Everyone new to the

Direct Payments continued scheme should meet, or talk, with one of their experienced advisers. They can:

• be at the end of a phone if you need advice.

• visit you at home to explain the help they can provide;

When you join the scheme, an independent adviser will be able to help you manage the paperwork for a short time until you are confident with it. They will support you through the whole process, from deciding whether or not to use Direct Payments to setting up everything to get going. The adviser is there to support you once your arrangements are set up and if you have any problems or concerns about employing your staff.

• discuss your responsibilities as a Direct Payment user; • help you set up a Direct Payment bank account; • help you recruit and employ staff; • give you health and safety advice; • help you set up a system for dealing with tax and National Insurance for someone you employ; • offer a payroll service, if needed; • help and explain how to keep financial records to show what you have spent; and

Paying for care homes Will you have to pay the costs of a care home?

If you have capital or savings of more than £23,250 you will have to pay for any care services you require, but you can still ask us for a needs assessment to make sure the care home is suitable. We will still provide help and advice.

If you have savings of less than £23,250 you may be entitled to funding from us (see adjacent).

If you are moving into a care home, you may be entitled to some of the following financial assistance and support.

If you ask for a Deferred Payment at a later date it will start from the date we receive your application and will not be backdated.

There is a standard fee of £500, to cover our administration and legal costs of setting up the Deferred Payment.

It is very important that you decide whether or not you wish to ask for a Deferred Payment before the end of the twelve-week period. We ask that you let us know by the end of the fourth week so that we have time to set up your

Twelve-week property disregard If your former home is included in your financial assessment but your other capital is less than £23,250 and your income is not enough to meet your care home fees, we will help with the cost during the first twelve weeks of permanent care, providing a social worker agrees that care is needed.

Deferred Payments After the twelve-week period, you can continue to receive financial help from us by asking for a Deferred Payment.

What is a Deferred Payment?

If your property is taken into account you may enter into a Deferred Payment agreement with us. This means we will continue to pay the part of your care fees you cannot afford after the twelve-week period ends, as a loan, to be repaid later. This allows anyone who doesn’t want to sell their property and wishes to explore others ways to pay their fees, or who can’t sell it quickly enough, to pay their care fees. The Financial Assessment and Benefits officer will discuss this with you.

Paying for care homes continued Deferred Payment. If we don’t hear from you we will stop paying towards your care after this. You will become selffunded and will have to pay the full cost of your care direct to the home.

If you apply for a Deferred Payment you, or your representative, will need to sign our Deferred Payment agreement. We will place a legal charge on your property and will charge interest on the loan.

You do not have to sell your home if you don’t want to. You may, for example, decide to use rental income to increase the amount that you pay each week. This will reduce the weekly payments made by us and the eventual Deferred Payments debt.

Running out of money If your capital is likely to reduce to £23,250, you must let us know well in advance, as we will step in to help with

your care fees. We suggest that you let us know when your capital drops to about £30,000 to give plenty of time to put arrangements in place. We will complete a care assessment and will normally make a contribution – this is described on page 43. If the home you have chosen charges more than this, you must find someone to help pay the difference – this is called a ‘top-up’ payment. Whoever does this, whether it is your family or a charity, they should realise that they may have to pay this for some time. If there is no one who can pay your top-up you could talk to the home owner to see if they will reduce your current cost, or you may have to find a cheaper home.

It is important that you understand your rights and obligations before moving into a care home. There are a number of financial products and specialist companies who may be able to help. It is important to seek advice before making a decision you cannot change.

NHS Nursing Care Contribution If you need a nursing home, a specially trained nurse will assess your nursing needs and the NHS will then pay the cost of the nursing care part of your fees to the home.

Currently this is a flat rate of £112.00 per week. The NHS does not pay for personal care.

This is not means-tested and is paid directly to the home.

NHS Continuing Care Funding If you live in a care home with nursing (usually known as a ‘nursing home’) and have a high level of nursing care needs, you may qualify for the full cost of your care home fees, including board and accommodation, under the National Framework for NHS Continuing Healthcare.

If you live in a residential home and need help from a registered nurse, this will be free, covered by the NHS again.

Do you have more than £23,250 in capital and savings?

The value of your home is included when assessing capital unless: • your partner; or • a relative who is over 60 or disabled; or • a child under 16 who you or a former partner maintain; still lives there.

No Your local authority will contribute financially towards your care costs if you cannot afford them.

How much?

To work out how much we will pay, you must first ask for a care needs assessment. This will make sure the homes you consider are suitable. To do this, phone us on: 0300 123 2224.

Third party payments If the care home you eventually choose costs more than the rate we usually pay for a person with your needs, someone will have to make up the difference. This extra payment is often referred to as a ‘top-up’ or ‘third party top-up’.

Except in limited circumstances, the law states that you are not allowed to make this additional payment yourself.

The responsibility for this often falls to a member of your family or a benevolent sponsor such as a charity. Once this person or organisation has been confirmed, they must sign an agreement formalising the arrangement.

Whoever agrees to do this for you, it is important they are made aware that the amount could increase in the future and they must be confident that any increases will be met.

If payments stop for any reason, you should seek help and advice immediately.

If a top-up stops, we may ask you to move to an alternative home that accepts our fee level.

Support for people who lack capacity The Mental Capacity Act requires us to assume that people have capacity and can make decisions themselves, unless otherwise established. A person will be given all practicable help to make specific decisions before being assessed as lacking capacity to make their own decisions. This includes what may be considered as an unwise or unusual decision, reflecting that everyone has their own values and beliefs and preferences which may not be the same as those of other people.

If we think a person may lack capacity to make a decision even after being offered practical support then a social worker or other suitably qualified person will carry out a capacity assessment in relation to the specific decision to be made.

Where it has been assessed that a person lacks capacity for a particular decision, decisions will be made in their best interest. Any restrictions as a result of this decision will be in the person’s best interest and will be proportionate to the likelihood of the person suffering harm as a result of the decision.

Planning will always continue to involve the person as far as possible, taking account of their wishes, feelings, values and aspirations as well as their needs and wellbeing.

They may be supported and represented by family and friends. If this is not possible an independent advocate will be appointed. The advocate will represent the person, speak for them and challenge the local authority’s decision if necessary.

Financial assessments and charging We will still charge people who have eligible care needs but lack capacity. We will make sure they are fully represented and decisions are in their best interest.

We will find out if there is an appropriate person to represent them. This could be through:

If there is absolutely no one else who can act for the person, we will make an application to the Court of Protection and assume the role as their deputy. The local authority will apply to be the person’s Corporate Appointee in respect of their welfare benefits with the Department of Work and Pensions. Until there is an appropriate person appointed and full access to the person’s financial affairs can be obtained no financial assessment will take place.

If there is no access to the person’s finances to make care and support payments we can make these payments as a loan until an appointee/deputy is in place and a full financial assessment can be completed.

The following will apply: • if capital is over £23,250, we will expect the full fee to be repaid; • if capital is less than £23,250 we will expect the assessed contribution to be repaid; • if benefits became available during the Corporate Appointee-ship application period, we will expect them to be repaid, backdated to when they became available; • If there is a top-up there must be a relative willing to pay.

Do not assume that the resident will have enough money to pay a top-up.

Specialist care Dementia care The term ‘dementia’ is used to describe a range of progressive organic brain diseases. It describes a collection of symptoms, including a decline in memory, reasoning and communication skills, mood changes and a gradual loss of the skills needed to carry out daily activities.

Alzheimer’s disease is the best-known form of dementia and accounts for 62 per cent of dementias in England.

However, there are over 100 types in total.

Dementia can affect anyone irrespective of gender,

ethnicity or social class. Although dementia is commonly seen as a condition of the elderly it also affects people under 65, and people with learning disabilities are a group at particular risk. The prevalence of early onset dementia is higher in men, with late onset higher in women. It is estimated that there are about 700,000 people in the UK with dementia. In Somerset, there are estimated to be 8,125 people aged 65 and over and 153 people under 65 with dementia. These figures are projected to increase to 13,088 people aged 65 and over and 175 people aged under 65 by 2025.

Dementia Services in Somerset These include services to support people with early onset dementia, and care homes that offer a type of care for people with dementia that is unique to Somerset called ‘Specialised Residential Care’. Somerset also has ‘Dementia Advisors’, who can provide information to people throughout their lives with dementia.

Mental health When someone is worried about their mental health they will usually visit their GP, who may refer them to Somerset Community RightSteps, or if they require a specialist mental health service, Somerset Partnership NHS Foundation Trust.

Somerset Community RightSteps is a free and confidential service to help people in Somerset access appropriate mental health support quickly and easily.

Referrals will normally be made by GPs and other health professionals, but if someone is concerned about their emotional health and wellbeing they can contact the service directly by phoning: 01278 727447.

We have a partnership agreement with Somerset Partnership NHS Foundation Trust to provide social care for adults of working age who have mental health conditions, to help them make the most out of their lives through a recovery-based approach. The Trust also provides mental health services for young people through a separate agreement with us (CAMHS service).

Staff provide support to help manage everyday living, organise activities, employment and training to build selfconfidence, and to give encouragement to become part of the local community and be more independent.

There is increased demand for home care for people with mental health conditions, arising from greater use of supported living instead of care home placements.

Like other specialist care services, this can be paid for (if the person is assessed as eligible) with Direct Payments, discussed on page 45.

Learning disability The Government says that services for people with learning disabilities should promote the following key principles: • respect people’s rights; • actively promote people’s independence; • make sure that people have, and are able to make choices, in all areas of their lives; and • create opportunities for people to be included, rather than separated from society.

In Somerset, the Community Teams for Adults with Learning Disabilities will assess people with a learning disability to help understand what support they may need. This could include, for example, help in the home,

access to work, or transitions from children’s services. The teams have a range of specialist health and social care professionals who can offer advice and guidance.

Increasingly in Somerset, adults with a learning disability are supported to live in the community, accessing ordinary housing and having support consistent with their individual need.

How solicitors can help A solicitor can give you impartial advice about wills, making gifts, estate planning and Powers of Attorney.

Some can also offer guidance on immediate and longterm care plans, making sure (if applicable) the NHS has made the correct contribution to your fees.

Lasting Powers of Attorney (LPA) allow you to appoint someone you trust to make decisions about your personal welfare, including healthcare and consent to medical treatment, and/or your property and financial affairs. An LPA is only valid once it is registered with the Office of the Public Guardian. It allows for a person of your choice to make decisions on your behalf at a time when you may be unable.

The Court of Protection can issue Orders directing the management of a person’s property and financial affairs if they are incapable of managing their own affairs, if they do not have a LPA. The Court procedure is presently very slow and the fees are quite expensive so preparing a LPA is always advisable, providing you have somebody sufficiently trustworthy to appoint as your attorney.

An ‘advance directive’ allows you to communicate your wishes about future medical treatment but it is not legally binding. You may instead wish to make a living will, officially known as an ‘advance decision’, setting out treatment that you do not want to receive in specified circumstances, which would legally have to be followed, even if you die as a result.

Any proposed gift out of your estate needs careful consideration of the benefits, risks and implications, particularly on any future liability for care costs or tax liability.

Long-term care: whether you remain in your own

home or move into sheltered or residential care, you may qualify for financial assistance in the form of social care and NHS-funded care and welfare benefits.

If you don’t have your own solicitor, ask your family or friends for their recommendations. Contact several firms, explain your situation and ask for an estimate of cost and an idea of timescales involved. Many firms will make home visits if necessary and will adapt their communications to meet your needs. It’s important to find a solicitor who specialises in this area of the law. The Citizen’s Advice Bureau offers an advice service and will be able to recommend solicitors in your area.

• Neglect and acts of omission – including ignoring medical, emotional or physical care needs, failure to provide access to appropriate health, care and support or educational services, the withholding of the necessities of life, such as medication, adequate nutrition and heating.

• Self-neglect – this covers a wide range of behaviour neglecting to care for one’s personal hygiene, health or surroundings and includes behaviour such as hoarding or not getting to medical appointments.

Making a complaint We hope you will be happy with the care and support you choose for yourself or someone who matters to you. However, there may be times when you wish to raise a concern, make a comment or suggestion, or a compliment about the support provided. This may be about anything, from the way you feel you are treated by staff to the food you are served.

You should feel free to make comments and suggestions about possible improvements to your surroundings and the services that have been provided.

All care providers are required to have a complaints procedure and are actively encouraged to record feedback received from service users. The best feedback about a service comes from the people who use it or have direct experience of it. If you have something you feel should be said, then there are a number of ways you can be heard.

All care providers should have their own easy to use individual complaints policy. If you are concerned about the care that you, or a friend or relative is receiving, it makes sense to speak to the manager of the service about your concerns before you take any further action.

The problem may be resolved quite easily once they are made aware of it. However, if you need to make a formal complaint, you should first contact the registered owners of the service.

If the care is being provided by an independent agency or organisation, ask to speak to the person who handles their complaints and feedback. They have a duty

When someone has died, there are a lot of people who need to be told, at a time when you probably feel least like doing it. Our Registration Service provides a service which we hope will make things easier for you. This service means that when you tell us about a death, we can then contact other organisations on your behalf.

How the service can help you After someone has died, one of the first things that must be done is to legally register the death with the Council’s Registration Service. Once that’s done, several other organisations may still need to be contacted and given the same information. To save you time and worry,

Somerset’s Registration Service offers a free service which can pass this information directly to a number of other government departments and local council services.

Healthwatch Somerset Healthwatch Somerset is the local consumer champion for people of all ages, taking equal account of both health and social care issues.

Court House Retirement Home, Cheddar Court House is a very special place to live, the very fact that it is made up of different areas of accommodation adds to its attraction.

The Main House with gracious large rooms with full en-suite looking out onto different aspects and personalized with Resident’s own furniture and pictures etc. A spacious Drawing Room with doors to a covered veranda and patio area leading onto lawns and flower beds.
